Bacterial co-infections with influenza A virus (IAV) are extremely serious and life-threatening. However, there exists limited understanding about the importance of fungal infections with IAV. Clinical case reports indicate that fungal co-infections do occur and suggest the IAV pandemic of 2009 had a propensity to predispose patients to secondary fungal infections more than previous IAV strains. IAV-fungal co-infections are marked by high mortality rates of 47 to 61% in previously healthy individuals between the ages of 20 and 60. Celiac Disease (CD) is an autoimmune disorder that emerges due to the ingestion of gluten, a protein found in a variety of common grains such as wheat, rye, and barley. Approximately 1 in 100 individuals in the US suffer from CD, making it the most commonly diagnosed gastrointestinal disorder (Ciclitira et. al., 2005). These proline-rich gluten peptides are resistant to proteolysis and accumulate in the duodenum of the small intestine. Once in the duodenum, these peptides illicit an autoimmune response resulting in villous atrophy. Current treatment for CD requires a rigorous adherence to a gluten-free diet. Antibiotic resistance is a consequence of evolution via natural selection and poses a serious threat to the world's population. Escherichia coli, and other bacterial species, have acquired genes encoding Metallo-β-Lactamases (MBLs). MBLs are zinc-dependent enzymes that degrade most β-lactam antibiotics, and have emerged as important antibacterial resistance determinants in multidrug resistant pathogens. There are several different families of MBLs, and the major clinically relevant ones are Verona Integrated-encoded Metallo-β-Lactamases (VIMs), Imipenemases (IMPs), and New Delhi Metallo-β-Lactamases (NDMs). This research is part of the Meiothermus ruber genome annotation project which aims to predict gene function with various bioinformatics tools. We investigated the function of Mrub_3019, which encodes the CasA protein involved in the multi-subunit effector complex for the CRISPR-Cas immunity system and predicted it to be an ortholog of E. coli K12 MG1655 b2760 (casA). We predicted that Mrub_3019 encodes the protein CasA, which is involved in PAM recognition of CRISPR interference pathway. In the past 20 years, the number of subterranean taxa discovered in Australia, especially in the Pilbara bioregion, has considerably increased due to incidental environmental surveys often associated with mining development. Bathynellidae are an important component of stygofauna and they occur in most Australian aquifers, but their collection and identification are difficult due to their habitat, and small and fragile bodies with conservative morphology. Ataxia telangiectasia mutated (ATM) and mutS homologue 2 (MSH2) are important DNA repair proteins that participate in DNA repair pathways to maintain genomic integrity. Mice