Recent advancements in nanotechnology have unfolded novel opportunities in medicine, especially in targeted therapeutics and imaging for cancer. However, the majority of the existing nanotechnologies for cancer suffer from shortcomings such as (i) rapid elimination from the systemic circulation before reaching the cancer tissue. (ii) poor tumor accumulation, targeting, and penetration due to inadequate vasculature and extensive extracellular matrix in the tumor. Many serious infections for which there are no existing vaccines, enter by a mucosal route such as HIV, influenza, and tuberculosis, etc. Therefore, designing safe and effective mucosal vaccines that elicit robust immune response at mucosal sites is still a major challenge without a safe mucosal adjuvant. Previously, our laboratory had discovered inulin acetate as a novel toll-like-receptor-4 agonist (adjuvant). Helicobacter pylori is a gram-negative bacteria that is responsible for causing chronic gastritis, ulcers in the stomach and intestine, and eventually even gastric lymphoma or cancer. Multiple treatment options have been recommended for treating an H. pylori infection based on kidney function, previous antibiotic exposure, and whether or not the infection is recurrent. The most common regimens used at St. Vincent's primary care centers closely mimic the American College of Gastroenterology's clinical guidelines. สปอร์เห็ดหลินจือมีสารสำคัญออกฤทธิ์กระตุ้นการทำงานของเซลล์ในระบบภูมิคุ้มกัน เซลล์เหล่านี้ใช้เวลาในการฟื้นตัวมากกว่า 9 เดือนหลังจากผู้ป่วยมะเร็งหยุดยาเคมีบำบัด จึงนำมาสู่การศึกษาครั้งนี้โดยมีวัตถุประสงค์เพื่อศึกษาผลของสปอร์เห็ดหลินจือที่มีต่อระบบภูมิคุ้มกันในผู้ป่วยมะเร็งเต้านม ผู้ป่วยมะเร็งลำไส้ใหญ่หรือทวารหนักหลังครบการรักษาด้วยยาเคมีบำบัดในช่วงเดือนพฤศจิกายน พ.ศ. 2561 ถึงธันวาคม พ.ศ. 2562 การศึกษานี้เป็นการศึกษาทางคลินิกแบบสุ่ม ปกปิด 2 ทาง มีการเปรียบเทียบกับกลุ่มควบคุม โดยกลุ่มตัวอย่างที่เข้าร่วมการศึกษาจนครบการศึกษาทั้งสิ้น 44 ราย แบ่งโดยการสุ่มเป็น 2 กลุ่มเท่า ๆ กัน ได้แก่ กลุ่มที่ได้สปอร์เห็ดหลินจือที่ผ่านการกะเทาะขนาด 250 มิลลิกรัมและกลุ่มที่ได้ยาหลอก รับประทานผลิตภัณฑ์ครั้งละ 1 แคปซูล วันละ 3 ครั้ง ก่อนอาหาร เป็นระยะเวลา 8 สัปดาห์ วัดระดับอิมมูโนโกลบูลิน (IgG, IgM, IgA และ IgE) ความสมบูรณ์เม็ดเลือด ค่าการทำงานของตับและไต อาการไม่พึงประสงค์ ณ สัปดาห์ที่ 0, 4 และ 8 รวมไปถึงประเมินคุณภาพชีวิตด้วยแบบประเมิน FACT-G ก่อนและหลังเข้าร่วมการศึกษา พบว่า สัปดาห์ที่ 8 กลุ่มที่ได้สปอร์เห็ดหลินจือมีการเปลี่ยนแปลงของระดับอิมมูโนโกลบูลิน เม็ดเลือดแดง เม็ดเลือดขาวชนิด neutrophil basophil eosinophil monocyte และเกล็ดเลือดไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก ในขณะที่จำนวนเม็ดเลือดขาวทั้งหมดและ absolute neutrophil count ในกลุ่มที่ได้สปอร์เห็ดหลินจือมีระดับเพิ่มขึ้นอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก (p-value= 0.031 และ 0.021 ตามลำดับ) ผู้เข้าร่วมการศึกษาทั้ง 2 กลุ่มมีการเปลี่ยนแปลงของค่าการทำงานของตับและไตอย่างไม่มีนัยสำคัญตลอดระยะเวลาการศึกษา อาการไม่พึงประสงค์ที่พบ เช่น ปากแห้ง คอแห้ง คลื่นไส้ เป็นต้น มีความรุนแรงระดับน้อย นอกจากนี้กลุ่มที่ได้สปอร์เห็ดหลินจือมีคุณภาพชีวิตไม่แตกต่างอย่างมีนัยสำคัญเมื่อเปรียบเทียบกับกลุ่มที่ได้ยาหลอก กล่าวโดยสรุป การรับประทานสปอร์เห็ดหลินจือในขนาดที่ศึกษามีความปลอดภัยสามารถเพิ่มจำนวนเม็ดเลือดขาวทั้งหมดและ ANC ของผู้ป่วยมะเร็งหลังได้รับยาเคมีบำบัดได้ แต่ไม่สามารถเพิ่มระดับอิมมูโนโกลบูลินได้อย่างมีนัยสำคัญ

