วัตถุประสงค์: เพื่อพัฒนาแบบประเมินศักยภาพพยาบาลในการส่งเสริมคุณภาพชีวิตของผู้ป่วยมะเร็งเต้านม\n \nรูปแบบการวิจัย: การพัฒนาเครื่องมือ\n \nวิธีดําเนินการวิจัย: กลุ่มตัวอย่าง คือ พยาบาลที่มีประสบการณ์การดูแลผู้ป่วยมะเร็งเต้านม โดยคัดเลือก จากพยาบาลวิชาชีพที่ดูแลผู้ป่วยมะเร็งเต้านม จํานวน 120 คน ดําเนินการสร้างแบบประเมินศักยภาพ พยาบาล ตามขั้นตอนดังนี้ 1) กําหนดคํานิยามเชิงปฏิบัติการ จากการศึกษาแนวคิดและทําการ สนทนากลุ่มกับพยาบาลที่ให้การดูแลผู้ป่วยมะเร็งเต้านม จํานวน 19 คน 2) สร้างข้อคําถามตามคํานิยาม ที่ได้ 6 มิติ จํานวนทั้งหมด 30 ข้อ ลักษณะคําตอบเป็นมาตรวัดประมาณค่า 5 ระดับ ตั้งแต่ ปฏิบัติประจํา (5 คะแนน) ถึง ไม่เคยปฏิบัติ (1 คะแนน) 3) ตรวจสอบความตรงเชิงเนื้อหาของแบบประเมินโดยผู้เชี่ยวชาญ 3 คน ได้ค่าดัชนีความตรงเท่ากับ .93 โดยไม่มีข้อใดถูกตัดออก 4) หาความเที่ยงโดยนําไปทดลองใช้กับ พยาบาลผู้ให้การดูแลผู้ป่วยมะเร็งเต้านม จํานวน 30 คน ได้สัมประสิทธิ์อัลฟ่าของครอนบาค เท่ากับ .80 และ 5) ตรวจสอบความตรงเชิงโครงสร้าง (Construct validity) โดยการวิเคราะห์ปัจจัย (Factor analysis) ของแบบประเมินที่นําไปใช้กับพยาบาลวิชาชีพที่ดูแลผู้ป่วยมะเร็งเต้านม จํานวน 120 คน\n\nผลการศึกษา: พบว่า แบบประเมินศักยภาพพยาบาลมีองค์ประกอบหลัก 3 องค์ประกอบ รวม 30 ข้อ คือ 1) ศักยภาพการดํารงตน ประกอบด้วยข้อคําถาม จํานวน 20 ข้อ สามารถอธิบายความแปรปรวนได้ ร้อยละ 67.14 2) ศักยภาพการดูแลวิถีสุขภาพของตนเอง ประกอบด้วยข้อคําถาม จํานวน 5 ข้อ สามารถ อธิบายความแปรปรวนได้ร้อยละ 4.53 และ 3) ศักยภาพการดูแลปัญหาเฉพาะโรค ประกอบด้วยข้อคําถาม …

วัตถุประสงค์: เพื่อศึกษาอาการ กลวิธีการจัดการกับอาการ และคุณภาพชีวิตของผู้ป่วยมะเร็งศีรษะและ คอเพศชายขณะได้รับรังสีรักษา และศึกษาความสัมพันธ์ระหว่างอาการ การสนับสนุนทางสังคม และคุณภาพชีวิตของผู้ป่วยมะเร็งศีรษะและคอเพศชายขณะได้รับรังสี\n \nรูปแบบการวิจัย: การวิจัยแบบบรรยายเชิงความสัมพันธ์\n \nวิธีดําเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยมะเร็งศีรษะและคอเพศชายขณะได้รับรังสีรักษา ณ ศูนย์มะเร็ง สุราษฎร์ธานี คัดเลือกตามสะดวก จํานวน 130 คน เก็บรวบรวมข้อมูลโดยใช้ แบบสอบถามข้อมูลส่วน บุคคล แบบสอบถามอาการแบบสอบถามกลวิธีการจัดการกับอาการ แบบสอบถามการสนับสนุนทางสังคม และแบบสอบถามคุณภาพชีวิตของผู้ป่วยมะเร็งศีรษะและคอที่ได้รับรังสีรักษา มีค่าดัชนีความตรงเชิง เนื้อหาเท่ากับ .80, .87, .80, และ .81 ตามลําดับ มีค่าสัมประสิทธิ์อัลฟาของครอนบาคเท่ากับ .80, .75, .80, และ .71 ตามลําดับ วิเคราะห์ข้อมูลโดยการหาค่าความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน\n \nผลการวิจัย: 1) อาการน้ำลายเหนียว/คอแห้ง มีความถี่และความรุนแรงของอาการมากที่สุด โดยจัดการ กับอาการด้วยการใช้น้ำยาบ้วนปาก การใช้ยาแก้ปวด และการใส่สายให้อาหาร มากที่สุด 2) คุณภาพ\nชีวิตของผู้ป่วยมะเร็งศีรษะและคอเพศชายภาคใต้ขณะได้รับรังสีรักษาอยู่ในระดับสูง (x̄ = 2.90, SD = 0.52) 3) ความถี่และความรุนแรงของอาการมีความสัมพันธ์ทางลบในระดับต่ำและระดับปานกลางกับคุณภาพ ชีวิตของผู้ป่วยมะเร็งศีรษะและคอเพศชายภาคใต้ขณะได้รับรังสีรักษา (r = -.28, = -.38, p<.05) และ 4) การสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกในระดับต่ำกับคุณภาพชีวิตของผู้ป่วยมะเร็งศีรษะและ คอเพศชายภาคใต้ขณะได้รับรังสีรักษา (r=.13, p<.05)\n \nสรุป: อาการต่างๆ ที่เกิดขึ้นกับผู้ป่วยส่งผลต่อคุณภาพชีวิตของผู้ป่วยในด้านร่างกายมากที่สุด ดังนั้นพยาบาล จึงควรมีการพัฒนากลวิธีการจัดการกับอาการที่เกิดขึ้นอย่างเหมาะสม โดยส่วนใหญ่เกิดในช่องปากที่มีทั้ง ความรุนแรงและความถี่ โดยให้สมาชิกในครอบครัวเข้ามามีส่วนร่วม โดยเริ่มตั้งแต่ก่อน ขณะ และหลัง สิ้นสุดการรักษา เพื่อให้เป็นแนวทางในการดูแลผู้ป่วยอย่างต่อเนื่อง\n

