Cytomegalovirus Gastrointestinal Disease In Patients With Aids: A Case Series, Akarin Hiransuthikul, Narin Hiransuthikul

Cytomegalovirus (CMV) is an important opportunistic infection in patients with acquired immunodeficiency syndrome (AIDS), usually in those with low CD4 cell counts. CMV colitis is the most common CMV end-organ diseases after CMV retinitis, though cytomegalovirus the gastrointestinal disease (CMV-GID) can occur at any region of gastrointestinal tract. Its clinical manifestations depend on the region of gastrointestinal tract involved. However, due to indistinguishable clinical features from other diseases, further investigations, preferably endoscopy and histopathological testing, are necessary for diagnosis. Prompt diagnosis and treatment are vital to decrease mortality and reduce morbidity in an otherwise fatal disease. Our case series consist …

โปรโตซัวบลาสโตซิสทิส เป็นหนึ่งในโปรโตซัวในลำไส้ที่พบได้ทั่วไปในมนุษย์และสัตว์ต่าง ๆทั่วโลก แม้ว่าจะมีการค้นพบโปรโตซัวชนิดนี้มานานกว่าร้อยปีแล้ว แต่ความสามารถในการก่อโรคของเชื้อยังคงไม่ชัดเจนและเป็นคำถามที่สำคัญในปัจจุบัน นอกจากนี้ยังพบการติดเชื้อบลาสโตซิสทิสจำนวนมากขึ้นเรื่อย ๆ ในช่วงหลายสิบปีที่ผ่านมา ซึ่งก่อให้เกิดอาการทางระบบทางเดินอาหารได้หลากหลาย ผู้ติดเชื้อส่วนมากอาจไม่ปรากฎอาการ หรือปรากฎอาการได้ทั้งแบบเฉียบพลันและแบบเรื้อรัง จากการศึกษาทางอณูชีววิทยาพบว่าโปรโตซัวบลาสโตซิสทิสมีความหลากหลายทางพันธุกรรมเป็นอย่างมาก จนถึงปัจจุบันนี้มีรายงานสายพันธุ์ต่าง ๆ แล้วถึง 17 สายพันธุ์ โดยสายพันธุ์ที่ติดเชื้อในคนคือสายพันธุ์ที่ 1 - 9 อย่างไรก็ตาม ความชุกและการกระจายตัวของสายพันธุ์ต่าง ๆ มีความแตกต่างกันไปในแต่ละประเทศทั่วโลก นอกจากนี้ จากความหลากหลายทางพันธุกรรมนี้ จึงสันนิษฐานว่ามีความสัมพันธ์กับการเกิดอาการทางคลินิกที่หลากหลายของผู้ติดเชื้อโปรโตซัวบลาสโตซิสทิสบทความฟื้นฟูวิชาการนี้ จึงรวบรวมความรู้ปัจจุบันของชีววิทยาของโปรโตซัวบลาสโตซิสทิส รวมถึงความชุกและการกระจายตัวของสายพันธุ์ต่าง ๆ ในแต่ละประเทศทั่วโลก ตลอดจนความสัมพันธ์ระหว่างสายพันธุ์ของโปรโตซัวบลาสโตซิสทิสกับอาการทางคลินิกของผู้ติดเชื้อ ซึ่งจะเป็นข้อมูลที่สำคัญเพื่อความเข้าใจความสามารถและบทบาทในการก่อโรคของโปรโตซัวบลาสโตซิสทิส และจะนำไปสู่การวางแผนการรักษาของแพทย์ ซึ่งจะทำให้สามารถป้องกันการแพร่กระจายของเชื้อและป้องกันการเกิดการดื้อยาของเชื้อได้ต่อไป.

Surgical Management Of Inflammatory Bowel Disease: A Low Prevalence, Developing Country Perspective, Sana Nasim, Tabish Chawla, Ghulam Murtaza Sheikh

OBJECTIVE:

To determine the outcomes of surgical management of inflammatory bowel disease.

