It is well documented that regular exercise can improve cardiopulmonary function and health-related fitness. However, major limitation of exercise therapy lies in physiologic and environmental factors, which is particularly true for patients with physical disability such as stroke. A standardized Buddhist meditation skill, Forshang Meditation (FM), was adopted; and this paper described the long-term effects of FM on cardiopulmonary fitness.Seventeen patients ( 1 female and 16 males; mean age 70.8 ± 4.8 years, range 61-78 years ) with a history of cerebrovascular disease and low extremity of hemiplegic side above Brunnstrom stage V were recruited. Hemifacial spasm is an intermittent, synchronous contraction of ipsilateral facial muscles. Botulinum toxin A injection is an effective and safe symptomatic treatment for hemifacial spasm. In the traditional method, physicians omitted the central upper eyelid area to prevent the development of ptosis, but there may remain some twitches.Here we report a case of Botulinum toxin A injection for left hemifacial spasm with complication of ptosis. Apraclonidine 0.5% ophthalmic solution was used in this patient to facilitate the recovery of ptosis. Acute transverse myelitis ( ATM ) is a rare but severe complication of systemic lupus erythema-tosus ( SLE ). Magnetic resonance imaging ( MRI ) is the leading imaging study for diagnosing ATM and monitoring the treatment and/or the progression of the disease. Lesions of the spinal cord of ATM are usually less than 4 spinal segments. Dislocation of the glenohumeral joint is a frequent disorder that occurs among traumatic injuries of the shoulder. There are differences in the dislocation between young and older patients. Recurrent instability is more common to follow traumatic anterior dislocation of the glenohumeral joint in young patients than older patients. Neurologic complications and associated soft tissue injuries after a glenohumeral dislocation occur more frequently in patients around 40 years of age and older.A 73-year-old patient suffered from dislocation of left glenohumeral joint after falling down. 脛後肌肌腱斷裂的病態生理現象至今仍未有廣泛的研究。本文的目的乃利用兔子動物模型來探討缺血在脛後肌肌腱退化中可能扮演之角色。我們利用40隻紐西蘭白兔將之分為四組,將一段脛後肌肌腱的內行性與外行性營養供應途徑阻斷其一或同時阻斷,三星期後將此段肌腱取出,做病理學之觀察。實驗結果顯示,缺血的確可以造成兔子脛後肌肌腱的退化甚至斷裂。而肌腱內行性的血流循環乃脛後肌肌腱主要的營養供應途徑,從外面滑膜囊液滲透進入肌腱的養份並不足以維持脛後肌肌腱的完整性。脛後肌肌腱所承受的張力會加重缺血對肌腱所造成的傷害。

This retrospective study explores the physical and emotional barriers faced by individuals with spinal cord dysfuntion when returning to school. All subjects lived in central Taiwan. The questionnaires of 20 cases were successfully completed. The ages of them at the time of the disorder ranged from 0 to 20 years (mean of 10.9 years). Regarding the education level, 7 of 20 were graduated from or remained in university or college, 8 in high school, 3 in junior high school, and 2 in kindergarten. 肌筋膜疼痛症候群(MPS)以激痛點(MTrP)的存在為其特徵,局部抽搐反應(LTR)和自發性電位活動(SEA)是激痛點之兩個很重要的客觀性指標。激痛點的基本單元,就是肌筋膜激痛點小點(MTrP locus),它包含了感覺性成分(感覺小點;或LTR小點)與運動性成分(活動小點;或SEA小點)。在動物實驗中發現,在高於支配該肌肉之下運動神經元處截斷其脊髓後,兔子的LTR在脊髓休克期時會被暫時性地壓抑,可見LTR顯然經由脊髓來做調節。在激痛點裡的SEA,是因為乙醯膽素(Acetylcholine)過量釋放所引起的不正常之終板電位(endplate potentials)。不過,由於SEA的出現率在脊髓截斷後並沒有很明顯的改變,SEA應該不是經由脊髓來做調節。本研究收集了十四隻紐西蘭成兔,來評估所支配之神經被切除後,其對激痛點的SEA所造成的影響。我們在兔子坐骨神經切除前、切除之後、切除後第一天、切除後第二天、切除後第一週、切除後第二週、切除後第三週與切除後第一個月,分別追蹤兔子股二頭肌上SEA的發生率。發現SEA在神經切除後還持續存在約一天,但從切除後第二天開始,SEA就消失不見了,直到本研究追蹤結束時。由本研究的結果可知,SEA的發生,神經脈衝傳導的神經性因素是比較不重要,而神經介質傳導的內分泌性因素則相當重要。此研究更進一步證實肌筋膜激痛點裡的SEA,是運動終板機能障礙所導致的一種局部性的現象。

In-line skating has grown to become a popular sport and recreational activity in the past few years. This study was designed to survey the sport injuries in in-line skaters and its relating factors. 240 subjects (136 males and 104 females) were sampled from two public parks in Taipei (Ta-An Park and Sun Yat-Sen Memorial Hall), which are the most representative of the in-line skaters’ population in Taipei city. Ages ranged from 3 to 50 (mean, 17) years old. 126 (52%) subjects had skated for more than one year, and 159 (66.5%) had injury while skating.Data collected revealed four most commonly …