Background: Although balance exercises are often performed in shallow pools, the efficacy of this practice is not well understood. Purpose: To quantify the effect of water depth and jet intensity on measures of balance in older and younger adults. Subjects: Sixteen older (age = 62.8 °” 9.56 yrs) and fifteen younger (age = 22.5 °” 1.85 yrs) adults. Methods: Posturography data were collected as participants stood quietly for 90 seconds on land and at various water depths and jet intensities. Results: Main effects < 0.001) and pairwise comparisons revealed that all measurements increased with increasing water depth and jet intensity,with older adults increasing statistically less than younger adults at higher jet intensities. Conclusions: Water immersion to the xiphoid process and water jet intensities at or above 40% seem to be environmental conditions that produce the greatest postural sway.

It is recommended that children with HL are assessed using standardized assessments normed on hearing peers (Houston & Caraway, 2009; Joint Commission on Infant Hearing (JCIH), 2007). However, as these assessments are more commonly administered to children with HL there is reason to further investigate the sensitivity of these assessments particularly in their ability to identify weaknesses specific to HL. Nonheterosexuals disproportionately experience mental illness when compared with heterosexuals. Although it has been well established that the apparent mental health disparity among sexually diverse populations is almost exclusively derived from minority stress (i.e., the excess stress associated with culturally maintained sexual stigma), our understanding of how specific aspects of minority stress lead to specific psychological syndromes (e.g., depression) remains limited. Physical, mental, and emotional changes can occur throughout the aging process, making it important to treat different age groups as separate populations when researching body image. Yet, very few instruments have been validated for middle-aged and older adults. An online survey was used to perform a validation of the Body Image Quality of Life Inventory (BIQLI) and examine demographic relationships, with a sample of 947 women, ages 40-79, from across the United States. This is the first study to examine how pastors lost authority over bodily healing in the nineteenth century. I argue that clergymen adapted to the scientific and cultural developments of the post-Darwin era by encouraging a separation of bodily healing and spiritual healing that was unprecedented in Christianity. In 1840, ministers wielded much more authority than physicians, who still practiced a heroic medicine whose foundations predated Christ. Ministers insisted that doctors' value lay in their willingness to be proselytizing Christians. The dissertation consists of three chapters exploring the effect of school program and neighborhood environment on childhood obesity outcome using individual panel data set of Arkansas public schoolchildren.

