The primary purpose of this project was to study exercise-induced leukocyte cytokine mRNA expression. Changes in plasma cytokine levels and blood leukocyte mRNA expression for interleukin-6 (IL-6), IL-8, IL- 10, and IL-1 receptor antagonist (IL-1Ra) were measured in 12 athletes following 2 h of intensive cycling (64% Wattsmax) while ingesting a carbohydrate or placebo beverage (randomized and double blinded). Blood samples were collected 30 min preexercise and immediately and 1 h postexercise. Carbohydate compared with placebo ingestion attenuated exercise-induced changes in plasma cortisol (8.8% vs. 62%, respectively), epinephrine (–9.2% vs. 138%), IL-6 (10-fold vs. 40-fold), IL-10 (8.9-fold vs. …

วัตถุประสงค์ เพื่อศึกษาชนิดและความชุกของภาวะฉุกเฉินทางอายุรกรรมที่เกิดขึ้นในขณะปฏิบัติงานทางทันตกรรมข้อมูลเชิงประจักษ์ที่ได้อาจใช้เป็นแนวทางให้ทันตแพทย์สร้างสมรรถนะในการป้องกัน และจัดการกับภาวะฉุกเฉินที่เป็นปัญหาจริงในคลินิกทันตกรรม และอาจเป็นประโยชน์สําหรับการพัฒนาการเรียนการสอนในระดับปริญญาบัณฑิต วัสดุและวิธีการ ศึกษาเชิงพรรณนาโดยส่งแบบสอบถาม ให้แก่กลุ่มตัวอย่างทันตแพทย์จํานวน 4,000 คน จาก ทะเบียนรายชื่อสมาชิกของทันตแพทยสมาคมแห่งประเทศไทย ช่วงระยะเวลารวบรวมแบบสอบถาม คือ เดือนมกราคม ถึงกุมภาพันธ์ พ.ศ. 2548 ผลการศึกษา ได้รับแบบสอบถามตอบกลับจํานวน 464 ฉบับ คิดเป็นร้อยละ 11.6 ภาวะฉุกเฉินที่พบมากที่สุดคือ ภาวะเป็นลมหมดสติชั่วคราว (พบ 16.16 รายต่อหนึ่งช่วงอายุทํางานของทันตแพทย์) รองลงมาได้แก่ ภาวะระบาย ลมหายใจเกิน (3.53) ภาวะความดันโลหิตสูงรุนแรง (2.99) ภาวะน้ําตาลในเลือด (1.44) ปฏิกิริยาไม่พึงประสงค์ จากยาชาเฉพาะที่ (1.14) อาการปวดเค้นหน้าอก (0.55) และภาวะชัก (0.42) ประมาณครึ่งหนึ่งของทันตแพทย์ (ร้อยละ 51.2) เชื่อว่าตนเองมีความสามารถในการใช้อุปกรณ์ช่วยหายใจชนิดแบ็กวาล์ฟมาสก์ หรือแอมบู รู้จัก การใช้อะดรีนาลินมีเพียงร้อยละ 29.1 ทันตแพทย์เกือบร้อยละ 80 ไม่สามารถวินิจฉัยสาเหตุของภาวะฉุกเฉิน ในขณะที่ร้อยละ 36.2 คิดว่าสามารถให้การรักษาเบื้องต้นแก่ผู้ป่วยที่มีภาวะฉุกเฉินทางอายุรกรรม สรุป ภาวะเป็นลมหมดสติชั่วคราวเป็นภาวะฉุกเฉินที่พบบ่อยที่สุด ภาวะฉุกเฉินทางอายุรกรรมชนิดอื่น ๆ พบอุบัติการณ์ รวมกันเพียง 10.7 ราย ในหนึ่งช่วงอายุการทํางาน ทันตแพทย์ส่วนใหญ่ยังไม่มั่นใจในการวินิจฉัยสาเหตุของ ภาวะฉุกเฉิน การใช้ยาเพื่อแก้ไขภาวะฉุกเฉิน และการช่วยชีวิตขั้นพื้นฐาน (ว ทันต จุฬาฯ 2549;29:171-182)

การระบุเชื้อพอร์ฟีโรโมแนส จิงจิวาลิส ที่นํามาจากร่องลึกปริทันต์อักเสบชนิดแอดวานซ์ โดยวิธีพีซีอาร์, จินตกร คูวัฒนสุชาติ

