When a woman discovers that she is pregnant, she begins a process of internal work to develop her mother role. This process has been outlined in the literature for the delivery of a healthy full-term baby, however little is known about the process for mothers of medically fragile babies. A threatened pregnancy and subsequent delivery of a medically fragile baby involves a different process of internal work by the mother to prepare for her role. Examining Generational Differences Across Organizational Factors That Relate To Turnover, Kimberly Asuncion

Turnover continues to pose a problem for all organizations across industries. This study examines the complex nature of turnover, by examining the relationship of turnover intentions with perceptions of distributive justice, procedural justice, growth opportunities, and recognition across age groups. Age groups will be used as a proxy for generational cohort membership. Results of the study confirm previous research that generational differences do exist; however, those differences are fairly small. Perceptions of distributive justice, procedural justice, growth opportunities, and recognition were found to be significant predictors of turnover intentions regardless of the age group.

Emergency departments are a major source of hospital admissions with patients at risk for pressure ulcer development. Yet, there is a paucity of literature in two key areas: emergency RNs' role in PU prevention and their knowledge, skills, attitudes and intentions toward implementation of PU prevention guidelines. Manuscript 1 was an integrative review that found multiple factors--knowledge, attitudes, and environmental--that affect nurses' use of PU prevention. Manuscript 2 was an integrative review that found the readiness for change construct as a precursor to implementing an organizational or individual change. Background: Prolonged sexual abstinence after childbirth is a socio-cultural practice with health implications, and is described in several African countries, including Tanzania. This study explored discourses on prolonged postpartum sexual abstinence in relation to family health after childbirth in low-income suburbs of Dar es Salaam, Tanzania.
Methods: Data for the discourse analysis were collected through focus group discussions with first-time mothers and fathers and their support people in Ilala, Dar es Salaam, Tanzania.

Background: Lifestyle risk factors like smoking, nutrition, alcohol consumption, and physical inactivity (SNAP) are the main behavioural risk factors for chronic disease. Primary health care is an appropriate setting to address these risk factors in individuals. Generalist community health nurses (GCHNs) are uniquely placed to provide lifestyle interventions as they see clients in their homes over a period of time. The aim of the paper is to examine the impact of a service-level intervention on the risk factor management practices of GCHNs.

Objective: This paper describes the socio-demographic and health-related characteristics of people with chronic disease attending an inter-professional student-assisted clinic in regional Queensland.
Design: A retrospective review of data collected during the first 10 months of operation of the clinic was conducted.
Setting, participants and outcome measures: Data was collected on up to 378 patients during an intake appointment at the Capricornia Allied Health Partner-ship (CAHP) community-based clinic and compared with normative reference groups where available. Background: While the detrimental effects of smoking among HIV positive patients have been well documented, there is a paucity of data regarding cigarette smoking prevalence among these patients in South Africa.
Objectives: To establish the frequency, demographics, and knowledge of harmful effects and of smoking cessation strategies among HIV-positive patients in Johannesburg, South Africa.
Methods: A prospective cross-sectional survey, using a structured questionnaire interview, of HIV-positive patients attending the HIV Clinic at the Charlotte Maxeke Johannesburg Academic Hospital between 1 July 2011 and 31 October 2011.

The practice of obtaining blood as part of the placement of a new peripheral venous access device (p-VAD) is a frequent practice in the emergency department (ED). Of the concerns related to this practice is the possibility of laboratory specimen rejection due to p-VAD catheter size, use of the wrong collection device, and the absence of a standardized collection process. Adolescent athletes with type 1 diabetes (T1DM) face unique challenges when compared to peers with and without diabetes. Continuous subcutaneous insulin infusion (CSII) provides a method of insulin delivery that can enhance flexibility in insulin regimens and lifestyle that may be especially appealing to the adolescent athlete. No studies have explored the impact of athletics in this population. This descriptive qualitative study explored and described the experiences of adolescent athletes using CSII as their primary insulin delivery method, with a focus on athletic participation and performance.

Background:
Dog walking is a strategy for increasing population levels of physical activity (PA). Numerous cross-sectional studies of the relationship between dog ownership and PA have been conducted. The purpose was to review studies comparing PA of dog owners (DO) to non-dog owners (NDO), summarize the prevalence of dog walking, and provide recommendations for research.
Methods:
A review of published studies (1990-2010) examining DO and NDO PA and the prevalence of dog walking was conducted (N=29). Studies estimating the relationship between dog ownership and PA were grouped to create a point-estimate using meta-analysis.

