Tennessee's registered nurse vacancy rate is the highest in five years and the nursing shortage has dramatically impacted all portions of the state. The purpose of this study was to describe organizational attributes of exemplar Tennessee hospitals, as perceived by Chief Nurse Executives, which may influence the recruitment and retention of registered nurses.

In this study, 52 Tennessee hospitals were identified as exemplars of quality patient care, organizational policy, and administration. Their Chief Nurse Executive's were surveyed utilizing a Hospital Characteristics Questionnaire and the Organizational Support Subscale from the Nursing Workforce Index Revised (NWI-R). Prenatal maternal attachment and the practice of health promoting behaviors during pregnancy are considered universal phenomena to women. Yet, the understanding of these phenomena from the lived experiences of pregnant women has not been well researched. The purpose of this phenomenological study was to describe the experience of maternal attachment to the unborn child and how that attachment might relate to the practice of these behaviors during pregnancy. The participants in this study were ten English speaking women, college educated, professionally employed, who were pregnant for the first time. Alzheimer's disease is a progressive dementia associated with increasing loss of memory, intellectual function, and disturbances in speech. It is the most common form of dementia. It is an incurable disease and has four stages which culminate in coma and death. The average survival rate for Alzheimer's is seven years. Presently, the only treatments approved in the United States for Alzheimer's are the drugs tacrine and donepezil. These drugs improve cognitive function in early Alzheimer's. Children ages 6 through 11 years are progressing in many aspects of their life. Of interest, children are establishing eating patterns that will be carried throughout their lives. Some of these patterns can be considered problematic and need to be addressed, these include: overfeeding or obesity, adequacy of major minerals and establishing healthy eating patterns. We conclude that while some children's eating patterns are problematic, there are means to resolve these trends. They include counseling, family involvement, physical exercise and nutrition education in school at a young age.

A retrospective chart review was conducted on pediatric patients at the Intermountain Cystic Fibrosis Center who had a percutaneous endoscopic gastrostomy (PEG) placed between 1993 and 1999. Height velocity improved significantly in the group of patients with a PEG placed; however, pulmonary function declined more significantly over time.

Questionnaires regarding attitude toward PEG placement were sent to patients enrolled in accredited cystic fibrosis centers in the mountain west region and to their parents. The overall response rate was 54.25% for the PEG questionnaire and 24% for the non-PEG questionnaire. Ninety-six percent of the patients with a PEG reported that weight …