วัตถุประสงค์ การศึกษาครั้งนี้ ต้องการนําวิธีพีซีอาร์ มาใช้ยืนยันการระบุเชื้อพอร์ฟีโรโมแนส จึงจิวาลิส ที่นํามาจาก ร่องลึกปริทันต์อักเสบชนิดแอดวานซ์ของผู้ป่วยจํานวน 3 ราย วัสดุและวิธีการ ทําการแยกเชื้อแบล็คพิคเม็นเต็ด แบคทีรอยดีส (3 ชนิด ประกอบด้วยเชื้อหมายเลข 1, 2 และ 3) จากเชื้อที่ได้จากร่องลึกปริทันต์ของผู้ป่วยโรคปริทันต์อักเสบชนิดแอดวานซ์ 3 ราย โดยผู้ป่วยเป็นชาวไทยที่มารับ การรักษาโรคปริทันต์จากภาควิชาปริทันตวิทยา คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ทําการเพาะเลี้ยง เชื้อที่แยกมาได้ (ทั้ง 3 ชนิดนี้) ลงบนจานอาหารเลี้ยงเชื้อพวกเบรนฮาร์ทอินฟิวชั่นยีสต์เอ็กแทรกบลัดชีพอาการใน ตู้อบที่ปราศจากออกซิเจนที่มีไนโตรเจนร้อยละ 80 ไฮโดรเจนร้อยละ 10 และคาร์บอนไดออกไซด์ร้อยละ 10 ที่อุณหภูมิ 37 องศาเซลเซียส เป็นเวลา 10 วัน แล้วนําเชื้อแบล็คพิคเม็นเต็ด แบคทีรอยดีส (ทั้ง 3 ชนิดที่เพาะเลี้ยง ขึ้นเรียบร้อยแล้ว) ไปทําการแยกดีเอ็นเอ สําหรับการทําพีซีอาร์ เพื่อทําการระบุชนิดของเชื้อต่อไป ผลการศึกษา ผลที่ได้จากการศึกษา ปรากฏว่า เชื้อแบล็คพิคเซ็นเต็ด แบคทีรอยดีส 3 ชนิด ที่ประกอบด้วยเชื้อ หมายเลข 1, 2 และ 3 นั้น เชื้อหมายเลข 1 กับเชื้อหมายเลข 2 ให้แถบกับไพรเมอร์พีจี (19+20) กับไพรเมอร์พีจี (21+22) ซึ่งตรงกับกลุ่มควบคุมบวกที่ให้แถบกับไพรเมอร์พีจี (19+20) กับไพรเมอร์พีจี (21+22) ส่วนเชื้อหมายเลข 3 ไม่ให้แถบกับไพรเมอร์พีจี (19+20) กับไพรเมอร์พีจี (21+22) สรุป วิธีพีซีอาร์เป็นวิธีที่เหมาะต่อการนํามาใช้ระบุเชื้อพีจีจากผู้ป่วยด้วยโรคปริทันต์อักเสบ เพราะสามารถระบุเชื้อพีจีได้รวดเร็วและแม่นยํา (ว ทันต จุฬาฯ 2549;29:161-170)

รายงานผู้ป่วยหนึ่งรายที่มีฟันตัดซี่ข้างบนหายไปแต่กําเนิดทั้งสองข้างซึ่งต้องอาศัยการรักษา แบบสหวิทยาการ สําหรับการจัดฟันเพื่อแก้ไขการเรียงตัวของฟัน แกนตามยาวรากฟัน และเตรียมช่องว่างที่พอ เหมาะระหว่างฟันตัดกลางบนและฟันเขี้ยว เพื่อให้สามารถบูรณะฟันโดยการฝังรากเทียมได้อย่างมีประสิทธิภาพ และสวยงาม ในรายงานนี้อธิบายถึงสิ่งที่ควรคํานึงเกี่ยวกับขนาดของครอบฟันและรากเทียม ขนาดของช่องว่าง ระหว่างตัวฟันและรากฟันข้างเคียง คุณภาพและปริมาณของกระดูกเบ้าฟัน ลักษณะของอวัยวะปริทันต์ รวมทั้ง เทคนิกทางทันตกรรมจัดฟัน (ว ทันต จุฬาฯ 2549;29:219-228)

พฤติกรรมการออกกําลังกาย ของนิสิตทันตแพทย์, ยุทธศักดิ์ เสียงฉ่ํา, รชฏ กลับประทุม, ภฑิตา ภูริเดช

