Nearly 50% of women experience back pain and other lower extremity pain during pregnancy, with many reporting lasting pain postpartum. Pregnant women experience changes to their pelvis and lower extremities that do not always return to pre-pregnancy baseline. Not much is known of the lingering effects of pregnancy related asymmetry and its relationship to pain. Therefore, the purpose of this study was to assess asymmetries of the pelvis and lower extremities to determine whether malalignment is related to areas of pain at the low back, hip, thigh, knee, leg, and foot/ankle. Workers with low back pain (LBP) may benefit from wearing a lumbar belt (LB), but the biomechanical and psychological mechanisms involved are not fully understood. Two types of flexible LB (extensible and non-extensible) were compared to a control condition (no LB) regarding pain-related (pain, fear of pain and catastrophizing) and biomechanical (range of motion – ROM) outcomes related to two tasks: maximal trunk flexion-extension and manual material handling. Healthy controls and participants with LBP were tested. During both tasks, the two LBs reduced the lumbar ROM in participants with LBP in the same way as healthy controls. This was observed …

Research Round-up: Cancer survivors frequently experience cancer treatment–related functional impairments and disability. While a growing body of evidence supports the effectiveness of rehabilitation interventions for these individuals, there are significant gaps in the existing literature and no interdisciplinary agreement on the priorities for cancer rehabilitation research. In 2016, the Oncology Section revised its strategic plan and set an objective to create an oncology rehabilitation research agenda for the physical therapy (PT) profession. To achieve this goal, the scope of the agenda, a development and review process, and dissemination plan need to be developed.

Background

Lumbar stabilization exercise programs should normalize the aberrant movements patterns often observed in patients with low back pain. This study aimed to determine the effect of an 8-week lumbar stabilization program on EMG/kinematics measures of the aberrant movement patterns in such patients. A secondary goal was to assess the 8-week test-retest reliability of these measures.

Methods

One main purpose was to explore the compensatory gait strategies of individuals with multiple sclerosis (MS). To address this purpose, we quantified the mechanical work generated by the lower extremity joints during walking. The outcomes from this investi­gation suggested that individuals with MS redistribute positive mechanical work during walking to the hip in order to compensate for a reduced ability of the ankle to generate positive mechanical work. There are nearly 26 million people with diabetes mellitus (DM) in the US, and half of chronic DMs develop somatosensory deficits due to diabetic polyneuropathy or diabetic peripheral neuropathy (DPN). The absence or impaired somatosensory feedback (e.g. touch sensation or joint proprioception) resulted from the damage of large nerve fiber, and motor deficits such as attenuated muscle strength and abnormal plantar pressure of lower extremity have been identified in DPN, and these sensorimotor impairments lead to an increased number of falls. Adequate neuromuscular control of the lumbar spine is required to prevent lumbar injuries. A trunk postural control test protocol, controlling for the influence of body size on performance, was implemented to carry out between-subject comparisons. The aim of this study was to assess the effect of sex and low back pain status with the use of two measures of trunk postural control, the first based on chair motion, and the second based on trunk motion.

The purpose of this study was to determine if body weight support influences the stride length-speed relationship. Additionally, the purpose was to determine if impact characteristics of running are influenced by body weight support and speed. Subjects (n=10; 6 female, 4 male) volunteered to participate in this study. All subjects were injury free and were comfortable running on a treadmill for 30 minutes. Previous literature has investigated the effects of using orthotics, textured insoles and textured surfaces on balance. Purpose: Barefoot running (BF) is popular in the running community. Biomechanical changes occur with BF, especially when initial contact changes from rearfoot strike (RFS) to forefoot strike (FFS). In addition, changes in lumbar spine range of motion (ROM), particularly involving lumbar lordosis, have been associated with increased low back pain (LBP). However it is not known how changing from RFS to FFS affects lumbar lordosis or LBP. Thus, the purpose of this study was to determine if a change from RFS to FFS would change lumbar lordosis, and/or decrease shock attenuation, and/or change comfort levels in healthy recreational/experienced runners.

Scapular dyskinesis refers to abnormal movement patterns of the scapula, which have been associated with injury. Previous investigations have validated the scapular dyskinesis test as a visual assessment method using physicians, physical therapists, and certified athletic trainers. Female athletes have a four-to-six times greater risk of sustaining a lower extremity injury than their male counterparts (Hewett, 2000). One explanation for the discrepancy in injury rates is increased dynamic knee valgus angles caused by neuromuscular imbalances of the lumbopelvic hip complex (LPHC). The purpose of this study was to determine if implementing a LPHC conditioning program improved neuromuscular deficiencies. Correct muscular activation of the body segments during an overhand throw is achieved when movement originates in the larger and more proximal legs and trunk and moves sequentially to the smaller, distal segments of the shoulder and arm. This sequence permits angular velocity to transfer progressively through the throw as part of an open kinetic chain. This study analyzed the effects of gender, strike technique, power level, and trial factors on tennis athletes performing backhand strokes with the incoming ball launched by a serving-stand. Five male and five female college tennis athletes volunteered as subjects. A previous study found that the patellofemoral joint compressive force (PFJRF) during backward running was less than during forward running at free speed. Therefore, the purpose of this study was to compare the patellofemoral joint compressive forces during backward and forward running at the same speed. Ten runners (4 females, 6 males) between the age of 20-25 (X=22, SD=2) ran in backward and forward directions at the slow speed of 2.3 mph (1.0 m.sec-1). This study investigated the biomechanical and symptomatic effects of a newly described taping technique to support the longitudinal arch in symptomatic plantar fasciitis patients. The subjects were 20 individuals all diagnosed by a physician with unilateral plantar fasciitis, ambulating full weight-bearing without an assistive device.

Measurements were taken pre and post-taping for calcaneal position, tibial position and navicular height, with the subjects serving as their own control. The purpose of this study resulted from a need to gather and analyze data on gait characteristics of normal children using the Peak Performance Technologies, Incorporated system, initiate a data base for the Motion Analysis Center at Old Dominion University and compare a computerized motion analysis system to studies using different methods. Fifteen children, aged four to six, were evaluated during gait using a video-based computerized motion analysis system manufactured by Peak Performance Technologies, Incorporated. Spasticity is often considered the primary limitation to function in children with spastic cerebral palsy. The purpose of this study was to study the relationship between hamstring spasticity and a functional activity, specifically gait. The gait parameters chosen were step length, stride length and velocity. A secondary purpose was to study the relationship between hamstring contracture and the same gait parameters. Reliability data were calculated for tone and ROM measurements. Eleven subjects (8 male and 3 female) between the ages of three years and fifteen years with a primary diagnosis of spastic diplegia were recruited for this study.
