This literature review focuses on the physiological development of obesity due to the consumption of diets rich in fats and the resulting pathophysiological development of hypertension. This review aims to identify causes of obesity and hypertension as well as current treatments to propose future areas of study. The literature review analyzed a wide variety of literature sources on obesity and hypertension published between the years 1995 and 2025. The review identifies high-fat diets as a cause of obesity and the resulting health effects of elevated leptin, insulin, and nervous system activity as contributors to the development of hypertension. International Journal of Exercise Science 18(6): 659-671, 2025. Firefighters (FFs) and law enforcement officers (LEOs) have heightened cardiovascular disease (CVD), with data suggesting that ≈45% of on-duty FF fatalities are related to CVD, while LEOs have a 1.7 times higher CVD prevalence than the general public. This study compared CVD risk biomarkers, fitness, and body composition between FFs and LEOs. Ninety-eight career, structural male FFs (age = 35.1±9.6 yrs; weight = 94.3±15.4 kg; height = 178.4±13.2 cm) and seventy-three career LEOs (age = 41.4±9.0 yrs; weight = 92.3±16.8 kg; height = 179.6±8.1 cm) from local departments were studied. Obesity is a condition that affects over 40% of American adults and contributes to numerous preventable health issues. This condition is caused by increased adipose tissue, which forms when stem cells transform into adipocytes. Mesenchymal stem cells, including human adipose-derived stem cells (hASC), are multipotent with the ability to differentiate into adipocytes, osteocytes, or chondrocytes. Many factors affect this transformation, including the peptide growth hormone-releasing hormone (GHRH). Nesfatin-1, a posttranslational product of the protein encoded by the nucleobindin 2 gene (NUCB2), was functionally identified as an appetite regulatory molecule in rat hypothalamic nuclei. In the years following the discovery, those findings have been corroborated and expanded upon, and we now know that nesfatin-1 is expressed throughout peripheral tissues and exerts physiological effects beyond feeding control. Literature indicates that adipose tissue is one of the peripheral sources of NUCB2/nesfatin-1, and in this setting, it has anti-inflammatory effects that have recently been implicated in regulating chronic inflammation associated with diet-induced obesity. The rising rates of obesity worldwide have increased the incidence of cardiovascular disease (CVD), making it the number one cause of death. Higher plasma bilirubin levels have been shown to prevent metabolic dysfunction and CVD. However, reducing levels leads to deleterious outcomes, possibly due to reduced bilirubin half-life that escalates the production of its catabolized product, urobilinogen, produced by gut bacteria and naturally oxidized to urobilin. Recent findings suggest that the involvement of the microbiome catabolism of bilirubin to urobilin and its absorption via the hepatic portal vein contributes to CVD, suggesting a liver-gut axis involvement. Diet-induced obesity and adipose tissue (AT) inflammation that ensues sets the stage for systemic metabolic inflammation, disrupted insulin signaling, and eventual type 2 diabetes (T2D) development. At the center of this pathology lies a disruption in endogenous inflammation resolution pathways characterized by an imbalance in lipid mediator production (greater abundance of pro-inflammatory to pro-resolving lipids). Additionally, obese states are characterized by adrenergic deficiency, especially in response to physical activity. RATIONALE: Serum amyloid A (SAA) is bound to high-density lipoproteins (HDL) in blood. Although SAA is increased in the blood of patients with asthma, it is not known whether this modifies asthma severity.

OBJECTIVE: We sought to define the clinical characteristics of patients with asthma who have high SAA levels and assess whether HDL from SAA-high patients with asthma is proinflammatory.

METHODS: SAA levels in serum from subjects with and without asthma were quantified by ELISA. HDLs isolated from subjects with asthma and high SAA levels were used to stimulate human monocytes and were intravenously administered to BALB/c mice.