deficient for ATM and MSH2 mice are viable. However, ATM-/- mice show growth retardation, neurological defects, and spontaneous lymphomagenesis. MSH2-/- mice suffer from aggressive lymphoid tumors between two to five months of age and have increased microsatellite instability, which predisposes MSH2-/- mice to carcinomas. โรคติดเชื้อที่มีสาเหตุมาจาก Mycobacterium avium complex (MAC) มีอุบัติการณ์เพิ่มสูงขึ้นทั่วโลกรวมทั้งในประเทศไทย เนื่องมาจากการเพิ่มขึ้นของจำนวนผู้ป่วยที่มีภาวะภูมิคุ้มกันของร่ายกายต่ำ การวินิจฉัยเชื้อ MAC ในปัจจุบันที่อาศัยการเพาะเลี้ยงเชื้อและทดสอบปฏิกิริยาชีวเคมี นอกจากใช้ระยะเวลานานแล้ว ยังไม่สามารถจำแนกเชื้อได้ถึงระดับสปีชี่ส์ โดยเฉพาะอย่างยิ่งเชื้อ Mycobacterium avium และเชื้อ Mycobacterium intracellulare ซึ่งเป็นเชื้อ MAC ที่สำคัญและเพาะแยกได้บ่อยจากสิ่งส่งตรวจ ดังนั้นวัตถุประสงค์ของการศึกษาในครั้งนี้ คือการพัฒนาเทคนิค Recombinase Polymerase Amplification ร่วมกับการอ่านผลด้วยสี SYBR Green I (RPA/SYBR) ในการตรวจวินิจฉัยเชื้อ MAC ซึ่งอาศัยการเพิ่มปริมาณสารพันธุกรรมภายใต้อุณหภูมิคงที่ ร่วมกับไพรเมอร์ที่จำเพาะต่อเชื้อกลุ่ม MAC เชื้อ M. avium และเชื้อ M. intracellulare และอ่านผลที่เกิดขึ้นด้วยตาเปล่าได้ในทันทีภายหลังการเติมสี SYBR Green I ผลจากการทดสอบกับตัวอย่าง DNA ของเชื้อ MAC จำนวน 120 ตัวอย่าง พบว่าเทคนิค RPA/SYBR สามารถตรวจวินิจฉัยจำแนกสปีชี่ส์ของเชื้อ M. avium และเชื้อ M. intracellulare ได้อย่างมีประสิทธิภาพ โดยมีความไว 100% และความจำเพาะตั้งแต่ 80.7% ขึ้นไป ซึ่งจัดว่ามีความสอดคล้องในเกณฑ์ดีมากเมื่อเปรียบเทียบกับวิธีหาลำดับเบส (สถิติ Kappa มีค่าตั้งแต่ 0.801 ขึ้นไป) ในขณะที่เมื่อเปรียบเทียบกับเทคนิค PCR มีความไวตั้งแต่ 89.2% ขึ้นไปและความจำเพาะตั้งแต่ 97.5% ขึ้นไป และมีความสอดคล้องในเกณฑ์ดีมากเช่นเดียวกัน (สถิติ Kappa มีค่าตั้งแต่ 0.863 ขึ้นไป) นอกจากนี้เทคนิค RPA/SYBR ยังมีค่าความเข้มข้นของ DNA ต้นแบบน้อยที่สุดที่สามารถตรวจได้ เท่ากับ 1 ng/µl และไม่เกิดปฏิกิริยาข้ามกลุ่มกับเชื้อส่วนใหญ่ของ Mycobacterium แม้ว่าเทคนิค RPA/SYBR ที่ได้พัฒนาขึ้น มีขั้นตอนการทดสอบที่ทำง่าย …

เชื้อ Escherichia coli ที่สร้างเอนไซม์ Extended-spectrum β-lactamase (ESBLs) เป็นปัญหาสำคัญที่พบในโรงพยาบาลทั่วโลก การตรวจจีโนไทป์ของเอนไซม์ ESBLs เป็นวิธีที่สำคัญและมีประโยชน์ในการควบคุมการแพร่กระจายของเชื้อดื้อยาและการเลือกใช้ยาต้านจุลชีพอย่างเหมาะสม การศึกษานี้มุ่งหวังจะพัฒนาเทคนิค Multiplex Recombinase polymerase amplification (Multiplex RPA) และ Helicase dependent amplification (HDA) สำหรับตรวจหายีนดื้อยา blaCTX-M, blaOXA, blaSHV และ blaTEM ในเชื้อ E. coli ในสถาบันมะเร็งแห่งชาติ ประเทศไทย เชื้อ E. coli จำนวน 144 ตัวอย่างที่แยกได้จากสิ่งส่งตรวจภายในงานจุลชีววิทยา สถาบันมะเร็งแห่งชาติ ช่วงระหว่างเดือนกุมภาพันธ์ 2017 ถึงเดือนกันยายน 2018 ถูกนำมาทดสอบความไวต่อยาต้านจุลชีพและตรวจคัดกรองการสร้างเอนไซม์ ESBLs ด้วยวิธี Disk diffusion ตามวิธีมาตรฐานของ CLSI ปี ค.ศ. 2017 เชื้อที่สร้างเอนไซม์ ESBLs จำนวน 95 ตัวอย่าง พบดื้อยาสูงสุดในยา Ampicillin (100%), Cefotaxime (100%), Cefdinir (100%) และ Ceftriaxone (99.9%) และเชื้อ E. coli จำนวน 144 ตัวอย่าง มียีน blaCTX-M (100%) รองลงมาคือ blaTEM (42.1%), blaOXA (30.5%) และ blaSHV (2.1%) ตามลำดับ โดยเป็นยีน ESBLs ชนิด CTX-M-15 สูงสุด (51.1%) รองลงมาคือ CTX-M-55 (18.1%), CTX-M-27 (17.0%), CTX-M-14 …

