Although protocols exist, it appears there are no standardized treatment guidelines for practicing fertility experts in Nigeria. The aryl hydrocarbon receptor (AHR) is a ligand-activated signaling molecule which controls tumor growth and metastasis, T cell differentiation, and liver development. Expression levels of this receptor protein are sensitive to the cellular p23 protein levels in immortalized cancer cell lines. As little as 30% reduction of the p23 cellular content can suppress the AHR function. Here we reported that down-regulation of the p23 protein content in normal, untransformed human bronchial/tracheal epithelial cells to 48% of its content also suppresses the AHR protein levels to 54% of its content. Solid tumors possess biological features that are different from those in healthy tissues, which provides opportunities of anticancer treatment by nanomedicines. Due to the presence of the fenestrated tumor vasculatures, nanomedicines can selectively accumulate in tumor tissues by the enhanced permeability and retention (EPR) effect. The acidic pH in tumor interstitium (pH 6.0-7.0) also provides a promising mechanism to trigger the nanomedicines to promote the cellular uptake of cargo drugs. Polymers with mucoadhesive properties are universally used in the development of mucoadhesive drug delivery system. Their physicochemical properties as well as the mechanisms related to their adhesive actions draw great attention for the modification of mucoadhesive properties.

In this study, relationships between physicochemical properties of hydroxypropyl methylcellulose (HPMC) compacts and mucoadhesive performance were investigated. Different commercial grades of HPMC (K3, E3, E5, E50, K4M, E4M and K15M) were prepared into compacts, and their surface hydrophilicity and hydration behavior were characterized. Cancer is the second leading cause of death and is responsible for approximately 9.6 million deaths worldwide in 2018. Among all oncological diseases, lung cancer claims the highest mortality (male: 23.5%; female: 22%) and the second most new cases (male: 13%; female: 12%) in the US. Approximately 40% of newly diagnosed lung cancer patients are in the advanced stage IV, for which platinum-based chemotherapy is the first-line treatment, either by itself or in combination with surgery or radiotherapy.

The dissociative photoionization reactions of two small, oxygenated organics, namely 1,3-dioxolane and methyl vinyl ketone, were studied by photoelectron photoion coincidence (PEPICO) spectroscopy. Experiments involving 1,3-dioxolane were carried out in the photon energy range of 9.5‒13.5 eV. The statistical thermodynamics model shows that a total of six dissociation channels are involved in the formation of three fragment ions, namely C3H5O2+ (m/z 73), C2H5O+ (m/z 45) and C2H4O+ (m/z 44), with two channels contributing to the formation of each. Targeting DNA has the advantage over proteins for cancer remediation because of the fewer copies of the ligands required for the desired therapeutic effect. Traditionally, covalent DNA binders like alkylating agents have been used to induce genetic instability through the formation of DNA lesions and strand breaks, leading to cellular apoptosis. The primary drawback of this treatment is the non-specific binding that affects both cancerous and non-cancerous cells. The availability of quality pharmaceutical products impacts the lives of the global population on a daily basis. The importance of product availability and quality within pharmaceutical manufacturing requires that quality systems put into place are capable of accurately and robustly capturing the quality compliance, or lack thereof, of the various stages of production. This review intends to analyze current expert-recommended best practices within key elements of a pharmaceutical manufacturing quality control system and provide recommendations throughout. The quality elements discussed include Quality by Design method validation, necessary equipment determination, and equipment qualification, followed by documentation control and data flow.

