Background:

Axillary lymph node involvement in primary breast cancer is one of its most important prognostic features. Thus any factors that may predict axillary lymph node involvement in this setting could be potentially helpful in treatment planning and other interventions.

Objective:

The objective of this study was to evaluate clinical, pathological and immuno-histochemical markers in univariate and multivariate analysis, which may be helpful predictors of axillary lymph node involvement in breast cancer.

Method:

A retrospective analysis of 555 cases. Of these 58% had axillary nodal positivity and 42% were negative.

Endophytic bacteria reside within plant hosts without causing disease symptoms. In this study, 853 endophytic strains were isolated from aerial tissues of four agronomic crop species and 27 prairie plant species. We determined several phenotypic properties and found approximately equal numbers of gram-negative and gram-positive isolates. In a greenhouse study, 28 of 86 prairie plant endophytes were found to colonize their original hosts at 42 days postinoculation at levels of 3.5 to 7.7 log₁₀ CFU/g (fresh weight). More comprehensive colonization studies were conducted with 373 corn and sorghum endophytes. In growth room studies, none of the isolates displayed pathogenicity, …

Though often difficult, ethical decision making is necessary when caring for surgical patients. Perioperative nurses have to recognize ethical dilemmas and be prepared to take action based on the ethical code outlined in the American Nurses Association's (ANA's) Code of Ethics for Nurses with Interpretive Statements. In this second of a nine‐part series that will help perioperative nurses relate the ANA code to their own area of practice, the author looks at the third provision statement, which addresses nurses' position as patient advocates. Home-Care Nurses' Knowledge And Perceived Preparedness To Counsel Patients On Advance Directives, Mary Moran Barr

This study examined home-care nurses’ knowledge and perceived preparedness to counsel patients on advance directives (ADs). Joyce Fitzpatrick’s “life perspective rhythm” model provided the framework for the thesis. Sixty-two registered nurses actively practicing in home-care in West Michigan were surveyed over a 4-week period. Each nurse completed a 16-question survey, which included an 11-question true-false test of knowledge of ADs. Survey results indicated that nurses who feel prepared to counsel patients are more knowledgeable than those who do not feel prepared to counsel (t=2.89, df=57, p=<0.05). Overall knowledge was high (87.4%) but only 40 nurses (64%) felt prepared to counsel patients on ADs. The study implies that factors other than knowledge may have an impact on preparedness to counsel. Possibilities for further study include more research on education of healthcare providers about ADs, patient education, and the need for mutual discussions on end-of-life care issues in relationship to completing ADs.

It is an incurable disease and has four stages which culminate in coma and death. The average survival rate for Alzheimer's is seven years. Presently, the only treatments approved in the United States for Alzheimer's are the drugs tacrine and donepezil. These drugs improve cognitive function in early Alzheimer's. Children ages 6 through 11 years are progressing in many aspects of their life. Of interest, children are establishing eating patterns that will be carried throughout their lives. Some of these patterns can be considered problematic and need to be addressed, these include: overfeeding or obesity, adequacy of major minerals and establishing healthy eating patterns. We conclude that while some children's eating patterns are problematic, there are means to resolve these trends. They include counseling, family involvement, physical exercise and nutrition education in school at a young age.

A retrospective chart review was conducted on pediatric patients at the Intermountain Cystic Fibrosis Center who had a percutaneous endoscopic gastrostomy (PEG) placed between 1993 and 1999. Height velocity improved significantly in the group of patients with a PEG placed; however, pulmonary function declined more significantly over time.

Questionnaires regarding attitude toward PEG placement were sent to patients enrolled in accredited cystic fibrosis centers in the mountain west region and to their parents. The overall response rate was 54.25% for the PEG questionnaire and 24% for the non-PEG questionnaire. Ninety-six percent of the patients with a PEG reported that weight …

BACKGROUND: Non-heart-beating donor (NHBD) livers represent an important organ pool, but are seldom utilized clinically and require rapid retrieval and implantation. Experimental work with oxygenated perfusion during preservation has shown promising results by recovering function in these livers. This study compared sanguinous perfusion with cold storage for extended preservation of the NHBD liver in a porcine model.

A method of modulating cytokine mediated hepatic injury by administering deltorphins to a mammal. Deltorphin I SEQ ID NO:1, deltorphin II SEQ ID NO:2 or combinations of deltorphins I SEQ ID NO:1 and II SEQ ID NO:2 may be administered. A deltorphin concentration in the range of about 0.5 mg/kg to about 20 mg/kg in a physiologically acceptable formulation blocks a cytokine cascade in a murine model of septic shock. A therapeutic method of modulating cytokine mediated acute inflammatory, trauma induced and toxin induced hepatic injury, particularly via tumor necrosis factor modulation, is thus disclosed.

The nature of the plant-animal interaction between wood-boring isopods and the red mangrove Rhizophora mangle L. has been controversial, with discussion ranging from the damage caused by the isopod being detrimental to beneficial for the mangroves they attack. Initiation of lateral roots by the mangrove in response to isopod burrowing has been one of the most commonly cited examples as support for the concept of beneficial herbivory. The objective of this case study is to determine the cost difference between using a conventional type of inventory system in the operating room versus using a pack system in the operating room. In this study three different ways of delivering disposables for a laminectomy procedure were considered: 1) multiple distributors of disposables, 2) single distributor of disposables, and 3) the pack system (CDS by Medline). The first comparison was of multiple distributors vs. a single distributor with a cost difference of $90.08 per procedure and $18,916.80 annually, multiple distributor costing less. Numerous studies have found a host of factors that are likely to result in more successful applications to graduate schools. This study was a retrospective examination of the variables that distinguish graduate students who believed they were better prepared for graduate school. We examined several of these factors, including variables associated with undergraduate education and the individual for their relation to self-rated preparedness for graduate school. Our findings highlighted quality faculty interactions and participation in research as important factors in graduate students’ post hoc ratings of their preparedness for graduate school. Cationic liposomes and the complexes they form with DNA (lipoplexes) constitute the most promising alternative to the use of viral vectors for gene therapy. One of the limitations to their application in vivo, however, is the inhibition of gene delivery by serum. In a previous study, we demonstrated that transferrin (Tf)-lipoplexes were superior to plain lipoplexes in transfecting HeLa cells in the presence of high concentrations of serum.
