The Prospects Of Car T-Cell Therapies In Changing The Therapeutic Algorithm Of Neurologic Autoimmunities, Marinos C. Dalakas
Department of Neurosurgery Faculty Papers
T cells, genetically modified to express chimeric antigen receptors (CAR T), successfully used in hemato-oncologic malignancies, are showing promising and sustained benefits in refractory autoimmune neurological diseases, including Myasthenia Gravis, stiff-person syndrome, neuromyelitis optica, myositis, autoimmune neuropathies, and multiple sclerosis. Several reported patients with a steadily progressive disease and evolving disability unresponsive to available therapies, including rituximab and new biologics, after 2-3 months of treatment with CARs targeting CD19-positive, antibody-secreting, long-lived plasma cells, and plasmablasts exhibit impressive, long-lasting, and drug-free clinical improvements with the potential for immune reset shifting immunity to a healthy state without the need for continuing more …

RNA origami enables genetically encoded, single-stranded RNA nanostructures that can self-assemble through co-transcriptional folding and are increasingly deployed as scaffolds for biosensing, synthetic biology, and nanomedicine. A recurring practical bottleneck is scalable, solution-phase readout of whether a designed scaffold has reached its intended base-paired architecture, whether it undergoes slow maturation or kinetic trapping, and how its stability is distributed across motifs. Here, we adapt microfluidic modulation spectroscopy (MMS) as a label-free structural biosensor for RNA folding by exploiting the rich 1760–1600 cm−1 vibrational fingerprints of RNA bases and base pairs. MMS alternates between sample and composition-matched buffer measurements in a …

Background/Objectives: Previous studies have suggested that lifestyle intervention (LSI) therapies involving diet and exercise can modulate DNA methylation; however, whether this occurs in severely obese hypogonadal men undergoing weight loss from diet and exercise remains unclear.
Methods: In this study, we investigated the effects of weight loss from diet and exercise on global DNA methylation as well as on the mRNA expression of specific demethylation enzymes, DNMT1, DNMT3A, and DNMT3B—in peripheral blood mononuclear cells (PBMCs) and DNA methylation markers in DNA of severely obese hypogonadal men. This is a secondary analysis of samples of severely obese (body …

Background
Solitary fibrous tumor (SFT) is a rare mesenchymal tumor usually occurring at the pleura. Primary hepatic SFT presents at the liver is extremely rare, and its nonspecific clinical-biological findings cause a considerable difficulty in differentiation from other hepatic tumors.
Case presentation
We reported a uncommon case of primary hepatic SFT, which was pathologically characterized by abundant multinucleated giant cells. A man in his 60 s was admitted for a long-standing right upper abdominal pain (more than several months). Magnetic resonance imaging (MRI) scan revealed a mass in the liver. A right hemihepatectomy was performed and the pathological diagnosis revealed …

Teaching and Learning in Communication Sciences & Disorders
Assessment is central to effective teaching, and the structure of exam questions significantly influences both learning and instruction. Multiple-choice (MCQ) and short-answer (SAQ) questions are widely used and frequently studied, each offering distinct advantages and limitations for formative and summative assessment. This study examined a modified multiple-choice format, multiple-choice questions with explanation (MCQwE), for which students selected an answer and briefly explained their reasoning. Designed to combine the efficiency of MCQs with the diagnostic value of open-ended responses, MCQwE were implemented in an undergraduate speech science course and compared to traditional MCQs and SAQs in terms of student performance. Findings …

Teaching Through Disruption: Trauma-Informed, Community-Engaged Learning In A Graduate Csd Course, Teresa L. Boggs
Teaching and Learning in Communication Sciences & Disorders
This article explores how a graduate course in communication sciences and disorders (CSD) was rapidly redesigned in response to a natural disaster to integrate trauma-informed and community-engaged learning. Following Hurricane Helene’s impact on Appalachian communities, a standard literature review assignment was transformed into a collaborative project where students created low-cost, developmentally appropriate language-learning kits for young children affected by the storm. Grounded in adult learning theory, trauma-responsive pedagogy, and high-impact educational practices, the revised course structure enabled students to apply clinical knowledge in real time while supporting families in crisis. Outcomes included increased student engagement, empathy, and professional competency. The …