วัตถุประสงค์ (1) เพื่อศึกษาพฤติกรรมการออกกําลังกายของนิสิตทันตแพทย์ และความสัมพันธ์ระหว่างนิสิต ทันตแพทย์ชั้นปีต่างๆกับเวลาที่มีสําหรับการออกกําลังกาย (2) เพื่อศึกษาความชุกของการปวดคอและปวดหลังของนิสิตทันตแพทย์ วัสดุและวิธีการ เก็บข้อมูลโดยใช้แบบสอบถามชนิดตอบด้วยตนเองจากนิสิตทันตแพทย์ชั้นปีที่ 1-6 คณะทันต แพทยศาสตร์ จุฬาฯ ปีการศึกษา 2548 กลุ่มตัวอย่างได้มาจากการสุ่มแบบแบ่งชั้น ผลการทดลอง นิสิตจํานวน 194 คน ตอบแบบสอบถาม คิดเป็นอัตราการตอบกลับเท่ากับ ร้อยละ 100 เป็นนิสิตชาย 57 คน หญิง 137 คน พบว่ามีนิสิตที่ออกกําลังกายเป็นประจําร้อยละ 14.4 โดยนิสิตปี 6 ออกกําลังกายมากที่สุด (ร้อยละ 27.3) และนิสิต ปี 3 ออกกําลังกายน้อยที่สุด (ร้อยละ 3.2) สาเหตุส่วนใหญ่ที่ทําให้ไม่สามารถออกกําลังกายได้คือไม่มีเวลา (ร้อยละ 66) นอกจากนี้อาการปวดคอและปวดหลังยังพบได้ร้อยละ 43.8 และร้อยละ 47.4 ตามลําดับ ทั้งนี้ความถี่ในการ ออกกําลังกายของนิสิตทันตแพทย์ชั้นปีที่ 1-6 มีความแตกต่างกันอย่างมีนัยสําคัญ (p < 0.05) นอกจากนี้เพศยังมี ความสัมพันธ์กับพฤติกรรมการออกกําลังกายอย่างมีนัยสําคัญ (p < 0.05) สรุป ชั้นปีการศึกษาต่างๆ กัน มีพฤติกรรมการออกกําลังกายแตกต่างกัน อาการปวดคอและปวดหลังพบได้ร้อยละ 43.8 และร้อยละ 47.4 ตามลําดับ นอกจากนี้นิสิตทันตแพทย์ชายจะมีความถี่และใช้เวลาในการออกกําลังกายสูงกว่า นิสิตทันตแพทย์หญิง (ว ทันต จุฬาฯ 2549;29:195-204)

วัตถุประสงค์ เพื่อศึกษาลักษณะ อาการของลิ้นแผนที่ และความสัมพันธ์ของอัตราการไหลของน้ําลายและการตรวจ พบเชื้อแคนดิดา อัลบิแคนส์ ในผู้ที่มีลิ้นแผนที่ วัสดุและวิธีการเป็นการศึกษาแบบวิเคราะห์และทดลองในผู้ที่มีลิ้นแผนที่จํานวน 25 รายและผู้ที่ไม่มีลิ้นแผนที่จํานวน 21 ราย การศึกษานี้เก็บข้อมูลลักษณะ อาการ จํานวนของลิ้นแผนที่ และใช้สถิติเชิงพรรณนาในการวิเคราะห์ การเก็บน้ําลายใช้การบ้วนทั้งในระยะที่ไม่ได้รับและได้รับการกระตุ้น ตรวจหาเชื้อแคนดิดา อัลบิแคนส์ ด้วยสีเพอร์ไอ- โอดิค อะชิด ชิฟฟ์และการเพาะเลี้ยงเชื้อ ใช้สถิติวิเคราะห์ unpaired t test ในการเปรียบเทียบอัตราการไหลของ น้ําลาย และใช้สถิติวิเคราะห์ chi-square test ในการหาความสัมพันธ์ของอาการของลิ้นแผนที่ การไหลของน้ําลาย และการมีเชื้อแคนดิดา ผลการศึกษา พบการหลุดลอกของปุ่มรับรสฟิลิฟอร์มเป็นหย่อมๆ เป็นรอยแดง ขอบส่วนใหญ่มีการหนาตัวของ เนื้อเยื่อเคราติน ผู้ที่มีลิ้นแผนที่ 20 ราย (ร้อยละ 80) มีรอยแดงหลายตําแหน่ง 14 ราย (ร้อยละ 56) มีอาการปวด แสบปวดร้อน การรับรสเปลี่ยนไปหรือมีอาการปากแห้ง พบความแตกต่างกันแต่ไม่มีนัยสําคัญทางสถิติของอัตรา การไหลของน้ําลาย รวมทั้งการตรวจพบเชื้อแคนดิดา อัลบิแคนส์ ระหว่างผู้ที่มีและไม่มีลิ้นแผนที่ และในระหว่างผู้ที่ มีลิ้นแผนที่ที่มีอาการและไม่มีอาการ สรุป ในการศึกษานี้ผู้ที่มีและไม่มีลิ้นแผนที่มีอาการแสดง อัตราการไหลของน้ําลาย และเชื้อแคนดิดา อัลบิแคนส์ แตกต่างกันแต่เมื่อเปรียบเทียบทางสถิติแล้วไม่พบค่าแตกต่างกัน และความสัมพันธ์ของอาการกับอัตราการไหลของน้ําลาย และการมีเชื้อแคนดิดาในผู้ที่มีลิ้นแผนที่ไม่มีนัยสําคัญทางสถิติ (ว ทันต จุฬาฯ 2549;29:205-218)