อะไมลอยด์เบต้า 42 (Aβ42) เป็นของเสียที่เกิดจากการย่อยอะไมลอยด์เบต้าพรีเคอเซอร์โปรตีน อาจเสี่ยงต่อการเกิดโรคอัลไซเมอร์เมื่อสะสมที่สมอง โดยมียีนบีเอซีอีวัน (BACE1) ควบคุมการสร้างเอนไซม์เบต้าซีครีเตส เพื่อย่อยอะไมลอยด์เบต้าพรีเคอเซอร์โปรตีน และมียีนเอโปอี (ApoE) เป็นยีนที่ถอดรหัสเป็น Apolipoprotein E ทำหน้าที่ขนส่งอะไมลอยด์เบต้าไปกำจัดที่ตับ คาดว่าการติดเชื้อไวรัสตับอักเสบเรื้อรังอาจส่งผลให้การกำจัดของเสียลดลงด้วย งานวิจัยนี้จึงมีวัตถุประสงค์เพื่อศึกษาระดับอะไมลอยด์เบต้า 42 ในเลือดด้วยหลักการ ELISA และศึกษาความหลากหลายทางพันธุกรรมของยีน BACE1 (rs638405) ด้วยหลักการ PCR-RFLP และยีน ApoE (rs429358 และ rs7412) ด้วยหลักการ real time PCR ในผู้ติดเชื้อไวรัสตับอักเสบบีเรื้อรัง (CHB, N=149) ซีเรื้อรัง (CHC, N=31) และคนปกติ (N=164) โดยเป็นผู้ติดเชื้อไวรัสตับอักเสบบีเรื้อรัง ซีเรื้อรังที่รักษา ณ โรงพยาบาลพระมงกุฎเกล้า และผู้บริจาคโลหิต ณ สถาบันพยาธิวิทยา ในช่วงเดือน สิงหาคม 2561 - มิถุนายน 2562 ผลการศึกษาพบว่าระดับอะไมลอยด์เบต้า 42 ในเลือดของผู้ป่วย CHB มีค่าสูงกว่าคนปกติ (CHB=46.86 pg/ml (N=22), CHC=43.27 pg/ml (N=15) และ คนปกติ =33.72 pg/ml (N=36), p-value=0.002) ข้อมูลจากกลุ่มตัวอย่างที่ตรวจวัดนี้มีจำนวนจำกัด จึงยังไม่สามารถนำไปอ้างอิงได้ นอกจากนี้ผู้วิจัยพบสัดส่วนของจีโนไทป์ของยีน BACE1 ในผู้ป่วย CHB (N=149), CHC (N=31) มีจีโนไทป์ C/G สูงกว่าในคนปกติ (N=164) (p-value = 0.044) และยีน ApoE ในผู้ป่วย CHB (N=149), CHC (N=31) มีจีโนไทป์ ε3ε4 สูงกว่าในคนปกติ (N=148) (p-value=0.001) …

สถานการณ์การดื้อยาปฏิชีวนะของแบคทีเรียเป็นปัญหาใหญ่ที่ส่งผลกระทบไปทั่วโลก จึงจำเป็นต้องศึกษาเพื่อพัฒนาสารทดแทนยาปฏิชีวนะ ตัวเงินตัวทอง (Varanus salvator) เป็นสัตว์เลื้อยคลานมีความสามารถในการอยู่รอดจากสภาวะที่เสี่ยงต่อการติดเชื้อ อาจเนื่องจากสัตว์เหล่านี้มีระบบภูมิคุ้มกันโดยกำเนิดซึ่งมีวิวัฒนาการมาแต่โบราณจึงเป็นจุดเด่นที่น่าศึกษาวิจัย งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาฤทธิ์ต้านเชื้อแบคทีเรียขององค์ประกอบเลือดจากตัวเงินตัวทองและโปรตีนหรือเปปไทด์ที่มีคุณสมบัติต้านจุลชีพ ซึ่งข้อมูลของลำดับกรดอะมิโนของตัวเงินตัวทองที่ได้ในการศึกษาวิจัยครั้งนี้จะเป็นพื้นฐานที่สำคัญใน โดยนำพลาสมาและซีรัมจากตัวเงินตัวทองมาทดสอบคุณสมบัติการยับยั้งเชื้อแบคทีเรียที่ทำให้เกิดโรคด้วยเทคนิค agar diffusion พบว่าพลาสมาสามารถยับยั้งการเจริญของเชื้อแบคทีเรียได้ดีที่สุดมีขนาดเส้นผ่านศูนย์กลางของวงใสยับยั้งเชื้อ (inhibition zone) เท่ากับ 17-20 มิลลิเมตร เมื่อทำการทดสอบหาค่าความเข้มข้นต่ำสุดของพลาสมาและซีรัมที่สามารถยับยั้งเชื้อแบคทีเรีย (minimal inhibitory concentration, MIC) ด้วยวิธี broth dilution พบว่าสารสกัดหยาบพลาสมาให้ผลการทดสอบกับเชื้อได้ดีที่สุดโดยมีค่าความเข้มข้นต่ำสุดที่สามารถยับยั้งเชื้อแบคทีเรีย (MIC) เท่ากับ 125 ไมโครกรัมต่อมิลลิลิตร จากผลของการแยกบริสุทธิ์บางส่วนของพลาสมาและซีรัมโดยเทคนิค ion exchange chromatography ด้วย Q - sepharose คอลัมน์ พบว่าทั้งพลาสมาและซีรัมสามารถแยกโปรตีนแฟรคชันได้ 5 