Idiopathic scoliosis is more frequently found in early adolescence. To prevent possible deterioration and thus, inevitable surgery, early diagnosis and treatment are strongly considered. Screening school children for scoliosis has been very popular in developed countries. The purpose of this study is to find (1) the best age to start screening and (2) the best way to screen. Using the forward bending test, one experienced physiatrist observed the backs of 5185 students from TaipeiMunicipal Dun-Hwa Primary School, which included 2618 boys and 2567 girls. Cardiopulmonary exercise testing is a powerful non-invasive mean for objectively assessing the exercise response of patients with cardiovascular or respiratory disease. The ventilatory equivalent for CO2(V E/V CO2)derived from gas exchange analysis is considered to be an estimate of dead space ventilation and provides an assessment of the efficiency of minute ventilation during exercise. Clark and associates showed that the V E/V CO2 ratio differentiated patients into subgroups in terms of the severity of congestive heart failure by measuring the maximal oxygen consumption and theduration of exercise time. There are several variations of nerve communications between the ulnar and the median nerves in the hand, which are referred as the Riche-Cannieu anastomoses. The anatomic and clinical significances of these anastomoses have not been clarified. Because of these variations, electric stimulation may travel from the ulnar nerve, through the communications, to the median nerve impulse and thus induces the contraction of thenar muscles. When performing electrophysiologicexaminations, additional attention should be paid to the variations to avoid misinterpretations. Drooling is a common problem among children with a neurological deficit. To establish the incidence of drooling and to determine the factors related to drooling in a population of the children with a neurological deficit, seventy-two children of Chang Gung Children Hospital were analyzed, prospectively. They were aged from 2 to 8 years. Exercise training, especially aerobic exercise, is beneficial to one's health. This is also true for end-stage renal failure patients. However, exercise programs prescribed on non-dialysis days are not usually carried out by the patients, which can decrease the expected effectiveness of exercise training.We present an uremic patient who first underwent a 3 month non-exercise control period, then immediately followed by another 3 months of exercise training during hemodialysis, with a stationary bike. We found the patient's compliance rate for exercise training was 94% (34/36). His maximal oxygen consumption increased by 37.8%, and he was able to discontinue his anti-hypertensive agents …

Myofascial trigger point (MTrP) is a quite common painful muscular disorder in clinical practice. When an MTrP is mechanically stimulated (needling or snapping palpation), a local twitch response (LTR) can be elicited, which is one of the characteristics of MTrP. Recent studies on humans and rabbits suggest that an MTrP is at the endplate zone. The purpose of this study is to map the distribution of sensitive loci from which an LTR can be elicited by mechanical stimulation of it in rat skeletal muscle. Electrical injuries, compared to fire or scald burns, occur infrequently but often result in amputations causing disabilities. A retrospective study was undertaken to present some statistical data and determine the correlation of serum creatinine kinase (CK) level and creatine kinase myocardial band isoenzyme (CK-MB) level with treatment, morbidity and outcome. From January 1995 to December 1996, thirty-six electrical burn patients were admitted to our hospital. Thirty-four of these patients were men and two were women, and the mean age was 31.3 years old. In clinical prosthetic experience, profuse sweating was a major problem when conventional sockets or inserts were used. However, the newly developed silicon suction socket could prevent that. We doubted that these differences came from the different nature of the silicon, pelite and polyethylene. So using the starch iodine sweating test, the forearm skins of the twenty normal subjects were sealed with a piece of 2cm×2cm×0.3cm silicon, pelite or polyethylene respectively. Surprisingly, not only silicon but also pelite and polyethylene could inhibit sweating. This meant that decreased sweating of the silicon suction socket might not be due to the chemical property …

Cerebrovascular accidents often produce alterations in the ability to control movement. Recovery of impaired motor function varies with the individual. We studied the correlation between the location as well as the size of lesions and the prognosis for motor recovery. Magnetic Resonance Images (MRI) of the brain were done in stroke patients with cortical, or subcortical infarcts or hemorrhage within 0.5 to 1.5 (mean 0.9±0.25) months after a stroke. The accessory deep peroneal nerve is a common anatomical variation in innervating the extensor digitorum brevis (EDB) muscle of the foot. Awareness of this anomaly is important in electrodiagnostic study of peroneal nerve lesions. One hundred healthy Taiwanese, including 50 men and 50 women, were tested for the presence of an accessory deep peroneal nerve by an electrodiagnostic method. The presence of a visible EDB twitch and an amplitude of the evoked muscle action potential of 0.2 mV or greater upon stimulation of the accessory deep peroneal nerve were taken as positive findings. Recurrent myofascial trigger points (MTrPs) may be caused by or associated with chronic soft tissue lesion(s) which can not be easily identified. We hereby report a case of recurrent myofascial trigger points in the upper back muscles, which were successfully controlled after one treatment of cervical facet injection.This patient was a 40-year-old man who had suffered from chronic left upper back pain and soreness with tingling sensation and back stiffness for six months. Communication between the median and ulnar nerves is a common anomaly in the forearm (Martin-Gruber anastomosis, MGA). The