With the increase in use of lingual appliances a more accurate assessment is needed of exactly where the brackets are being placed. This study was undertaken in order to see if it makes any difference who places the brackets. Three groups of participants were selected. One, the commercial laboratory which offers a bracket placement service. Two, clinicians experienced with the lingual appliance, and three, clinicians with no clinical experience with the appliance.

This article reviews a solitary, well-defined tumor of the floor of the mouth. The tumor presented as a raised, elevated, sessile mass that was freely movable and soft to. the touch. The case illustrates the importance of performing a thorough head and neck examination, and formulating a differential diagnosis if any abnormality is detected. The clinical features, histopathology and responsibility of the dental hygienist in performing a thorough head and neck examination is reviewed.

Double-Blind Comparison Of Rectally Administered Diazepam To Placebo For Pediatric Sedation: The Cardiovascular Response, Catherine M. Flaitz, Arthur J. Nowak, M. John Hicks

The sedative and cardiovascular effects of rectally administered diazepam (0.6 mg/kg) were compared to placebo in uncooperative children who required sedation during dental treatment. Twelve healthy preschool children, who required amalgam restorations, were treated during two standardized restorative appointments in a double-blind, crossover study. Blood pressure and pulse were obtained during four specified intervals during the appointment. The behavior of the children during the treatment visits was videotaped and later statistically analyzed using a kinesics/vocalization instrument. Behavioral ratings of cooperation were significantly improved during the treatment visit following diazepam. All interfering bodily movements, patient vocalizations and operator commands for the …

รายงานผู้ป่วย : ผู้ป่วยไลเคน พลานัส ในช่องปากกับโรคเบาหวาน, อารีย์ เจนกิตติวงศ์

บทความนี้เป็นรายงานผู้ป่วย ไลเคน พลานัส ในช่องปากที่ได้รับการรักษาและควบคุม อาการของโรคไว้ดีแล้ว ต่อมาเมื่อระดับน้ำตาลในเลือดของผู้ป่วยสูงกว่าปกติ ปรากฏว่ารอย โรคในช่องปากกำเริบขึ้น ขยายขอบเขตไปยังบริเวณต่าง ๆ ในช่องปากและมีอาการรุนแรง การรักษาโดยให้สเตียรอยด์เฉพาะที่ร่วมกับการควบคุมโรคเบาหวาน ปรากฏว่า เมื่อสามารถ ลดระดับน้ำตาลในเลือดให้อยู่ในสภาวะปกติสามารถควบคุมอาการของโรคได้ดี จึงน่าที่จะมี ความสัมพันธ์ระหว่าง ไลเคน พลานัสในช่องปากกับโรคเบาหวาน ในแง่ที่โรคเบาหวานส่งเสริม ให้ อาการของไลเคน พลานัส รุนแรงขึ้น

สภาวะอนามัยช่องปากของเด็กอายุ 6-12 ปี จากครอบครัวที่มีระดับเศรษฐกิจต่ำในเขตกรุงเทพมหานคร, ทรงธรรม สุคนธาภิรมย์ ณ พัทลุง, ยุทธนา ปัญญางาม