วัตถุประสงค์ เพื่อเปรียบเทียบค่าความแข็งของเคลือบฟันระหว่างเคลือบฟันปกติ เคลือบฟันที่ถูกสึกกร่อนด้วย เครื่องดื่มโคลา กับเคลือบฟันที่ถูกส่งเสริมให้มีการสะสมแร่ธาตุกลับคืนด้วยเคซีนฟอสโฟเปปไทด์ - อะมอร์ฟัส แคลเซียมฟอสเฟต (ซีพีพี - เอซีพี) และเพื่อเปรียบเทียบผลของซีพีพี - เอซีพีและน้ําลายเทียมต่อการเปลี่ยนแปลง ค่าความแข็งเคลือบฟันที่ถูกสึกกร่อนด้วยเครื่องดื่มโคลา วัสดุและวิธีการ เตรียมชิ้นตัวอย่างโดยตัดจากเคลือบฟันของฟันกรามน้อยมนุษย์ที่ถูกถอนจํานวน 11 ซี่ ทําการ สุ่มตัวอย่างโดยแบ่งเป็น 4 กลุ่มทดลองเพื่อแช่ในสาร 4 ประเภทดังนี้ 1) ซีพีพี - เอซีพี 2) น้ําลายเทียม 3) ซีพีพี - เอซีพีร่วมกับน้ําลายเทียม 4) น้ําปราศจากไอออน วัดค่าความแข็งของเคลือบฟันบริเวณหน้าตัดด้านใน ของฟัน โดยกําหนดตําแหน่งกดที่ระยะห่างจากขอบนอกของเคลือบฟัน 200 ไมครอนจํานวน 5 รอยกัดต่อการ ทดสอบแต่ละครั้ง ด้วยเครื่องวัดความแข็งผิวแบบจุลภาคที่ใช้หัวกดวิกเกอร์ส 3 ครั้งต่อ 1 ชิ้นตัวอย่าง คือ ก่อน การทดลอง หลังการสึกกร่อนด้วยเครื่องดื่มโคลา และหลังการทดลอง นําค่าความแข็งที่ได้มาทดสอบด้วยสถิติแพร์ แซมเปิล ที เทสท์ และการวิเคราะห์ความแปรปรวนทางเดียว ผลการศึกษา หลังการสึกกร่อนด้วยเครื่องดื่มโคลา ค่าความแข็งของเคลือบฟันมีค่าลดลงอย่างมีนัยสําคัญ ในขณะ ที่ซีพีพี - เอซีพี น้ําลายเทียมและซีพีพี - เอซีพร่วมกับน้ําลายเทียมมีผลทําให้เคลือบฟันที่ถูกสึกกร่อนด้วยเครื่อง ดื่มโคลามีค่าความแข็งเพิ่มขึ้นอย่างมีนัยสําคัญ โดยซีพีพี - เอซีพีและซีพีพี - เอซีพีร่วมกับน้ําลายเทียม มีผลทําให้ค่าความแข็งของเคลือบฟันเพิ่มขึ้นได้มากกว่าน้ําลายเทียมอย่างมีนัยสําคัญ สรุป ซีพีพี - เอซีพีมีผลทําให้เคลือบฟันที่ถูกสึกกร่อนด้วยเครื่องดื่มโคลามีค่าความแข็งเพิ่มขึ้น และซีพีพี - เอซีพี มีความสามารถในการเพิ่มค่าความแข็งของเคลือบฟันที่ถูกสึกกร่อนด้วยเครื่องดื่มโคลามากกว่าน้ําลายเทียม (ว ทันต จุฬาฯ 2549;29:183-194)

Laboratory Experiments Collection

Finding ways to minimize pain and distress in research animals is a continuing goal in the laboratory animal research field. Pain and distress, however, are not synonymous, and often measures that alleviate one do not affect the other. Here, the authors provide a summary of a meeting held in February 2004 that focused on distress in laboratory animals. They discuss the difficulties associated with defining ‘distress,’ propose methods to aid in recognizing and alleviating distressful conditions, and provide recommendations for animal research conduct and oversight that would minimize distress experienced by laboratory animals.

