This article concerns the correlation between children with dyslexia and brain morphology. Computed tomography (CT) and magnetic resonance imaging (MRI) studies suggest that five major differences exist between dyslexic and typical children. Primarily, the genu of the corpus callosum is significant smaller in dyslexic children. Secondly, the corpus callosum in dyslexic children with family history is larger and thicker. Thirdly, dyslexics demonstrate more deviations from normal patterns of left and right hemisphere asymmetry. Fourthly, posterior superior surface of temporal lobe, planum temporal surface area and brain volume in left brain are smaller in dyslexics. Finally, six brain regions account for …

The alien hand syndrome was diagnosed in six stroke patients treated in our hospital during the period from 1993 to 1998. Left cerebral artery occlusion was responsible for the stroke episode in five patients and hydrocephalus subsequent to intracerebral hemorrhage in one patient. All of the patients were right handed and exhibited behavior of forced grasping and impulsive groping, with their dominant hand, of objects entering their field of vision. A survey of utilization and demand of Rehabilitation Information System (RIS) for out-patient clinic service was done by questionnaires from July 1999 to December 1999. One hundred and seventeen responses (52.0% of total samples) were adopted for analysis. The questionnaires included data base, utilization of rehabilitation information system for outpatient clinic service and demands of specific system . The results showed that 77.8% of rehabilitation clinics used computer system for outpatient clinic service; and 67.5% of them had electric chart record. ”Training of sitting balance is one of the major treatment programs for rehabilitation of complete thoracic spinal cord injured patients. In the literature, there are only a few objective and quantitative tests for sitting balance, and they are not appropriate for patients with spinal cord injury. The Functional Reach Test was modified by Lynch and colleagues to measure sitting balance. ”From January 1993 to June 1996, one hundred and fifty-four spinal cord injury (SCI) patients received urodynamic studies, including urosonography, radionuclide renography, voiding cystoureth-ragrphy, intravenous urography, and/or cystometry. Among the subjects, 125 patients were male (81.2%) and 29 were female (18.8%); there are 133 patients (86.4%) with upper motor neuron lesion and 21 (13.4%) with lower motor neuron lesion.One hundred and eleven patients (72.7%) had the history of symptomatic urinary tract infection, and urolithiasis was noted in 3 patients (1.9%). The primary purpose of this qualitative study was designed to identify, from a client and medical personnel perspective, factors that associated with the quality of life of patients with spinal cord injuries (SCI). Three focus groups were recruited and conducted: newly injured patients, patients with a chronic SCI, and medical personnel who worked with these patients. Responses were audio taped, transcribed, and analyzed by pattern coding according to domains and facets of Taiwan version of WHOQOL-BREF. Hiccups are a common phenomenon though their mechanism and physiological meaning are still controversial. No special treatment is necessary for most patients with hiccup. However, the hiccup might last for more than one month in rare cases, defined as an intractable hiccup, as one of the clini-cal manifestations of respiratory, gastrointestinal or neurological diseases.A case with an ischemic brain stem stroke is investigated herein. The intractable hiccup disturbed the patient’s appetite, impaired his nutritional and dehydration status, and even delayed rehabilitation. The main function of corpus callosum is to connect bilateral hemispheres. Patients with mech-anical injuries to corpus callosum, e,g., tumor compression, anterior or posterior cerebral artery infarction may have in neurological defects including left apraxia, left alexia or anomia to colors. Nevertheless, these symptoms may not occur in congenital corpus callosum agenesis, complete or incomplete. Brain stem tumors are relatively rare but predominantly diseases of childhood. Most of them are astrocytomas or ependymomas histologically. Intracranial teratomas arise in the midline, most comm-only in the pineal and suprasellar regions. Only one documented paper illustrated a teratoma arising in the brain stem. Articular cartilage is a highly compressive load-bearing tissue by virtue of the functional interplay between a 3-dimensional (3-D) structure of collagen fibrils and the entrapped water-swollen proteoglycan macromolecules. Any loss of interconnectivity in the collagen network that might reduce the constraints on the swelling tendency of the proteoglycan domain will lead to swelling and low matrix stiffness. In order to understand the prevalence of limitation in activities of daily living in elderly people and the relationship between limitation in activities of daily living and the cardiovascular risk factors among elderly people, we conducted a cross-sectional study in Chung-Shing-Shin-Tseun community of Taiwan in May 1998. All of individuals aged 65 and over were collected. A total of 1093 persons, out of a 1774 registered residents, finished questionnaires. The response rate was 61.6%. However, only 586 respondents had blood tests and completed questionnaires. Analysis in this study was based on these 586 subjects. Our results showed that 66.2% were …

One hundred and thirty-four babies who were clinically diagnosed with postural wry neck were enrolled in this study. All of them received sonographic examination of both sternocleidomastoid muscles with a 7.5 MHz linear array transducer connected to the Acuson 128 10M ultrasonographic machine. Three (2.2%) of the 134 babies were noted to have echogenic changes in one of their sternocleidomastoid muscles, and were diagnosed as having muscular type torticollis. Periprosthetic humeral fractures constituted only 2 % of all the complications during or after a shoulder hemiarthroplasty or total shoulder arthroplasty. Most of the fractures occurred intraoperatively. Postoperative fractures were caused by falls or traffic accidents. To our knowledge, they have never been caused by stretching exercises after a shoulder hemiarthroplasty. We present a 57-year-old female patient who had a periprosthetic humeral shaft spiral fracture caused by stretching exercises after a shoulder hemiarthroplasty because of proximal humeral fracture. The fracture was close reduced and fixed with a sugar-tong splint. Three months later, the fracture successfully healed with good alignment. Possible …