METHODS:

The retrospective case series was conducted at Aga Khan University Hospital, Karachi, and comprised medical record of adult patients operated between January 1986 and December 2010 for inflammatory bowel disease. Outcomes consisted of complications till last follow-up and 30-day mortality (disease or procedure related). Functional status of patients with ileal pouch was determined via telephone. SPSS 16 was used to analyse data.

RESULTS:

Ovarian malignancy is a serious disease affecting women of all ages, more so above 50 years, and they are still difficult to treat, partly because no truly effective therapy has yet been developed although presentation is often vague and non-specific, the symptoms are definitely present. It is important to recognize the symptoms so far, there is no test yet available, which is truly specific and suitable for screening and early detection of epithelial ovarian carcinoma. So, it is concluded that for prognosis and patient survival, early detection and treatment is mandatory.

Long non-coding RNAs (lncRNAs) have been recently shown to play an important role in gene regulation and normal cellular functions, and disease processes. However, despite the overwhelming number of lncRNAs identified to date, little is known about their role in cancer for vast majority of them. The present study aims to determine whether lncRNAs can serve as prognostic markers in human breast cancer. We interrogated the breast invasive carcinoma dataset of the Cancer Genome Atlas (TCGA) at the cBioPortal consisting of ~ 1,000 cases. Because of the potential link between the ongoing Zika virus outbreak and a surge in the number of cases of congenital microcephaly, officials in Latin America have recommended that women postpone pregnancy until this association is firmly established or the outbreak subsides. We examined the pharmacokinetic properties of vancomycin conjugated to a bone-targeting agent (BT) with high affinity for hydroxyapatite after systemic intravenous administration. The results confirm enhanced persistence of BT-vancomycin in plasma and enhanced accumulation in bone relative to vancomycin. This suggests that BT-vancomycin may be a potential carrier for the systemic targeted delivery of vancomycin in the treatment of bone infections, potentially reducing the reliance on surgical debridement to achieve the desired therapeutic outcome.

Primary Hepatic Carcinoid Tumor (PHCT) represents an extremely rare clinical entity with only a few cases reported to date. These tumors are rarely associated with metastasis and surgical resection is usually curative. Herein, we report two cases of PHCT associated with poor outcomes due to late diagnosis. Both cases presented late with non-specific symptoms. One patient presented after a 2-week history of symptoms and the second case had a longstanding two years symptomatic interval during which he remained undiagnosed and not properly worked up. Background: Intrathecal adjuvants are added to local anaesthetics to improve the quality of neuraxial blockade and prolong the duration of analgesia during spinal anaesthesia. Used intrathecally, fentanyl improves the quality of spinal blockade as compared to plain bupivacaine and confers a short duration of post-operative analgesia. Intrathecal midazolam as an adjuvant has been used and shown to improve the quality of spinal anaesthesia and prolong the duration of post-operative analgesia. No studies have been done comparing intrathecal fentanyl with bupivacaine and intrathecal 2 mg midazolam with bupivacaine.

Although the effect of experimental surface modifications on various properties (e.g., fatigue, wear) on coated files have been tested in the past, there is no report for the coating quality of commercially available TiN coated files. The aim of this study was to characterize the surface and cross section of TiN coated endodontic files.

Materials and methods

TiN coated nickel–titanium endodontic files (EasyShape) were surface and cross-sectionally analyzed via scanning electron microscopy backscattered electron imaging and energy-dispersive X-ray spectroscopy analysis in spot, area, and line scan modes.