แฟรคชันเท่ากัน เมื่อวิเคราะห์ด้วยเทคนิค sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE) พบขนาดโมเลกุลอยู่ระหว่าง 16 – 200 กิโลดัลตัน แต่เมื่อทดสอบคุณสมบัติการต้านเชื้อ พบบางแฟรกชันจากพลาสมา (P) และซีรั่ม (S) ซึ่งกำหนดเป็น P1 / P2 / P5 และ S1 / S2 / S4 ตามลำดับ ที่แสดงฤทธิ์ต้านเชื้อแบคทีเรีย เมื่อวิเคราะห์องค์ประกอบของสารออกฤทธิ์ทางชีวภาพด้วยเทคนิค liquid chromatography mass spectrometry (LC-MS) สามารถระบุชนิดโปรตีนได้ทั้งหมด 33 ชนิดและพบโปรตีนที่มีศักยภาพเป็นเปปไทด์ต้านจุลชีพ 15 ชนิด มีเพียง 3 ชนิด จากส่วนของพลาสมา (P1) 2 ชนิด และ (P2) 1 ชนิด ที่มีโครงสร้างเป็น α-helical peptide …

งานวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาองค์ประกอบทางพฤกษเคมีและฤทธิ์ต้านเชื้อรา Candida albicans ของน้ำมันเสม็ดขาวจากใบและกิ่งอ่อนของ Melaleuca cajuputi Powell ที่พบในพื้นที่ภาคตะวันออกของประเทศไทย รวมทั้งประเมินผลกระทบของน้ำมันเสม็ดขาวต่อการแสดงออกของยีนที่เกี่ยวข้องกับกลไกการดื้อยาฟลูโคนาโซลในเชื้อราทดสอบ การวิเคราะห์องค์ประกอบทางพฤกษเคมีด้วยวิธีแก๊สโครมาโทกราฟี-แมสสเปกโตรมิทรี (gas chromatography-mass spectrometry, GC-MS) พบว่าน้ำมันเสม็ดขาวมีองค์ประกอบหลัก 6 ชนิด ได้แก่ 1,8-Naphthyridine derivatives (ร้อยละ 10.46), alpha-Pyrone (ร้อยละ 10.11), Terpinolene (ร้อยละ 9.26), gamma-Terpinene (ร้อยละ 8.00), trans-Caryophyllene (ร้อยละ 6.36) และ beta-Elemene (ร้อยละ 5.09) จากการทดสอบฤทธิ์เบื้องต้นของน้ำมันเสม็ดขาวในการยับยั้ง C. albicans สายพันธุ์คลินิกที่ดื้อต่อยาฟลูโคนาโซล จำนวน 16 ไอโซเลท ด้วยวิธี Disc diffusion พบว่าดิสก์บรรจุน้ำมันเสม็ดขาว ขนาด 1 ไมโครลิตรต่อดิสก์ แสดงฤทธิ์ยับยั้งการเจริญของเชื้อทดสอบทุกไอโซเลท โดยมีขนาดเส้นผ่านศูนย์กลางบริเวณยับยั้งเชื้อเฉลี่ยอยู่ระหว่าง 6.67 ± 0.58 ถึง 10.00 ± 0.00 มิลลิเมตร เมื่อทำการทดสอบหาค่าความเข้มข้นต่ำสุดของน้ำมันเสม็ดขาวที่สามารถยับยั้งเชื้อทดสอบ (minimal inhibitory concentration, MIC) และฆ่าเชื้อทดสอบ (minimum fungicidal concentration, MFC) ด้วยวิธี Broth macrodilution พบว่าน้ำมันเสม็ดขาวแสดงฤทธิ์ฆ่าเชื้อราทดสอบทุกไอโซเลท โดยมีค่า MIC และ MFC อยู่ในช่วง 0.31-1.25 และ 0.63-2.5 ไมโครลิตรต่อมิลลิลิตร ตามลำดับ การทดสอบการเสริมฤทธิ์กันของสารผสมน้ำมันเสม็ดขาวและยาฟลูโคนาโซลในการต้านเชื้อราทดสอบด้วยวิธี Checkerboard microdilution พบว่าสารผสมดังกล่าวมีการเสริมฤทธิ์กันในการยับยั้งเชื้อราทดสอบส่วนใหญ่ (ร้อยละ 60) โดยค่า MIC ของยาฟลูโคนาโซลและน้ำมันเสม็ดขาวจะลดลงประมาณ 8-64 และ 8-16 เท่า ตามลำดับ …

ผลิตภัณฑ์เกล็ดโลหิตที่มีการปนเปื้อนของเชื้อแบคทีเรีย อาจก่อให้การติดเชื้อในกระแสโลหิตที่รุนแรงและนำไปสู่การเสียชีวิตได้ ดังนั้นจึงมีการกำหนดมาตรฐานให้งานธนาคารโลหิต ทำการตรวจการปนเปื้อนเชื้อแบคทีเรียในผลิตภัณฑ์เกล็ดโลหิตก่อนนำไปใช้กับผู้ป่วย เพื่อลดอาการไม่พึงประสงค์ที่อาจเกิดขึ้น วิธีการตรวจการปนเปื้อนเชื้อแบคทีเรียในผลิตภัณฑ์เกล็ดโลหิตในปัจจุบันยังมีข้อจำกัดที่พบได้บางประการ อาทิเช่น การเพาะเลี้ยงเชื้อแบคทีเรียที่ไม่สามารถปฏิบัติได้ในห้องปฏิบัติการทางธนาคารโลหิต การทดสอบที่ใช้ระยะเวลานาน ซึ่งไม่เหมาะสมกับอายุการเก็บรักษาผลิตภัณฑ์เกล็ดโลหิตที่สั้นประมาณ 5 วัน และต้นทุนของการทดสอบที่มีราคาแพง