The story of ‘Le Petit William’ (Contes anglais, 1964) is based on Ferron’s experiences as a general practitioner in the Gaspé in 1946. A medical event, use of the maternal left lateral position by a sage-femme to deliver a baby boy, becomes allegory. The sage-femme had learned the technique from a visiting Anglophone doctor. A simple joke, which superficially appears to be the story’s culmination, takes on a sombre political tone when considered in the light of the Latin epigraph. In 2003, an outbreak of a highly pathogenic H5N1 strain of avian influenza (AI) in Southeast Asia, notably in Vietnam but also in Thailand, focused extensive local and international media coverage on the disease and its potential human health consequences. The media coverage has followed subsequent AI outbreaks in the People’s Republic of China, the Republic of Korea, and Japan, and westward through Russia into Eastern Europe and the Middle East. A study was conducted to determine the effect Therapeutic Listening® intervention has on auditory attention in children when used in conjunction with sensory integration therapy. Single subject experimentation took place with three participants over a ten week period of time. Pre-test and post-test results on standardized tests were compared as well as graphical representation of change during the intervention phase of this study to determine effect. Positive intervention results were concluded when graphical and test changes were compared. Subclavian artery occlusive disease is usually secondary to persistent compression caused by thoracic outlet syndrome (TOS) and rarely due to focal atherosclerosis. Emboli from diseased vessel can flow retrograde to the vertebral or carotid arteries to produce ischemic infarct with or without neurological deficit. We are reporting two cases of distal subclavian artery disease presenting with cerebral embolization, an unusual manifestation. Such surgically correctable lesions producing cerebral emboli and stroke needs consideration while evaluating patients with unusual presentation to prevent further occurrence of stroke.

Background Psychosocial and behavioral interventions trials targeting a broad range of complex social and behavioral problems such as smoking, obesity and family caregiving have proliferated in the past 30 years. At the same time the use of Data and Safety Monitoring Boards (DSMBs) to monitor the progress and quality of intervention trials and the safety of study participants has increased substantially. Most of the existing literature and guidelines for safety monitoring and reporting of adverse events focuses on medical interventions. Consequently, there is little guidance for investigators conducting social and behavior trials.

Objective: To determine the effect on survival after transarterial chemoembolization (TACE) in patients with unresectable hepatocellular carcinoma (HCC).Design: Longitudinal cohort study.PLACE AND DURATION OF STUDY: Radiology Department, The Aga Khan University Hospital, Stadium Road, Karachi, from December 1997 to September 2005.PATIENTS AND Methods: Patients undergoing TACE procedure for HCC were prospectively followed. Fortythree patients were enrolled from December 1997 to March 2003 in the study and subjected to chemoembolization therapy. Eight out of 43 patients were excluded from the study, who lost to follow-up. All the patients were followed till their death. Median and mean survival were calculated.Results: The median …

The newborn kidney is vulnerable to vasomotor acute renal failure (ARF) from adverse perinatal events or complications of prematurity. Nitric oxide (NO) vasodilation is vitally protective in this type of ARF, but its relationship with other vasoactive factors, such as angiotensin II (AII) has not been examined. In the immature kidney, nitric oxide synthase (NOS) isoforms, specifically eNOS and nNOS, are developmentally regulated, but their specific role and regulation are unknown.

The enhanced vasodilatory role of NO in the immature kidney was hypothesized to be attributed to regulatory, expressional, and functional differences in eNOS and nNOS isoforms from the adult. …

This study investigates the following two research questions: To what extent, if any, do traumatic experiences contribute to juvenile delinquency? To what extent, if any, does gender affect the relationship between trauma and juvenile delinquency? The extent of trauma was reviewed to include various studies relating to the impact trauma has on juveniles. Robert Agnew's General Strain Theory provided support for the differences that exist between males and females when it comes to trauma contributing to juvenile delinquency. The purpose of the data analysis was to, in the theoretical framework of Compromise Theory, examine the connection between micro-level structural conditions (i.e. immediate situational conditions) and individuals' values, attitudes, and beliefs. Objectives: To assess the capability of spiral computed tomography (CT) scan in preoperative evaluation of colorectal carcinoma (CRC).Methods: A cross sectional study on 52 patients with recent histopathologic diagnosis of CRC was conducted over a period of one year at the Aga Khan University Hospital, Karachi. All these patients underwent spiral CT with oral and intravenous contrast administration. Surgery was subsequently done in all cases and surgical specimens were sent for detailed pathologic analysis. The radiologic findings on the CT scans were compared with the pathological findings.Results: The results proved that spiral CT had 60% sensitivity and 83% specificity for …
