Seventeen patients underwent treatment for retained common bile duct stones. In 7 patients the stones were removed via a T-tube tract using steerable catheters while 5 patients underwent ERCP and sphincterotomy and 5 underwent surgical re-exploration. Considering morbidity, mortality, success rate and patients' stay in the hospital, non-operative modalities should be the treatment of choice for retained common bile duct stones.

Interleukin 4 (IL-4), insulin, and insulin-like growth factor I (IGF-I) efficiently induced DNA synthesis in the IL-3-dependent murine myeloid cell lines FDC-P1 and FDC-P2. Although these factors could not individually sustain long-term growth of these lines, a combination of IL-4 with either insulin or IGF-I did support continuous growth. The principal tyrosine-phosphorylated substrate observed in FDC cells stimulated with IL-4, previously designated 4PS, was of the same size (170 kDa) as the major substrate phosphorylated in response to insulin or IGF-I. These substrates had phosphopeptides of the same size when analyzed by digestion with Staphylococcus aureus V8 protease, and each …

Inmost countries of the world some form of family medicine is being practiced by members of the medical profession. In over fifty countries distinct practitioners of this form of medicine are recognized and in some countries formal designation as specialists is accorded to them. In certain countries the physicians are called family practitioners and in others they are called general practitioners. Both terms are correct as these physicians have broad-based clinical skills (generalists) and also take care of the families. Access to medical care is an issue of acute and increasing importance in the United States, a country in which the most promising of ground-breaking technologies may be available to only the privileged few. Although debate about the problem of unequal access to medical care typically centers on financial obstacles to advanced therapies and the obvious inequity of allowing patients' ability to pay to drive treatment decisions, issues of equitable access for patients of both genders and all racial and ethnic backgrounds increasingly have come into focus. A formulation for use in detecting and/or determining peroxidase activity comprises a mixture in solution of tetramethylbenzidine, hydrogen peroxide, a buffering agent and bacitracin as a stabilizing agent. The formulation is used as a peroxidase substrate that is stable in solution for an extended period of time and provides enhanced color sensitivity.

Researchers have begun to employ social-skills training in efforts to change the sexual behavior of adolescents. However, despite the promise of social-skills training, little is known about how social skills are related to the sexual practices of adolescents. The present article reviews the current literature and proposes a conceptual framework for understanding the relations between social skills and sexual behavior of adolescents by (a)examining the relationship between sexual activity and social skills, (b) examining the development of sexual-interaction skills and deficits, (c) summarizing what is currently known about the relations between social skills and sexual behavior, (d) discussing current issues …

The transcriptional enhancers of the Moloney and Friend murine leukemia viruses (MLV) are important determinants of viral pathogenicity. We used electrophoretic mobility shift and methylation interference assays to study nuclear factors which bind to a region of these enhancers whose sequence is identical between Moloney and Friend viruses and particularly highly conserved among 35 mammalian type C retroviruses whose enhancer sequences have been aligned (E. The core-binding factor (CBF) binds the conserved core motif in mammalian type C retrovirus enhancers. We analyzed the phosphate contacts made by CBF on the Moloney murine leukemia virus enhancer by ethylation interference assay. The phosphate contacts span 9 bp centered around the consensus core site. To examine the sequence preferences for CBF binding, we employed the technique of selected and amplified binding sequence footprinting (T. K. Blackwell and H. Weintraub, Science 250:1104-1110, 1990). The consensus binding site for CBF defined by selected and amplified binding sequence footprinting is PyGPyG GTPy.

A Longitudinal Investigation Of The Relationship Between Educational Investment And Adolescent Sexual Activity, Christine Mccauley Ohannessian, Lisa J. Crockett

The relation between educational investment and sexual behavior was examined longitudinally in a sample of 479 rural adolescents. Surveys were administered to the subjects twice—once when they were in the 8th, 9th, and 10th grades, and again 2 years later when they were in the 10th, 11th, and 12th grades. Results indicated that among girls, educational investment predicted sexual activity, with academic grades in school significantly predicting reported frequency of sexual activity 2 years later. In contrast for boys, sexual involvement predicted later educational investment, particularly involvement in academic activities. These findings suggest different developmental patterns for boys and girls. …

Mammalian type C retrovirus enhancer factor 1 (MCREF-1) is a nuclear protein that binds several directly repeated sequences (CNGGN6CNGG) in the Moloney and Friend murine leukemia virus (MLV) enhancers (N. R. Manley, M. O'Connell, W. Sun, N. A. Speck, and N. Hopkins, J. Virol. 67:1967-1975, 1993). In this paper, we describe the partial purification of MCREF-1 from calf thymus nuclei and further characterize the binding properties of MCREF-1. MCREF-1 binds four sites in the Moloney MLV enhancer and three sites in the Friend MLV enhancer. Clonal expansion of antigen-reactive T lymphocytes is driven by the lymphokine interleukin 2 (IL-2). To further elucidate the mechanisms of IL-2 action, we have utilized a differential hybridization procedure to clone IL-2-induced immediate-early genes from an IL-2-stimulated human T-cell cDNA library. To increase the frequency of IL-2-induced transcripts represented in the library, the protein synthesis inhibitor cycloheximide was included during the 2-hr IL-2 stimulation to superinduce gene expression, and the uridine analogue 4-thiouridine was utilized to enable selective purification of newly synthesized transcripts. From the enriched library, we have isolated eight IL-2-induced genes, six of which represent previously unrecognized human …

Transtracheal jet ventilation (TTJV) using a percutaneously inserted intravenous (IV) catheter for the patient who cannot be ventilated or tracheally intubated or, using a jet stylet for changing endotracheal tubes (ETT) in patients for whom subsequent ventilation and/or tracheal reintubation may be difficult, are extremely valuable therapeutic options. The jet ventilation system must have a sufficiently high pressure-oxygen source to drive oxygen through noncompliant tubing and through relatively small IV catheters and/or jet stylets in order to achieve adequate ventilation and oxygenation. There is no evidence that using the common gas outlet of an anesthesia machine by activating the flush …

Tracking Plasma Lactate Concentration In Vivo With A Catheter-Tip L-Lactate Sensor, Brett T. Weinzapfel, Mark D. Ball, Lee R. Waite, Nacer E. Abrouk, Shun P. Lim

To circumvent the problems of repeated blood sampling for in vitro analysis, a catheter-tip L-lactate sensor has been developed. The sensor was tested in anesthetized pigs (n=6). The sensor in vivo tracked the lactate concentration non-linearly, seeming to obey Michaelis-Menten kinetics. Calibration time was short, typically 1.5 min per lactate standard. Furthermore, time drift was small, typically -1.3% to -3.3% per hour of in vivo use.

This paper describes the curricula contents, and presents data to evaluate the implementation, process and immediate post-test knowledge of Project Towards No Tobacco Use (Project TNT). Four different school-based tobacco use prevention curricula were developed to counteract the effects of three types of tobacco use acquisition variables typically addressed within a comprehensive social influences program: (1) peer approval for using tobacco (normative social influence), (2) incorrect social informational provided about tobacco use (information social influence) and (3) lack of knowledge or misperceptions about physical consequences resulting from tobacco use.