วัตถุประสงค์ของการศึกษาครั้งนี้เพื่อพัฒนาเทคนิค Helicase dependent amplification ควบคู่กับการอ่านผลด้วยตาเปล่าโดยใช้สี SYBR Green I (HDA/SYBR Green I) สำหรับตรวจหาการปนเปื้อนเชื้อแบคทีเรียในผลิตภัณฑ์เกล็ดโลหิต โดยใช้ Primer ที่ถูกออกแบบให้จำเพาะต่อยีน 16s rRNA ซึ่งเป็น Universal gene ของเชื้อแบคทีเรีย ผลการทดสอบกับตัวอย่างผลิตภัณฑ์เกล็ดโลหิตที่ปนเปื้อนเชื้อแบคทีเรียแบบจำลอง ณ วันต่าง ๆ จำนวน 96 ตัวอย่าง พบว่าเทคนิค HDA/SYBR Green I มีความไวและความจำเพาะสูง เท่ากับ 96% และ 100% ตามลำดับ และมีความสอดคล้องกันในระดับดีมาก เมื่อเปรียบเทียบกับเทคนิค HDA/AGE เมื่อเปรียบเทียบกับเทคนิค PCR การนับจำนวนโคโลนี และเครื่องอัตโนมัติ BacT/Alert System พบว่าเทคนิค HDA/SYBR Green I มีความจำเพาะสูงเท่ากับ 100% แต่มีความไวอยู่ในช่วงระหว่าง 63-76% จึงทำให้มีความสอดคล้องกันในระดับปานกลางถึงดี อย่างไรก็ดี เมื่อนำเทคนิค HDA/SYBR Green I ตรวจการปนเปื้อนเชื้อแบคทีเรียในผลิตภัณฑ์เกล็ดโลหิตเฉพาะวันที่ 2 ถึง 5 ของการเก็บรักษา พบว่ามีความไวที่สูงขึ้น เท่ากับ 88% และมีความสอดคล้องกันในระดับดีมาก เทคนิค HDA/SYBR Green I มีค่าความเข้มข้นน้อยที่สุดของ DNA ต้นแบบที่สามารถตรวจได้ เท่ากับ 1 ng และไม่เกิดปฏิกิริยาข้ามกลุ่มกับสิ่งมีชีวิตอื่น ๆ ที่มีโอกาสพบการปนเปื้อนได้ในผลิตภัณฑ์เกล็ดโลหิต เทคนิค HDA/SYBR Green I …

การทดสอบความไวต่อยา Pyrazinamide (PZA) ด้วยวิธีมาตรฐาน (culture-based susceptibility tests) นั้นทำได้ยาก เนื่องจากสภาวะเป็นกรดในอาหารเลี้ยงเชื้อสามารถทำให้เกิดผลลบและผลบวกลวง การศึกษานี้มุ่งเน้นศึกษาและพัฒนาชุดทดสอบสำหรับใช้วินิจฉัยเชื้อดื้อต่อยา PZA ในเชื้อ Mycobacterium tuberculosis ด้วยชุดทดสอบ colorimetric biphasic medium assay ที่ผสม Nicotinamide (NIC-CBMA) ความเข้มข้น 250, 500, 1000 และ 2000 µg/mL และวิเคราะห์การกลายพันธุ์ที่สัมพันธ์กับการดื้อยา PZA ในยีน pncA, rpsA และ panD ผลการทดสอบในเชื้อวัณโรคดื้อยาทั้งหมด 150 สายพันธุ์ แบ่งเป็นเชื้อดื้อต่อยา PZA 40 สายพันธุ์ และเชื้อไวต่อยา PZA 110 สายพันธุ์ เมื่อทดสอบด้วยวิธี BACTEC MGIT 960 พบว่าความเข้มข้นของ NIC ≥ 1000 µg/mL เหมาะสำหรับใช้วินิจฉัยเชื้อดื้อยา PZA ด้วยวิธี NIC-CBMA ซึ่งมีความไวของชุดทดสอบเท่ากับ 100% ความจำเพาะเท่ากับ 96.36% เมื่อเทียบกับวิธีมาตรฐาน BACTEC MGIT 960 นอกจากนี้พบการกลายพันธุ์ในยีน pncA ที่สัมพันธ์กับการดื้อยา PZA จำนวน 28 แบบ ในเชื้อจำนวน 37 จาก 40 สายพันธุ์ที่ดื้อยา PZA เป็นการกลายพันธุ์แบบ Nonsynonymous 57.89% การกลายพันธุ์แบบ Indels 21.05% การกลายพันธุ์บริเวณ Promoter 13.16% การกลายพันธุ์แบบ Nonsense 5.26% และการกลายพันธุ์แบบ Double mutation 2.63% นอกจากนี้พบการกลายพันธุ์ใหม่จำนวน 4 แบบ …

ปริมาณโลหิตและส่วนประกอบโลหิตที่เพียงพอในธนาคารเลือดนั้นมีความสำคัญในการรักษาผู้ป่วย ดังนั้นธนาคารเลือดทุกแห่งต้องส่งเสริมให้ผู้บริจาคโลหิตมีสุขภาพดีและมีคุณสมบัติเหมาะสมในการบริจาคโลหิตอย่างสม่ำเสมอจากข้อมูลของศูนย์บริการโลหิตแห่งชาติ สภากาชาดไทยพบผู้ถูกปฏิเสธการบริจาคโลหิตประมาณร้อยละ 15-20 จากจำนวนผู้ประสงค์จะบริจาคโลหิตทั้งหมด ส่วนใหญ่มีสาเหตุจากค่าฮีโมโกลบิน (hemoglobin; Hb) ไม่ผ่านเกณฑ์ นำไปสู่ความผิดหวังความไม่พอใจเพราะผู้บริจาคโลหิตต้องเสียเวลา ค่าใช้จ่ายในการเดินทางแต่ไม่ได้บริจาคโลหิตและส่งผลกระทบต่อการจัดหาโลหิตโดยตรง หากสามารถพยากรณ์ผลตรวจ Hb ได้ล่วงหน้าจะช่วยลดผลกระทบปัญหา เป็นประโยชน์ทั้งต่อผู้บริจาคโลหิต ธนาคารเลือดและผู้ป่วย การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาและเปรียบเทียบประสิทธิภาพเทคนิคการเรียนรู้ของคอมพิวเตอร์ในการพยากรณ์จำแนกกลุ่มผลตรวจ Hb ของผู้บริจาคโลหิต โดยเก็บข้อมูล 44 ตัวแปรของผู้บริจาคโลหิตจำนวน 2,180 รายจากภาคบริการโลหิตแห่งชาติ 12 แห่งและสถานีกาชาดหัวหินเฉลิมพระเกียรติตั้งแต่ 1 ต.