Sickle cell anemia (SCA) is an inherited hemolytic anemia with compensatory reticulocytosis. Recent studies have shown that increased levels of reticulocytosis during infancy are associated with increased hospitalizations for SCA sequelae as well as cerebrovascular pathologies. In this study, absolute reticulocyte counts (ARC) measured prior to transfusion were analysed among a cohort of 29 pediatric SCA patients receiving chronic transfusion therapy (CTT) for primary and secondary stroke prevention. A cross-sectional flow cytometric analysis of the reticulocyte phenotype was also performed. Mean duration of CTT was 3.1 ± 2.6 years. Background:
Many studies have explored smokers’ behavioral response to cigarette prices at the individual level, but none have factored in regional variation and determinants. This study addresses these research gaps in the Chinese context.
Methods:
A cross-sectional multistage sampling process was used to recruit participants in 21 cities in China. Individual-level information was collected using standardized questionnaires. City-level variables were retrieved from a nationall database. Multilevel logistic regression analysis was used to assess price sensitivity variation at both individual and city levels.

Extracorporeal Photochemotherapy (ECP) is a widely used immunotherapy for cutaneous T cell lymphoma, as well as an immunomodulatory treatment for graft versus host disease (GVHD) and rejection of allografts. We hypothesized that ECP’s physiologic induction of large-scale monocyte-to-dendritic antigen presenting cell (APC) conversion is mechanistically responsible for both its anti-cancer effect and its tolerogenic impact in the transplant setting. To interrogate this possibility in an experimental system, we developed an ECP device that is scalable from mouse to man and tested its capacity to produce APCs that, when advantageously tuned and tumor antigen-loaded, can limit the growth of otherwise lethal …

Diagnosis of lung cancer (LC) has been fraught with difficulty and by the time of definitive diagnosis, most patients are in later stages of the disease. Epidemiological studies have demonstrated that lifestyle behaviors play an etiological role in LC risk; however data in the literature on this topic often appears inconclusive or require further study. BACKGROUND: Minimally invasive surgery (MIS) for trauma in pediatric cases remains controversial. Recent studies have shown the validity of using minimally invasive techniques to decrease the rate of negative and nontherapeutic laparotomy and thoracotomy. The purpose of this study was to evaluate the diagnostic accuracy and therapeutic options of MIS in pediatric trauma at a level I pediatric trauma center.

One of the challenges faced by the practising physician is the interpretation of patient-reported outcomes (PROs) in clinical trials and the relevance of such data to their patients. This is especially true when caring for patients with progressive diseases such as COPD. In an attempt to incorporate the patient perspective, many clinical trials now include assessments of PROs. These are formalized methods of capturing patient-centered information. BACKGROUND: Pirfenidone was recently approved for treatment of idiopathic pulmonary fibrosis. However, the therapeutic dose of pirfenidone is very high, causing side effects that limit its doses and therapeutic effectiveness. Understanding the molecular mechanisms of action of pirfenidone could improve its safety and efficacy. Because activated fibroblasts are critical effector cells associated with the progression of fibrosis, this study investigated the genes that change expression rapidly in response to pirfenidone treatment of pulmonary fibroblasts and explored their contributions to the anti-fibrotic effects of pirfenidone.

BACKGROUND: Roflumilast, a once-daily, selective phosphodiesterase-4 inhibitor, reduces the risk of COPD exacerbations in patients with severe COPD associated with chronic bronchitis and a history of exacerbations. The RE(2)SPOND study is examining whether roflumilast, when added to an inhaled corticosteroid/long-acting β2-agonist (ICS/LABA) fixed-dose combination (FDC), further reduces exacerbations. The methodology is described herein.
METHODS: In this Phase IV, multicenter, double-blind, placebo-controlled, parallel-group trial, participants were randomized 1:1 (stratified by long-acting muscarinic antagonist use) to receive roflumilast or placebo, plus ICS/LABA FDC, for 52 weeks. Clothing can provide safety and comfort for persons exposed to both cold and hot thermal environments. To assess the potential impact of clothing moisture and wetness on fabric cooling, a series of wind-tunnel tests was conducted to quantify the evaporative cooling capacity of selected fabric samples. Single-layer cotton, polyester, nylon and silk were evaluated. The results showed that onset and magnitude of evaporative cooling was determined by the amount of water contained in a fabric sample. The results also showed that an exposed "skin" exhibited more cooling when covered with a fabric than when it was not. The information obtained …

Determining the language-dominant hemisphere is essential for planning epilepsy surgery. A 60-year-old righthanded woman with epilepsy since age 16 failed a partial right anterior lobectomy at age 21. Later, a brain MRI found extensive right-sided cortical dysplasia and periventricular heterotopia. Subsequently, prolonged videoEEG monitoring localized her seizures to the right temporoparietal region. Functional MRI was inconclusive in lateralizing her language, prompting a Wada test, which strongly lateralized language to the right. This unique case of atypical language representation in a right-handed individual with an extensive right-hemispheric congenital malformation and seizure focus illustrates the important thorough presurgical language assessment.

Objective. To examine sex differences in complementary and alternative medicine (CAM) use among adults with multiple chronic conditions. Methods. This study used a cross-sectional design with data from the 2012 National Health Interview Survey. The participants were interviewed in 2012 and the reference period for the questions in the survey varied from 1 week to 12 months prior to the interview date. The study included adults (age > 21 years) with no missing data on CAM use variables and who had multiple chronic conditions. In eukaryotes, pre-mRNA splicing is important step for gene expression. Splicing is a two-step process which is carried out by a multi-megadalton molecular weight ribonucleoprotein (RNP) machinery called spliceosome. Spliceosome converts pre- mRNA to mRNA by removing non-coding sequence (introns) and splice together coding sequence (exons). Mammalian pre-mRNA are spliced by two different class of spliceosomes which are known as U2- and U12- dependent spliceosomes. U12 dependent spliceosome is composed of five small nuclear RNAs (snRNA). Since its discovery over 100 years ago, malaria has been the most important parasitic disease in the world. Malaria affects about 300 to 500 million people every year; killing 5 million people every year. The most virulent causative agent in humans is Plasmodium falciparum; it is an important member of the phylum Apicomplexa, which also includes Toxoplasma gondii. These organisms contain specialized, unique structures, including rhoptry organelles. In Plasmodium and Toxoplasma gondii, they assist invasion into the host cell. The cell surface expresses a variety of carbohydrates such as glycoproteins, proteoglycans and glycolipids. They play critical roles in pathological and physiological processes, including cell signaling, immune responses, pathogen-host interactions, tumor metastasis, and other cellular events. Due to its function and uses in cell communication, it is a curious subject of observation for scientific research. The molecular mechanism of carbohydrate recognition is still undetermined, and lectins, sugar binding proteins, are used to analyze the particular structures expressed on the cell surface.
