Individuals with intellectual and developmental disabilities (IDDs) experience disproportionately low employment rates despite expressing a strong desire for community integrated employment. This study examined whether discrepancies exist between the current work capacity of adolescents and young adults with IDDs and the physical exertion demands of their desired careers, and explored the role of physical therapy (PT) in addressing these gaps. Forty-five participants (ages 14–24) in the Nebraska EmployABILITY project identified their top career choice. Extracellular vesicles (EVs) are nano-sized, membrane-delimited, particles released by cells that carry signaling macromolecules. A major pathway of EV production is potentiated by neutral sphingomyelinase 2 (SMPD3/nSMAse2), an enzyme that generates ceramide from sphingomyelin. In our attempt to study this pathway in adipocytes of male mice, we discover that the elimination of SMPD3 from adipocytes in vivo triggers a signal to surrounding immune cell-like preadipocytes to release EVs that carry SMPD3 mRNA. This results in a widespread increase in SMPD3 mRNA in purified null adipocytes without a change in the transcripts of other enzymes involved in ceramide metabolism. These results …

Septal ring assembly protein ZapB (EMR18431.1) is involved in Z-ring formation, and nucleoid segregation during the bacterial cell division . It promotes cell division in the initial stages of the cell cycle through direct interaction with FtsZ, thus stimulating Z-ring assembly. The ZapB inhibition can make bacterial cell susceptible to primary antibiotics, and therefore considered as an important approach for adjuvent therapy against MDR bacterial infections. Objectives Relatively few young adults with fragile X syndrome (FXS) meet the normative expectations of adulthood, although there is heterogeneity in this regard. The present study examined the preparation for the transition to adulthood experienced by young adults with FXS and their parents.

Methods Thirty-nine young adults with FXS and their primary caregiving parents participated. Those with FXS were in their last year of, or had recently left, high school or a formal transition program. Parents completed surveys about the formal transition planning process. We examined the role of the young adults’ adaptive behavior skills in transition planning.

Acute pyelonephritis is a bacterial infection of the renal parenchyma that requires prompt recognition and antibiotic therapy to prevent complications such as sepsis and chronic renal inflammation. We present a case of a 26-year-old woman with poorly controlled type 1 diabetes mellitus and a documented beta-lactam allergy who presented with atypical symptoms, including flank pain without urinary complaints. Laboratory evaluation demonstrated leukocytosis with a negative urinalysis. CT of the abdomen revealed left lobar nephronia with early contralateral involvement of the right kidney. Background: Nitroglycerin (NTG) is a medication commonly used for chest pain, often administered in prehospital settings during acute myocardial infarctions (AMI). Current guidelines from the American Heart Association (AHA) and European Society of Cardiology (ESC) recommend avoiding NTG in suspected right ventricular myocardial infarctions (RVMI).

Objectives: Review the hemodynamic effects of nitroglycerin in acute myocardial infarction subtypes to reevaluate the validity of its contraindications.

Method: PubMed and Google Scholar databases were searched, yielding five publications that retrospectively analyzed the effects of NTG on blood pressure (BP) in AMI.

The most common hand fracture occurs in the fifth metacarpal, and is known as a “boxer’s fracture.” These fractures typically result from direct trauma from punching an object with a closed fist, falls, or motor vehicle accidents. Boxer’s fractures are diagnosed using X-rays to confirm the injury and assess severity. However, extensive bruising and tissue swelling often hides the characteristic depression of the metacarpal head, making clinical evaluation challenging.