Biased and stigmatizing language is pervasive in healthcare records and has negative effects on clients’ care. While some research has studied how medical students may be taught to reduce stigmatizing language in their writing, the impacts of such trainings on clinical writing are rarely directly assessed, and little has been done to address this problem in speech-language pathology (SLP). This study assessed how a training module on biased language in clinical writing impacted SLP masters students’ ability to identify and correct stigmatizing language in a clinical report. Across two years, 120 students took part in this module. During the second …

Endometriosis is a chronic gynecological disorder marked by the growth of endometrial-like tissue outside the uterus, often leading to pelvic pain, inflammation, and infertility. Despite its global prevalence, diagnosis remains delayed, and effective non-surgical treatments are lacking. While recent transcriptomic studies have identified mRNA transcript changes in ectopic lesions, the contribution of alternative pre-mRNA splicing, a key posttranscriptional regulatory layer, remains largely unknown. In this study, we performed a comprehensive analysis of alternative splicing (AS) events in endometriotic lesions using transcriptomic data. We uncovered distinct alterations in alternative splicing events are associated with endometriosis, highlighting a previously underappreciated layer of …

International Arab Journal of Dentistry
Objective: This study aims to assess the impact of presurgical mandibular incisor inclination on the stability of surgical class III patients.
Methods: An electronic search was conducted on PubMed, Scopus, Web of Science, EBSCO, and the Cochrane Library from 2014 to February 2025, initially identifying 351 publications relevant to our review.
Results: Four retrospective studies were included in this review, with the risk of bias ranging from low to serious. A preoperative retroinclined incisal axis leads to an increased post-surgical overjet and inadequate interincisal contact, indicating that retroclined mandibular incisors may affect skeletal stability. However, Overcorrection of mandibular incisor inclination …

Intergenerational Hypertension Transmission Before And After South Africa's Democratic Transition, Winfred A. Avogo
Faculty Publications – Sociology and Anthropology
Background: Apartheid's structural violence produced profound cardiovascular inequalities among Black South Africans through processes that accelerate biological aging ("weathering") and transmit health disadvantage across successive generations. Whether South Africa's 1994 political transition interrupted this intergenerational transmission remains unknown. This study tests whether parent-to-offspring hypertension transmission weakened among cohorts born after apartheid's end, using the 1994 transition as a natural experiment. Methods: This study analyzed 30,438 parent-offspring dyads from five waves (2008–2017) of South Africa's National Income Dynamics Study, representing 11,655 unique offspring aged 18 years and older. Hypertension was defined as blood pressure ≥140/90mmHg or use of antihypertensive medication. Logistic …

Currently, there are very few efficient treatment options for hepatobiliary pancreatic cancer (HPC), which comprises pancreatic ductal adenocarcinoma (PDAC), biliary tract cancer (BTC), and hepatocellular carcinoma (HCC). The HPC tumors are the most lethal malignant tumors in the world. Traditional chemotherapy offers little survival benefit and is associated with notable systemic toxicity, which has made antibody–drug conjugates (ADCs) a hopeful treatment option. Strong cytotoxic drugs combine with monoclonal antibodies to attack tumor-associated antigens. This review discusses the benefits and current developments of Antibody–Drug Conjugates (ADCs) in treating HPC. It also covers their mechanisms of action, ongoing clinical trials, and the …

Cemented Femoral Fixation In A High-Risk Periprosthetic Fracture Cohort – A Case Series, John Spak, Philip C. Aurigemma, Yaser C. Rahim
The Lahey Journal
Introduction:
Periprosthetic femur fractures (PPFx) are a major cause of early revision after total hip arthroplasty (THA), accounting for nearly 20% of early failures. Cementless fixation, though widely used, has been associated with an increased risk of early PPFx, particularly in elderly or osteoporotic patients. Cemented femoral fixation may reduce this risk in high-risk populations.
Case Presentation:
We performed a retrospective review of 430 consecutive primary THAs conducted between July 2022 and July 2025 at a single institution. Cementless fixation was used in 390 patients and cemented fixation in 40 high-risk patients (mean age 81.7 vs. 65.9 years; BMI 24.5 …

