Interferon (IFN)-γ–producing CD8⁺ T cells are important for the successful resolution of the obligate intracellular parasite Toxoplasma gondii by preventing the reactivation or controlling a repeat infection. Previous reports from our laboratory have shown that exogenous interleukin (IL)-15 treatment augments the CD8⁺ T cell response against the parasite. However, the role of endogenous IL-15 in the proliferation of activated/memory CD8⁺ T cells during toxoplasma or any other infection is unknown. In this study, we treated T. gondii immune mice with soluble IL-15 receptor α (sIL-15Rα) to block the host endogenous IL-15. The treatment markedly reduced the ability …

Nearly 12 million children in the United States do not have health insurance, and therefore often lack access to health care. In response, Congress enacted the State Children’s Health Insurance Program (SCHIP) in August 1997, the largest expansion of health insurance coverage since the inception of the Medicare and Medicaid programs. The SCHIP provides states with federal matching funds for children’s insurance either by expanding the existing Medicaid program, by creating a separate state program, or a combination of both.

Changes in reimbursement and advances in technology have resulted in a decreased hospital stay for patients undergoing cardiac surgery. These patients are being discharged into the community setting earlier, resulting in an increased demand for transitional specialty home care. This article relates the experience of a Midwest home care agency charged with the development of specialty home care for these complex patients. It offers a new practice model for professional nursing staff delivering this level of care. Laurence Leonard is one of the most prolific and well-respected researchers in the area of specific language impairment (SLI) in children, and he is well qualified to write a book surveying the topic. SLI is a disorder of unknown origin, which appears to have a genetic component, causing delays and disorders of language development in children of normal nonverbal intelligence with no significant medical, emotional, or sensory deficits. OBJECTIVE: To determine if patient satisfaction with ambulatory care visits differs when medical students participate in the visit.

DESIGN: Randomized controlled trial.

SETTING: Academic general internal medicine practice.

PARTICIPANTS: Outpatients randomly assigned to see an attending physician only (N = 66) or an attending physician plus medical student (N = 68).

MEASUREMENTS AND MAIN RESULTS: Patient perceptions of the office visit were determined by telephone survey. Overall office visit satisfaction was higher for the “attending physician only” group (61% vs 48% excellent), although this was not statistically significant (P = .16). Under contract with the Department of Children and Families (DCF) and in accordance with the requirements of Senate Bill (SB)1258, the Louis de la Parte Florida Mental Health Institute (FMHI), University of South Florida, is conducting an ongoing formative evaluation of the financing strategies authorized to be implemented by the legislation. The demonstration sites that were selected were DCF District 1, including Escambia, Okaloosa, Santa Rosa and Walton Counties and DCF District 8, including Charlotte, Collier, Glades, Hendry and Lee Counties. Elbow disarticulation(ED) is very rare in upper limb amputation. Traditionally prosthetic fabrication of ED is fitted with a medial window on the socket for the bulbous distal residual limb to pass through. Afterwards, closing of the medial window is followed for suspension, which usually raises cosmetic concerns on the client's part. This casereport aims to introduce a double socket fabricating method to address the above-mentioned issue to promote the client's life quality and ADL independence.

This 25 years old female was found comatous for 24 hours after she injected heroin for attempting suicide. She had no history of drug abuse and no history of inhalation or injection of heroin. She received naloxone therapy at first. After her general condition became stable, she was transferred to rehabilitation department. Background and purpose: Magnets had been applied in the relief of musculoskeletal pain for years. We conducted a double blind study to assess the efficacy of magnets on knee osteoarthritis. Methods: Twenty subjects were recruited in our study. All patients wore braces over the painful knee areas all day long for 1 week except the time of bathing.Ten bands containing magnets and the other 10 bands containing placebos were used in our study. 4 times of measurements were performed before application, 1 hour, 1 day, and 1 week after application. The purpose of our study was to evaluate the effects of graded head-up tilting on common carotid blood flow volume (CCBFV), pulsatility index (PI), blood pressure (BP), and heart rate (HR) of patients with SCI and of healthy subjects.Six patients with an SCI above T6 level and 15 healthy volunteers were recruited in our study. The purpose of the study was to reveal any abnormal distribution of the static and dynamic plantar pressure in the diabetic patients with advanced neuropathy. Sixteen diabetic patients with advanced neuropathy (Group N) and 16 diabetic patients without advanced neuropathy (Group D) were recruited. Advanced neuropathy was determined by inability to perceive the 5.07 Semmes-Weinstein monofilament at one or more sites in each foot. Static plantar pressure was measured during standing, and dynamic plantar pressure was measured during walking. Static plantar pressure was significantly higher (p＜0.05) at the posterior part of bilateral central lateral metatarsal heads, while dynamic plantar pressure …