ดัชนีอนามัยช่องปาก (oral hygiene index) ของเด็กนักเรียนระดับประถมศึกษา อายุ 6-12 ปี ที่สุ่มมาจากโรงเรียนในสังกัดกรุงเทพมหานคร 24 แห่ง จากทุกเขตการปกครอง จำนวน 1,508 คน มีค่าเฉลี่ยเป็น 2.1 ซึ่งจัดอยู่ในระดับพอใช้ ค่าดัชนีอนามัยช่องปากของ เด็กเหล่านี้ถูกกำหนดโดยอิทธิพลของดัชนีแผ่นคราบฟัน (debris index) ซึ่งมีค่าเฉลี่ย 1.9 มากกว่าจะเป็นผลจากอิทธิพลของดัชนีหินน้ำลาย (calculus index) ซึ่งมีค่าเฉลี่ย 0.2 ดังนั้น จึงเห็นได้ว่าการเลือกใช้วิธีแปรงฟันที่เหมาะสมและให้ประสิทธิภาพสูงในการกำจัดแผ่นคราบฟัน น่าจะทำให้สภาวะอนามัยช่องปากของเด็กเหล่านี้เปลี่ยนไปอยู่ในระดับดีได้ อย่างไรก็ดี พบว่าความแตกต่างระหว่างเพศจะมีอิทธิพลต่อดัชนีแผ่นคราบฟันและดัชนีอนามัยช่องปากอย่างมีนัย สำคัญทางสถิติ (p < 0.01 และ p < 0.1) โดยกลุ่มเด็กผู้หญิงจะมีค่าเฉลี่ยของดัชนีอนามัย ช่องปากและดัชนีแผ่นคราบฟันเท่ากับ 2.06 และ 1.84 ตามลำดับ ในขณะที่กลุ่มเด็กผู้ชาย จะมีค่าเฉลี่ยของดัชนีทั้งสองเท่ากับ 2.13 และ 1.95 ตามลำดับ ขณะเดียวกันก็พบว่าเด็กกลุ่ม อายุน้อยจะมีดัชนีแผ่นคราบฟันและดัชนีหินน้ำลายต่ำ และเพิ่มขึ้นเมื่ออายุมากขึ้น แต่ความ แตกต่างนี้ไม่มีนัยสำคัญทางสถิติ นอกจากนี้เมื่อทำการทดสอบทางสถิติโดยคำนวณหาค่า สัมประสิทธิ์สหสัมพันธ์ระหว่างดัชนีอนามัยช่องปากกับอัตราฟันผุ ถอน อุด พบว่าค่าที่ได้ต่ำ มาก อย่างไรก็ดีกลุ่มที่มีสภาวะอนามัยช่องปากระดับดีจะพบมีอัตราฟันผุ ถอน อุด ต่ำกว่า กลุ่มที่มีสภาวะอนามัยช่องปากระดับเลว ทั้งในฟันแท้และฟันน้ำนม

การตรวจหาเชื้อแคนดิดา ในช่องปากมีหลายวิธีซึ่งให้ผลแตกต่างกัน วิธีการตรวจที่ ให้ผลถูกต้องเชื่อถือได้ จะเป็นสิ่งที่ช่วยในด้านการให้การวินิจฉัยโรคที่ถูกต้องตลอดจนการ วางแผนการรักษาทั้งโรคในช่องปาก และโรคทางระบบของร่างกาย วิธีเพาะเลี้ยงแบบอิมพรินท์ เป็นวิธีการตรวจหาเชื้อแคนดิดาในช่องปากแบบใหม่ ที่ง่ายและสะดวก และให้ผลดี ถูกต้อง เชื่อถือได้มากที่สุดและมีความไวต่อการตรวจพบเชื้อสูงที่สุด เมื่อเปรียบเทียบกับวิธีการตรวจแบบอื่น ๆ เหมาะสำหรับใช้ตรวจหาเชื้อแคนดิดาทั้งในแง่ของการวินิจฉัยโรคในคนที่มีแผล ติดเชื้อในช่องปาก และในคนที่มีโรคทางระบบของร่างกายที่มีจำนวนเชื้อแคนดิดาในช่องปาก สูงรวมทั้งที่มีแนวโน้มของการเพิ่มสูงขึ้นของเชื้อแคนดิดาในช่องปาก ตลอดจนแม้ในคนที่มี สุขภาพในช่องปากปกติ ก็สามารถที่จะตรวจพบเชื้อแคนดิดา ได้โดยวิธีนี้

การทดสอบการออกฤทธิ์ของนอร์อะเดรนาลีน (noradrenaline) ในยาชาลิโดเคน (Lidocaine) ใช้ปลากัดลูกหม้อเพศผู้จำนวน 44 ตัว พบว่าระยะเวลาการออกฤทธิ์ของนอร์-อะเดรนาลีนในยาชาที่เก็บไว้ในตู้เย็น และยาชาประเภทเดียวกันนี้ที่เก็บไว้ในห้องปรับอากาศ ยาวกว่ายาชาที่เก็บไว้ ณ อุณหภูมิห้องอย่างมีนัยสำคัญทางสถิติ (p < 0.01) เนื่องจากนอร์-อะเดรนาลีนเป็นสารบีบหลอดเลือดที่มีอิทธิพลต่อการออกฤทธิ์ของยาชา คือจำนวนสารนี้ลดลง จะทําให้ยาชาหมดฤทธิ์เร็ว ดังนั้น ควรเก็บยาชาที่มีนอร์อะเดรนาลีนไว้ในตู้เย็น หรือห้องปรับ อากาศ เพื่อป้องกันการทำลายนอร์อะเดรนาลีน ซึ่งจะส่งผลไปถึงการออกฤทธิ์ของยาชาด้วย

