Parasite abundance and community structure could vary depending on conditions that are both extrinsic and intrinsic to the host. Variation in parasite burden, resulting from heterogeneous exposure to parasites or heterogeneous establishment of parasites in the host, often drives transmission dynamics. Changes in parasite community structure influence parasite species diversity and thus, diversity of infection within a single host or entire host population. I sought to quantify relative roles of exposure and establishment in producing patterns of infection using bats, Eptesicus fuscus, and their helminths. Aims: To evaluate the relationship between self-reported head injury and cognitive impairment, dementia, mortality, and Alzheimer's disease (AD)-type pathological changes. Methods: Clinical and neuropathological data from participants enrolled in a longitudinal study of aging and cognition (n = 649) were analyzed to assess the chronic effects of self-reported head injury. Results: The effect of self-reported head injury on the clinical state depended on the age at assessment: for a 1-year increase in age, the OR for the transition to clinical mild cognitive impairment (MCI) at the next visit for participants with a history of head injury was 1.21 and 1.34 …

Abstract
INTRODUCTION:
This study compares the effectiveness and cost of trans-catheter verses surgical closure of secundum atrial septumdefect (ASD). ASD accounts for 10% of congenital cardiac defects. Trans-catheter closure of secundum ASD is increasingly used as the primary intervention. Surgical repair is advised in a proportion of secundum type defects which are unsuitable for device closure.
METHODS:
We reviewed the clinical course of 176 patients who underwent closure of isolated secundum ASD. The patients were assigned to either the device or surgical group depending upon the treatment they received. Successful closure was assessed immediately after the procedure. The following outcomes …

Background—Data on risk associated with 24-hour ambulatory diastolic (DBP24) versus systolic (SBP24) blood pressure are scarce.
Methods and Results—We recorded 24-hour blood pressure and health outcomes in 8341 untreated people (mean age, 50.8 years; 46.6% women) randomly recruited from 12 populations. We computed hazard ratios (HRs) using multivariable-adjusted Cox regression. Over 11.2 years (median), 927 (11.1%) participants died, 356 (4.3%) from cardiovascular causes, and 744 (8.9%) experienced a fatal or nonfatal cardiovascular event. Isolated diastolic hypertension (DBP24≥80 mm Hg) did not increase the risk of total mortality, cardiovascular mortality, or stroke (HRs≤1.54; P≥0.18), but was associated with a …

BACKGROUND: Heart failure (HF) patients experience impaired functional status, diminished quality of life, high utilization of healthcare resources, and poor survival. Yet, the identification of patient-centered factors that influence prognosis is lacking.
METHODS AND RESULTS: We determined the association of 2 measures of self-rated health with healthcare utilization and skilled nursing facility (SNF) admission in a community cohort of 417 HF patients prospectively enrolled between October 2007 and December 2010 from Olmsted County, MN. Patients completed a 12-item Short Form Health Survey (SF-12). Low self-reported physical functioning was defined as a score ≤ 25 on the SF-12 physical component. The …

