66-Year-Old Male Dies In Tractor Overturn While Mowing, Kentucky Injury Prevention And Research Center
Fatality Case Reports--Tractors/Logging
A 66-year-old male (the victim) was killed while mowing his farm land. He was driving a Case tractor model 1212 with a front-end loader and 4-foot rotary mower attachments. The tractor had a homemade canopy attached for sun protection but it was not intended to function as a Rollover Protective Structure (ROPS). Tracks in the field show that he had made several passes and had likely worked for about three hours when the incident occurred. It was about 1 p.m. when he began mowing along the edge of the woods that bordered the field. Although the field was relatively flat, …

Solid-state fermentation has the potential to produce inexpensive enzymes for use in high-volume industrial applications. Process parameters such as substrate moisture content and length of fermentation can have a significant effect on the amount and timing of enzyme production. This study was conducted in two stages, a screening stage and an optimization stage, to determine the effects of moisture content of the substrate, surfactant addition upon inoculation, depth of the substrate, and duration of fermentation on xylanase activity produced by Trichoderma longibrachiatum. The Location Of The Carboxy-Terminal Region Of Γ Chains In Fibrinogen And Fibrin D Domains, Michael W. Mosesson, Kevin R. Siebenlist, David A. Meh, Joseph S. Wall, James F. Hainfeld

Elongated fibrinogen molecules are comprised of two outer “D” domains, each connected through a “coiled-coil” region to the central “E” domain. Fibrin forms following thrombin cleavage in the E domain and then undergoes intermolecular end-to-middle D:E domain associations that result in double-stranded fibrils. Factor XIIIa mediates crosslinking of the C-terminal regions of γ chains in each D domain (the γXL site) by incorporating intermolecular ɛ-(γ-glutamyl)lysine bonds between amine donor γ406 lysine of one γ chain and a glutamine acceptor at γ398 or γ399 of another. This study compares academic performance, behavior, and commitment of basketball and volleyball athletes and non-athletes in a rural Canadian high school. It compares mid-term and final grades in each school discipline; visits to an administrator for disciplinary visits; and demerit points for improper behavior; and estimates the mean weekly time commitment for athletes in each sport.

Prediction Of Health Patterns From General Appraisal, Attributions, Coping, And Trait Anxiety, Scott Hemenover, Richard A. Dienstbier

We examined the relationships among general appraisal style, attributional style, trait anxiety, coping styles, and health status (i.e., depression, hostility, and flu-like symptoms) in a study for which we also examined the validity of a trait measure of general appraisal. Participants completed personality measures at the beginning of an academic semester, and health assessments at regular intervals throughout the semester. Consistent with our predictions, after removing the influence of neuroticism and attributional style, general appraisal style led to more negative, and less positive affect 2 weeks later, and to more stressful and threatening appraisals of a life event occurring 3 …

The chemokine receptors CCR5 and CXCR4 are used by human immunodeficiency virus type 1 (HIV-1) in conjunction with CD4 to infect cells. In addition, some virus strains can use alternative chemokine receptors, including CCR2b and CCR3, for infection. A polymorphism in CCR2 (CCR2-V64I) is associated with a 2- to 4-year delay in the progression to AIDS. To investigate the mechanism of this protective effect, we studied the expression of CCR2b and CCR2b-V64I, their chemokine and HIV-1 coreceptor activities, and their effects on the expression and receptor activities of the major HIV-1 coreceptors. Patients on cytotoxic therapy often develop neutropenia and fever. Our interest was to identify the common pathogens isolated from such patients and to study the sensitivity patterns of these organisms to the antibiotics used in their treatment. Thus, guidelines can be established by hospitals to identify which antibiotics can be used in the treatment of these patients when the results of cultures and sensitivities are not available. We conducted a retrospective study of neutropenic pediatrics presenting to AKUH from July, 1990 to June, 1996. A total of 153 isolates in 35 different patients were studied. Few studies of dentofacial and orthodontic structural relationships relative to temporomandibular joint (TMJ) dysfunction have been reported. We undertook this investigation to determine any correlation of orthodontic and dentofacial characteristics with TMJ bilateral disc displacement. The population of patients was selected from a TMJ clinic where a control group of asymptomatic volunteers had been previously established and standardized. Differences in skeletal structural features were determined among three study groups: (1) asymptomatic volunteers with no TMJ disk displacement, (2) symptomatic patients with no TMJ disc displacement, and (3) symptomatic patients with bilateral TMJ disk displacement. The latency-related transcript (LRT) of bovine herpesvirus 1 (BHV-1) is the only abundant viral RNA detected during latency. A previous study (A. Hossain, L. M. Schang, and C. Jones, J. Virol. 69:5345–5352, 1995) concluded that splicing of polyadenylated [poly(A) +] and splicing of nonpolyadenylated [poly(A) -] LRT are different. In this study, splice junction sites of LRT were identified. In trigeminal ganglia of acutely infected calves (1, 7, or 15 days postinfection [p.i.]) or in latently infected calves (60 days p.i.), alternative splicing of poly(A) + LRT occurred. Objective: The role of group therapy in treatment of posttraumatic stress disorder (PTSD) has been traditionally restricted to issues of self-esteem and interpersonal relationships, rather than primary symptoms of the disorder. In this study, the authors examined the effectiveness of a 16-week trauma-focused, cognitive-behavioral group therapy, named Interactive Psychoeducational Group Therapy, in reducing primary symptoms of PTSD in five groups (N=29) of multiply traumatized women diagnosed with chronic PTSD. Method: The authors made assessments at baseline, at 1-month intervals during treatment, at termination, and at 6-month follow-up by using self-report and structured interview measures of PTSD and psychiatric symptoms. Histologic diagnosis ultimately determines the prognosis and treatment of lesions causing spinal cord compression. Modem imaging techniques have revolutionized the procedure of localizing lesions pre¬senting with signs and symptoms of spinal cord compression. As a result, these lesions are more accessible for fine needle aspiration and biopsy. A quick diagnosis is possible if cytologic preparation is made. Similarly, intraoperative frozen section facility not only provides rapid diagnosis, but also offers oppor tunity of appropriate management decision there and then. Soft tissue sarcomas (STS) are a diverse and heterogeneous group of tumours The sub-classification of these tumours is of importance for both prognosis and treatment. Classically, sub- categorization is based purely on histomorphological grounds, but as new techniques evolve, a more, conclusive and accurate diagnosis can be made. This study describes the prevalence of soft tissue sarcomas in adults diagnosed at The Aga Khan University Hospital (AKUH) and the impact of immunohistochemistry(IHC) on the precise sub-categorization of these tumours. Methodological Considerations In Pretrial Publicity Research: Is The Medium The Message?, Jeffrey R. Wilson, Brian H. Bornstein

