Negotiating The Healthy Self: A Theory Of Health Behavior In Older Adults, Barbara Schwitz White

Researchers have identified discrete behaviors of older persons to maintain or improve health and prevent or treat illness. Few have developed theories of health behavior specific to this population. The purpose of this study was to develop a substantive theory to explain how health and illness are integrated into and influence behaviors in the daily lives of community-residing older persons. Using grounded theory design and analysis, 26 participant observations involving 186 individual encounters, and seven semistructured [sic] one-on-one interviews were conducted at two senior centers in urban southern California, between 1992 and 1994.

Endothelial dysfunction has been found prior to any signs of atherosclerotic plaque formation in patients with a variety of risk factors for atherosclerosis. Elevated serum cholesterol has repeatedly been associated with endothelial dysfunction as demonstrated by impaired vasoreactivity to vasodilating stimuli.
The aim of this research was to determine whether short term lowering of serum cholesterol levels by life style changes or by cholesterol lowering medication could reverse or reduce the endothelial dysfunction. The brachial artery diameter was measured by high-resolution ultrasound at rest and during reactive hyperemia pre and postintervention. The effect of the Edgewise Bioprogressive Herbst appliance, as described by Dr. Terry Dischinger, was investigated on 24 Class II, Division 1 malocclusions. Mean pretreatment age of this sample was 11.2 months. All patients were treated by Dischinger with a Bioprogressive Herbst appliance. Lateral cephalographs taken before, after Herbst appliance therapy, and after edgewise treatment were traced, then digitized into Quick Ceph™. Measurements were computed by the computer software. The following results were found: 1) Average treatment time was 9 months. 2) An overcorrected Class I relationship was achieved. 3) Class II malocclusion was corrected by 4.5 mm skeletal alteration …

The accuracy of the computer video imaging treatment prediction was investigated using Quick Ceph Image.™ Thirty one surgical mandibular advancement/genioplasty orthognathic cases were examined to determine if the soft tissue profile prediction was representative of the actual result. All patients were over the age of 18. Lateral cephalographs and slides were taken immediately pre-surgically, and six months post-surgically. The slides were duplicated into 4X6 photographs. The cephalographs were digitized and the photographs captured by video on the Quick Ceph Image software program. A prototype multimedia medical database has been developed to provide image and textual data for thoracic oncology patients undergoing treatment of advanced malignancies. The database integrates image data from the hospital pieture archiving and communication system with textual reports from the radiology information system, alphanumeric data contained in the hospital information system, and other electronic medical data. The database presents information in a timeline format and also contains visualization programs that permit the user to view and annotate radiographic measurements in tabular or graphic form. The database provides an efficient and intuitive display of the changing status of oncology patients. …

A secondary TRAM flap was prefabricated in a rat model using the rectus muscle. The right rectus muscle was elevated as an inferiorly pedicled muscle flap in one experimental group (n = 6) and as a superiorly pedicled muscle flap in a second group (n = 5) and then sandwiched between a silicone sheet and the abdominal skin. Two weeks later, the abdominal skin and attached rectus muscle were elevated over the silicone sheet as a secondary TRAM flap. Removal of extracellular Cl- has been shown to suppress light-evoked voltage responses of ON bipolar and horizontal cells, but not photoreceptors or OFF bipolar cells, in the amphibian retina. A substantial amount of experimental evidence has demonstrated that the photoreceptor transmitter, L-glutamate, activates cation, not Cl-, channels in these cells. The mechanism for Cl-free effects was therefore reexamined in a superfused retinal slice preparation from the mudpuppy (Necturus maculosus) using whole-cell voltage and current clamp techniques. In a Cl-free medium, light-evoked currents were maintained in rod and cone photoreceptors but suppressed in horizontal, ON bipolar, and OFF bipolar cells. Changes …

BACKGROUND: Platelet glycoprotein IIb/IIIa is a membrane receptor for fibrinogen and von Willebrand factor, and it has an important role in platelet aggregation. It is known to be involved in the pathogenesis of acute coronary syndromes. Previously, we found a high frequency of a particular polymorphism, PlA2, of the gene encoding glycoprotein IIIa in kindreds with a high prevalence of premature myocardial infarction.
METHODS: To investigate the relation between the PlA2 polymorphism and acute coronary syndromes, we conducted a case-control study of 71 case patients with myocardial infarction or unstable angina and 68 inpatient controls without known heart disease. The …