การวิจัยนี้เป็นการวิจัยเชิงวิเคราะห์แบบเก็บข้อมูลย้อนหลัง เพื่อประเมินการสั่งใช้ยากลุ่มสแตตินในทางปฏิบัติเพื่อป้องกันโรคหัวใจและหลอดเลือด และศึกษาประสิทธิผลการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ได้รับยาต้านไวรัส ณ โรงพยาบาลกระทุ่มแบน มีผู้ป่วยเข้าร่วมการวิจัย 551 ราย โดยผู้ป่วยส่วนใหญ่เป็นเพศชาย ร้อยละ 52.5 เมื่อประเมินการสั่งใช้ยากลุ่มสแตตินตามแนวทางเวชปฏิบัติการใช้ยารักษาภาวะไขมันผิดปกติเพื่อป้องกันโรคหัวใจและหลอดเลือด พ.ศ. 2559 มีผู้ป่วยที่ควรได้รับยากลุ่มสแตตินตามคำแนะนำเพื่อป้องกันโรคหัวใจและหลอดเลือด 198 ราย (ร้อยละ 35.9) แต่ได้รับยากลุ่มสแตตินเพียง 37 ราย (ร้อยละ 6.7) และไม่ได้รับยากลุ่มสแตตินสำหรับป้องกันปฐมภูมิและทุติยภูมิ 155 ราย (ร้อยละ 28.1) และ 6 ราย (ร้อยละ 1.1) ตามลำดับ เมื่อจำแนกตามระดับความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในการป้องกันโรคหัวใจและหลอดเลือดชนิดปฐมภูมิ พบว่า ผู้ป่วยที่ระดับความเสี่ยงปานกลางและสูงถึงสูงมากได้รับการสั่งใช้ยากลุ่มสแตตินไม่ตรงตามคำแนะนำมากกว่าตรงตามคำแนะนำในแนวทางเวชปฏิบัติฯ อย่างมีนัยสำคัญทางสถิติ (P<0.01 และ P<0.01 ตามลำดับ) สำหรับข้อมูลประสิทธิผลของการลดระดับไขมันในเลือดของยากลุ่มสแตตินในผู้ติดเชื้อเอชไอวีที่ระยะเวลา 3, 6 และ 12 เดือน พบว่า กลุ่มผู้ป่วยที่ไม่ได้รับยากลุ่มสแตตินและกลุ่มผู้ป่วยที่ได้รับยากลุ่มสแตตินมีผลต่างของระดับแอลดีแอลในเลือด ซึ่งมีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P<0.01) ผลจากการศึกษานี้แสดงให้เห็นว่า อัตราการสั่งใช้ยากลุ่มสแตตินเพื่อป้องกันโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวีที่ไม่เหมาะสมตามเกณฑ์คิดเป็นร้อยละ 37.7 แม้ว่ายากลุ่มสแตตินจะมีประสิทธิผลในการลดระดับไขมันในเลือดและมีความปลอดภัยในการใช้ร่วมกับยาต้านไวรัส ดังนั้นควรมีการพิจารณาเพิ่มการสั่งใช้ยากลุ่มสแตตินอย่างสมเหตุผลตามเกณฑ์ เพื่อป้องกันความเสี่ยงต่อการเกิดโรคหัวใจและหลอดเลือดในผู้ติดเชื้อเอชไอวี

ยาไมโครฟีโนลิกชนิดมีสารเคลือบเป็นยากดภูมิคุ้มกันใช้ป้องกันการปฏิเสธไตในผู้ป่วยที่ได้รับการปลูกถ่ายไต ยามีความผันแปรภายในตัวบุคคลและระหว่างบุคคลค่อนข้างสูง (high intra-and inter-individual variation) จึงควรตรวจติดตามระดับยาโดยหาจุดเวลาที่เหมาะสมที่สัมพันธ์ดีกับพื้นที่ใต้กราฟ (AUC คือ MPA exposure) รวมทั้งยังไม่มีรายงานการหาจุดตัด (cut-off point) ความเข้มข้นที่เหมาะสมที่บ่งถึงค่า MPA exposure ≥ 30 ไมโครกรัม•ชั่วโมง/มิลลิลิตรในผู้ป่วยปลูกถ่ายไต การศึกษานี้มีวัตถุประสงค์คือหาความสัมพันธ์ของพื้นที่ใต้กราฟกับความเข้มข้นของ MPA และเมทาบอไลท์ทั้ง total และ free form แต่ละจุดเวลา และหาค่าจุดตัด (cut-off point) ความเข้มข้นที่บ่งถึงค่า MPA exposure ≥ 30 ไมโครกรัม•ชั่วโมง/มิลลิลิตร จึงทำการศึกษาในผู้ป่วยปลูกถ่ายไตจำนวน 20 ราย ที่ได้รับยา EC-MPS ขนาด 1,080 มกต่อวัน ร่วมกับทาโคลิมุสและเพรดนิโซโลน เก็บเลือดผู้ป่วยที่ 0 ก่อนรับประทานยาและ 0.5, 1, 1.5, 2, 2.5, 3, 3.5, 4, 6, 8, 10 และ 12 ชั่วโมงภายหลังรับประทานยา EC-MPS ในวันที่ 3 วัดความเข้มข้นของ MPA และเมทาบอไลท์ทั้ง total และ free form ด้วยวิธี LC-MS/MS จากการศึกษาพบว่าจุดเวลาที่ความเข้มข้นสัมพันธ์ดีที่สุดกับพื้นที่ใต้กราฟของ MPA total form C4 (r2 = 0. 50) MPA free form C8 (r2 = 0. 41) MPAG total form C6 (r2 = 0. 97) …

Paclitaxel is recommended first-line therapy for patients with advanced non-small cell lung cancer. Genetic polymorphisms of Paclitaxel’s metabolic pathway may affect treatment outcomes and adverse events. The purpose of this study was to determine the association of genetic polymorphisms and clinical outcomes. A total of 58 Thai NSCLC patients with Paclitaxel-based chemotherapy were enrolled for this cohort study. The prevalence of variant allele of SLCO1B3 (rs7311358), ABCB1 (rs2032582) and CYP3A5 (rs776746) were 72.4%, 40.5% and 67.5%, respectively. A total of 831 moderately halophilic bacteria isolated from various samples in Thailand were screened