In the early 1970s, the National Head Injury Foundation identified 422,000 adults with permanent brain damage caused by traumatic head injury. It is estimated that 400,000 new cases of varying severity are treated in hospitals each year, the majority of whom are previously employed young adult men. Although many persons with traumatic brain injuries are able to return to productivity, approximately 35% of the adults who have been rated as mildly head injured on the Glascow Coma Scale (Teasdale & Jennet, 1974) never return to work and have difficulty reentering society after restorative efforts are discontinued (Rosenthal, Griffith, Bond, & …

Serial Casting: Clinical Applications For The Adult Head-Injured Patient, Cynthia Zablotny, Maureen Forte Andric, Charlotte Gowland

Serial casting has been used for over a decade to manage soft tissue contractures in patients with traumatic head injuries. Its use has also extended to the prevention of contractures in extremities exhibiting potentially deforming spasticity. This article will review the causes of soft tissue contractures and discuss how serial casting can manage such deformities. It will also identify how a serial casting program can be modified and integrated with other therapeutic efforts as the head-injured patient progresses physically and cognitively. Finally, it will address the timing and priority of casting in the patient's overall program.

The steroid hormone, 1 alpha,25-dihydroxyvitamin D3 (calcitriol), has been shown to inhibit T cell proliferation, primarily through inhibition of interleukin 2 (IL-2) production. In these experiments, we show that calcitriol also markedly inhibited production of the lymphokine, gamma interferon (IFN-gamma), by activated human T lymphocytes. Regulation of both IL-2 and IFN-gamma production as well as transferrin receptor (TfR) expression by calcitriol was apparent at the messenger RNA (mRNA) level as determined by Northern blotting. The decrease in IL-2 and IFN-gamma mRNA that occurred with calcitriol treatment was coordinate and not apparent up to 12 h after phytohemagglutinin stimulation, whereas decreased …

Using Neuromuscular Electrical Stimulation To Facilitate Limb Control In The Head-Injured Patient, Cynthia Zablotny

Neuromuscular electrical stimulation (NMES) has been used to treat a variety of extremity problems in head-injured patients. Specifically, NMES has been used to gain motor control, reduce joint contractures, and reduce muscle hypertonicity.1 Such uses rely on intact peripheral nerve excitability to generate the desired muscular response. This article will present some clinically relevant methods of maximizing the effectiveness of NMES programs for facilitating limb control in the head-injured patient. Stimulator features that help ensure treatment success will be identified. The special cognitive considerations that must be addressed when using NMES on the head-injured patient will also be discussed. The …

Tyrosine protein kinases, in addition to their roles as viral transforming proteins and growth factor receptors, have been suggested to have specialized functions in tissue specific processes and in differentiation. High levels of soluble tyrosine kinases have been found in human serum and plasma. To determine if the level of tyrosine kinase activity is development tally expressed in human serum, we assayed sera from 214 individuals of different ages from newborns to 90 years. We found that serum tyrosine kinase levels are high in newborns and the levels closely parallel skeletal growth until late adolescence. The serum tyrosine kinase levels …

Resolving Superfund Disputes Using Mediation, R. Steven Konkel

Superfund clean-up disputes are difficult to resolve because they involve multiple issues in addition to multiple parties. Environmental Protection Agency (EPA) and waste generator negotiators often disagree about how to interpret technical or scientific information. Examples of difficult issues include company shares of the clean-up cost, potential threat to groundwater, and what environmental standards apply to clean-up; "how clean is clean" is not at all well understood. Industry typically focuses on fairness of the allocation of cost and liability and the cost and responsibility for clean-up.

Bone loss in women accelerates immediately after menopause, often leading to osteoporosis and an increased risk of bone fractures. In 1985,247,000 hip fractures occurred in people over 45 years of age. Among those who live to be 90, one-third of women and one-sixth of men will experience hip fractures. Six months after fracture, 25% of victims will not be able to carry out their usual activities, and 50% will need assistance to do so. Hip fractures are not only a serious cause of morbidity but they also increase the risk of mortality in older people. Because the proportion of the …

Detection Of Human Dna Polymorphisms With A Simplified Denaturing Gradient Gel Electrophoresis Technique., Walter W. Noll, Mary Collins

Single base pair differences between otherwise identical DNA molecules can result in altered melting behavior detectable by denaturing gradient gel electrophoresis. We have developed a simplified procedure for using denaturing gradient gel electrophoresis to detect base pair changes in genomic DNA. Genomic DNA is digested with restriction enzymes and hybridized in solution to labeled single-stranded probe DNA. The excess probe is then hybridized to complementary phage M13 template DNA, and the reaction mixture is electrophoresed on a denaturing gradient gel. Only the genomic DNA probe hybrids migrate into the gel. Differences in hybrid mobility on the gel indicate base pair …

Release of fibrinopeptide B from fibrinogen by copperhead venom procoagulant enzyme results in a form of fibrin (beta-fibrin) with weaker self-aggregation characteristics than the normal product (alpha beta-fibrin) produced by release of fibrinopeptides A (FPA) and B (FPB) by thrombin. We investigated the ultrastructure of these two types of fibrin as well as that of beta-fibrin prepared from fibrinogen Metz (A alpha 16 Arg----Cys), a homozygous dysfibrinogenemic mutant that does not release FPA. At 14 degrees C and physiologic solvent conditions (0.15 mol/L of NaCl, 0.015 mol/L of Tris buffer pH 7.4), the turbidity (350 nm) of rapidly polymerizing alpha …

Noncariogenic Sweeteners: Sugar Substitutes For Caries Control, Curt S. Ayers, Richard A. Abrams

The evidence is clear that the incidence of dental caries is related to the frequency of eating sugar. The use of sugar substitutes is a suggested way of reducing sugar intake. A variety of noncariogenic sweeteners exists, but most have no practical value for caries control because of their technical or safety problems, taste, or cost. Urinary bladder tumorigenic effects have been reported in experimental animals treated with saccharin and cyclamates. Because of concerns for human safety, cyclamates were banned in the U.S., and saccharin use was permitted only by special legislation. The polyalcohols sorbitol and xylitol are important sugar …

One of the most useful genres of personal computer software for human service administrators is the electronic spreadsheet. Despite its status as the single most important reason for the commercial success of the personal computer (or PC), spreadsheets have received scant attention in human service administration. An extended search of the major electronic bibliographies and information services turned up no references to spreadsheets in the human service literature. We are already building an extensive knowledge and experience base in the application of spreadsheets to the problems of human service administration. Maternal And Nutritional Contributions To Infant Rats’Activational Responses To Ingestion, Brian H. Bornstein, Leslie M. Terry, Joseph A. Browde Jr.

Infant rats deprived of food, maternal care, and the opportunity to suckle display a dramatic behavioral activation and vigorously ingest when provided milk through oral cannulas. These experiments assessed which components of deprivation are important in producing these responses to milk. Nutritional deprivation alone, with or without the presence of an active maternal female, appears to be sufficient to produce ingestion. Behavioral activation, on the other hand, appears to require both nutritional deprivation and deprivation from a maternal female. The effect of maternal stimulation on later behavioral reactivity was not a function of the pups’ opportunity to suckle. However, active …