Spinal epidural abscess is an uncommon disease, and is rarely reported as a complication of acupuncture. We present two cases of spinal epidural abscess which developed after acupuncture treatment. Case 1 was 61 years old male who suffered from progressive pain and weakness over right upper arm after cervical manipulation and acupuncture. He became qudriplegia with urine retention 6 days later. Cervical MRI showed C3-6 epidural abscess. After operation and antibiotics treatment, he recovered to qudriparesis and could walk with the spastic gait 9 months later. The clinical presentation of psoas abscess is variable and nonspecific. The triad of fever, flank pain, and limitation of hip movement, which is specific for psoas abscess, is present in only less than 30% of the cases. Therefore, delayed diagnosis or misdiagnosis, resulting in the high mortality and morbidity, is not uncommon. A primary psoas abscess has no obvious focus of infection. It is believed that hematogeneous spread occurs from an occult source, analogous to acute osteomyelitis. Hematoma due to trauma may be the pathogenesis. The most common pathogen is staphylococcus. Interferon α may be used as a single agent therapy for metastatic malignant melanoma or as an adjuvant to chemotherapy. Delivery of interferon α by gene therapy offers an alternative to recombinant protein therapy. Electrically mediated delivery enhances plasmid expression in a number of tissues, for instance skin, liver, muscle and tumors including melanomas. Here we compare the effect of delivery of a plasmid encoding mouse interferon α on growth of visible B16 mouse melanomas following electrically mediated delivery to muscle or directly to the tumor. Computer-based support for the incorporation of clinical practice guidelines and protocol into daily practice has recently attracted a lot of research interest within the healthcare informatics area. The aim is not only to provide support for the flexible specification and execution of clinical guidelines or protocols but also the dynamic management of these guidelines or protocols. This paper presents a framework and architecture for the management of clinical protocols whose specification and execution models are based on the event-condition-action (ECA) rule paradigm.

The purpose of this study was to improve breast cancer diagnosis by reducing the number of benign biopsies performed. To this end, we investigated modular and ensemble systems of machine learning methods for computer-aided diagnosis (CAD) of breast cancer. A modular system partitions the input space into smaller domains, each of which is handled by a local model. An ensemble system uses multiple models for the same cases and combines the models' predictions.

Transportation arrangements are an integral but fragile element in the effectiveness of adult day care services in Vermont and nationwide. Almost by definition, adult day center participants generally cannot drive due to cognitive and/or physical limitations. Since adult day care services are congregate in nature and serve community-residing elders, this long-term care option is feasible only when there are arrangements to transport elders to and from service centers. Transportation is therefore a major issue for adult day care services.

In this article, the authors discuss the Maryland Court of Appeals decision in the case of Grimes v. Kennedy Krieger Institute, Inc. and its implications for the tort duty owed by researchers, in particular public health researchers, to their subjects. The Opinion resulted from two lawsuits alleging lead poisoning of children enrolled in a study conducted by the Kennedy Krieger Institute, a world renown pediatric research and treatment facility. The opinion shocked the research establishment with its scathing characterization of researchers and its apparent holding that in Maryland a parent cannot consent to the participation of a child in "nontherapeutic …
