A 48-year-old man with relapsing multiple myeloma (MM) presented with serum monoclonal protein (IgG l) of 5.93 g/dl. He was initially treated with four cycles of VAD (vincristine, adriamycin and dexamethasone) but due to deterioration of his renal functions with disease relapse, he was placed on hemodialysis. A combined treatment with thalidomide at a daily dose of 200 mg and 4-d cycles of dexamethasone was started for relapsing MM. Thalidomide dose was increased to 400 mg after two weeks. Early Onset Stroke Due To Fibromuscular Dysplasia In A Child, Selçuk Çomoğlu, Yavuz Gürer

Turkish Journal of Medical Sciences

Fibromuscular dysplasia (FMD) is a rare, idiopathic and nonatheromatous disease. Definitive diagnosis is made only by angiography, which is characterized by a "string of beads" appearance. In this article, we present the rare entity of early-onset thromboembolic stroke due to FMD in a case who was found to have a carotid artery lesion in angiography. Antiaggregant therapy was given (300 mg/oral/day) and partial improvement was observed on follow-up.

We report in detail the preparation of two substituted terephthalic acids: 2‐sulfomethylterephthalic acid (1) and 2‐phosphonoterephthalic acid (2). Efficient, short syntheses have been developed for both compounds. They are potentially useful monomers for construction of acid‐pendant polymer chains.

We are presenting the preparation, characterization and density functional theory (DFT) studies {B3LYP/6-31+G(d)) of several reiated classes of N-nitrosocarbamates and N-nitrosoureas. The iong-range goal is the design and preparation of compounds, which would undergo photochemical or hydrolytic decomposition, to yield stabilized cyclic cations that can serve as alkylating agents at various nucleophilic centers, including DNA bases.

Folic acid is an essential vitamin utilized in the one-carbon metabolism pathway for the synthesis of purine and thymidine nucleotides, which are necessary for cell growth and proliferation. As a result, the enzymes that participate in the metabolism of folic acid have been good targets for cancer chemotherapy. Folylpoly-γ-glutamate synthetase (FPGS) is an enzyme in the folate metabolism pathway that catalyzes the addition of glutamic acid to the naturally occurring folates, thereby allowing the retention of folate cofactors in cells. There is significant sequence homology and anatomical co-distribution between cannabinoid (CB1) and sphingosine-1-phosphate (S1P) receptors in the CNS, but potential functional relationships between these lysolipid receptors have not been examined. Therefore, to investigate possible relationships between these two systems at the level of G-protein activation, agonist-stimulated [35S]GTPγS binding and autoradiography were conducted. Autoradiographic studies were first performed to localize receptor-mediated G-protein activation in mouse brain. Coronal brain slices were processed for stimulation of [35S]GTPγS binding using the synthetic cannabinoid agonist WIN 55,212-2 (WIN) or SIP. The exponential integrate-and-fire (EIF) model was introduced by Fourcaud-Trocme et al. (2003) as an extension of the standard leaky integrate-and-fire model (LIF). Here, the nonlinearity in the EIF model’s temporal response to square-wave inputs is investigated. Comparing the time course of onset and offset responses revealed that offset responses have a steeper initial slope, but a slower approach to equilibrium. A linear systems analysis performed for these square-wave inputs indicates that at frequencies above ~40 Hz, gain was slightly smaller for square-wave inputs, but phase did not change significantly relative to simulations in which the corresponding sinusoids were presented in …

BACKGROUND: Protein acetylation is increasingly recognized as an important mechanism regulating a variety of cellular functions. Several human protein acetyltransferases have been characterized, most of them catalyzing epsilon-acetylation of histones and transcription factors. We recently described the human protein acetyltransferase hARD1 (human Arrest Defective 1). hARD1 interacts with NATH (N-Acetyl Transferase Human) forming a complex expressing protein N-terminal alpha-acetylation activity. RESULTS: We here describe a human protein, hARD2, with 81 % sequence identity to hARD1. The gene encoding hARD2 most likely originates from a eutherian mammal specific retrotransposition event. hARD2 mRNA and protein are expressed in several human cell lines. …