Obesity is a multifactorial disease with many comorbidities leading to multiorgan dysfunction. Current studies point towards dysregulation of platelet signaling as a driver for increased risk of cardiovascular disease and stroke. The exact mechanisms remain elusive due to the complex interplay of factors involved, warranting the need to further understand how platelets are affected. Our preliminary data evaluating changes in platelet and coagulation parameters from bariatric patients showed that obesity did not affect platelet count or clotting time as demonstrated in INR values. Recently, a collaboration between Regeneron Pharmaceuticals and the Schwartzman-Garcia labs at New York Medical College published an exome sequencing study of individuals across the United Kingdom, United States, and Mexico which concluded that individuals possessing non-functioning, truncated mutations to the orphan g protein coupled receptor (GPCR), GPR75, had lower BMI and 54% reduced likelihood of obesity. Bilirubin levels in obese humans and rodents have been shown to be lower than in their lean counterparts. Some studies have proposed that the glucuronyl UGT1A1 enzyme that clears bilirubin from the blood increases in the liver with obesity. UGT1A1 clearance of bilirubin allows more conjugated bilirubin to enter the intestine, where it is catabolized into urobilin, which can be then absorbed via the hepatic portal vein. We hypothesized that when bilirubin levels are decreased, the urobilin increases in the plasma of obese humans, as compared to lean humans. Within the human body, there are two types of adipose tissue: white adipose tissue (WAT) and brown adipose tissue (BAT). WAT is beneficial for insulation whereas BAT has thermogenic capacity. BAT activation increases energy consumption via heat generation. Increased energy expenditure is associated with improved glucose and lipid metabolism. Therefore, BAT activation research has primarily been aimed at its potential use in the treatment of obesity, diabetes, and other metabolic disorders. In this literature review, BAT's thermogenic mechanisms, techniques of activation, potential therapeutic targets, and future research topics are explored.

The exposome encompasses the concept that an individual’s health is a combination of genetic, environmental, and occupational exposures. A worker’s exposomal profile could potentially influence their susceptibility to hazardous occupational exposures. Over 35% of adults in the United States have metabolic dysfunction (MetDys), yet interactions between MetDys and hazardous occupational exposures are generally unknown. MetDys is associated with HFWD-consumption and obesity and induces a chronic low grade systemic inflammation and alters serum adipokine profiles. While obesity can often be treated using diet and exercise, there are complex genetic interactions driving obesity that don’t always respond to such changes. One important research focus for addressing genetic factors is activating brown adipose tissue which increases thermogenesis, the process of burning stored fats to generate heat. Cold exposure is a known way to activate brown adipose tissue through b3-adrenergic signaling. In the present study, we utilized rats genetically selected for high capacity running (HCR) and low capacity running (LCR) to determine pathways in the liver that aerobic exercise modifies to control plasma bilirubin. The HCR rats, compared to the LCR, exhibited significantly higher levels of plasma bilirubin and the hepatic enzyme that produces it, biliverdin reductase-A (BVRA). Obesity and psychological stress are major risk factors for cardiovascular disease and have a positive correlation with vascular dysfunction. This functional deficit has been linked to a pro-inflammatory milieu, exacerbated by an upregulation of pro-inflammatory cytokines. Febuxostat, a xanthine oxidase inhibitor, has been shown to rescue vascular impairment in these models. The purpose of this study was to examine the individual and combined effects of obesity and chronic stress on neuroimmune cell populations, and assess if febuxostat is able mediate immune alterations.

Nineteen overweight or obese females completed three 10-minute experimental trials including a self-selected pace shallow water walking trial, a matched heart rate response land walking trial, and a self-selected pace land walking trial. Energy expenditure (kcal·min-1)was computed from expired gases assessed via indirect calorimetry. Results showed energy expenditure was lower (p= 0.046) during shallow water walking (6.46 ± 1.38 kcal·min-1) compared to matched heart rate response land walking trial (7.26 ± 1.29 kcal·min-1), with no significant difference in between shallow water and self-selected pace land walking (6.92 ± 1.61 kcal·min-1). …
