Background: Upper extremity return to sport (RTS) assessments are not standardized with respect to which metrics to use, if the metrics are appropriate for all populations and levels of competition, and what constitutes good or poor test performance. Subsequently, clinicians may utilize suboptimal metrics to evaluate RTS readiness, or forgo objective criteria altogether. The purpose of this study was to examine the psychometric properties (reliability, agreement/measurement error, hypothesis testing/construct validity, criterion/predictive validity, responsiveness) of upper extremity functional tests used to assess RTS readiness.

Background: Flow diverters (FDs) are increasingly used for cerebral aneurysms, including distal anterior cerebral artery (DACA) aneurysms, but comparative data between devices in this challenging location are limited.
Objective: To compare the safety and efficacy of Pipeline, Silk Vista Baby (SVB), and FRED Jr. FDs for unruptured DACA aneurysms and identify predictors of complete occlusion.
Methods: We retrospectively analyzed 166 patients treated with FDs at 39 centers in 14 countries (2018–2022) from the CRETA registry. Outcomes included aneurysm occlusion (O’Kelly–Marotta [OKM] scale), complications, retreatment, modified Rankin Scale (mRS) scores, and independent predictors of complete occlusion using multivariable Cox regression.

Background: The relationship between media portrayal of psychedelic drugs, scientific research and drug policy is an area of debate.
Aims: To apply artificial intelligence technology to measure trends in media sentiment towards the therapeutic potential of psychedelic drugs.
Method: Up to 300 of the most relevant articles from Google News searches for the term 'psychedelics' were sampled for each year from 2000 to 2025. A large language model, ChatGPT, evaluated subject matter and sentiment.

Introduction: Menstrual disorders—including premenstrual syndrome (PMS), premenstrual dysphoric disorder (PMDD), and dysmenorrhea—are highly prevalent among women of reproductive age and are associated with impaired academic performance, psychological distress, and reduced social functioning. Nursing students are particularly vulnerable due to the combined demands of intensive academic schedules and clinical training, yet prevalence estimates and institutional responses remain inconsistent.

The 3D images derived from non-contrast Computed Tomography (NCCT) scans provide a glimpse into the intra- and extracranial structures, based on the ability of electromagnetic X-rays to penetrate through various tissue densities. The gray matter, for example, possesses a higher density than the white matter, absorbs more X-rays, allows less penetration (attenuation), and thus has larger Hounsfield units (HU), the mathematical value of attenuation coefficient. The CT scanner’s wide availability and ease of use allow for quick assessment of acute neurosurgical abnormalities, such as hemorrhages, mass effect, hydrocephalus, and traumatic injuries. Introduction: Total knee arthroplasty (TKA) is a common orthopedic procedure that is increasing in incidence and prevalence and is rising in younger populations. Social determinants of health (SDOH) training is a critical component of medical education, equipping physicians to address factors such as economic stability, education, and health care access. Despite its importance, the evaluation of SDOH training often focuses on lower-level outcomes, such as learner satisfaction and knowledge assessment, with limited emphasis on behavioral change and patient outcomes. Kirkpatrick’s model offers a validated framework for addressing this gap by categorizing training outcomes into four levels: Reaction, Learning, Behavior, and Results.

