BACKGROUND: Sepsis, a leading cause of morbidity and mortality, is not a homogeneous disease but rather a syndrome encompassing many heterogeneous pathophysiologies. Patient factors including genetics predispose to poor outcomes, though current clinical characterizations fail to identify those at greatest risk of progression and mortality.

METHODS: The Community Acquired Pneumonia and Sepsis Outcome Diagnostic study enrolled 1,152 subjects with suspected sepsis. We sequenced peripheral blood RNA of 129 representative subjects with systemic inflammatory response syndrome (SIRS) or sepsis (SIRS due to infection), including 78 sepsis survivors and 28 sepsis non-survivors who had previously undergone plasma proteomic and metabolomic profiling. This article compares the International Nursing Association of Clinical Simulation and Learning (INACSL) memberships' thoughts and discussions about the use of simulation for high- stakes evaluation in nursing education to a similar town hall discussion, five years ago. Data on the topic of high-stakes testing were collected through a survey before the INACSL conference in June 2014. During a town hall meeting at the conference, the survey data was presented, attendees shared their thoughts and reacted to prepared comments by selected simulation leaders. Patellar tendon tears often occur in patients less than 40 years old with forced flexion of the knee. Surgical repair of the tendon is most commonly the treatment following a tear; additionally, performing surgery to repair the tendon in a timely fashion is an important prognostic factor. The purpose of this case report is to outline the deficits following surgical repair of a left patellar tendon rupture, describe specific physical therapy interventions used during 12 weeks of outpatient rehabilitation, and report the outcomes of physical therapy.

Pseudomonas aeruginosa infects every type of host that has been examined by deploying multiple virulence factors. Previous studies of virulence regulation have largely focused on chemical cues, but P. aeruginosa may also respond to mechanical cues. Using a rapid imaging-based virulence assay, we demonstrate that P. aeruginosa activates virulence in response to attachment to a range of chemically distinct surfaces, suggesting that this bacterial species responds to mechanical properties of its substrates. Surface-activated virulence requires quorum sensing, but activating quorum sensing does not induce virulence without surface attachment. This study investigated the possibility of quantifying hearing aid benefit by means of the Audibility Index. The Audibility Index (AI) is defined as the amount of the audible speech spectrum available to a listener. Twenty-one participants with bilateral sensorineural hearing losses, each required to wear bilateral hearing aids, were recruited for this study. Utilizing FM tone stimuli, audiometric thresholds were established for each individual conventionally under earphones for the right and left ears, unaided in a sound field, and aided in a sound field. The thresholds were placed on a count-the-dot audiogram in order to calculate AIs for each individual. …

The 'Retinoic Acid-Inducible G-protein-coupled receptors' or RAIG are a group comprising the four orphan receptors GPRC5A, GPRC5B, GPRC5C and GPRC5D. As the name implies, their expression is induced by retinoic acid but beyond that very little is known about their function. In recent years, one member, GPRC5A, has been receiving increasing attention as it was shown to play important roles in human cancers. Background: Central line–associated bloodstream infections (CLABSIs) result in increased length of stay, cost, and patient morbidity and mortality. One CLABSI prevention method is disinfection of intravenous access points. The literature suggests that placing disinfectant caps over needleless connectors decreases CLABSI risk.

Methods: A quasi-experimental intervention study was conducted in a >430-bed trauma I center. In addition to an existing standard central line bundle, a new intervention consisting of a luer-lock disinfectant cap with 70% alcohol was implemented in all intravenous (IV) needleless connectors on patients with peripheral and central lines. Children born preterm show persisting impairments in cognitive functioning, school achievement, and brain development. Most research has focused on implications of birth prior to 37 gestational weeks; however, the fetal central nervous system continues to make fundamental changes throughout gestation. Longer gestation is associated with reduced morbidity and mortality even among infants born during the period clinically defined as full term (37–41 gestational weeks). The implications of shortened gestation among term infants for neurodevelopment are poorly understood. Background: Unicompartmental knee arthroplasty (UKA): replacement of one compartment of the tibiofemoral joint. A balance between an ambitious plan of care and mindfulness of the acute inflammatory process poses a challenge for acute care physical therapists. There is a lack of literature that addresses the acute care physical therapy management of patients who have undergone this procedure. Specifically, little is known about the acute care physical therapy management of former elite athletes who undergo UKA in middle age. Mortality from stroke is the 4th leading cause of death in the US, and is a leading cause of long-term severe disability among adults. Incidences of disabilities include: hemiparesis, unable to walk without assistance, dependent in ADLs, aphasia, and depression. Stroke survivors represent the largest group admitted to rehabilitation hospitals and approximately 1 in 4 patients with stroke are placed in long-term care. The purpose of this case report is to be a description of practice and interventions targeting functional mobility and gait retraining for patients within inpatient rehabilitation setting.

Oscillatory breathing (OB) patterns are observed in pre-term infants, patients with cardio-renal impairment, and in otherwise healthy humans exposed to high altitude. Enhanced carotid body (CB) chemoreflex sensitivity is common to all of these populations and is thought to contribute to these abnormal patterns by destabilizing the respiratory control system. OB patterns in chronic heart failure (CHF) patients are associated with greater levels of tonic and chemoreflex-evoked sympathetic nerve activity (SNA), which is associated with greater morbidity and poor prognosis. Enhanced chemoreflex drive may contribute to tonic elevations in SNA by strengthening the relationship between respiratory and sympathetic neural outflow. …

This review presents the mechanistic underpinnings of corticospinal tract (CST) development, derived from animal models, and applies what has been learned to inform neural activity-based strategies for CST repair.We first discuss that, in normal development, early bilateral CST projections are later refined into a dense crossed CST projection, with maintenance of sparse ipsilateral projections. Using a novel mouse genetic model, we show that promoting the ipsilateral CST projection produces mirror movements, common in hemiplegic cerebral palsy (CP), suggesting that ipsilateral CST projections become maladaptive when they become abnormally dense and strong.We next discuss howanimal studies support a developmental “competition rule” …

Background Over the past ten years, there has been an explosion of microbiome research. Many software packages for analyzing microbial sequences such as the 16S gene from 454 sequencers and Illumina platforms are available. But for a new researcher, it is difficult to know which package to choose. We present a systematic review of packages for the analysis of molecular sequences used to describe and compare microbial communities. This review gives students and researchers information to help choose the best analytic pipeline for their project. To the best of our knowledge, this is the first review of such software.