Two methodological issues within the pretrial publicity (PTP) literature were examined in the present experiment: the effect of emotional versus factual PTP and the effect of presenting PTP through different media. Emotional and factual PTP were constructed that differed in level of emotionality, but produced the same degree of bias. The PTP was presented in either a videotaped or written format. Although there was a significantly biasing effect of PTP overall compared to a control condition, no significant difference was found either between factual and emotional PTP or between video and written PTP.

The Legionnaires' disease bacterium, Legionella pneumophila, is a facultative intracellular pathogen that invades and replicates within two evolutionarily distant hosts, free living protozoa and mammalian cells. Invasion and intracellular replication within protozoa are thought to be major factors in the transmission of Legionnaires' disease. We have recently reported the identification of a galactose/N-acetyl-d-galactosamine (Gal/GalNAc) lectin in the protozoan host Hartmannella vermiformis as a receptor for attachment and invasion by L. pneumophila (Venkataraman, C., B.J. Ingestion of arsenic-contaminated drinking water is associated with an increased risk of several cancers, including skin and bladder malignancies; but it is not yet clear whether such adverse effects are present at levels to which the U.S. population is exposed. In New Hampshire, detectable levels of arsenic have been reported in drinking water supplies throughout the state. Therefore, we have begun a population-based epidemiologic case-control study in which residents of New Hampshire diagnosed with primary squamous cell (n = 900) and basal cell (n = 1200) skin cancers are being selected from a special statewide skin cancer incidence survey; patients …

Certain forms of the heavy metals arsenic and chromium are considered human carcinogens, although they are believed to act through very different mechanisms. Chromium(VI) is believed to act as a classic and mutagenic agent, and DNA/chromatin appears to be the principal target for its effects. In contrast, arsenic(III) is considered nongenotoxic, but is able to target specific cellular proteins, principally through sulfhydryl interactions. We had previously shown that various genotoxic chemical carcinogens, including chromium (VI), preferentially altered expression of several inducible genes but had little or no effect on constitutive gene expression. The purpose of the present study was to investigate the effect of attending to a speech stimulus on the amplitude of the distortion product otoacoustic emission (DPOAE). The distortion product otoacoustic emission is a measurement of the activity of the inner ear, specifically the outer hair cells in the ear. The activity of the outer hair cells depends upon the auditory nervous system; when it is stimulated, the outer hair cell activity is modified. Normal outer hair cells will create a large DPOAE amplitude. When a stimulus is presented to the opposite ear, the auditory nervous system acts upon the …

OBJECTIVES: This study was undertaken to investigate the incidence of posttransplant recoarctation of the aorta, delineate the mode of presentation, identify risk factors that predict recoarctation and examine the results of intervention for posttransplant recoarctation.
BACKGROUND: Patients with aortic arch hypoplasia require extended arch reconstruction at transplant, with an inherent possibility of subsequent recoarctation of the aorta.

Dynamic decision analysis concerns decision problems in which both time and uncertainty are explicitly considered. We present a new dynamic decision analysis framework, called DynamoL, that supports graphical presentation of the decision factors in multiple perspectives. To alleviate the difficulty in assessing conditional probabilities over time in dynamic decision models, DynaMoL incorporates a Bayesian learning system to automatically learn the probabilistic parameters from large medical databases. We describe the DynaMoL modeling and learning architecture through a medical decision problem on the optimal follow-up schedule for patients after curative colorectal cancer surgery.
