A 51-year-old male demolition foreman (the victim) died from head injuries sustained when he fell through a hole in a flat roof to a concrete floor. At about 3:25 pm the victim was on the roof to retrieve a four by eight sheet of 3/4" plywood to be used at another location on the plant site. After moving the plywood, partially exposing the hole, the victim fell 35 feet to a concrete floor. There were no witnesses to the fall. He was found by the site supervisor approximately one-half hour later. Emergency medical personnel were called. Co-workers at the scene …

The text of my presentation at the WHO meeting in St Petersburg Russia held at the former KGB Sanatorium to address the problems of those involved in the cleanup after the accident at Chernobyl. My wife Karen came with me on this trip. Cyclic GMP hydrolysis by the phosphodiesterase (PDE) of retinal rod outer segments (ROS) is a key amplification step in phototransduction. Definitive estimates of the turnover number, kcat, and of the Km are crucial to quantifying the amplification contributed by the PDE. Published estimates for these kinetic parameters vary widely; moreover, light-dependent changes in the Km of PDE have been reported. The experiments and analyses reported here account for most observed variations in apparent Km, and they lead to definitive estimates of the intrinsic kinetic parameters in amphibian rods. Arm autotomy in asteroids, as tail autotomy in vertebrates, typically occurs when the animal is attacked and appears to facilitate escape. One assumes autotomy has a cost, but it rarely has been demonstrated in the field in vertebrates and never demonstrated in asteroids. The concentration of lipid was 40 % less and the amount of kilojoules 85 % less in the pyloric caeca of the asteroid Stichaster striatus Muller & Troschel (Echinodermata: Asteroidea) with autotomized arms than in those of individuals with intact arms collected in the field. as the pyloric caeca are used as nutrient reserves, individuals with autotomized …

Currently, electrical stimulation is an accepted method used clinically to promote chronic wound healing. A literature review revealed that similar therapeutic current has been shown to suppress growth of common wound pathogens in vitro and in vivo. To date, little has been reported on the factors contributing to the antibacterial effects of microamperage direct current (μADC) stimulation. The purpose of this project was to investigate the role of electric field strength, current density, pH, and type of electrode used in vitro, to gain a better understanding of how these factors contribute to inhibiting growth of select wound pathogens. …

An immunohistochemical assay for the measurement of estrogen receptor (ER) has been evaluated on 290 consecutive human breast biopsy and mastectomy specimens in the year 1992 at The Aga Khan University Hospital laboratories. Immunohistochemical localization of estrogen receptor on frozen/paraffin section was scored in a semi-quantitative fashion incorporating both the intensity and the distribution of specific staining. Histologic grading of the tumour was performed according to Bloom’s method. In this study, 21% of the tumours were estrogen receptor negative, 15% were weak positive, 25% intermediate positive and 39% strong positive. This study examined the effect of a speech-language pathologist using consultative coaching with the primary caregivers (PCG) of two expressive language delayed preschool children. The coaching was designed to improve the PCG’s use of specific communication behaviors and strategies to facilitate growth in the child’s communicative behaviors. Specifically, the consultative coaching encouraged the PCG to (1) follow the child’s lead (2) use self and parallel talk (3) expand on the child’s verbal productions and (4) use open-ended questions. Two dyads, made up of expressive language delayed preschool children and their primary caregivers, were the subjects in single subject multiple-baseline designs. …

During the period January, 1987 to June, 1991, 208 adults with mechanical bowel obstruction were managed at The Aga Khan University Hospital (AKUH). Post-operative adhesions accounted for 34% of the cases and were the most frequent cause of intestinal obstruction. External hemia (16%), malignancy (13.5%) and tuberculous stricture (10%) were the next most frequent causes. The predominance of adhesive intestinal obstruction at AKUH shows a trend towards a Western disease pattern. Socioeconomic status was a significant determinant of the cause of obstruction. The proportion of self paying to welfare patients was significantly lower in tuberculous obstruction as compared with adhesive …

Metabotropic glutamate receptors (mGluRs) include receptors sensitive to L-2-amino-4-phosphonobutyrate (L-AP4) and 1S,3R-1-aminocyclopentane-1,3-dicarboxylic acid (1S,3R-ACPD). To determine whether 1S,3R-ACPD is an agonist at retinal L-AP4 receptors, whole cell voltage clamp recordings were obtained from mudpuppy ON bipolar cells in a superfused retinal slice and L-AP4 and 1S,3R-ACPD were bath applied. Both compounds evoked similar outward currents which reversed near 0 mV and were accompanied by an increased input resistance. Responses to both agonists washed out in parallel suggesting they act through the same second messenger pathway(s). Inhibitors of cGMP-PDE activity suppressed responses to both L-AP4 and 1SR,3RS-ACPD, suggesting that both compounds …

Violent events are main causes of mortality among children and include intentional (e.g., homicide) and unintentional (e.g., accidents) circumstances. This study investigated the prediction of the self-reported occurrence of 14 violent events among eighth-grade youth from psychosocial variables measured in these same youth in seventh grade. Psychosocial variables ineluded tobacco and alcohol use, demographic variables, interpersonal variables such as family conflict, and intrapersonal variables such as risk taking. An iterative procedure, involving selection of a set of predictors and a test of the correlation of the set of predictors to the set of events, provided support for an extension of …