for their lipolytic activity. Three hundred and forty-nine isolates showed activity on agar plate while 322 isolates exhibited in broth. The grouping and identification of 322 isolates based on DNA-fingerprinting using (GTG)5+ERIC2 primer and 16S rRNA gene sequence, 180 DNA patterns could be generated. The result indicated that the representative isolates belonged to genera Bacillus, Halobacillus, Lentibacillus, Marinobacter, Thallossobacillus, and Virgibacillus. Lusianthridin is a phenanthrene derivative isolated from Dendrobium venustum. Some phenanthrene compounds have antiplatelet aggregation activities via undefined pathways. The objective of this study is to determine the inhibitory effects and potential mechanisms of lusianthridin on platelet aggregation. Turbidometric aggregometry method was used to measure the extent of platelet aggregation, then cyclic adenosine monophosphate (cAMP) levels were measured by ELISA kit. Inhibitory effect of lusianthridin on cyclooxygenase enzymes was analyzed by COX fluorescent inhibitor screening assay kit. Chemical investigation of two orchids native to Thailand led to the isolation of eighteen compounds. From the whole plants of Cymbidium finlaysonianum Wall. ex Lindl, a new dihydrophenanthrene, 1-(4-hydroxybenzyl)-2,7-dihydroxy-4,6-dimethoxy-9,10-dihydrophenan-threne, and nine known stilbenoids, including a phenanthrenequinone, cymbinodin-A, a dihydrophenanthrenequinone, ephemeranthoquinone B, a bibenzyl, batatasin III, two phenanthrenes, 2,4-dimethoxy-3,7-dihydroxyphenanthrene and 3,7-dihydroxy-2,4,6-trimethoxyphenanthrene, and four dihydrophenanthrenes i.e. coelonin, 6-methoxycoelonin, flavanthridin and lusianthridin, were obtained. Indomethacin (INDO) and diclofenac (DIC) cause intestinal cell death through reactive oxygen species (ROS), endoplasmic reticulum stress (ER stress) and mitochondrial dysfunction. This study investigated the cytoprotective effects of certain natural polyphenolic antioxidants against INDO- and DIC-mediated Caco-2 cell death as well as underlying mechanisms. In this study, caffeic acid (CAF), curcumin (CUR), epigallocatechin gallate (EGCG), gallic acid (GAL), hypophyllanthin (HYPO), naringenin (NAR), phyllanthin (PHY), piperine (PIP), quercetin (QUE), rutin (RUT) and silymarin (SLY) were able to decrease ROS production in Caco-2 cells treated with INDO and DIC. Human immunodeficiency virus (HIV) and opioid abuse have been described as synergistic epidemics. Pharmacologically, it has been found that opioids have the capacity to enhance HIV infection and replication. Research has shown that activation of the mu-opioid receptor (MOR) elevates the expression of the HIV-1 entry co-receptor CXCR4 on T-lymphocytes in the peripheral nervous system, thus allowing for enhanced viral entry and invasion. Although the exact mechanism for opioid modulation of CXCR4 expression and subsequent exacerbation of HIV is unknown, several hypotheses exist. Rhinacanthin-C (RN-C) is a major bioactive naphthoquinone ester found in Rhinacanthus nasutus Kurz (Acanthaceae). This compound has potential therapeutic value as an anticancer agent. Rhinacanthin-C was reported to enhance doxorubicin cytotoxicity in breast cancer cells through direct inhibition of P-gp activity. However, its ability in reversing multidrug resistance (MDR) in cancer cells through the down-regulation of P-gp expression, alteration of detoxification