Polymorphisms in the genes encoding cytochrome p450 (CYP) and thiopurine S-methyl transferase (TPMT) enzymes catalyze the metabolic reactions of several drugs. These polymorphisms might be responsible for adverse drug reactions. Turkish population data for these genes still needs to be elucidated. We aimed to detect the allele frequencies of thiopurine S-methyl transferase (TPMT), cytochrome p4503A4*1B (CYP3A4*1B) and cytochrome p4503A4*3 (CYP3A5*3) gene variants in the Turkish population. The encapsulated follicular variant of papillary thyroid carcinoma constitutes the most common source of consultation material in thyroid pathology. We present our five consecutive cases to illustrate this issue. The diagnosis of the encapsulated follicular variant of papillary thyroid carcinoma is considerably controversial. These tumors are encapsulated neoplasms and have the nuclear features of papillary thyroid carcinoma. Although the criteria were described for the diagnosis, a considerable subjectivity is present among the pathologists. Therefore, the categories of "well differentiated carcinoma" and "well differentiated tumor of uncertain malignant potential" were included in the diagnostic spectrum of these neoplasms. We prospectively studied the characteristics of 401 patients with first-ever-in-a lifetime stroke who have been admitted consecutively into stroke care unit. During the evaluation of distribution according to age, patients were divided into 7 age groups with 10-year intervals. It was determined that ischemic stroke was mostly seen in the age group 71-80 (32.2%) and the age group 61-70(28.2%). The major risk factor of ischemic stroke was hypertension, followed by diabetes mellitus, ischemic heart disease, smoking and hypercholesterolemia. The aim of this study is to evaluate factors influencing utilization of health services by women aged 15 years and older having any health symptoms within the previous 15 days in a city center located in the eastern part of Turkey. In this cross-sectional study, subjects (1,000 households) were sampled from the "city-information-system" based on household satellite photographs of the city. Pleural effusions are very common in general population. There are more than 50 reasons for the etiology. There is limited data on the prevalence of pleural effusions in a population of pregnant women. We prospectively screened 45 pregnant women between May and October 2006. All the pregnant women underwent thoracic ultrasonography during their routine check. Five out of 45 subjects (11%) had minimal and bilateral pleural effusion. None of the subjects was symptomatic. Aim: Owing to the widespread use of antibiotic drugs in both human and animals, we thought it would be important to determine the effect of these on lactoperoxidase (LPO; E.C. 1.11.1.7) activity during lactation. The in vitro effects of gentamicin sulfate and ampicillin sodium drugs on bovine LPO enzyme activity in breast-milk were investigated. 3,4-Methylenedioxymethamphetamine (MDMA), also known as "ecstasy", is a synthetic, psychoactive drug chemically similar to the stimulant methamphetamine and the hallucinogen mescaline. It can cause cognitive, physical, psychological and neurological side effects. Ecstasy-induced acute renal failure, liver damage and brain edema are reported in the literature. We report an MDMA-induced toxic hepatitis case which was thought to have viral origin.

The goal of this study was to elucidate the molecular mechanism by which type I IFN inhibits assembly and release of HIV-1 virions. Our study revealed that the IFN-induced ubiquitin-like protein ISG15 mimics the IFN effect and inhibits release of HIV-1 virions without having any effect on the synthesis of HIV-1 proteins in the cells. ISG15 expression specifically inhibited ubiquitination of Gag and Tsg101 and disrupted the interaction of the Gag L domain with Tsg101, but conjugation of ISG15 to Gag or Tsg101 was not detected. Porphyromonas gingivalis, an important periodontal pathogen, is an effective colonizer of oral tissues. The organism successfully invades, multiplies in, and survives for extended periods in primary gingival epithelial cells (GECs). It is unknown whether P. gingivalis resides in the cytoplasm of infected cells throughout the infection or can spread to adjacent cells over time. We developed a technique based on flow cytofluorometry and fluorescence microscopy to study propagation of the organism at different stages of infection of GECs. The genetic diversity among globally circulating human immunodeficiency virus type 1 (HIV-1) strains is a serious challenge for HIV-1 vaccine design. We have generated a synthetic group M consensus env gene (CON6) for induction of cross-subtype immune responses and report here a comparative study of T-cell responses to this and natural strain env immunogens in a murine model. Three different strains of mice were immunized with CON6 as well as subtype A, B, or C env immunogens, using a DNA prime-recombinant vaccinia virus boost strategy. FoxP3 is a lineage specific marker for regulatory T-cells (Treg). We have generated FoxP3 knock-in (KI) mice by introducing a bicistronic GFP reporter into the endogenous FoxP3 locus, allowing us to faithfully track T-reg in vivo. Recently, we have also generated a MOG 35-55/IAb-tetramer. The combination of these two technologies enables us to study the in vivo behavior of myelin specific T-reg and effector T-cells (T-eff) during EAE. Upon immunization with MOG 35-55, we identified a population of MOG-tetramerreactive T-reg in the peripheral lymphoid compartment. T-cell immunoglobulin and mucin (TIM) family Members are differentially expressed on Th1 and Th2 cells. Polymorphisms of TIM-1 have been associated with susceptibility to asthma; however, its role in regulating autoimmunity has not been studied. Here, we have used an agonistic antiTIM-1 antibody (Ab, Clone 3B3) which has previously been shown to costimulate T-cell activation and expansion, to analyze the role of TIM-1 in the development and regulation of experimental autoimmune encephalomyelitis (EAE). Several questions need to be addressed to evaluate whether Tannerella forsythia is to be considered a periodontal pathogen. T. forsythia has been detected in periodontal health and disease, so could it be a pathogen? The species was not detected in many studies despite finding other putative pathogens, so could it be important in pathogenicity? The challenges of working with T. forsythia include its fastidious and anaerobic growth requirements for cultural detection. Thus, studies associating T. forsythia with periodontal and other oral infections have used noncultural approaches (immunoassays and DNA-based assays) in addition to cultural approaches. We feel …
