Background: Cervical lymphadenitis (LAD) and deep neck space abscesses (DNSA) are common pediatric infections caused by similar bacteria. We sought to determine differences in presentation, diagnosis, and treatment between LAD and DNSA to identify antimicrobial and diagnostic stewardship opportunities.
Methods: Charts were obtained using ICD9/10 codes for retropharyngeal and parapharyngeal abscessed (DNSA), and LAD between 1/1/10-12/31/20 from two pediatric centers. 1981 charts were identified. Charts were excluded if the diagnosis was not a bacterial infection (e.g. Kawasaki disease), if the LAD was not in the neck, or if caused by less common bacteria (e.g. tuberculosis). The SARS-CoV-2 virus, responsible for the covid-19 pandemic, has had profound effects on countries worldwide. As the pandemic progressed, clinical and patient data continued to mount. A subset of symptoms named “Long Covid Syndrome” persisted in patients after recovering from infection. One commonly reported but understudied symptom was a deficit in memory function. Although commonly reported, prevalence of ‘brain fog’ has yet to be characterized using patient data. Using Rowan Medicine electronic patient data, we were able to collect information on patients before and after the emergence of the coronavirus. Cavitary pneumonia can be caused by several different mechanisms. Such causes include suppurative necrosis (pyogenic lung abscess), caseous necrosis (tuberculosis), ischemic necrosis (pulmonary infarction) cystic dilatation of lung structures (ball value obstruction and Pneumocystis pneumonia) as well as malignant processes. Causes are subject to both environmental factors and host susceptibility. The most common cause of infectious process is tuberculosis with its propensity to cause extensive caseous necrosis. The potential for secondary infection after an individual develops a cavitary pneumonia is also great as it provides a shelter for various pathogens the flourish in an environment otherwise usually well-guarded by host …

SARS-CoV-2, which causes coronavirus disease 2019 (COVID-19), was first discovered in December 2019 in Wuhan, China when adults began presenting with severe pneumonia of an unknown cause. SARS-CoV-2 can cause a wide range of clinical manifestations. SARS-CoV-2 can penetrate the olfactory mucosa and may enter the brain through the cribriform plate along the olfactory tract, through vagal or trigeminal pathways, or pass through the blood-brain barrier. Once inside the brain, levels of inflammatory cytokines are increased and can lead to altered learning, memory, neuroplasticity, hallucinations, nightmares, cognitive and attention deficits, new-onset anxiety and depression, and psychosis. Since the start of the COVID-19 pandemic and the launch of the vaccine, healthcare workers have been encouraging people to get vaccinated to prevent the spread of the virus. However, there is a significant number of people who are vaccine hesitant, or uncertain about receiving the COVID-19 vaccine and its booster. Vaccine hesitancy represents a state of opportunity to make positive change; therefore, it is crucial for healthcare workers to identify those who are in this population. We present a case of necrotizing fasciitis initially presenting as septic hypoxic pneumonia, and discuss imaging modalities and diagnostic evaluation. Necrotizing soft tissue infection (NSTI) is a rapidly progressive surgical emergency with a mortality rate of 30%. In approximately 80% of cases, it is introduced through a break in the skin. It can also occur after any invasive procedure or surgery. It is most commonly associated with skin flora including staph and strep, though polymicrobial infections are most common. Background:
The COVID-19 pandemic caused contributed to significant increases in ICU nurse turnover. To restore the ICU nursing workforce and rebuild a healthy work environment, evidence-based strategies are needed to provide social support and ongoing clinical skills development for large cohorts of ICU residents and fellows.
Purpose:
The purpose of the Clinical Coach program was to increase retention of ICU residents and fellows and foster a healthy work environment at a ministry in Southern California.