Purpose: Dog owners represent 40% of the population, a promising audience to increase population levels of physical activity. The purpose of this study was to develop and test the psychometric properties of a new instrument to assess social cognitive theory (SCT) constructs related to dog walking. Methods: Dog owners (N=431) completed the Dogs and WalkinG Survey (DAWGS). Survey items assessed dog walking behaviors, and self-efficacy, social support, outcome expectations, and outcome expectancies for dog walking. Background: Papanicoloau (Pap) smear screening has helped to reduce cervical cancer rates significantly through the detection of premalignant cells (Bray et al, 2005). Uptake among women who use alcohol and other drugs (AOD) is known to be low (Chau et al, 2002) so they are at increased risk of being under-represented in the adequately screened population. AOD-using women experience disproportionately increased morbidity and mortality from cervical dysplasia and cancer (Nogara et al, 2013). Pregnancy may provide the midwife an opportunity to offer this vital screening test.

Critical-care nurses often look after three or more critically-ill patients during a shift. The workload and emotional stress can lead to disharmony between the nurse’s body, mind and spirit. Nurses with a high emotional intelligence have less emotional exhaustion and psychosomatic symptoms; they enjoy better emotional health; gain more satisfaction from their actions (both at work and at home); and have improved relationships with colleagues at work. Objectives: The study aims to describe the characteristics of patients presenting to an ED with a fall and evaluate multidisciplinary Care Coordination Team (CCT) referrals on patient outcomes.
Methods: A single-centred retrospective analysis of electronic data at an adult tertiary hospital was performed using data from 2004 to 2009 of presentations for patients aged 65 years or over with a fall. The primary outcome measure was representation to hospital within 30 days, comparing patients referred to CCT and those not referred. Secondary outcomes were differences in demographic characteristics, mode of arrival, triage score and readmission.

The purpose of this phenomenological study is to uncover the meaning of transition to adult centered care as experienced by Young Adults with Cerebral Palsy (YA-CP) through the research question: What are the lived experiences of young adults with cerebral palsy transitioning from pediatric to adult healthcare?

In the United States, 40% of the contemporary nursing workforce is comprised of women of childbearing age, 65% of whom are employed full-time. Hence, the likelihood of pregnancy occurring for this population at some point in their employment is high. A holistic exploration of how nurses integrate pregnancy and full-time employment has been lacking. The purpose of this research was to explore how primiparous nurses managed pregnancy and full-time employment. Objective(s): The purpose of this study was to explore the effectiveness of CHW/ nursing student teams in promoting secondary protection and improving access to care for residents of three urban underserved neighborhoods. The study also sought to measure CHW and resident satisfaction of such a program. Design and Sample: Quasi-experimental and nonexperimental designs were used for the study. Convenience samples consisted of residents who participated in the CHW program during 2005–2006, CHWs and residents who participated in the CHW program during 2005–2007 and a systematic random sample of residents across the three neighborhoods. อัตมโนทัศน์ทางเพศเป็นปัจจัยด้านจิต-สังคมที่มีอิทธิพลต่อพฤติกรรมทางเพศของบุคคล เนื่องจากเป็นความรู้สึกที่มีต่อพฤติกรรมทางเพศที่เกี่ยวกับตนเองและมีอิทธิพลต่อการสร้างความสัมพันธ์ กับผู้อื่นรวมทั้งการรับรู้เกี่ยวกับคุณลักษณะความสามารถของตนต่อพฤติกรรมทางเพศ การรับรู้อัตมโนทัศน์ ทางเพศ (Sexual self-concept) ประกอบด้วย 4 ด้าน คือ 1) ความพึงพอใจในการมีเพศสัมพันธ์ (Sexual satisfaction) 2) ความวิตกกังวลในการมีเพศสัมพันธ์ (Sexual anxiety) 3) การรับรู้สมรรถนะแห่ง ตนในการมีเพศสัมพันธ์ (Perceived sexual self-efficacy) และ 4) ภาวะซึมเศร้าในการมีเพศสัมพันธ์ (Sexual depression) แนวคิดดังกล่าวได้ประยุกต์ใช้กับผู้ป่วยโรคหัวใจวายและโรคกล้ามเนื้อหัวใจตายพบว่า อัตมโนทัศน์ทางเพศมีความสัมพันธ์ทางบวกกับพฤติกรรมทางเพศ เมื่อพิจารณารายด้าน พบว่าความพึงพอใจในการมีเพศสัมพันธ์และการรับรู้สมรรถนะแห่งตนในการมีเพศสัมพันธ์มีความสัมพันธ์ทางบวกกับ พฤติกรรมทางเพศของผู้ป่วยโรคหัวใจวายและโรคกล้ามเนื้อหัวใจตาย ขณะที่ความวิตกกังวลในการมีเพศสัมพันธ์และภาวะซึมเศร้าในการมีเพศสัมพันธ์มีความสัมพันธ์ทางลบกับพฤติกรรมทางเพศของผู้ป่วย โรคหัวใจวายและโรคกล้ามเนื้อหัวใจตาย