The first chapter (Section 2) investigates how the Fresh Fruit and Vegetable Program (FFVP), a program that provides funding for the distribution of free fresh fruits and vegetables to students in participating schools, affects childhood obesity. We combine matching methodology and difference-in-differences analysis to estimate the effect of the FFVP on childhood BMI outcomes. Worldwide, Campylobacter is one of the leading causes of foodborne bacterial gastroenteritis causing an estimated 1.3 million infections in the United States alone. Consumption and/or cross-contamination of raw or undercooked poultry products have been linked as the most common source of Campylobacter infection, making the poultry industry a target for Campylobacter reduction strategies. Campylobacter is prevalent in most poultry flocks in the United States, with as many as 90% of flocks Campylobacter-positive at the time of slaughter. Endogenous cannabinoids (endocannabinoids) exert a wide range of biological effects. In addition to having their well-known neurobehavioral effects, a role for the major endocannabinoid anandamide (N-arachidonoyl ethanolamide; AEA), in the cardiovascular system in various pathological conditions has been reported. The aim of this thesis is to explore the effects of AEA on contractility, Ca2+ signaling, and action potential (AP) characteristics in rat ventricular myocytes. A video edge detection system was used to measure myocyte shortening. Intracellular Ca2+ was measured in cells loaded with the fluorescent indicator fura-2 AM. Patients typically undergo physiotherapy with the help of a physiotherapist who teaches, guides, and corrects the patients as they perform exercises. It would be nice if people could repeat these exercises at home, potentially improving their recovery rate. However, without guidance and/or corrective feedback from a physiotherapist, the patient will not know whether they are doing their exercises correctly. To address this problem, we implemented a prototype that guides patients through pre-recorded exercise movements using visual guides overlaid atop a mirror-view of the patient on a wall-mounted display. 3D volumetric medical images, such as MRIs, are commonly explored and interacted with by medical imaging experts using systems that require keyboard and mouse-based techniques. These techniques have presented challenges for medical imaging specialists: 3D spatial navigation is difficult, in addition to the detailed selection and analysis of 3D medical images being difficult due to depth perception and occlusion issues. Group exercise provides motivation to follow and maintain a healthy daily exercise schedule while enjoying beneficial encouragement and social support from friends and exercise partners. However, mobility and transportation issues frequently prevent seniors from engaging in group activities. To address this problem, we investigated the exercise needs of seniors and developed a prototype remote exercise system. Our system uses haptic feedback to simulate assistive pushing and pulling of limbs when exercising with a partner. The aggregation of amyloidogenic proteins is a critical event in the pathology of a variety of neurodegenerative diseases, including Alzheimer’s disease (AD) and Parkinson’s disease (PD). The proteins α-synuclein (αS) and amyloid-β (Aβ) are involved in the formation of amyloid lesions observed in PD and AD, respectively. Both PD and AD exhibit a significant amount of co-pathology in clinical settings, and the αS and Aβ proteins have been shown to interact in vitro. Activation of the innate immune system is a prerequisite for the induction of adaptive immune responses to both infectious and non-infectious agents. Toll-like receptors (TLRs) are a family of proteins important for recognizing pathogen associated molecular patterns. MyD88 is an adaptor molecule whose function is critical in TLR signaling. MyD88-deficient (MyD88-/-) mice exhibit heightened susceptibility to infections, even by attenuated strains of Salmonella enterica serovar Typhimurium. Paradoxically, despite their hypersusceptibility, infected MyD88-/- mice produce elevated serum levels of anti-Salmonella antibodies regardless of the route of infection. The majority of diabetics develop neuropathy, which can be debilitating, but the underlying pathophysiological mechanisms are poorly understood. Diabetic neuropathy progresses in a distal to