Background: Fenestrated and branched endovascular aneurysm repair can be complicated by branch vessel occlusion in the absence of structural stenosis. We hypothesized that computational flow simulation could identify adverse hemodynamic features associated with postfenestrated and branched endovascular aneurysm repair branch occlusion.
Methods: Patients undergoing 4-vessel fenestrated and branched endovascular aneurysm repair for Extent II to IV thoracoabdominal aortic aneurysms were retrospectively reviewed. Branches that occluded without identifiable kinking or stenosis on computed tomography were included, along with an equal cohort of anatomy-matched patent controls. Introduction: Human immunodeficiency virus (HIV) infection is associated with various comorbidities, including macrocytic anemia, though the role of the infection is unclear. HIV has been implicated in microbial translocation and altered immune responses. This study aimed to establish the relationship between gut microbial translocation and immune response, and macrocytic anemia among people with HIV (PWH).
Materials and methods: Fifty-five PWH on combination antiretroviral therapy (cART) were age-matched with 55 HIV-negative individuals. Understanding how rare genetic variants influence complex traits remains a major challenge, particularly when these variants lie in noncoding regions of the genome. The effects of variants within candidate cis-regulatory elements (cCREs) often depend on the cell type, making interpretation difficult. Here we introduce cellSTAAR, which integrates whole-genome sequencing data with single-cell assay for transposase-accessible chromatin using sequencing data to capture variability in chromatin accessibility across cell types via the construction of cell-type-specific functional annotations and regulatory elements. Background: Sporotrichosis is a dimorphic fungal infection of increasing global relevance. Although usually cutaneous or lymphocutaneous, disseminated disease occurs in immunocompromised hosts. Large-scale data on its epidemiology, treatment and outcomes remain limited.
Objectives: To characterise the clinical features, comorbidities, antifungal prescribing patterns and one-year mortality associated with sporotrichosis, with emphasis on patients with HIV and other forms of immunosuppression.
Patients/methods: We performed a retrospective global cohort study using the TriNetX Research Network. Objectives: The Harmonized Cognitive Assessment Protocol (HCAP) neuropsychological assessment approach aims to support the collection of harmonizable data on cognitive function for cross-national cognitive aging and dementia research. As the measurement of cognition is sensitive to differences in contextual, cultural, educational, linguistic, social, and other factors that may influence cognitive test performance, HCAP requires adaptation to be appropriate for the contexts in which it is administered. Dysregulated ribosome biogenesis and p53 mutations are known to play oncogenic roles in various cancers, including pancreatic cancer. In this study, we demonstrated the therapeutic potential of BMH-21, a pharmacologic inhibitor of RNA polymerase I, against pancreatic cancer by uncovering a novel molecular mechanism involving RPA194-mediated ubiquitination of mutant p53 without affecting the ubiquitination of wild-type p53. Our key findings are that (i) BMH-21 selectively induces apoptosis and cell growth inhibition of pancreatic cancer cells with no effect on normal human pancreatic ductal epithelial cells; (ii) BMH-21 degrades RPA194; (iii) BMH-21 inhibits recruitment of both RPA194 and RPA135 on rDNA …

The rapid integration of artificial intelligence (AI) into neurosurgical practices in the United States is transforming how diagnoses are interpreted, how surgical plans are developed, and how guidance is provided during operations as clinical needs continue to grow. Background: Lucid dreaming (LD), which involves being aware of and controlling one's dreams, is of particular interest in the study of consciousness and has potential therapeutic applications. In this narrative review, we highlight the neurobiological aspects of LD, specifically its mechanisms associated with the activation of prefrontal and parietal areas, increased gamma oscillations, and the influence of cholinergic and dopaminergic signaling, primarily during rapid eye movement (REM) sleep.
Objective: To evaluate human data on brain mechanisms and clinical effects, synthesizing the neurobiological foundations and therapeutic possibilities of LD, including remarkably controlled lucid dreams.

Background: Neurodegenerative diseases (NDs) such as Parkinson's disease (PD), Alzheimer's disease (AD), amyotrophic lateral sclerosis (ALS), Huntington's disease (HD) are set apart by progressive neuronal loss and concomitant functional decline. Traditional therapies are equipped with only symptomatic relief, devoid of neurorestorative properties. Stem-cell-based therapies have the potential to revolutionize neurological care by replenishing lost cells, mitigating inflammation, and fostering a neuroprotective environment.

