Evaluation of surgical antibiotic prophylaxis at a large, tertiary medical center

Surgical site infections (SSI) are a significant cause of morbidity, prolonged hospitalization, and mortality. The cost of a single SSI is estimated to be upward of $25,000 and increases a patient’s length of hospital stay by ~10 days. Evaluating the utility of CYP2C19 genetic testing for P2Y12 inhibitor prescribing within an inpatient setting

The emergence of pharmacogenomics offers the opportunity to practice precision medicine across health care. In patients presenting with acute coronary syndrome (ACS) and undergoing percutaneous coronary intervention (PCI), dual antiplatelet therapy (DAPT) is routinely prescribed for 12 months. DAPT consists of a P2Y12 inhibitor (clopidogrel, ticagrelor, or prasugrel) plus aspirin. Of the P2Y12 inhibitors, clopidogrel is uniquely impacted by CYP2C19 status. The author ran Linear Probability Model and Logistic Model regressions on data from the 2017 Medical Expenditures Panel Survey to determine demographics of opioid prescriptions. The dependent variable was opioid prescription received (Y/N) and independent variables included age, education level, relative income, race, insurance status, etc. Results of the regressions showed that people with public or private insurance are more likely to be prescribed an opioid, that the problem mainly affects white people, and that as educational attainment increases, likelihood of opioid prescription decreases.

Introduction: Recently, research has revealed the presence of cannabinoid receptor 2 (CB2R) in the central nervous system (CNS); since then, the receptor has emerged as a promising therapeutic target for treating multiple neurodegenerative diseases (NDD) such as Alzheimer’s (AD), Parkinson’s (PD), Human Immunodeficiency Virus (HIV) and Multiple Sclerosis (MS). Focus: This study focused on pharmacy students’ perception of the value of writing goals and how this perception changes as students progress in the program.

Methods: Annually, all students in the pharmacy program are required to reflect on the previous year’s goals and develop new goals. Following this assignment, a survey was sent out to a random sample (n=162) of students. Based on the feedback, the response was classified as indicating “value” or “no value”.

Expanding pharmacist-driven antimicrobial stewardship efforts in the emergency department (ED) can improve antibiotic management for both admitted and discharged patients. We piloted a pharmacist-driven culture and rapid diagnostic technology (RDT) follow-up program in patients discharged from the ED. This was a single-center, pre- and post-implementation, cohort study examining the impact of a pharmacist-driven culture/RDT follow-up program in the ED. Adult patients discharged from the ED with subsequent positive cultures and/or RDT during the pre- (21 August 2018–18 November 2018) and post-implementation (19 November 2018–15 February 2019) periods were screened for inclusion. Death by drug poisoning is the leading cause of death due to injury in the state of Utah (UDOH, 2014). From 2013-2015, Utah ranked seventh in the nation for drug overdose deaths, the majority of which were opioid-related (CDC, 2018a). In 2017, the age-adjusted mortality rate from drug overdose in Utah was 15.5 per 100,000, putting the state above the national average of 14.9 per 100,000 (CDC, 2018b). The neuropathological outcomes of a viral infection in the brain are often age-dependent. Infection of the immature CNS can lead to impairment of neural/stem progenitor cell (NSPCs) activity. NSPCs are multi-potent progenitor cells of the CNS and are responsible for populating the brain with neurons, astrocytes, and oligodendrocytes during development. Viral infections may alter NSPC activity via two mechanisms; directly by infection of the NSPC or indirectly due to the anti-viral immune response. Recent reports indicate that 7% of pregnant mothers in North America use cannabis. This is concerning given that in utero exposure to Δ9-tetrahydrocannabinol (Δ9-THC), the main psychoactive component in cannabis, causes fetal growth restriction and may alter replication and survival of pancreatic β-cells in the offspring. Accordingly, we hypothesized that maternal exposure to Δ9-THC during pregnancy would impair postnatal glucometabolic health of offspring. To test this hypothesis, pregnant Wistar rats were treated with daily intraperitoneal injections of either 3 mg/kg Δ9-THC or vehicle from gestational day 6 to birth. Seventy-three Staphylococcus spp. isolates were recovered from bacterial samples including poultry, foods, sewage, and soil. Antibiotic susceptibility tests were conducted on all environmental isolates for Methicillin and Vancomycin. The results showed that 39 Staphylococcus spp. isolates (53.42%) were resistant to Methicillin, while 26 isolates (35.61%) were resistant to Vancomycin. The minimum inhibitory concentration (MIC) range of Vancomycin was found to be between 2-512 µg/mL. Plasmid DNA extraction from Methicillin and Vancomycin-resistant isolates revealed that some isolates had one plasmid band. The Clinical Laboratory Standards Institute lowered the fluoroquinolone minimum inhibitory concentration (MIC) susceptibility breakpoints for and glucose non-fermenting Gram-negative bacilli in January 2019. This retrospective cohort study describes the impact of this reappraisal on ciprofloxacin susceptibility overall and in patients with risk factors for antimicrobial resistance. Gram-negative bloodstream isolates collected from hospitalized adults at Prisma Health-Midlands hospitals in South Carolina, USA, from January 2010 to December 2014 were included. Matched pairs mean difference (MD) with 95% confidence intervals (CI) were calculated to examine the change in ciprofloxacin susceptibility after MIC breakpoint reappraisal. © 2020 The Author(s). Postoperative shivering and cold hypersensitivity are major side effects of acute and chronic opioid treatments respectively. TRPM8 is a cold and menthol-sensitive channel found in a subset of dorsal root ganglion (DRG) nociceptors. Deletion or inhibition of the TRPM8 channel was found to prevent the cold hyperalgesia induced by chronic administration of morphine. Here, we examined the mechanisms by which morphine was able to promote cold hypersensitivity in DRG neurons and transfected HEK cells. Drug delivery to triple negative breast cancer (TNBC) remains a formidable challenge. Given the lack of expressions of estrogen (ER), progesterone (PR) and human epidermal growth factor receptor (HER2), there is no specific prognostic marker for targeting this aggressive cancer. In spite of rapid advancement in breast cancer with agents such as Trastuzumab in HER2 positive, chemotherapy with cytotoxic agents remains the mainstay treatment for TNBC. Although these cytotoxic agents are potent in managing the tumor progression, yet they pose adverse effects on healthy tissues.
