Increasing multidrug resistance in Enterococcus faecalis, a nosocomial opportunist and common cause of bacterial endocarditis, emphasizes the need for alternative therapeutic approaches such as immunotherapy or immunoprophylaxis. In an earlier study, we demonstrated the presence of antibodies in E. faecalis endocarditis patient sera to recombinant forms of 9 E. faecalis cell wall-anchored proteins; of these, we have now characterized an in vivo-expressed locus of 3 genes and an associated sortase gene (encoding sortase C; SrtC). Here, using mutation analyses and complementation, we demonstrated that both the ebp (encoding endocarditis and biofilm-associated pili) operon and srtC are important for biofilm production …

BACKGROUND: Renal involvement is a serious manifestation of systemic lupus erythematosus (SLE); it may portend a poor prognosis as it may lead to end-stage renal disease (ESRD). The purpose of this study was to determine the factors predicting the development of renal involvement and its progression to ESRD in a multi-ethnic SLE cohort (PROFILE).

METHODS AND FINDINGS: PROFILE includes SLE patients from five different United States institutions. The purpose of this study was to explore the effects of vital tooth whitening on tooth color satisfaction, beliefs about dentofacial appearance, and self-esteem in a population 50 years of age and older. A two-group, randomized, pre-test, post-test, single-blind design was utilized following !RB-approval. Sixty-two participants were enrolled. Fifty-three participants (N=53) completed the study with no adverse events reported. Both the control and experimental groups received instructions for a threeweek oral self-care regimen, a toothbrush, toothpaste, and floss. The experimental group also received a tooth whitening product (independent variable) to be used during the same three-week period. Dependent variable measures …

Although domestic violence is a significant societal problem, which continues to receive public and private sector attention, intervention and treatment programs have proven inconsistent in their success. This paper reviews the published literature on domestic violence treatment efficacy and post-treatment recidivism and explores the related factors. In addition, challenges in the assessment of domestic violence are briefly discussed. Finally, recent developments are discussed along with their potential benefits, and an appeal is made for the need to study domestic violence in the broader context of family violence.

The definitions of a biologic interaction and causal interdependence are reconsidered in light of a sufficient-component cause framework. Various conditions and statistical tests are derived for the presence of biologic interactions. The conditions derived are sufficient but not necessary for the presence of a biologic interaction. Through a series of examples it is made evident that in the context of monotonic effects, but not in general, the conditions which are derived are closely related but not identical to effect modification on the risk difference scale.

Sufficient-component causes are discussed within the potential outcome framework so as to formalize notions of sufficient causes, synergism and sufficient cause interactions. Doing so allows for the derivation of counterfactual conditions and statistical tests for detecting the presence of sufficient cause interactions. Under the assumption of monotonic effects, more powerful statistical tests for sufficient cause interactions can be derived. The statistical tests derived for sufficient cause interactions are compared with and contrasted to interaction terms in standard statistical models.

Substantial evidence indicates that the locus ceruleus (LC)–norepinephrine (NE) projection system regulates behavioral state and state-dependent processing of sensory information. Tonic LC discharge (0.1–5.0 Hz) is correlated with levels of arousal and demonstrates an optimal firing rate during good performance in a sustained attention task. In addition, studies have shown that locally applied NE or LC stimulation can modulate the responsiveness of neurons, including those in the thalamus, to nonmonoaminergic synaptic inputs. Many recent investigations further indicate that within sensory relay circuits of the thalamus both general and specific features of sensory information are represented within the collective firing patterns …

A unifying meta-theory of nursing is suggested, building from the foundation of Ken Wilber’s AQAL (integral) framework. A definition of nursing as situated caring is presented. Historical discussion of contemporary nursing epistemology and theory are provided for context. Implications for practice, education, and research are discussed. A unifying meta-theory of nursing is needed to most benefit from the diversity in nursing education, practice, theory and research. A unifying meta-theory will enable nurses at both the practical and academic levels to appreciate the complexity and simplicity of nursing, allowing them to articulate confidently what we do and why we do it.