The entrapment syndrome of the intermediate dorsal cutaneous branch of the superficial peroneal nerve is rarely reported. Here we reported a 17-year-old male patient, who suffered from left dorso-lateral foot numbness after prolonged local compression of left lateral foot. The electrophysiological studies of the left intermediate dorsal cutaneous nerve, 10 days after onset of symptoms, revealed normal sensory nerve conduction velocity with significantly reduced amplitude of nerve action potential. These findings were compatible with the diagnosis of entrapment syndrome of left intermediate dorsal cutaneous nerve. In the past, attempts to evaluate arch height from footprint parameters have yielded conflicting results. This could be caused by their failure to take anatomy into consideration in the proposed footprint parameters. Acute traumatic spinal cord injury (SCI), a devastating event, results in huge costs to patients and society. Current management emphasizes general supportive care and stabilization of associated spinal columnar injury to prevent worsening of the cord lesions. Most of the numerous experimental studies to investigate pathophysiological changes following acute SCI suggest a two-step mechanism of damage to the spinal cord in which the primary (direct) or mechanical injury caused by trauma initiates secondary (indirect) or progressive autodestructive injury of the cord. Spinal cord injury (SCI) patients confined to a wheelchair for long periods are prone to pressure sores occurring at ischial tuberosity. This study used Expert-Seat pressure evaluation pad to record the pressure of the ischial tuberosities. Twenty-nine spinal cord injury subjects and twenty normal subjects participated in this study. The pressure distributed over ischial tuberosities during sitting was measured five times, including one measured without cushion. A 44-year-old female Taiwanese developed T5 transverse myelitis (TM) in 1984, eight years before the diagnosis of systemic lupus erythematosus (SLE). She responded very well to prednisolone and azathioprine administration and had complete neurological recovery. In August 1997, a relapse of TM at vicinal level, T6, occurred. Magnetic resonance image of the spinal cord revealed long segment involvement from T4 to T8. High titer of antiphospholipid antibodies and various serological markers of lupus activity were found to correlate with this relapse of TM. Electrical burns dramatically affect the nervous system, muscles, bones, and other tissues within the human body. Myelopathy after an electrical burn has seldom been mentioned.This article describes two cases of myelopathy admitted to our burn center after a high-voltage electrical burn incurred in the workplace, with symptoms onset around 16th and 4th day, respectively. The patients both experienced a course of deterioration, followed by a slow and partial recovery. The possible mechanisms and pathophysiology associated with electric burns are discussed.

Traumatic carotid artery dissection is often not recognized at the time of admission, possibly leading to cerebral infarction. This article reports on a 77-year-old man suffering from headache and neck pain after a traffic accident. Although the initial head computed tomography ( CT ) scan revealed no intracranial lesion, progressive left limbs weakness, slurred speech and blurred vision of his right eye developed on the next day after the accident. This study attempts to define the reliability of ice-water test (IWT) in detecting the detrusor reflex in patients with chronic spinal cord lesion (SCL) and further explore its clinical application. 220 patients with SCL over a period of four years were included in this study. 105 of the patients were rated as the acute group with the injury less than six months. 115 patients with lesions over six months were grouped as the chronic one. Uroflowmetry and ultrasonic residual urine measurements had been done to 85 urologically asymptomatic diabetic patients and 20 control subjects. The prevalence of voiding dysfunction was 60% (51/85), and increased residual urine was found in 27% (23/85) of diabetic patients. The abnormal findings included increased residual urine (23/85), interrupted urine stream (9/85), voiding with abdominal strain (31/85), low flow rate (28/85), and enlarged bladder capacity (6/85). The group with voiding dysfunction had lower maximal and average flow rate, longer voiding time, and larger residual urine amount than the diabetic patients with normal voiding function and the healthy controls. A total of 827 Functional Independence Measure (FIM) scores, which from three separate cohorts research from 1995 to 1998 in Taiwan, were conducted the structural analysis of FIM. The 872 subjects included residents of nursery center, outpatients of neurology and neuro-surgery clinics as well as inpatients and outpatients of rehabilitation department. Both principle factor analysis and Rasch model analysis were able to identified two major factors of FIM in Taiwan. ”Astasia(inability to stand)” is a popular and immediate result after onset from stroke. Health professionals usually label astasia as ”poor” standing performance. However, few studies substantiate the falling tendency of astasia. The result of inability to stand is to fall toward one specific direction; therefore, this study used ”falling direction” as a medium to analyze the falling tendency of astasia. This study evaluates how selective posterior rhizotomy (SPR) affects the motor control pattern of children diagnosed with cerebral palsy (CP) by using polyelectromyography and gait performance. Twenty-four spastic children diagnosed with CP aged 3 to 16 years old were included in this study. Another twenty children diagnosed with CP who had only undergone rehabilitation were selected as the control group. The children diagnosed with CP received polyelectromyography and gait analysis within one month before treatment (SPR or only rehabilitation) and 9 months to 1 year after treatment. There are many techniques and methods for gait analysis, but the data are not easy for the clinician to read and to correlate clinically in patients with neurological deficits such as stroke. In this study, we tried to simplify the kinetic gait data of 102 functionally ambulant stroke patients in a more simple way through pattern recognition in addition to conventional gait parameters and compared them with 50 healthy subjects.