Critical illness presents with the classic response to stress, including hypermetabolism and increased catabolism, resulting in a negative energy and nitrogen balance. These harmful events initiate the immunological response, starting with the systemic inflammatory response syndrome, which, if not resolved, may lead to multiple organ dysfunction syndrome (MODS). Furthermore, patients who do not develop early MODS manifest a compensation anti-inflammatory response syndrome, which suppresses immunity and predisposes the patient to sepsis, thereby ­increasing the risk of late MODS and ultimately death. Clinical Characteristics Of Pulmonary Tuberculosis In Adult Pakistani Patients With Co-Existing Diabetes Mellitus, A Jabbar, S F. Hussain, A A. Khan

A review was made of the records of all patients with pulmonary tuberculosis and diabetes mellitus admitted over a 5-year period to a teaching hospital in Karachi, Pakistan. Among 42358 patients, the total number with both tuberculosis and diabetes was 173. The prevalence of tuberculosis in diabetic patients was 10-times higher than in non-diabetic patients and prevalence increased with duration of diabetes. The lower lung field was most frequently involved, followed by the upper and middle. Bilateral involvement was seen in half and an associated pleural effusion in one-third of the patients.   • Before you operate a forklift, make sure that you are trained and certified.
  • Remember if you are on a sit-down type forklift, and it starts to over-turn, lean in the opposite direction, stay with the forklift; don’t jump.
  • When operating a forklift on site, slow the truck and sound the horn at cross aisles, exits, and other employee traverse points.
  • Before dismounting, set the parking brake, lower the forks or lifting carriage, and neutralize controls.

Newly Identified Vitamin K-Producing Bacteria Isolated From The Neonatal Faecal Flora, Gordon Cooke, John Behan, Mary Costello

Fat-soluble vitamin K is an essential component of the blood clotting process. Menaquinones are the naturally occurring form of vitamin K identified in bacteria. Lipid extracts were made from three bacteria originally isolated from the human neonatal gut and identified as Enterobacter agglomerans, Serratia marcescens and Enterococcus faecium. Following preparative thin layer chromatography (TLC), the lipid extracts were subjected to liquid chromatography-mass spectrometry (LC-MS) analysis. Peak analysis of the LC-MS data showed that the three bacteria produce various forms of menaquinone.

In herpesvirus infections, the virus persists for life but is contained through T-cell-mediated immune surveillance. How this immune surveillance operates is poorly understood. Recent studies of other persistent infections have indicated that virus persistence is associated with functional deficits in the CD8(+) T-cell response. To test whether this is the case in a herpesvirus infection, we used a mutant murine gammaherpesvirus that is defective in its ability to persist in the host. By comparing the immune response to this virus with a revertant virus that can persist, we were able to dissect the changes in the antiviral CD8(+) T-cell response …

This paper describes a University-sponsored community physical education program and the feedback received about it from teachers, children, and the college students who oversaw it. The program, called Fitness, Friendship, and Fun, was staffed by 65 first-year student interns from the University of Dayton; four graduate assistants; and two university professors. It began with 65 male and female elementary students, primarily from the fifth and sixth grades at a nearby elementary school.

Currently, the emphasis throughout health care is on providing evidence-based practice. Occupational therapy practitioners are involved in critical examination of the profession's assumptions and beliefs and are conducting research that supports its theoretical underpinnings. In many areas of practice, practitioners have created bodies of literature to support interventions; and in other areas, we rely on literature from fields outside of the profession. Pediatric occupational therapy is no exception. Although exciting research is currently under way, existing research from outside the profession supports many of our methods and beliefs. La0.76Sr0.24MnO3 + δ particles, prepared by performing a traditional, solid-state method of synthesis, were coated by uniform layers of silica via initiating hydrolysis and condensation of TEOS in aqueous–alcoholic alkali environment. The eventually obtained samples exhibited Curie temperature at ∼40 °C, and comprised core-shell particles of ∼250 nm in diameter. By varying stoichiometric ratio of cations within manganite cores of the particles, Curie point of the resulting material can be varied too, thus opening a way for the simple design of biocompatible, temperature-self-regulating particles for application in hyperthermia treatments, with Curie point thereof adjusted to …