The population of older adults is increasing rapidly and is expected to reach 83.7 million by the year 2050. Previous research demonstrates that greater resourcefulness is associated with better quality of life and life satisfaction. The purpose of this pilot study was to evaluate the effects of a resourcefulness training intervention on positive cognitions, resourcefulness, relocation adjustment, and adaptive functioning among older adults who have relocated to retirement communities. Enjoyment is an important construct for understanding physical activity participation, and it has not been examined in adults with functional limitations. This secondary analysis reported the reliability and validity of the Physical Activity Enjoyment Scale (PACES) in a convenience sample of 40 adults with functional limitations. The participants completed the PACES, Center for Epidemiological Studies Depression Scale (CES-D), and the Late Life Function and Disability Instrument (LLFDI) prior to beginning a 12-week feasibility dance intervention study. We undertook a cross-sectional survey of antimicrobial stewardship clinicians in North America and Australasia regarding practices, goals, and barriers to implementation of stewardship for pediatric oncology patients. Goals and barriers were similar regardless of clinician or institutional characteristics and geographic location. Strategies addressing these factors could help optimize antimicrobial use.

The cytochrome P450-2D6 (CYP2D6) enzyme metabolizes ∼25% of common medications, yet homologous pseudogenes and copy number variants (CNVs) make interrogating the polymorphic CYP2D6 gene with short-read sequencing challenging. Therefore, we developed a novel long-read, full gene CYP2D6 single molecule real-time (SMRT) sequencing method using the Pacific Biosciences platform. Long-range PCR and CYP2D6 SMRT sequencing of 10 previously genotyped controls identified expected star (*) alleles, but also enabled suballele resolution, diplotype refinement, and discovery of novel alleles. Coupled with an optimized variant-calling pipeline, CYP2D6 SMRT sequencing was highly reproducible as triplicate intra- and inter-run nonreference genotype results were completely concordant. OBJECTIVES: To investigate whether walking mediates neighborhood built environment associations with weight status in middle- and older-aged women.

METHODS: Participants (N=5085; mean age=64 ± 7.7; 75.4% White non-Hispanic) were from the Women's Health Initiative San Diego cohort baseline visits. Body mass index (BMI) and waist circumference were measured objectively. Walking was assessed via survey. The geographic information system (GIS)-based home neighborhood activity supportiveness index included residential density, street connectivity, land use mix, and number of parks.

RESULTS: BMI was 0.22 units higher and the odds ratio for being obese (vs. normal or overweight) was 8% higher for every standard deviation …

Many people living with HIV desire childbearing, but low cost safer conception methods (SCM) such as timed unprotected intercourse (TUI) and manual self-insemination (MSI) are rarely used. We examined awareness and attitudes towards SCM, and the correlates of these constructs among 400 HIV clients with fertility intentions in Uganda. Measures included awareness, self-efficacy, and motivation regarding SCM, as well as demographics, health management, partner and provider characteristics. Just over half knew that MSI (53%) and TUI (51%) reduced transmission risk during conception, and 15% knew of sperm washing and pre-exposure prophylaxis. Hydroxyurea (HU) is a crucial therapy for children with sickle cell anemia, but its off-label use is a barrier to widespread acceptance. We found HU exposure is not significantly altered by liquid vs capsule formulation, and weight-based dosing schemes provide consistent exposure. Inherited renal disease is a leading cause of morbidity and mortality in pediatric nephrology. High throughput advancements in genomics have led to greater understanding of the biologic underpinnings of these diseases. However, the underlying genetic changes explain only part of the molecular biology that contributes to disease manifestation and progression. Other omics technologies will provide a more complete picture of these cellular processes. This review discusses these omics technologies in the context of pediatric renal disease.

Introduction: Although sexual dysfunction is common after prostate cancer, men's decisions to seek help for sexual concerns are not well understood.

Aim: Describe predictors of actual prior help-seeking and intended future medical help-seeking for sexual dysfunction in prostate cancer survivors.

Methods: A cross-sectional survey of 510 prostate cancer survivors assessed masculine beliefs, attitudes, support/approval from partner/peer networks (subjective norm), and perceived control as predictors of medical help-seeking for sexual concerns. A theory of planned behavior (TPB) perspective was used to examine actual prior and planned future behavior and contributing factors. Statistical analyses included multiple and logistic regressions.