Strokes are the leading cause of adult disability and death in China. The prevalence is 6 - 9% among adults over 65 in urban areas. It is estimated that currently there are more than 7.5 million stroke survivors with an annual increase of 2.5 million new cases. The purpose of this study was to assess the correlation of gingival recession to the following parameters in fresh cadavers: gingival thickness, buccal bone loss, buccal bone thickness, shape of bony dehiscence defect, and age. A secondary aim was to evaluate predictors for gingival recession.
Sixteen fresh cadavers were used in this study. Gingival recession, facial gingival thickness, alveolar bone loss, and buccal bone thickness were measured at teeth #6-#11 and #22-#27. Objective Vegetarian dietary patterns represent longstanding, real-world diets consumed by a minority of persons. Studies of important health outcomes of such diets, particularly for all-cause mortality and colorectal cancer risk, have yielded inconsistent results. We sought to examine these outcomes (mortality and colorectal cancer incidence) in a large North-American cohort. We also sought to further characterize potentially important differences in the food consumption patterns of these diets. Systemic Lupus Erythematosus and Rheumatoid Arthritis are B cell-mediated autoimmune diseases that afflict millions of people worldwide. B cell-targeted therapies for these diseases result in variable clinical outcomes. Thus, a need exists to better understand the dynamics of human B cell production and function. The mouse model has provided a foundation for understanding the mechanisms involved in human B cell development and autoimmune disease. Developmental programming of health and disease correlates the adverse intrauterine environment and the risk of disease later in life as cardiovascular disease, metabolic diseases, renal dysfunction or neurobehavioral disorders among others. Maternal hypoxia is one of the most common intrauterine growth restricted environment during pregnancy. Spatial and temporal regulation of developmental gene transcription often involves regulatory sequences found in noncoding DNA separate from the gene’s promoter. Conservation of a noncoding region (CNR) across divergent species may indicate a regulatory region critical to basic morphogenesis. CNR-green fluorescent protein (GFP) constructs can demonstrate spatial and temporal CNR activity by microscopy visualization. However, characterization of regulatory modules within a CNR requires quantitative, in addition to qualitative, analysis. There is a need for culturally attuned approaches for couple’s therapy with Mexican/ Mexican-American people (Falicov, 2009). However, there have been few theoretical frameworks to conceptualize cultural attunement, and attunement has been primarily understood as a skill or as an attitude rather than as a relationship or as a series of clinical concepts with steps to follow (Hoskins, 1999; Oakes, 2011). Older adults are the fastest growing segment of the population in Jamaica. With an increase from 10% (252,225) of the total Jamaican population in 1995 to 11% (279,051) in 2005, the rate is expected to rise to 25% by 2025. Objective: Depression is a significant global public health burden, and older adults may be particularly vulnerable to its effects. Among other risk factors, interpersonal conflicts, such as perceived criticism from family members, can increase risk for depressive symptoms in this population. We examined family criticism as a predictor of depressive symptoms and the potential moderating effect of optimism and pessimism.
Methods: One hundred five older adult, primary care patients completed self-report measures of family criticism, optimism and pessimism, and symptoms of depression. We hypothesized that optimism and pessimism would moderate the relationship between family criticism and depressive symptoms.

Peripheral arterial occlusive disease (PAOD) occurs due to the build up of atherosclerotic plaque and reduces blood flow to cause chronic ischemia. Patients with PAOD may experience intermittent claudication, or the pain in limb skeletal muscles due to a decease in blood flow. Collateral arteries can act as a natural bypass and improve blood flow to hypoxic tissue by creating an alternate route for blood to flow, but not all patients with PAOD have pre-existing collateral networks. Certified Child Life Specialists’ understanding of family-centered care, and their beliefs and practices involving siblings of chronically/critically ill children were examined using an online survey. Participants were Certified Child Life Specialists recruited form the Child Life Council Forum. Relationships between utilization of certain words and support programs offered to siblings, were examined. Findings revealed a relationship in one area of coded language and support program for siblings, but not the other. Findings also revealed that all CCLS believe in the inclusion of siblings during the treatment process, however they are offered less support services than parents. A 1999 evaluation of case studies performed by staff from the Institute of Medicine found that between 40,000 and 98,000 patients died from preventable errors, while 43,598 individuals died in car accidents that year. A 2011 report increased that estimate nearly 10 times. Widespread preventable patient harm still occurs despite an increase in healthcare regulations. High-reliability organization theory has contributed to improved safety and may potentially reverse this trend. There is evidence that industrialization and urbanization has led to an increase of immigration to urban areas for employment and has led to a change of family structure and connection between parents and adult children in Chinese societies. However, little research has compared different Chinese societies, as well as rural and non-rural regions. This study compared the adult children’s current level of filial piety, between non-rural and rural regions, and between males and females using data from an international study of countries in East Asia.