This study compared retrospective reports of childhood sexual and physical abuse as assessed by two measures: the Childhood Trauma Questionnaire (CTQ), which uses a Likert-type scaling approach, and the Computer Assisted Maltreatment Inventory (CAMI), which employs a behaviorally specific means of assessment. Participants included 1,195 undergraduate students recruited from three geographically diverse universities. Agreement was high across the two measures in the classification of victim status (92% and 80% for sexual and physical abuse, respectively). However, the CTQ classified more participants as sexually abused than did the CAMI, whereas the opposite trend was found for physical abuse. For child physical …

Standard prospective logistic regression analysis of case-control data often leads to very imprecise estimates of gene-environment interactions due to small numbers of cases or controls in cells of crossing genotype and exposure. In contrast, modern ``retrospective'' methods, including the celebrated ``case-only'' approach, can estimate the interaction parameters much more precisely, but they can be seriously biased when the underlying assumption of gene-environment independence is violated. The Kansas v. Hendricks (1997) decision, in which the Supreme Court authorized post-sentence civil commitment for certain sex offenders, appeared to be constitutionally legitimized by limiting the class of offenders eligible for this special form of civil commitment to those who are “unable to control” their dangerousness. Nowhere in the available record, however, did the Court elucidate what they meant by this notion of volitional impairment. Background

Preliminary evidence suggests that the physical environment and transportation are associated with youth physical activity levels. Only a few studies have examined the association of physical environmental factors on walking and bicycling to school. Therefore, the purpose of this study was (1) to examine the test-retest reliability of a survey designed for youth to assess perceptions of physical environmental factors (e.g. safety, aesthetics, facilities near the home) and transportation, and (2) to describe the associations of these perceptions with both physical activity and active transport to school.

The cholinergic system has long been implicated in learning and memory, yet its specific function remains unclear. In the present study, we investigated the role of cortical acetylcholine in a rodent model of declarative memory by infusing the cholinergic muscarinic receptor antagonist scopolamine into the rat perirhinal cortex during different stages (encoding, storage/consolidation, and retrieval) of the spontaneous object recognition task. Presample infusions of scopolamine significantly impaired object recognition compared with performance of the same group of rats on saline trials; this result is consistent with previous reports supporting a role for perirhinal acetylcholine in object information acquisition. Marginal structural models (MSMs) allow one to form causal inferences from data, by specifying a relationship between a treatment and the marginal distribution of a corresponding counterfactual outcome. Following their introduction in Robins (1997), MSMs have typically been fit after assuming a semiparametric model, and then estimating a finite dimensional parameter. van der Laan and Dudoit (2003) proposed to instead view MSM fitting not as a task of semiparametric parameter estimation, but of nonparametric function approximation. Some professionals have hypothesized that the observed problems in children who have been maltreated may actually be a result of the combination of risk factors surrounding the child, including but not limited to direct results of the abuse itself. This article examines the complexity of hypothesized pathways to poor adjustment after physical and sexual abuse. The literature on the impact of major risk factors associated with maltreatment is reviewed in an attempt to clarify the potential contributions of these contextual variables on the development of psychopathology in physically and sexually abused children. Heteroscedastic data arise in many applications. In a heteroscedastic regression model, the variance is often taken as a parametric function of the covariate or the regression mean. This paper presents a kernel-smoothing based nonparametric test for checking the adequacy of such a postulated variance structure. The test does not need to specify a parametric distribution for the random errors. It has an asymptotical normal distribution under the null hypothesis and is powerful against a large class of alternatives. Numerical simulations and an illustrative example are provided.

It is estimated that over 60 million people are currently suffering with some level of developmentally related cognitive impairment (American Disability Act ADA, 2000). Mental retardation occurs in 2.5-3% of the general population. About 6-7.5 million mentally retarded individuals live in the United States alone (ADA, 2000). The structures of the canine, rabbit, bovine and equine EIF2AK2 genes were determined. Each of these genes has a 5' non-coding exon as well as 15 coding exons. All of the canine, bovine and equine EIF2AK2 introns have consensus donor and acceptor splice sites. In the equine EIF2AK2 gene, a unique single nucleotide polymorphism that encoded a Tyr329Cys substitution was detected. Regulatory elements predicted in the promoter region were conserved in ungulates, primates, rodents, Afrotheria (elephant) and Insectifora (shrew). This paper investigates the partial differential equation for the evolving distribution of prostate-specific antigen (PSA) levels following radiotherapy. We also present results on the behavior of moments for the evolving distribution of PSA levels and estimate the probability of long-term treatment success and failure related to values of treatment and disease parameters. Results apply to a much wider range of parameter values than was considered in earlier studies, including parameter combinations that are patient specific.

Background

Diabetes mellitus is an important risk factor for increased vein graft failure after bypass surgery. However, the cellular and molecular mechanism(s) underlying vessel attrition in this population remain largely unexplored. Recent reports have suggested that the pathological remodeling of vein grafts may be mediated by mechanically-induced activation of the mitogen activated protein kinase (MAPK) signaling pathways and the MAPK-related induction of caspase-3 activity. On the basis of these findings, we hypothesized that diabetes may be associated with alterations in how veins "sense" and "respond" to altered mechanical loading.