แอสตาแซนทีนเป็นสารต้านออกซิเดชันที่มีฤทธิ์แรงซึ่งมีผลิตภัณฑ์จำหน่ายในรูปแบบเม็ดและแคปซูลสำหรับรับประทาน อย่างไรก็ตาม แอสตาแซนทีนมีข้อจำกัดด้านการดูดซึมเข้าสู่ทางเดินอาหาร จึงมีการนำระบบนาโนอิมัลชันมาใช้ในการพัฒนาเป็นระบบนำส่งแอสตาแซนทีน โดยทำการศึกษาผลของชนิดและความเข้มข้นของสารทำอิมัลชันต่อคุณสมบัติและความคงตัวของตำรับแอสตาแซนทีนนาโนอิมัลชันชนิดรับประทาน โดยใช้น้ำมันรำข้าว และใช้สารทำอิมัลชันเดี่ยว (โซเดียมเคซิเนต 1, 3, 5% หรือ พอลิซอร์เบต 80 4, 6, 8%) และใช้สารทำอิมัลชันผสมทั้งสองชนิด ผลการศึกษาพบว่า ขนาดหยดอนุภาคของตำรับนาโนอิมัลชันเปล่าที่ใช้สารทำอิมัลชันเดี่ยวโซเดียมเคซิเนตใหญ่กว่าตำรับที่ใช้สารทำอิมัลชันเดี่ยวพอลิซอร์เบต 80 และการเพิ่มความเข้มข้นของพอลิซอร์เบต 80 ส่งผลให้ขนาดหยดวัฏภาคภายในลดลงอย่างมีนัยสำคัญ (p<0.05) สำหรับตำรับที่ใช้สารทำอิมัลชันผสม การเพิ่มความเข้มข้นของสารทำอิมัลชัน ทำให้ขนาดหยดวัฏภาคภายในมีขนาดเล็กลงและมีค่าศักย์ไฟฟ้าซีต้าอยู่ระหว่างการใช้สารทำอิมัลชันเดี่ยว 2 ชนิด จากการศึกษาความคงตัวทางกายภาพภายใต้สภาวะเร่งร้อน-เย็น จำนวน 6 รอบ พบว่า ทุกตำรับที่ใช้สารทำอิมัลชันเดี่ยวและตำรับที่ใช้สารทำอิมัลชันผสมที่มีโซเดียมเคซิเนต 1% มีความคงตัวทางกายภาพที่ดี จากนั้น เลือกใช้สารทำอิมัลชันที่ความเข้มข้นน้อยที่สุดและให้ความคงตัวที่ดี นำมาเตรียมนาโนอิมัลชันที่บรรจุแอสตาแซนทีน โดยเลือกใช้สารทำอิมัลชันเดี่ยว 1% โซเดียมเคซิเนต, 4% พอลิซอร์เบต 80 และสารทำอิมัลชันผสมทั้งสองชนิดที่ความเข้มข้นเดียวกัน จากการศึกษาความคงตัวทางกายภาพและเคมี ที่อุณหภูมิ 5 °C และ 25 °C เป็นเวลา 100 วัน พบว่า ที่อุณหภูมิ 5 °C ตำรับแอสตาแซนทีนนาโนอิมัลชันมีความคงตัวทางเคมีดีกว่าที่อุณหภูมิ 25 °C และตำรับที่ใช้สารทำอิมัลชันผสมมีความคงตัวทางเคมีดีกว่าตำรับที่ใช้สารทำอิมัลชันเดี่ยวโซเดียมเคซิเนต และพอลิซอร์เบต 80 ตามลำดับ ผลการศึกษาที่ได้สามารถนำไปประยุกต์ใช้ในการพัฒนาระบบนำส่งแอสตาแซนทีนในรูปแบบนาโนอิมัลชันสำหรับรับประทานต่อไป

Cancer metastasis is the major cause of about 90% of cancer deaths. As epithelial-to-mesenchymal transition (EMT) is known for potentiating metastasis. In the present study aimed to elucidate the effect of ovalitenone on the suppression of EMT and metastasis-related behaviors, including cell movement and growth under detached conditions, and cancer stem cells (CSCs), of lung cancer cells. Ovalitenone, as a flavonoid was extracted from roots of Millettia erythrocalyx Gapnep., was the first reported to show the anti-migration activity of H460 and A549 lung cancer cells. However, information regarding to the molecular mechanisms underlying the apoptosis-inducing effects of CEP in CRC cells is still limited. The shifting of autophagy, a catabolic process for cell survival, toward death could be a potential mode of anti-cancer