This investigation was an examination of four Hispanic children's responses to a videotaped language sampling procedure, and was designed to determine bilingual (English/Spanish) and monolingual (English) children's expressive language development. Participants consisted of four normally developing Hispanic monolingual and bilingual males from the Rio Grande Valley between the ages of 8–6 and 9–2 years. Language samples were evoked using a videotape appropriate for children entitled Frankenweenie © . Stuttering is a multidimensional disorder that is influenced by hereditary factors, as well as environmental. Numerous methods of treatment exist that address stuttering. However, the Smooth Talking Program (Radford 2001) is unique in its inclusion of mediated learning and delayed auditory feedback. Narrative samples from Spanish-speaking children will be analyzed using a wordless picture book and standard storybook. Ages of participants will be between 6.0 and 7.11 years and demonstrate: normal hearing and vision, no familiarity with books used in study, and receptive and expressive language skills 1.5 to 2 standard deviations below the mean as predetermined by a licensed Speech-Language Pathologist. The first story will be orally presented. Afterwards, children will respond to questions and retell the story. In 1999, the Supreme Court ruled that school districts were financially responsible to provide related services including nursing services, to medically fragile students. This decision applied to children attending public schools who have complex health problems and are tracheostomy dependent. The purpose of this grounded theory study was to explore role perceptions of eighteen school nurses caring for these children in classrooms. Six dimensions emerged from the data: Promoting Family/Nurse Connections, Adapting the Environment, Claiming Authority, “Standing Out There”, Striking a Balance and Experiencing Success. Aged widowers are at risk for increased incidence of health problems and higher mortality rates following the death of a spouse. Mens' abilities for dealing with loss have been questioned, but little research has been done with this group. Surviving widowers provided significant insights illuminating the experience of resilience and providing relevant information regarding this population. Utilizing interpretive phenomenology, a methodology advocated by van Manen, in-depth interviews were conducted researching the lived experience of resilience among nineteen elderly widowers between 71 years and 100 years of age. Cigarette smoking is the leading preventable cause of death in the United States. Smoking also accounts for significant morbidity for others exposed to environmental tobacco smoke. Many women stop smoking during pregnancy. Most women relapse to smoking following delivery, yet postpartum smoking issues have received little attention. The purpose of this study was to develop and test an instrument, the Postpartum Smoking Questionnaire (PPSQ) using Ajzen's Theory of Planned Behavior. One of the high-tech modalities which foster dependence of these infants and children is parenteral nutrition. The objective of this case study is to determine the cost difference between using a conventional type of inventory system in the operating room versus using a pack system in the operating room. In this study three different ways of delivering disposables for a laminectomy procedure were considered: 1) multiple distributors of disposables, 2) single distributor of disposables, and 3) the pack system (CDS by Medline). The first comparison was of multiple distributors vs. a single distributor with a cost difference of $90.08 per procedure and $18,916.80 annually, multiple distributor costing less. This retrospective study analyzed the three muscles involved in knee extension: vastus medialis, rectus femoris, and vastus lateralis. lt was conducted to determine the reliability of normalization procedures when evaluating the EMG response detected from a muscle. During a maximal voluntary isometric contraction {MVIC), the EMG response of the muscle was recorded and the iEMG moment per unit area was determined. The iEMG response ofthe quadriceps muscles during dynamic exercise was normalized using the MVIC. This study examined the relationships between two psychological type dimensions (Extraversion-Introversion and Thinking-Feeling) and defense mechanism preferences. Psychological Type Theory was used as a conceptual framework for the generation of hypotheses. Specific hypotheses between the two psychological type dimensions and defense mechanism preferences were tested. Further, Extraversion-Introversion and Thinking-Feeling were combined, resulting in the formation of a quaternary personality model consisting of four groups (Introverted Thinking, Introverted Feeling, Extraverted Thinking, and Extraverted Feeling). Hypotheses that certain quaternary groups would display specific relationships with defense mechanism preferences were tested. Benzo[α]pyrene is a ubiquitous pollutant produced from the incomplete combustion of organic material such as fossil fuels. It is found in the workplace, urban air, drinking water, and the food supply. Recently, it has been proposed that benzo[α]pyrene may be the causative agent in the formation of lung adenocarcinomas among some Taiwanese women exposed to cooking oil fumes without adequate ventilation. In this study, calf thymus DNA was modified in vitro with benzo[α]pyrene-diol epoxide (BPDE) to a level consistent with that found in biological samples. Nuclear factor-kappa B (NF-kB) is an important transcription factor that participates in the genetic regulation of inflammation in many tissues. Cocaine and heroin are drugs of abuse associated with renal and cardiac pathology. The purpose of this study was to test the hypothesis that exposure to cocaine or morphine (a metabolite of heroin) would activate NF-kB in renal epithelial cells (COS-7) and cardiac myocytes (H9c2) grown in culture. COS-7 and H9c2 cells were co-transfected with an experimental reporter specific for NF-kB activation and a control reporter with constitutive activity. This thesis presents a novel electrocardiogram (ECG) processing algorithm design based on a Multiple Model Adaptive Estimator (MMAE) for a physiological monitoring system. Twenty ECG signals from the MIT ECG database were used to develop system models for the MMAE. The P-wave, QRS complex, and T-wave segments from the characteristic ECG waveform were used to develop hypothesis filter banks. By adding a threshold filter-switching algorithm to the conventional MMAE implementation, the device mimics the way a human analyzer searches the complex ECG signal for a useable temporal landmark and then branches out to find the other key wave components and …

The environmental carcinogens classified as heterocyclic amines (HCA’s) and the tobacco-specific nitrosamine NNK generally require internal enzymatic activation. This activation occurs via cytochrome P450 (CYP) enzymes and leads to metabolites that bind to human DNA which may in turn cause mutations which may then in turn lead to carcinogenesis. Certain nonnutritive plant compounds termed phytochemicals may protect against HCA and NNK-induced mutagenesis and carcinogenesis. We investigated the potential protective effects of five different phytochemicals of the flavonoid class which naturally occur in citrus. Human immunodeficiency virus type1 (HIV- 1) infectivity is dependent on env gene expression. HIV-1 Rev protein binds to a higher ordered RNA structure within the env transcript termed the Rev Binding Element (RBE). Mineral trioxide aggregate (MTA) has been shown to promote regeneration of periradicular tissues when used as a root-end filling material. The purpose of this study was to compare the effect of fresh MTA with set MTA on hard tissue healing following periradicular surgery.

Under general anesthesia, the root canals of twenty-four mandibular premolars in four 2-year-old beagle dogs were filled with MTA. This study addresses diabetes knowledge and attitudes of West Virginia dietitians as they relate to Medicare certified provider guidelines. A questionnaire was developed using the Diabetes Attitude Scale, questions from A CORE Curriculum for Diabetes Education, and demographic questions addressing length and type of practice, route to registration, and specialty credentials. Responses were analyzed to determine overall knowledge and attitude as well as how demographic information affects knowledge and attitude. Results indicated demographic information (except for specialty credentials and hours working with diabetes) did not have a significant effect on the dietitian’s diabetes knowledge and attitude.