ค. 2561 ถึง 31 พ.ค. 2562 นำมากลั่นกรองข้อมูล คัดเลือกตัวแปร พัฒนาตัวแบบทางเหมืองข้อมูลได้แก่ ต้นไม้ตัดสินใจ ซัพพอร์ตเวกเตอร์แมชชีน การจำแนกแบบเบส์อย่างง่ายและโครงข่ายประสาทเทียม จากนั้นเปรียบเทียบประสิทธิภาพการจำแนกกลุ่มของตัวแบบพยากรณ์ พบว่าต้นไม้ตัดสินใจเป็นตัวแบบการจำแนกกลุ่มที่เหมาะสมที่สุดโดยให้ค่าความถูกต้อง ค่าความไว ค่าความจำเพาะ ค่าการพยากรณ์ผลบวก ค่าการพยากรณ์ผลลบสูงที่สุดและค่า AUC เท่ากับร้อยละ 92.20, 82.98, 94.74, 81.25, 95.29 และ 0.943 ตามลำดับ ซึ่งต้นไม้ตัดสินใจที่ได้จากศึกษานี้อาจนำไปพัฒนาต่อเป็นระบบประเมินออนไลน์ก่อนเดินทางมาบริจาคโลหิตได้

โรคติดเชื้อไวรัสมะเร็งเม็ดเลือดขาวในแมว (Feline Leukemia Virus : FeLV) เป็นโรคติดเชื้อไวรัสในแมวที่สำคัญที่เป็นสาเหตุของโรคมะเร็งเม็ดเลือดขาว และ มะเร็งต่อมน้ำเหลือง การตรวจหา FeLV RNA และโปรไวรัส ด้วยวิธีทางอณูชีววิทยามีความสำคัญในการแยกระยะต่างๆของโรค ซึ่งวิธี Rapid immunochromatographic assay ที่นิยมใช้ตรวจแอนติเจนนั้นสามารถตรวจได้เพียงการติดเชื้อระยะ Progressive ที่มี Viremia เท่านั้น งานวิจัยนี้จึงได้พัฒนาเทคนิค Recombinase polymerase amplification (RPA) และ Nested RPA เพื่อตรวจ exogenous FeLV Provirus DNA และ RT-RPA เพื่อตรวจ FeLV RNA และทดสอบตัวอย่างเลือดแมวจำนวน 122 ตัวอย่าง ผลการศึกษาพบว่าเทคนิค RPA สามารถตรวจหา FeLV Provirus ได้ที่ขีดจำกัด 1 ng/µl และไม่เกิดปฏิกิริยาข้ามกลุ่มกับเชื้อก่อโรคในแมว มีความไว ความจำเพาะ เทียบกับเทคนิค PCR ที่ร้อยละ 95.89 และ 100 ตามลำดับ ขณะที่ Nested RPA เทียบกับเทคนิค Nested PCR มีความไว ความจำเพาะ ที่ร้อยละ 98.89 และ 96.88 ตามลำดับ การตรวจหา FeLV RNA ด้วยเทคนิค RT-RPA มีขีดจำกัดที่ 0.1 ng/µl เมื่อเทียบกับเทคนิค RT-PCR และ Rapid Immunochromatographic assay มีความไว ร้อยละ 93.75 และ 95.24 ตามลำดับ และ ความจำเพาะร้อยละ 100 ค่าความสอดคล้องของเทคนิค RPA …

เชื้อ Klebsiella pneumoniae ที่สร้างเอนไซม์ carbapenemases (KPCs) เป็นปัญหาสำคัญของการติดเชื้อในโรงพยาบาลโดยเฉพาะผู้ป่วยเด็ก เนื่องจากมักดื้อยาหลายขนาน ทำให้มียาที่ใช้รักษาได้จำกัด เป็นสาเหตุทำให้มีอัตราการเสียชีวิตสูงขึ้น งานวิจัยนี้มีวัตถุประสงค์เพื่อพัฒนาเทคนิค multiplex Recombinase Polymerase Amplification (multiplex RPA) สำหรับตรวจหายีนที่สร้างเอนไซม์ carbapenemase ของเชื้อ K. pneumoniae และเปรียบเทียบความไวและความจำเพาะกับ วิธี Modified Carbapenem Inactivation Method Test (mCIM) และ วิธี nucleotide sequencing กับตัวอย่างเชื้อ K. pneumoniae ในสถาบันสุขภาพเด็กแห่งชาติมหาราชินี ประเทศไทย ตั้งแต่เดือนมิถุนายน ถึง ธันวาคม 2018 โดยเก็บเชื้อที่ผลการคัดกรองพบดื้อยา carbapenem แบ่งเป็นจาก perianal ในงานเฝ้าระวังเชื้อดื้อยา จำนวน 129 ตัวอย่าง และจากสิ่งส่งตรวจของผู้ป่วยเด็ก จำนวน 21 ตัวอย่าง