คนไข้ที่รับการบำบัดรักษาทางทันตกรรมจัดฟัน ถ้าไม่ดูแลความสะอาดในช่องปาก ให้เพียงพอ อาจได้รับผลข้างเคียงที่ไม่ต้องการคือ เกิดการละลายสูญเสียแร่ธาตุของผิวเคลือบ ฟัน ซึ่งก่อให้เกิดการผุของฟันได้ ผลของการบำบัดรักษาจะไม่ดีเท่าที่ควร หรือทำให้เวลาของ การบำบัดรักษานานขึ้น การพิจารณาเลือกใช้สารประกอบฟลูออไรด์ในรูปแบบต่าง ๆ ที่เหมาะสม เพื่อเสริมสร้างความทนทานต่อการละลายผิวเคลือบฟันแก่คนไข้เหล่านั้น จะช่วยลดผลข้างเคียงที่ไม่ต้องการนี้ได้ โดยเฉพาะคนไข้อายุน้อยที่ฟันแท้เพิ่งโผล่ขึ้นสู่ช่องปาก และการสะสมของเกลือแร่ต่าง ๆ ที่ผิวเคลือบฟันยังไม่สมบูรณ์

รายงานผู้ป่วย : กระดูกโคโรนอยด์ไฮเปอร์เพลเซีย, สิทธิชัย ทัดศรี

การเจริญเติบโตที่ผิดปกติของกระดูกโคโรนอยด์ ไม่ค่อยปรากฏบ่อยนัก ไม่เหมือน กระดูกคอนไดล์ (condyle) ซึ่งพบได้บ่อยกว่า ดังนั้นการวินิจฉัยขั้นต้นมักผิดเพราะคิดว่าเป็น ที่กระดูกคอนไดล์ ได้รายงานผู้ป่วย 1 รายที่มีอาการอ้าปากได้แคบลงและขัดขวางการเคลื่อน ไหวของขากรรไกรล่าง จากประวัติ การตรวจร่างกายและภาพรังสีแสดงถึงการขยายใหญ่ของ กระดูกโคโรนอยด์ ผลการตรวจทางพยาธิวิทยาพบว่าเป็นไฮเปอร์เพลเซียของกระดูกโคโรนอยด์ การรักษาโดยการทำศัลยกรรมเอากระดูกโคโรนอยด์ออก และเข้าทำศัลยกรรมทางช่องปากจะ เป็นวิธีที่ง่าย หลีกเลี่ยงการเกิดแผลเป็นบนใบหน้า อันตรายน้อย และได้ผลดีที่สุด

รายงานนี้ แสดงผลของการเปรียบเทียบคุณภาพของภาพรังสีในตำแหน่งเดียวกันบน ฟิล์มโกดัก (Kodak film) กับฟิล์มอินสแตนท์ (Instant film) ในเรื่องความดำของภาพ (Density) การตัดกันของสีขาวและสีดำ (Contrast) ความชัดแจ๋ว (Sharpness) รายละเอียด (Detail) และความแน่นอน (Definition) ความบิดเบือนของภาพ (Distortion) โดยทำ การถ่ายภาพรังสีด้วยวิธีแบ่งครึ่งมุม (Bisecting the angle technic) ผู้ป่วยที่มารับการรักษา ที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย 40 คน อายุระหว่าง 12 ถึง 50 ปี ผลปรากฏว่า เมื่อใช้เครื่องมือวัดความดํา (Densitometer) หาค่าความดํา (Density) และ การตัดกันของสีขาวและสีดํา พบว่า แตกต่างกันอย่างไม่มีนัยสําคัญ ส่วนการอ่านฟิล์มด้วยตา พบว่า ความชัดแจ๋วแตกต่างกันอย่างมีนัยสําคัญ และความบิดเบือนแตกต่างกันอย่างไม่มีนัย สําคัญทางสถิติทั้งหมดที่ ๕ α 0.05, t = 2.447

The purpose of this investigation was to determine if a significant difference exists between dental health attitudes of children who are exposed to a dental health fair and children who are not exposed to a dental health fair. All consenting students, in grades four through six . from two Hampton public schools, completed a Dental Health Questionnaire designed to assess attitudes toward dental health. One day after the experimental group participated in the dental health fair, both groups were post-tested using the same questionnaire. The purpose of this study is to evaluate the relative corrosion rates of 316-L stainless steel cast metal, cold worked, and powdered metal brackets. This is done using potentiodynamic anodic polarization to measure the voltage difference between the samples and a reference electrode. Corrosion layers effect current flow and by plotting how the current responds to an applied voltage it is possible to determine the corrosion rate of a sample.