drug action. In addition, autophagic cell death (ACD) is an alternative death mechanism in resistant malignant cancer cells. This study, we demonstrated how PE5, a stilbene compound, exhibits potent ACD-promoting activity and apoptosis in lung cancer cells that may offer an opportunity for novel cancer treatment. It was found that PE5 exerted a higher toxic effect toward cancer cells compared with its effect on normal cells. Due to its several mechanisms of action, curcumin (CUR) has been employed as adjuvant therapy in treating psoriasis. Limited oral bioavailability was the problem for the development of CUR for therapeutic purposes. Therefore, many approaches, including the prodrug strategy, have been devised to address Cur's drawbacks. CUR was conjugated with mycophenolic acid (MPA) as a new prodrug in this research. Characterization of MPA-CUR structure was done by FT-IR, 1H-NMR, 13C-NMR, and MS. MPA-CUR showed water solubility at 0.73 μM and log P at 2.33, respectively. Phytochemical investigation of a methanol extracts prepared from Dendrobium pachyglossum and Dendrobium heterocarpum, Orchidaceae, led to the the isolation of thirteen compounds. One new bizbibenzyl compound and 6 known compounds including 4,5-dihydroxy-2,3-dimethoxy-9,10-phenanthrene, moscatilin, gigantol, 4,5,4'-trihydroxy-3,3'-dimethoxybibenzyl, dendrocandin T, and isovitexin were separated from D. pachyglossum. From D. heterocarpum, 6 compounds were isolated including amoenylin, methyl 3-(4-hydoxyphenyl) propionate, 3,4-dihydroxy-5,4'-dimethoxybibenzyl, dendrocandin B, dendrofalconerol A, and syringaresinol, respectively. The structure of isolated compounds were determined by analysis of spectroscopic data (NMR and HR-ESI-MS). เพื่อศึกษาปัจจัยต่าง ๆ ที่สัมพันธ์กับปฏิกิริยาเสมือนการแพ้เฉียบพลันต่อสารทึบรังสีที่มีไอโอดีนเป็นส่วนประกอบประเภทไม่แตกตัวเป็นประจุ และอุบัติการณ์ของอาการไม่พึงประสงค์แบบล่าช้า การศึกษาจากผลไปหาเหตุแบบเก็บข้อมูลไปข้างหน้า ในผู้ป่วยที่มารับการตรวจด้วยสารทึบรังสีฯ ที่ศูนย์ภาพวินิจฉัย โรงพยาบาลศิริราช ระหว่างพฤศจิกายน พ.ศ. 2562-มีนาคม พ.ศ. 2563 มีผู้ป่วย 434 ราย เข้าร่วมการศึกษา เป็นกลุ่มควบคุม 347 ราย และกลุ่มศึกษา 87 ราย จากผลการวิเคราะห์ด้วยสถิติวิเคราะห์พหุถดถอยลอจิสติก พบ ปัจจัยที่เพิ่มความเสี่ยงต่อการเกิดปฏิกิริยาเสมือนการแพ้เฉียบพลันอย่างมีนัยสำคัญทางสถิติ ได้แก่ อายุ≤60 ปี (p=0.032), ปริมาณสารทึบรังสีฯ≥100 มิลลิลิตร (p=0.037), อัตราเร็วในการบริหารสารทึบรังสีฯ≥5 มิลลิลิตรต่อวินาที (p=0.022) การที่ไม่เคยได้รับสารทึบรังสีฯ มาก่อน (p=0.036) การที่เคยมีผื่นลมพิษไม่ทราบสาเหตุ (p<0.001) และมีโรคหัวใจเป็นโรคร่วม (p=0.045) พบการเกิดอาการไม่พึงประสงค์แบบล่าช้าร้อยละ 14.1 จากกลุ่มควบคุม ร้อยละ 15.6 และกลุ่มศึกษา ร้อยละ 8 ดังนั้น ในขั้นตอนการคัดกรองความเสี่ยงจากการใช้สารทึบรังสีฯ ก่อนการตรวจเอกซเรย์คอมพิวเตอร์ ควรประเมินปัจจัยเสี่ยงต่าง ๆ ที่พบได้จากการศึกษานี้ ในผู้ป่วยที่มีปัจจัยเสี่ยงข้างต้น ควรมีการพิจารณาให้ได้รับสารทึบรังสีฯ ในปริมาณน้อยที่สุดและให้ด้วยอัตราเร็วที่ช้าที่สุดเท่าที่จะเป็นไปได้ เพื่อการตรวจวินิจฉัยที่ยังมีประสิทธิภาพและมีความปลอดภัยสูงสุด นอกจากนี้ ควรพิจารณาให้คำแนะนำแก่ผู้ป่วยทุกรายเกี่ยวกับอาการไม่พึงประสงค์แบบล่าช้า แม้ว่าผู้ป่วยจะเคยเกิดอาการเสมือนการแพ้เฉียบพลันมาก่อนหรือไม่ก็ตาม

ผลกระทบของการรักษาด้วยยาต้านรีโทรไวรัสต่ออุบัติการณ์โรคปอดอักเสบนิวโมซิสติสปฐมภูมิในผู้ใหญ่ติดเชื้อเอชไอวี, พิรุณ คำลี

การวิจัยนี้มีวัตถุประสงค์เพื่อเปรียบเทียบอุบัติการณ์การเกิดโรค PCP ปฐมภูมิระหว่างกลุ่มผู้ติดเชื้อเอชไอวีที่ได้รับและไม่ได้รับยาป้องกันโรค PCP ปฐมภูมิตามเกณฑ์เมื่อผู้ป่วยเริ่มรักษาด้วยต้านไวรัสที่มีประสิทธิภาพสูงในช่วงระยะเวลา 48 สัปดาห์ และรายงานอัตราการเกิดอาการไม่พึงประสงค์จากการใช้ยาป้องกันโรค PCP ปฐมภูมิ ใช้วิธีการศึกษารูปแบบ retrospective cohort study โดยทำการเก็บข้อมูลย้อนหลังจากเวชระเบียนผู้ป่วยนอกที่เข้ารับการรักษาการติดเชื้อเอชไอวี ณ โรงพยาบาลศิริราช ผลการศึกษา พบว่า มีกลุ่มตัวอย่างทั้งสิ้น 217 ราย มีค่ามัธยฐานของอายุ 39 ปี เป็นเพศชายร้อยละ 63.1 มีค่ามัธยฐานของระดับ CD4+ เริ่มต้น เท่ากับ 83 เซลล์ต่อไมโครลิตร และมีระยะเวลาติดตามประวัติในการรักษาผู้ป่วยรวมทั้งสิ้น 197.35 คน-ปี มีผู้ป่วยได้รับยาป้องกันโรค PCP ปฐมภูมิตามเกณฑ์ จำนวน 104 ราย (ร้อยละ 47.93) ลักษณะทั่วไประหว่างกลุ่มผู้ป่วยที่ได้รับและไม่ได้รับยาป้องกันโรค PCP ปฐมภูมิ มีค่ามัธยฐานของอายุและระดับ CD4+ เริ่มต้น เท่ากับ 41 ปี : 38 ปี และ 48 เซลล์ต่อไมโครลิตร : 133 เซลล์ต่อไมโครลิตร ตามลำดับ พบผู้ป่วย 1 รายในกลุ่มที่ไม่ได้รับยาป้องกันโรคดังกล่าวเป็นโรค PCP ปฐมภูมิ คิดเป็นอุบัติการณ์โดยรวม เท่ากับ 0.51 ต่อ 100 คน-ปี ผลการวิเคราะห์เปรียบเทียบโอกาสการปลอดโรค PCP ปฐมภูมิระหว่างผู้ป่วย 2 กลุ่มด้วยสถิติ Log-rank test เมื่อปรับค่าตามระดับ CD4+ พบว่า ไม่มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ (P-value = 0.133) และพบผู้ป่วยเกิดอาการไม่พึงประสงค์จากการใช้ยาป้องกัน PCP ปฐมภูมิ ร้อยละ 10.58 ของกลุ่มที่ได้รับยาป้อง PCP ปฐมภูมิ โดยส่วนใหญ่พบผู้ป่วยมีอาการข้างเคียงทางผิวหนังและมีอาการข้างเคียงในระดับรุนแรง คือ Stevens-Johnson syndrome …