พบให้ผลบวกกับวิธี mCIM จำนวน 149 ตัวอย่าง (99.3%) และพบเป็นยีน blaNDM-1 สูงสุดถึงร้อยละ 89.3 รองลงมาคือยีน blaOXA-232 ร้อยละ 8 และยีน blaNDM-1 ร่วมกับ blaOXA-232 ร้อยละ 2 ตามลำดับ เชื้อที่แยกได้จากสิ่งตรวจพบมีการดื้อยาหลายขนาน โดยดื้อยา carbapenem ร่วมกับ gentamicin, trimethoprim/sulfamethoxazole, ciprofloxacin ขณะที่เชื้อที่แยกจากแผนกผู้ป่วยต่าง ๆ ในงานเฝ้าระวังเชื้อดื้อยา พบการแพร่กระจายของยีน blaNDM-1 สูงสุดใน 4 แผนก จาก 9 แผนก การพัฒนาเทคนิค multiplex RPA พบว่าสามารถเพิ่มปริมาณยีน blaKPC, blaNDM, …

The intestinal microbiome of mammals plays a significant role in host health and response to environmental stimuli and can include both beneficial native bacteria as well as parasitic worms. In this study, I examined the intestinal cestode and bacterial communities of two closely related species of shrew, Sorex monticola and Sorex cinereus, over a six month period in 2016. Specimens were collected approximately every three weeks from May to October from the Sangre de Cristo Mountains in Cowles, New Mexico. Borrelia burgdorferi, the causative agent of Lyme disease, survives in nature through a cycle that alternates between ticks and vertebrates. To facilitate this defined lifestyle, B. burgdorferi has evolved a gene regulatory network that ensures transmission between those hosts, along with specific adaptations to niches within each host. Expression of vast repertoires of antigen receptors by lymphocytes, with each cell expressing a single receptor, requires stochastic activation of individual variable (V) genes for transcription and recombination. How this occurs remains unknown. Using single-cell RNA sequencing (scRNA-seq) and allelic variation, we show that individual pre-B cells monoallelically transcribe divergent arrays of Vκ genes, thereby opening stochastic repertoires for subsequent Vκ-Jκ recombination. Transcription occurs upon translocation of Vκ genes to RNA polymerase II arrayed on the nuclear matrix in transcription factories. Inbred mouse strains differ in their susceptibility to infection with the facultative intracellular bacterium Listeria monocytogenes, largely due to delayed or deficient innate immune responses. Previous antibody depletion studies suggested that neutrophils (polymorphonuclear leukocytes [PMN]) were particularly important for clearance in the liver, but the ability of PMN from susceptible and resistant mice to directly kill L. monocytogenes has not been examined. In this study, we showed that PMN infiltrated the livers of BALB/c/By/J (BALB/c) and C57BL/6 (B6) mice in similar numbers and that both cell types readily migrated toward leukotriene B4 in an in vitro chemotaxis assay. Alternative first exons diversify the transcriptomes of eukaryotes by producing variants of the 5′ Untrans- lated Regions (5′UTRs) and N-terminal coding se- quences. Accurate transcriptome-wide detection of alternative first exons typically