The purposes of this study were to explore the feasibility of exercise training during hemodialysis for end-stage renal failure patients, and the effect of exercise training on cardiopulmonary endurance, blood and biochemical changes. The purposive sample was recruited from a medical center in southern Taiwan. A total of 20 hemodialysis patients underwent a 30-50 minutes stationary bicycle training ( 3 times a week for 12 weeks ) within the first two hours of the hemodialysis period. This preliminary report demonstrated a new technique, “intramuscular sensory neurolysis” (IMSN), to control the muscle spasticity. Similar to myofascial trigger point (MTrP) injection, the needle were rapidly inserted, for a short distance (1 mm) each thrust, into the MTrP region (in the endplate zone) to elicit local twitch responses to identify the terminal sensory fibers for phenol injection. Immediate relief of spasticity was noticed both subjectively and objectively after IMSN. The objectives of this study were to assess the gait patterns of stroke patients, and to evaluate the correlation between gait variables and clinical status of stroke patients. A Motion Analysis System was used to collect gait data of temporal-spatial and kinematic variables from stroke patients and age-matched controls. Twenty-three ambulatory stroke patients (age 55±10 years; time since stroke 9.7±7.1 months) were analyzed. Nine age-matched healthy elderly subjects were recruited as a control group. The goal of this study was to explore knowledge, attitude and behavioral intention towards traditional Chinese medicine (TCM) among people with spinal cord injury (SCI). 155 valid sample questionnaires were collected and the following results were drawn from descriptive and inferential statistical analysis. The item with highest percentage being answered accurately was ”traditional Chinese drugs are made in different forms, such as boiled soup, pills, or condensed powder”. The purpose of this study is to investigate the effects of magnets on nerve conduction studies (NCS) in human. Methods: After excluding subjects with peripheral neuropathy or diabetic neuropathy, 12 normal subjects were recruited (mean age is 30.0 ± 5.2 years old; 5 are female and 7 are male). The goal of the study is to assess the prevalence, severity, and correlation with sex and age associated with scoliosis in elementary school children in Taitung city. A total of 5,611 children were screened characteristically for scoliosis using Adam’s forward bending test. Those schoolchildren who had positive forward bending test were referred to hospital for whole spinal radiographs, so that the Cobb angle could be quantified to evaluate the severity of scoliosis. 115 children had positive signs in screening test for scoliosis; the positive rate was 2.05% (girls: 2.46%, boys: 1.68%); and the positive rate of the children above 10 …

Infantile torticollis is a common condition in rehabilitation clinics. Acute torticollis due to atlanto-axial rotatory subluxation, however, is an uncommon condition in children. The diagnosis is usually delayed due to difficulty in the evaluation of roentgenograms, as well as to clinicians’ unawareness of this disorder. Delayed unrecognized atlanto-axial subluxation may make the treatment more complicated. Here, we review three cases who had atlanto-axial rotatory subluxation successfully treated with manipulation.

Although it is not common in the patients with cerebral infarction, the incidence of intractable hiccup seems to be higher in brainstem stroke. Because of intractable hiccup, the patients will be severely affected physically and emotionally during feeding, sleep activities, and even rehabilitation program. The purpose of this study is first to analyze the temporal-stride parameters and forces applied on the cane, and both feet in stroke patients walking with cane-assistance. Three force plates, six cam-eras, and an instrumented cane were integrated to conduct an analysis of the cane-assisted gait of 15 hemiplegic patients. Temporal-stride parameters, and peak vertical, anterior (propulsive)-posterior (braking) and lateral shear forces, and propulsive-breaking impulses, were analyzed.All patients walked with a relatively slow walking speed, the average is 16.1 cm/sec, ranging from 4.2 to 35.8 cm/s. The purpose of the study was to explore the effects of exercise training on cardiopulmonary endurance and depression status of hemodialysis patients. Hyperbaric oxygen (HBO) effectively decreases the morbidity and mortality of carbon monoxide poisoning . Immediate HBO treatment is indicated as it yields a more rapid and greater neuropsychological recovery. HBO significantly decreases the half life of carboxy hemoglobin (COHb) and increases the tissue clearance of residual carbon monoxide, thus decreasing the neuropsychological deficit. HBO enhances the outcome even in the subacute phase of CO poisoning as in this case. Posttraumatic spinal cord cyst is a late sequelae of spinal cord injury. The cyst has no or little inf-luence on the patient’s neurological functions. We report three patients of spinal cord injury complica-ted with posttraumatic spinal cord cyst whose motor functions were compromized due to increased spasticity. The incidence of the cyst may be up to 50%. The cyst may come from liquefaction of the cord tissue after local edema, necrosis and hematoma, or a myelomalacia due to local ischemia. A case of delayed diagnosis of spinal dural arteriovenous fistula is presented. A 65-year-old male presented to our clinic for progressive stiffness and numbness over bilateral lower limbs for 6 months. Neurological examination was suggestive of thoracic spinal cord lesion with signs of both upper and lower motor neuron disorders. Electromyographic examination showed evidence of denervation and re-innervation in the bilateral paraspinal muscles of middle thoracic level. Scheuermann's disease is one of the common causes of structural abnormality in the thoracic or thoracolumbar spines. Many theories have been proposed for the etiologies of Scheuermann's disease, but the true etiology remains unclear. The disease has a strong familial tendency which may be inh-erited in an autosomal dominant mode. This article describes a 21-year-old male patient who was suff-ering from low back pain for 2 years. Ankylosing spondylitis is an inflammatory disease of the axial skeleton. Spinal ankylosis reduces vertebral flexibility, exacerbates osteoporotic change, and leads to a loss of the biomechanical function of the spine. Thus, even small impacts might cause vertebral fracture or damage of the spinal cord in the patients with ankylosing spondylitis.This retrospective study included 11 patients with ankylosing spondylitis who were admitted to a hos-pital between 1991 and 1999 for rehabilitative service after spinal cord injury.