วัตถุประสงค์: วัตถุประสงค์หลัก ประเมินคุณภาพชีวิตด้านการบำบัดด้วยยาและด้านสุขภาพในกลุ่มผู้ป่วยโรคมะเร็งที่ได้รับการบริบาลทางเภสัชกรรมเปรียบเทียบกับกลุ่มที่ได้รับการบริบาลตามปกติ วัตถุประสงค์รอง เปรียบเทียบปัญหาจากการใช้ยาและผลลัพธ์ทางคลินิกจากการแก้ไขปัญหาการใช้ยาในกลุ่มผู้ป่วยโรคมะเร็งที่ได้รับการบริบาลทางเภสัชกรรมกับกลุ่มที่ได้รับการบริบาลตามปกติ วิธีการวิจัย: การวิจัยเชิงทดลองแบบสุ่มที่มีกลุ่มควบคุมแบบปกปิดฝ่ายเดียว ศึกษาในผู้ป่วยโรคมะเร็งที่ได้รับยาเคมีบำบัด โรงพยาบาลสมุทรปราการ ระหว่างเดือนพฤศจิกายน 2563 ถึงเดือนกุมภาพันธ์ 2564 จำนวนทั้งสิ้น 237 คน สุ่มผู้ป่วยเป็น 2 กลุ่ม คือ กลุ่มที่ได้รับการบริการตามปกติของโรงพยาบาล และกลุ่มที่ได้รับการบริบาลทางเภสัชกรรม ประเมินคุณภาพชีวิตด้านการใช้ยาและด้านสุขภาพของผู้ป้วยด้วยเครื่องมือ PROMPT-QoL และ FACT-G ตามลำดับ ผลการวิจัย: กลุ่มที่ได้รับบริบาลทางเภสัชกรรมมีคะแนนคุณภาพชีวิตด้านการใช้ยาและด้านสุขภาพดีขึ้นอย่างมีนัยสำคัญทางสถิติเมื่อเทียบกับกลุ่มควบคุม (p < 0.001) มีผลลัพธ์ทางคลินิกของการแก้ไขปัญหาการใช้ยาดีกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p < 0.001) สรุปผลการวิจัย: ผลของการบริบาลทางเภสัชกรรมช่วยให้คุณภาพชีวิตด้านการใช้ยาและด้านสุขภาพของผู้ป่วยดีขึ้น โดยให้การบริบาลที่เน้นการดูแลผู้ป่วยเป็นศูนย์กลาง และอาศัยเครื่องมือ PROMPT-QoL ที่สามารถวัดคุณภาพชีวิตด้านการใช้ยาของผู้ป่วยได้โดยตรงมาวัดผลของการบริบาลทางเภสัชกรรมได้

โคเอนไซม์คิวเทน (CoQ10) เป็นที่ทราบกันดีว่าเป็นสารต้านอนุมูลอิสระที่มีประสิทธิภาพสูงใช้ในอุตสาหกรรมอาหารและเครื่องสำอาง โคเอนไซม์คิวเทนสามารถผลิตได้โดยการหมักแบคทีเรีย Methylobacterium organophilum NBRC 15689T และ Methylobacterium sp. LRY1-08 ด้วยวิธีการสกัดโคเอนไซม์คิวเทนจากเซลล์ การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลของวิธีการสกัด สายพันธุ์ Methylobacterium และปัจจัยทางกระบวนการหมัก ต่อปริมาณโคเอนไซม์คิวเทนที่ได้ การวิเคราะห์ปริมาณโคเอนไซม์คิวเทนทำโดยใช้เทคนิคโครมาโตรกราฟีของเหลวสมรรถนะสูง (HPLC) การศึกษาปัจจัยทางกระบวนการหมักแบคทีเรีย ได้แก่ ความแตกต่างของแหล่งคาร์บอนและแหล่งไนโตรเจนและระยะเวลาของการหมัก ผลการศึกษา พบว่า วิธีการทำให้เซลล์แตกก่อนโดยใช้เมทานอลและโซเดียมคลอไรด์ 0.3% (10:1 v/v) ที่มีส่วนประกอบของไตรตอนเอ็กซ์-100 1% ควบคู่กับการใช้คลื่นเสียงความถี่สูง แล้วสกัดต่อด้วยไอโซโพรพานอลและเฮกเซน (3:5) พบว่า ให้ปริมาณโคเอนไซม์คิวเทนสูงสุดในทั้งสองสายพันธุ์ โดย Methylobacterium organophilum NBRC 15689T ให้ปริมาณโคเอนไซม์คิวเทนสูงกว่า Methylobacterium sp. LRY1-08 ซึ่งสอดคล้องกับน้ำหนักเซลล์แห้งที่มากกว่า ผลการศึกษาแสดงให้เห็นว่าวิธีการสกัดนี้สามารถใช้เพื่อสกัดโคเอนไซม์คิวเทนจากเยื่อหุ้มเซลล์ของแบคทีเรียได้ และผลการศึกษาปัจจัยของกระบวนการหมัก พบว่า กลูโคสความเข้มข้น 10 กรัมต่อลิตร และสารสกัดยีสต์ความเข้มข้น 10 กรัมต่อลิตร เป็นแหล่งคาร์บอนและแหล่งไนโตรเจนที่เหมาะสมที่สุด โดยหมักเป็นเวลา 96 ชั่วโมง ส่งผลต่อปริมาณโคเอนไซม์คิวเทนที่ผลิตได้จากแบคทีเรีย Methylobacterium organophilum NBRC 15689T ซึ่งให้ค่า CoQ10 production เท่ากับ 2.7667 ± 0.26 mg/L ค่า Specific CoQ10 content เท่ากับ 0.7663 ± 0.07 mL/g ของน้ำหนักเซลล์แห้ง และน้ำหนักเซลล์แห้งเท่ากับ 3.62 ± 0.20 g/L