Red, inflamed cutaneous nodules are frequently diagnosed as epidermal inclusion cysts in both primary care and dermatology settings. However, several benign and malignant entities can closely mimic an inflamed cyst, leading to delayed diagnosis and suboptimal management. We present four patients who initially presented with erythematous nodules clinically suspected to be cysts; final histopathologic diagnoses included dermatofibroma, amelanotic melanoma, dermatofibrosarcoma protuberans, and undifferentiated pleomorphic sarcoma. For each case, we describe the clinical presentation, histopathology, and management. The presence of a central punctum and superficial mobility strongly favors an epidermal inclusion cyst. Cutaneous manifestations of T-cell lymphomas may resemble common dermatologic or infectious conditions, increasing the risk of delayed recognition. We report a 78-year-old woman with known peripheral T-cell lymphoma (PTCL) who presented with a rapidly progressive, diffuse erythematous rash accompanied by cervical lymphadenopathy. Initial evaluation prioritized more common etiologies; however, inpatient medicine broadened the differential diagnosis, prompting early hematology and oncology consultation. Contrast-enhanced CT of the neck demonstrated multiple enlarged, partially necrotic left cervical lymph nodes consistent with disease progression. After multidisciplinary discussions and consideration of prognosis, the patient elected palliative-focused management with oncology follow-up. This case underscores the importance of …

Boerhaave’s syndrome is a rare and life-threatening form of spontaneous esophageal perforation, typically triggered by forceful vomiting and often misdiagnosed due to nonspecific clinical features. Although Mackler’s triad (vomiting, chest pain, and subcutaneous emphysema) is classically associated with the condition, it is infrequently observed in full. We present the case of a 32-year-old man with a history of ulcerative colitis (UC) who presented to the emergency department with acute chest pain and repeated vomiting following dinner. Learning and memory, as fundamental components of human cognition, are heritable traits that are highly variable between individuals and within populations. Investigation into the genetic basis of cognition is a prominent area of research, with genetic associations being previously reported for a wide range of cognitive phenotypes. Here we utilise a genome-wide association study (GWAS) approach to evaluate the contribution of genetic variation to learning and memory phenotypes in a comprehensively phenotyped, well-characterised, healthy, and unrelated cohort of individuals (n = 613). Aims/hypothesis: Osteomyelitis is a common complication of diabetic foot ulcers and a leading cause of lower limb amputations globally. Accurate diagnosis of diabetic foot osteomyelitis (DFO) is essential for effective infection management and improving prognosis. This study aimed to compare the performance of white blood cell (WBC)-single photon emission computed tomography (SPECT)/computed tomography (CT) and conventional MRI in the initial diagnosis of DFO and in detecting residual infection during treatment monitoring, using bone biopsy as the reference standard.

Background: Although combat-deployed soldiers are at high risk for developing trauma-related psychopathology, most will remain resilient for the duration and aftermath of their deployment tour. The neural basis of this type of resilience is largely unknown, and few longitudinal studies exist on neural adaptation to combat in resilient individuals for whom a pre-exposure measurement was collected. Here, we delineate changes in the architecture of functional brain networks from pre- to postcombat in psychopathology-free, resilient participants.

Myeloid/lymphoid neoplasms with eosinophilia and tyrosine kinase fusions (M/LN-eo-TK) are uncommon but highly treatable disorders. Among them, FIP1L1::PDGFRA–driven disease is distinguished by marked eosinophilia and multisystem involvement that can rapidly reverse with targeted therapy. We describe a 50-year-old man with uncontrolled diabetes who presented with progressive dyspnea, abdominal discomfort, and painful necrotic scrotal ulcers. Laboratory testing revealed leukocytosis with a striking absolute eosinophil count of 22.1 × 10³/µL, while imaging showed pulmonary infiltrates, small-bowel inflammation, and splenomegaly. Bone marrow examination demonstrated hypercellularity with prominent eosinophilic proliferation. Neuroanatomical findings on panic disorder (PD) are typically difficult to replicate, with inconsistent effects. These concerns prompted a paradigm shift towards large-scale collaborations, focused on harmonized data extraction and processing for robust examination of PD brain correlates. Hence, leveraging the largest-ever multi-site neuroimaging database on PD (Age: 10–66 years; global sites: 28), compiled by the ENIGMA-Anxiety Working Group, we report on cortical and subcortical differences in individuals with PD (N = 1146) versus healthy controls (HC: N = 3778).