The ability of Toxoplasma gondii to evade the host immune response during primary infection in humans is poorly understood. In murine toxoplasmosis, infected spleen macrophages release soluble factors that mediate a transient immunosuppression, which may allow the parasite to become established. When an enriched population of human monocytes from seronegative individuals was incubated with toxoplasmas in vitro, soluble factors that mediated market suppression of mitogen-induced lymphocyte DNA synthesis were released. Irradiated tachyzoites that do not undergo replication were sufficient stimuli for near-maximal soluble factor release. Up to 50% of the soluble factor-mediated suppression is attributable to a gamma interferon (IFN-gamma)-dependent …

The purpose of this study was to explore whether procedures used to establish target heart rates (THRs) for running are applicable to front crawl swimming. Eight male and 22 female fitness swimmers from Western Michigan University participated in this study. Their exercise durations under three experimental conditions were compared. The conditions were: (a) Condition 1, a treadmill run at an intensity equal to a THR of 85% of heart rate reserve (HRR); (b) Condition 2, a front crawl swim at an intensity equal to 85% of HRR; and (c) Condition 3, a front crawl swim at an intensity equal to …

Background: The nutritional quality of soybeans (Glycine max) is compromised by a relative deficiency of methionine in the protein fraction of the seeds. To improve the nutritional quality, methionine-rich 2S albumin from the Brazil nut (Bertholletia excelsa) has been introduced into transgenic soybeans. Since the Brazil nut is a known allergenic food, we assessed the allergenicity of the 2S albumin.

Normal brain tissue in an abnonnal configuration, occurring at a site away from the cranial cavity, is temied an Extranasal Glioma. Since its first description in 1852 by Reid2, only 140 patients with this entity have been reported. Heterotopic brain tissue when present in a confined area may cause obstmction, pressure and pain. Its occurrence in the nasopharyngeal region is relatively rare and only 19 cases have been reported so far2, the majority presenting with respiratory distress in the early neonatal period. ABSTRACT
Background Acute changes in cerebral function after elective coronary bypass surgery are a difficult clinical problem. We carried out a multicenter study to determine the incidence and predictors of — and the use of resources associated with — perioperative adverse neurologic events, including cerebral injury.
Methods In a prospective study, we evaluated 2108 patients from 24 U.S. institutions for two general categories of neurologic outcome: type I (focal injury, or stupor or coma at discharge) and type II (deterioration in intellectual function, memory deficit, or seizures).
Results Adverse cerebral outcomes occurred in 129 patients (6.1 percent). A total of …

BACKGROUND
Insulin resistance and increased ovarian cytochrome P450c17α activity are both features of the polycystic ovary syndrome. P450c17α, which is involved in androgen biosynthesis, has both 17α-hydroxylase and 17,20-lyase activities. Increased activity of this enzyme results in exaggerated conversion of progesterone to 17α-hydroxyprogesterone in response to stimulation by gonadotropin. We hypothesized that hyperinsulinemia stimulates ovarian P450c17α activity.
METHODS
We measured serum steroid concentrations during fasting and the response of serum 17α-hydroxyprogesterone to leuprolide, a gonadotropin-releasing hormone agonist, and performed oral glucose-tolerance tests before and after oral administration of either metformin (500 mg three times daily) or placebo for four to …

The resorptive capacity of osteoclasts in vitro has been used as an indicator of bone resorption. However, the kinetics of osteoclast development and senescence is not well understood. It has been noted that mononuclear precursors migrate to calcified substrate and after multi-nucleation become competent bone resorbing cells. Osteoclasts, once finished a wave of resorption, become senescent. In order to determine the survival characteristics of osteoclast populations involved in calcified tissue destruction, we have investigated the time course of the recruitment to dentin and apoptosis of osteoclasts. Mineralization and subsequent calculus formation is a common complication of biofilm infections. In the urinary tract, these infected calculi often arise from infections by urease-producing bacteria. Ammonia, liberated by bacterial urease activity, increases urine pH, resulting in the precipitation of Ca and Mg as carbonateapatite {Ca10(PO4,CO3)6(OH,CO3)2} and struvite (NH4MgP04·6H2O). These minerals become entrapped in the organic matrix which surrounds the infecting organisms and ultimately grow into mature calculi. Weight bearing (WB) is an important factor influencing bone remodelling. The present study evaluates the effects of weight bearing and non-weight bearing (S) (achieved by tail suspension and hindlimb elevation) on the healing of a fibular osteotomy in adult male rats. The use of backscattered electron (BSE) imaging as a tool for the qualitative measurement of mineral content in bone has been well documented. The challenge still remains to develop BSE imaging as a tool for quantitative mineral content analysis in bone. The limiting factor has been the ability to standardize the BSE signal within and between laboratories. Energy dispersive x-ray spectrometry (EDX) has been proposed as a method to standardize the BSE operating environment.