proximal manner. Previous studies have shown that glutamate, the most common excitatory neurotransmitter, plays a role in the pathogenesis of neuropathy. The reason why the role of glutamate in nociception becomes a problem in diabetes and the mechanisms that are involved are unknown. Myocardial infarction is the most serious manifestation of coronary artery disease and the cause of significant levels of mortality and morbidity worldwide. Galectin-1 (GAL-1) and Galectin-3 (GAL-3) are beta galactoside binding lectins with diverse functions. Hypoxia inducible factor-1 alpha (HIF-1α) is a transcription factor mediating early and late responses to myocardial ischemia. The pathological features of Alzheimer’s disease (AD) have been researched and documented extensively, however the causes of these features are still unknown. The following studies sought to determine if epigenetic methylation alterations contribute to AD. Two studies were sequentially carried out, first using an IMR-32 model and then using a transgenic mouse model overexpressing beta-amyloid. Adoptive immunotherapy can retarget T cells to CD19, a tumor-associated antigen (TAA) expressed on B-cell malignancies, by the expression of a chimeric antigen receptor (CAR). Infusion of CAR-modified T cells for the treatment B-cell malignancies has demonstrated promise in preclinical and clinical trials. These data highlight the ability of infused CD19-specific T cells to be synchronously activated by large burdens of CD19+ leukemia and lymphoma. Epithelial malignant tumors, such as breast, colon, ovarian and pancreatic cancers have been shown to overexpress proteins that control cell mitosis, growth, and proliferation. The survival of patients with osteosarcoma lung metastases has not improved in the last 20 years. Novel alternative therapies are needed. The purpose of this investigation was to evaluate the efficacy of combining natural killer [NK] cell therapy with aerosol interleukin-2 [IL-2] for the treatment of osteosarcoma lung metastasis. The expression of NKG2D ligands was analyzed in five different human osteosarcoma cell lines and 103 patient samples (47 from primary tumors and 56 from lung metastases). The epidermal growth factor receptor (EGFR) and fibroblast growth factor receptor (FGFR) are activated by gene amplification, mutation and overexpression in bladder cancer, which drives tumor development and progression. Both EGFR and FGFR inhibitors are currently being tested in clinical trials. However, bladder cancer (BC) cells show remarkably heterogeneous sensitivities to both inhibitors, and the molecular determinants of this heterogeneity are presently unclear. Among radiopharmaceuticals of positron emission tomography (PET), 18F-Fluorodeoxyglucose (18F-FDG) made from commercialized automated synthesis module is the most frequently used in tumor diagnoses. But the false positive findings, such as infectious tissues and post-operative surgical conditions, show strong uptake of 18F-FDG in PET scans which requires extra clinical procedures to confirm the results. Moreover, the false negative findings, such as low glycolytic activity tumors, reduce the accuracy of PET scans. Therefore, there will be new PET radiopharmaceuticals to redeem the defects of 18FDG-PET applications.

Retinitis pigmentosa (RP) is an inherited retinal disease that leads to degeneration of the retina through loss of rod and cone photoreceptor cells and subsequent loss of vision. RP affects approximately 1.5 million people world-wide. Mutations causing autosomal dominant retinitis pigmentosa (adRP) have been identified in 23 different genes. However, these mutations only account for approximately 70% of known adRP cases in Caucasians of Western-European origin and for an even smaller percentage of cases in other ethnicities. SCFFBXW7 is a tumor suppressor E3 ligase protein that targets numerous oncoproteins such as Cyclin E, c-Myc, c-Jun and MCL1. The deregulation of these proteins often leads to the proliferation of cancer cells. Thus, intracellular stability and functional activity of FBXW7 is critical for regulating cancer. However, there is a gap of knowledge about the intracellular signaling pathway or if there is another ubiquitin ligase that regulates FBXW7 stability. Here, I identify a novel mechanism of FBXW7 stability regulation which involves constitutive photomorphogenic 1 (COP1), AKT and CSN6 (i.e. the COP9 signalosome 6).