system, and increase in apoptosis proteins remains unreported. The Chapter II focuses on formulation of an optimized and robust amphotericin B ocular formulation with prolonged precorneal residence and ocular tissue concentration at par or superior to the marketed preparations. Additionally, demonstrating stability of amphotericin B formulations in presence of preservative in comparison to marketed preparation, making it an ideal choice of formulation for multi-dosing. As mentioned earlier, we optimized PEGylated nanostructured lipid carriers (PEG-NLC-AmB) for ocular delivery of amphotericin B. The thioredoxin (Trx) system is an endogenous antioxidant system that affects cell function and survival through controlling cellular redox status. Trx and TrxR are the main enzymes in this system while thioredoxin interacting protein (TXNIP) is a negative regulator. This study’s goal was to better understand the Trx system’s involvement in the cardiovascular disease and modulate the pathway through drug treatment. Purpose: Outpatient parenteral antimicrobial therapy (OPAT) is a therapeutic option meant to conserve healthcare resources when treating infections requiring the administration of IV antibiotics over a prolonged treatment course. In November 2016 at Franciscan Alliance Indianapolis, a dedicated pharmacist was hired to build a formal OPAT program for all patients discharged on IV antimicrobials under the care of the infectious disease physician group. We demonstrate a novel inorganic-organic crystalline nanoconstruct, where gold atoms were imbedded in the crystal lattices as defects of camptothecin nanocrystals, suggesting its potential use as simultaneous agents for cancer therapy and bioimaging. The incorporation of gold, a potential computed tomography (CT) contrast agent, in the nanocrystals of camptothecin was detected by transmission electron microscope (TEM) and further quantified by energy dispersive X-ray spectrometry (EDS) and inductively coupled plasma-optical emission spectrometers (ICP-OES). Due to gold's high attenuation coefficient, only a relatively small amount needs to be present in order to create a good noise-to-contrast ratio in CT imaging. The imbedded …

The role and functional anatomy of the blood-brain barrier (BBB) is summarized to enable the investigator to appropriately address evaluation of nanomaterial interaction with, and distribution across, it into brain tissue (parenchyma). Transport mechanisms across the BBB are presented, in relation to nanomaterial physicochemical properties. Measures and test substances to assess BBB integrity/disruption/permeation are introduced, along with how they are used to interpret the results obtained with the presented methods. Cerium dioxide nanoparticles (NPs) were synthesised using a novel precipitation procedure and their function explored in a range of applications. The impact of different factors on CeO2 NPs synthesis including temperature and reactant concentration were evaluated experimentally and optimised. The resulting nanoparticles (with an average size of 35 nm) was then characterised by a range of physical and spectroscopic techniques including dynamic light scattering, scanning electron microscopy in conjunction with energy dispersive spectrometry, X-ray diffractometry,Brunauer–Emmett–Teller isotherm measurement, Raman spectroscopy, UV-visible spectroscopy and X-ray photoelectron spectroscopy.