Background: Past studies suggested an association between human papillomavirus (HPV) and cervical intraepithelial neoplasia (CIN). In 1987, University of North Carolina (UNC) Hospitals Family Practice Center clinicians were approached for a control population to study this association. Methods: One hundred fives patients attending the UNC Hospitals Neoplasia Clinic with biopsy-proven CIN 2 or 3 and 268 control patients attending the UNC Family Practice Center for a routine Papanicolaou smear were enrolled in this case-control study. Case and control patients consented to having an additional cervical specimen taken and to being interviewed. The cervical specimens were classified by the Southern blot …

SecY, SecE, and band 1 copurify as the SecY/E integral membrane domain of Escherichia coli preprotein translocase. To measure the in vivo association of these polypeptides and assay possible exchange, plasmid-borne secY and secE genes were placed under control of the ara regulon and fused to DNA encoding the influenza hemagglutinin epitope. Cells were incubated with [35S]methionine, grown for a "chase" period, and then induced with arabinose to express epitope-tagged, nonradioactive SecY and SecE. Both the wild-type and epitope-tagged polypeptides assembled into functional, heterotrimeric SecY/E complex. However, immunoprecipitation with antibody to the epitope tag did not cross-precipitate radiolabeled SecY or …

OBJECTIVE: To determine the use effectiveness of the Creighton model ovulation method in avoiding and achieving pregnancy. DESIGN: Prospective, descriptive. SETTING: A natural family planning clinic at a university nursing center. PARTICIPANTS: Records and charts from 242 couples who were taught the Creighton model. The sample represented 1,793 months of use of the model. MAIN OUTCOME MEASURE: Creighton model demographic forms and logbook. RESULTS: At 12 months of use, the Creighton model was 98.8% method effective and 98.0% use effective in avoiding pregnancy. It was 24.4% use effective in achieving pregnancy. Biopsy is the definitive diagnostic procedure for breast cancer. The fear of cancer may make the breast biopsy itself a distressing experience for women and their significant others. Information seeking is believed to be a primary mode of coping with distressful events. The purpose of this study was to describe the information needs of women and their significant others prior to a breast biopsy and to explore the influence of information on the breast biopsy experience. Humic acids (HA) and fulvic acids (FA) are common forms of organic matter in marine sediments, and are routinely ingested by deposit- and suspension-feeding animals. These compounds may be a sink for metals, implying that once metals are bound to humic substances they are no longer available to food webs. A series of experiments was conducted to quantitatively examine this premise using 2 estuarine bivalves from San Francisco Bay, USA: the suspension feeder Potarnocorbula arnurensis and the facultative deposit feeder Macoma balthica. We studied cytotoxic capabilities of newborn polymorphonuclear leukocytes (PMNs) and monocytes and their enhancement by cytokines and antibodies. Umbilical cord PMNs were assessed for their ability to kill various target cells spontaneously, after activation with phorbol myristate acetate, in the presence of antiserum (antibody-dependent cellular cytotoxicity), and in the presence of dually specific antibody (heteroantibody-mediated cytotoxicity). Target cells included the K562 cell line (natural killer cell target), chicken erythrocytes (CRBCs), and herpes simplex virus-infected CEM cell lines. Newborn PMNs were equivalent to adult PMNs in their cytotoxic capacity in several cytotoxicity assays. Neither adult nor newborn PMNs lyse tumor cell …

Protective immunity against Toxoplasma gondii is mediated by the host cellular immune response. Interleukin-12 (IL-12), a recently described cytokine that stimulates NK cells to produce gamma interferon (IFN-gamma), is able to enhance host protection against this parasite in SCID mice. Administration of IL-12 to A/J mice significantly increased survival over that of control mice when IL-12 was delivered early in the course of acute infection. If it was administered at day 3 or thereafter, there was no observed difference in mortality between treated and control mice. Antibody depletion of IL-12 increased susceptibility to infection, as measured by mortality, only when …

Social networks and daily activities of children and adolescents living and/or working on the streets of a large Brazilian city were examined. Drawing on data collected through structured surveys, in-depth interviews, focus group discussions, and field observations, we described street youths’ family situation, social resources, institutional experiences, survival activities, and problem behaviors and investigated differences attributable to age, gender, and living situation (at home or on the street). To gain a further understanding of the regulation of human type I collagen gene expression under physiologic and pathologic conditions, we characterized 5.3 kilobase pairs (kb) of the human alpha 1(I) procollagen gene promoter. A series of deletion constructs containing portions of the alpha 1(I) procollagen 5'-flanking region (with end points from -5.3 kb to -84 base pairs (bp)) ligated to the chloramphenicol acetyltransferase (CAT) reporter gene were transiently transfected into NIH/3T3 cells. Maximal CAT activity was observed with constructs having 5' end points from -804 to -174 bp. A further 5' deletion to -84 bp caused a marked reduction …

The purpose of this study was to identify the information inner-city, African-American caregivers wanted in order to provide better care for their children. Thirty-five African-American caregivers of children, aged three to seven years, were interviewed. The gender, relationship to child, number of children cared for regularly, and the desired informational needs were identified. Frequencies and descriptive statistics were used for data analysis. Results showed that parenting-discipline and safety were each identified by approximately 25% of the respondents as desired informational needs.
