Mycobacterium tuberculosis (Mtb) has recently surpassed HIV/AIDS as the leading cause of death by a single infectious agent. The standard therapeutic regimen against tuberculosis (TB) remains a long, expensive process involving a multidrug regimen, and the prominence of multidrug-resistant (MDR), extensively drug-resistant (XDR), and totally drug-resistant (TDR) strains continues to impede treatment success. An underexplored class of natural products—the capuramycin-type nucleoside antibiotics—have been shown to have potent anti-TB activity by inhibiting bacterial translocase I, a ubiquitous and essential enzyme that functions in peptidoglycan biosynthesis. Alzheimer’s disease (AD) is the most prevalent neurodegenerative disorder in the world, and intracellular neurofibrillary tangles and extracellular amyloid-beta protein deposits represent the major pathological hallmarks of the disease. Currently available treatments provide some symptomatic relief but fail to modify primary pathological processes that underlie the disease. Erythropoietin (EPO), a hematopoietic growth factor, acts primarily to stimulate erythroid cell production, and is clinically used to treat anemia. EPO has evolved as a therapeutic agent for neurodegeneration and has improved neurological outcomes and AD pathology in rodents. Understanding the functional relevance of G protein-coupled receptor (GPCR) homodimerization has been limited by the insufficient tools to assess asymmetric signaling occurring within dimers comprised of the same receptor type. We present unmatched bivalent ligands (UmBLs) to study the asymmetric function of melanocortin homodimers. UmBLs contain one agonist and one antagonist pharmacophore designed to target a melanocortin homodimer such that one receptor is occupied by an agonist and the other receptor by an antagonist pharmacophore. Folate metabolism is essential for DNA synthesis and a validated drug target in fast-growing cell populations, including tumors and malaria parasites. Genome data suggest that Plasmodium has retained its capacity to generate folates de novo. However, the metabolic plasticity of folate uptake and biosynthesis by the malaria parasite remains unresolved. Here, we demonstrate that Plasmodium uses an aminodeoxychorismate synthase and an aminodeoxychorismate lyase to promote the biogenesis of the central folate precursor para-aminobenzoate (pABA) in the cytoplasm. Sigma-1 receptors (Sig-1Rs) are integral ER membrane proteins. They bind diverse ligands, including psychoactive drugs, and regulate many signaling proteins, including the inositol 1,4,5-trisphosphate receptors (IP3Rs) that release Ca2+ from the ER. The endogenous ligands of Sig-1Rs are unknown. Phospholipase D (PLD) cleaves phosphatidylcholine to choline and phosphatidic acid (PA), with PA assumed to mediate all downstream signaling. We show that choline is also an intracellular messenger. Choline binds to Sig-1Rs, it mimics other Sig-1R agonists by potentiating Ca2+ signals evoked by IP3Rs, and it is deactivated by metabolism. Background: On February 21st 2019, FDA added a boxed warning for increased risk of death with febuxostat compared to allopurinol. Purpose: To compare the incidence of cardiovascular events and efficacy between febuxostat and allopurinol use. Methods: A retrospective cohort study was conducted in hyperuricemic/gout patients admitted between 2013 to 2017. Study cohort (febuxostat/allopurinol/both) and control cohort (drugs other than febuxostat/allopurinol) were selected based on inclusion and exclusion criteria. The primary outcomes were various cardiovascular events which were compared and analysed using IBM SPSS version 20 and p<0.05 was considered statistically significant. Results: Mean reduction in level of serum urate after febuxostat and allopurinol use was 2.754 mg/dl [p=0.000; CI (95%) 2.209-3.299] and 1.137 mg/dl [p=0.000; CI (95%) 0.792-1.482], respectively. However, there was no statistically significant difference between the groups. The mean survival time for febuxostat was 37 months [CI (95%) 24-51], whereas for allopurinol was 107 months [CI (95%) 52-162] and for both users was 38 months [CI (95%) 20-56]. However, there was no significant difference among the groups (p=0.762). Cardiovascular events occurred in 4 (5.63%) patients in febuxostat group (n=71), 6 (6.45%) patients in allopurinol group (n=93) and 3 (30%) patients in group with both drugs (n=10). The number needed to harm (NNH) for febuxostat, allopurinol and with both drugs were 22, 19 and 3, respectively. Conclusion: Both febuxostat and allopurinol are significantly efficacious in reducing serum urate levels. Lesser mean survival time in febuxostat group indicates faster occurrence of cardiovascular events. On the other hand, incidence of cardiovascular events was more with allopurinol than febuxostat without statistical significance. We observed these cardiovascular adverse events of allopurinol and febuxostat are duration depended.

Alert fatigue is a complex problem that many health institutions face when using an electronic health record (EHR). The addition of user-filtered warnings (UFW) is a physicians’ proposed intervention at Inova Health System (IHS), a large 5-hospital health system in Northern Virginia, that allows prescribers to filter out specific drug-drug interactions and pregnancy and lactation medication alerts for a 30-day period. Introduction
- Infusion-related reactions (IRRs) are well-known adverse drug reactions of many biological agents and antineoplastic medications
- While typically mild-to-moderate in intensity, IRRs can be severe, with potentially life-threatening consequences requiring urgent interventions
- Appropriate use of pre-treatment medications, such as corticosteroids, antihistamines, intravenous fluids, and antiemetics reduces the incidence and severity of IRRs