วัตถุประสงค์: เพื่อศึกษาคุณภาพชีวิตของผู้ป่วยภาวะหัวใจล้มเหลว และทำนายความรู้สึกไม่แน่นอนใน ความเจ็บป่วย ความแตกฉานด้านสุขภาพ และการสนับสนุนทางสังคมที่มีต่อคุณภาพชีวิตของผู้ป่วย ภาวะหัวใจล้มเหลว \nรูปแบบการวิจัย: การวิจัยเชิงความสัมพันธ์พยากรณ์ \nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้ป่วยภาวะหัวใจล้มเหลวอายุมากกว่าหรือเท่ากับ 18 ปี ที่มา ติดตามรับการรักษา ณ คลินิกโรคหัวใจ โรงพยาบาลสรรพสิทธิประสงค์ จังหวัดอุบลราชธานี จำนวน 85 ราย เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสอบถามข้อมูลส่วนบุคคล แบบสอบถามความ รู้สึกไม่แน่นอนในความเจ็บป่วยฉบับชุมชน แบบประเมินความแตกฉานด้านสุขภาพขั้นพื้นฐานสำหรับ ผู้ป่วยภาวะหัวใจล้มเหลว แบบวัดการช่วยเหลือทางสังคมแบบพหุมิติ และแบบสอบถามวิถีชีวิตของ ผู้ป่วยภาวะหัวใจล้มเหลว ผ่านการตรวจสอบความตรงตามเนื้อหาและวิเคราะห์ความเที่ยงแล้ว มีค่า ความเที่ยงมากกว่า 0.80 วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา สัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน \nและการวิเคราะห์ถดถอยแบบเข้าพร้อมกัน \nผลการวิจัย: \n1. ผู้ป่วยภาวะหัวใจล้มเหลวมีคุณภาพชีวิตอยู่ในระดับปานกลางร้อยละ 54.1 \n2. ผู้ป่วยภาวะหัวใจล้มเหลวมีความแตกฉานด้านสุขภาพอยู่ในระดับไม่เพียงพอร้อยละ 47.1\n3. ความรู้สึกไม่แน่นอนในความเจ็บป่วยเพียงตัวแปรเดียวที่สามารถทํานายคุณภาพชีวิตของ ผู้ป่วยภาวะหัวใจล้มเหลวได้อย่างมีนัยสำคัญทางสถิติ (B = 651, p < .001) \n4. ความรู้สึกไม่แน่นอนในความเจ็บป่วย ความแตกฉานด้านสุขภาพ การสนับสนุนทางสังคม สามารถร่วมกันทำนายคุณภาพชีวิตของผู้ป่วยภาวะหัวใจล้มเหลวได้ร้อยละ 46.8 อย่างมีนัยสําคัญทางสถิติ AWANG (R2 = .468, p < .001) \nสรุปและข้อเสนอแนะ: \nการศึกษาครั้งนี้พบว่าความรู้สึกไม่แน่นอนในความเจ็บป่วยสามารถทำนายคุณภาพ ชีวิตได้สูง จึงควรนำผลการวิจัยที่ได้มาพัฒนาโปรแกรมการดูแลผู้ป่วย และมีการทดสอบประสิทธิผลของ โปรแกรม เพื่อลดความรู้สึกไม่แน่นอนในความเจ็บป่วยที่จะส่งผลให้ผู้ป่วยภาวะหัวใจล้มเหลวมีคุณภาพชีวิต ที่ดีต่อไป

Cardiovascular disease (CVD) remains among the leading causes of death in the United States despite widespread knowledge about risk factors as well as effective primary prevention strategies. Risk perception is a complex phenomenon that plays an important role in how persons view disease and ultimately how they make health behavior choices. Surveillance of ventilator-associated pneumonia (VAP) has been the common outcome measurement used for internal and external benchmarking for mechanically ventilated patients; and although not a clinical definition, it is commonly used as an outcome measurement for research studies. Criteria in the VAP definition include both subjective and objective components, leading to questions of validity. In addition, recent legislation has mandated the public reporting of healthcare-associated infections, including VAP, in many states. Nurse education programs are implementing standardized assessments without evaluating their effectiveness. Graduates of associate degree nursing programs continue to be unsuccessful with licensure examinations, despite standardized testing and stronger admission criteria. This problem is also prevalent for LPN-to-RN education programs due to a lack of research on this group of graduates, who by all accounts should be successful in nursing education based on their work experience and prior nursing education.