A 36-year-old Mexican immigrant male presented to an outside hospital after being found down in a sugarcane field. On initial presentation he was febrile to 104.7, hypotensive with a GCS of 3. He soon had an episode of emesis, with new onset seizure activity. He was refractory to fluid resuscitation, started on pressors and intubated before being transferred to UMC. Blood cultures drawn and returned positive for Brucella. While being one of the most common zoonotic diseases worldwide, Brucella infections are reported 100 to 200 times annually in the United States. BACKGROUND: Severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) Omicron variant B.1.1.529 and subvariant BA.2 were first detected in late 2021. The Omicron variant quickly became the most widespread in the United States due to its increased transmissibility compared to previous SARS-CoV-2 variants. The BA.2 sub-lineage has been shown to be particularly communicable and was the most prevalent strain in the United States as of March 2022. It is unknown if this increased infectivity of BA.2 is due to depressed antibody quantities following vaccination against wild-type virus or previous infection with other strains of SARSCoV-2. Background
New graduate nurses (NGNs) in acute care settings are particularly vulnerable to the stressors inherent in nursing, due to the immense amount of experiential learning required at the bedside during the first year of practice. The high amount of stress experienced by NGNs is reflected in high turnover rates. Nurses with less than two years of experience accounted for almost half (47.5%) of all nurse turnovers in 2019. Furthermore, the COVID-19 pandemic has caused much physical and emotional turmoil in the nursing profession due to the increased workload and emotional distress. Background:
Hospital chaplains are trained to provide spiritual support to inpatients, family members, and support persons across the continuum of care. Patients receiving a chaplain visit during their hospital stay report higher satisfaction, quality of life, and spiritual well-being. Chaplain consultation during hospitalization is related to higher rates of hospice enrollment and lower rates of in-hospital deaths. Additionally, evidence suggests nurses who are employed in hospitals with staff chaplains report lower stress and compassion fatigue based on frequency of interactions with chaplains during worked shifts. Background:
Occupational burnout, a symptom of chronic work-related stress, can lead to negative health outcomes such as anxiety, depression, diabetes, insomnia, and increased risk for cardiovascular disease. Burnout also augments negative occupational risks such as increased sick days, decreased productivity, disability claims, and turnover. Nurses working in the hospital setting report high levels of work-related stress and burnout related to patient acuity levels and short staffing, and rates have further worsened in the context of the COVID-19 pandemic. Background: Throughout the COVID-19 pandemic, Operating Room (OR) nurses were redeployed to patient care units during times of maximal surge capacity in hospitals caring for patients with the novel coronavirus. Hospital bed and staffing needs caused hospitals to cancel elective surgeries, allowing for conservation of bed space and nurses to accommodate the patient influx. Despite the highly specialized nature and skill set of OR nursing, OR nurses were expected to function in traditional patient care roles, where levels of self-assessed competence vary based on experience and training. Background: Registered nurses (RN) working in acute-care hospitals during the Covid-19 pandemic reported unprecedented stress. Walking between 7000-10,000 steps each day offers positive benefits such as improved mood, reduced stress, and reduction in overall mortality risk. Additionally, the American College of Sports Medicine’s (ACSM) Physical Activity Guidelines recommends adults should engage in 75 -150 minutes per week of moderate to intense activity for health benefits. We report the case of a 38-year-old male physician who was vaccinated with the Pfizer vaccine. He experienced arm pain and swelling on the vaccinated arm as well as systemic symptoms, including fever, chills, headache and weakness. The systemic symptoms improved after the use of oral prednisone. A review of the literature on the COVID vaccine is presented.

Acute sinusitis in the pediatric population is a common presentation in the emergency department. Most cases are viral and can be managed conservatively and will resolve spontaneously in 7-10 days. Even in cases of bacterial sinusitis, patients can typically be discharged with oral antibiotics and close follow up with the pediatrician.
There is however a small subset of patients who will develop a rare lethal complication, an intracranial empyema. While the incidence in the developed world has decreased as antibiotics become more available, it is still estimated to occur in up to 5% of severe cases requiring hospitalization (1) . …

Improving Antibiotic Durations For Skin And Soft Tissue Infections In Pediatric Urgent Care Clinics, Megan Hamner

Problem Statement/Question: Although national guidelines recommend 5-7 days of oral antibiotics for most skin and soft tissue infections (SSTIs), in 2019, CMH urgent care clinics (UCCs) prescribed > 7-day courses for 42% of patients diagnosed with SSTIs. Prolonged antibiotic courses result in increased patient burden, cost and adverse events which could be prevented by prescribing shorter courses.
Background/Project Intent (Aim Statement): To increase the percentage of patients receiving 5- 7 days of oral antibiotics for SSTIs from 58% to 75% by December 31st, 2021, in CMH UCCs.