Simulated Hearing Loss In Healthy Young And Older Adults Results In Poor Reaction Time In Older Adults., Victoria Kowalewski, Nicoleta Bugnariu, Rita M. Patterson, Jessica Hartos
Pacific Journal of Health
Emerging evidence links hearing loss to increased fall risk in older adults, yet mechanisms remain unclear. This study examined the impact of simulated hearing loss on reactive balance control in healthy young and older adults. Participants performed dual-task scenario involving auditory sentence repetition and unexpected surface translations. Hearing loss was simulated using Adobe Audition, applying frequency-specific attenuation based on moderate sensorineural hearing loss levels from the literature.
Speech-in-noise performance significantly declined under simulated hearing loss for both age groups (p < .001), confirming effective simulation, but not significant differences were found between age groups. Maximum COP-COM displacement (the scalar distance between Center of Pressure and Center of Mass) increased with perturbation intensity (Level 1 vs. Level 2: p < .001), but no significant differences were found between hearing conditions or age groups (p > .05), suggesting compensatory step execution was preserved.
Reaction time analysis revealed a significant interaction between hearing condition and perturbation level …

Well-designed randomized controlled trials (RCTs) have demonstrated safe and effective use of oral antimicrobial therapy for bone and joint infections. Application of data for implementation into real-world practice, however, has inherent challenges. This retrospective cohort study compared real-world use of intravenous versus oral antimicrobial therapy in bone and joint infections within a large healthcare system comprising both academic and community medical centers. The primary outcome was the proportion of treatment failure. Key secondary outcomes included the proportion of patients with logistical failure and risk factors associated with treatment and logistical failure. Among 166 patients included, 136 (82%) and 30 (18%) …

IL-22, a signature cytokine for type 3 lymphoid cells, including T helper 17/22 (Th17/22) and type 3 innate lymphoid cells (ILC3), mediates epithelial homeostasis and protective pathogen responses in barrier tissues. Upon dysregulation, IL-22 can drive chronic inflammatory diseases, yet little is known about transcriptional elements modulating its expression. Here, we identify two enhancers, E22-1 and E22-2, with distinct capacities for regulating Il22 expression in type 3 lymphoid cells. Both enhancers are necessary for protection from Citrobacter rodentium infection and for the onset of IL-22-mediated psoriasis. E22-2 is specifically required for IL-22 expression in ILC3s, while E22-1 functions in both …

Background and objectives: Re-irradiation with stereotactic radiosurgery (SRS) is an increasingly used treatment for recurrent high-grade glioma (HGG). We analyzed the patterns of failure after salvage SRS to better identify which patients would most benefit.
Methods: We reviewed all patients who received SRS for recurrent HGG over 12 years at our institution. Pattern of failure was defined by the distance of recurrence from the SRS isodose line. Overall-survival (OS), progression-free survival (PFS), and local-failure-free survival (LFS) were estimated using Kaplan-Meier methods, and regression model analysis was performed to analyze clinical predictors of failure.
Results: 146 patients with HGG underwent SRS …

Background: Tenosynovial giant cell tumour (TGCT) is a rare, locally aggressive neoplasm that affects otherwise healthy adults. There are few systemic treatment options, highlighting an unmet need. We report the results of part 1 of the MANEUVER trial, which aimed to evaluate the efficacy and safety of pimicotinib, a highly selective, potent, colony-stimulating factor-1 receptor inhibitor, in patients with TGCT.
Methods: MANEUVER is a randomised, placebo-controlled, phase 3 study done in 40 specialised hospitals in Asia, Europe, and North America. Patients aged 18 years and older with unresectable, symptomatic TGCT (patient-reported worse stiffness or worst pain of at least 4 …

Objectives: Cancer is still one of the leading causes of death worldwide and a high percentage of patients with cancer face emotional challenges such as guilt and self-blame. Early maladaptive schemas (EMSs) can make these feelings even stronger. This study aimed to explore how guilt and self-blame relate to EMSs in individuals with cancer, while taking overall psychological distress into account. Methods: This cross-sectional study was conducted at Rasoul-e-Akram and Firoozgar hospitals in Tehran, Iran, from October 2022 to June 2023. Patients with cancer answered the Early Maladaptive Schema Questionnaire-Short Form, Eysenck's Guilt Questionnaire (2007) and the Cognitive Emotion Regulation …