knowledge of these anastomoses and the resulting anomalous innervation patterns is of crucial importance to the clinicians and electromyographers in the performance of nerve conduction and electromyographic studies in normal patients and those with peripheral nerve lesions. One hundred forearms (fifty cases) were examined for median-to-ulnar or ulnar-to-median nerve anastomoses using surface electrodes. The life expectancy of individuals with untreated Duchenne muscular dystrophy(DMD) is around 18-19 years. The cause of their death is mostly from respiratory complications. Early detection of respiratory function will provide the opportunity of early management and prolong their life span.This study included 30 patients with DMD in a period of three years and took a cross-section design. The condition of medical and rehabilitation services for children with cerebal palsy(CP)in Taiwan has rarely been investigated. Such information is crucial for the health services planning. To understand the current status of medical and rehabilitation cares of CP children and related influencing factors, we sent 2435 copies of questionnaires to the main caregivers of possible CP children in the northern Taiwan. Severe spasticity often causes functional impairment in children with cerebral palsy. It is theissue that professionals have been working on to reduce the spasticity effectively and permanently. This study wants to find out the comparative effectiveness between the selective posterior rhizotomy and phenol intramuscular neurolysis on the treatment of the children with cerebral palsy. We collected 31 children with spastic cerebral palsy, aged 3 to 17, and divided the children into three groups. Group A received selective posterior rhizotomy and rehabilitation. Epidural lipomatosis is resulted from abnormal accumulation of unencapsulated fat in the epidural space. A case with idiopathic spinal lipomatosis is commonly seen in the following situations: steroid treatment, endocrinopathy, and obesity. Interestingly, a young male without the previous three conditions developed idiopathic spinal epidural lipomatosis. He was observed with lower limb length discrepancy and functional scoliosis.This patient was diagnosed by the method of magnetic resonance imaging and later received a variety of treatments according to his symptoms. Once RSD is established, successful outcome depends upon early recognition and therapy. This study demonstrates a technique to measure the mixed nerve conduction velocity (MNCV) of the ulnar and median component of the brachial plexus and its application on the diagnosis of brachial plexus lesion. Twenty patients with clinically confirmed pectoralis minor syndrome (Group I) and ten patients with traumatic brachial plexus injury due to traffic accident (Group II) were studied. Mini-Mental State Examination (MMSE) is a simple and effective bed-side screening test for cognitive function. Cognitive defect is frequently encountered clinically in stroke patients, but several sophisticated cognitive function tests are not proper for them. MMSE is used as a tool to investigate the correlation between cognitive function and the severity of brain damage of the stroke patient. The purpose of this study is designed to assess the superficial peroneal sensory nerve conduction of bicyclists since some bicyclists experience tingling or numbness over the dorsum of the foot during prolonged bicycling. Bilateral superficial peroneal nerve conduction studies were performed on 17 bicyclists compared with 20 normal subjects. Compound nerve action potentials were recorded with surface electrodes over the medial and intermediate dorsal cutaneous branches of the superficial peroneal nerves located at the anterior aspect of the ankle. Fifteen individuals with neuromuscular disease and ventilatory insufficiency received non-invasive respirator(NIR)for their respiratory failure, severe sleep disordered breathing, or respiratory distress. By providing the NIR, thirteen patients successfully maintained their ventilation for a mean duration of 13.5:t 11.8 months (range: 2 to 36); ten of them still used NIR effectively at .he time of report. The four patients failed in using NIR were all motor neuron disease. The reason for discontinuing NJR was difficulty in handling pulmonary secretions. To determine the prevalence of low compliance bladder and its effects on the upper urinary tract, 431 persons with neurogenic bladder have been evaluated over the past 3 years. Among them, 34 persons (7.9%) had low compliance bladder. Thirty-two persons who had lower motor neuron type neurogenic bladder served as controls. Bladder capacity (280 ml vs. 420 ml; P < 0.05) and creatinine clearance rate (53 ml/min vs. 83 ml/min; P < 0.05) of the low compliance bladder group were significantly lower than those of controls. Fifty patients with primary spinal tumors operated in Mackay memorial hospital between 1985 and 1995 were reviewed retrospectively. The commonest diagnoses were nerve sheath cell tumors (neurilemmomas and schwannoma) and meningioma. The mean delay in diagnosis was 1.32 years (range: half month to 7 years).The commonest intial symptoms were back pain and numbness over limbs, but later muscle weakness and sphincter disturbances came to predominate. Most patients visited orthopaedic surgeons and neurologist at beginning, and were initially diagnosed as spondylosis or herniated intervertebral disc. The purpose of this study was to compare medical quality of department of rehabilitation in Northern Taiwan area by using a new evaluation form designed by quality-oriented model for hospital accreditation. In this study, the questionnaire was mailed to twenty-six teaching hospitals in this area and was responsed from twenty-three hospitals including seven medical centers, eleven regional teaching hospitals and five district teaching hospitals (responsible rate 88.5%). Comparison was done among medical center, regional teaching hospitals and district teaching hospitals, and was also done between board-certified physiatrist training hospitals and non-training hospitals.