requires specialized experimental approaches that are designed to iden- tify the 5′ ends of transcripts. We developed a compu- tational pipeline SEASTAR that identifies first exons from RNA-seq data alone then quantifies and com- pares alternative first exon usage across multiple bi- ological conditions. ปัจจุบันพบเชื้อ Escherichia coli ที่สร้างเอนไซม์ extended-spectrum beta-lactamase (ESBL) แพร่กระจายในชุมชนอย่างกว้างขวาง เชื้อแบคทีเรียดังกล่าวสามารถพบได้ว่าเป็นเชื้อประจำถิ่นในลำไส้ของคน และสัตว์ รวมไปถึงสามารถพบได้ในสิ่งแวดล้อมทั่วไป ซึ่งอาจกลายเป็นแหล่งเก็บกักเชื้อแบคทีเรียดื้อสารต้านจุลชีพ ที่สามารถแพร่กระจายในชุมชนต่างๆ ทั่วโลก การศึกษาครั้งนี้ทำการจำแนกลักษณะทางโมเลกุลของเชื้อ E. coli ที่สร้างเอนไซม์ ESBL ซึ่งแยกได้จากสุกรในฟาร์มปศุสัตว์และประชากรในชุมชน ในพื้นที่อำเภอเมือง จังหวัดลำพูน เพื่อศึกษารูปแบบของยีนที่ควบคุมการสร้างเอนไซม์ ESBL และศึกษาหาความสัมพันธ์ของเชื้อในกลุ่มตัวอย่างทั้งสอง เชื้อ E. coli ที่สร้างเอนไซม์ ESBL จำนวนทั้งสิ้น 212 สายพันธุ์ แยกได้จากอุจจาระของสุกรและประชากรที่มีสุขภาพดี และทำการจำแนกยีนที่ควบคุมการสร้างเอนไซม์ ESBL ได้แก่ ยีน blaCTX-M ยีน blaTEM และยีน blaSHV ด้วยเทคนิค multiplex-PCR และเทคนิค whole genome sequencing และศึกษาความสัมพันธ์ของเชื้อ E. coli ที่สร้างเอนไซม์ ESBL ที่แยกได้จากทั้งสองกลุ่มตัวอย่างด้วยเทคนิค multilocus sequence typing ผลการศึกษาพบว่าเชื้อ E. coli ที่สร้างเอนไซม์ ESBL ส่วนใหญ่เป็นเชื้อดื้อยาหลายขนาน และมักดื้อยากลุ่ม fluoroquinolones ยากลุ่ม aminoglycosides และยา trimethoprim/sulfamethoxazole ในขณะที่เชื้อทุกสายพันธุ์ดื้อต่อยา ampicillin ยา ceftriaxone และยา cefotaxime ยีนที่ควบคุมการสร้างเอนไซม์ ESBL ที่พบมากที่สุดได้แก่ ยีน blaCTX-M ซึ่งพบได้ 95.75% รองลงมาคือยีน blaTEM และยีน blaSHV พบได้ 62.73% และ 2.40% ตามลำดับ โดย subgroup ของยีนที่ควบคุมการสร้างเอนไซม์ ESBL ที่พบมากที่สุด ได้แก่ ยีน blaCTX-M-55 …

Sickle cell anemia is a disease that affects red blood cells, specifically the hemoglobin protein. An amino acid mutation in the gene that encodes β-globin leads to malformation of the β subunit of hemoglobin.3, Valine becomes glutamic acid in the mutated β-globin gene.9 The mutation malformation leads to the red blood cells becoming sickle shaped, or crescent shaped.3,4 The sickle shape of red blood cells in individuals with sickle cell disease leads to vaso-occlusive crisis.9 Vaso-occlusive crisis includes complications such as blood clotting, chronic pain, organ failure, organ death, and possibly early mortality.9 The Centers for Disease Control and Prevention …

Development of Pneumocystis pneumonia (PCP) is a common problem among immunosuppressed individuals. There are windows of opportunity in which vaccination would be beneficial, but to date, no vaccines have made it to clinical trials. Significant hurdles to vaccine development include host range specificity, making it difficult to translate from animal models to humans. Discovery of cross-reactive epitopes is critical to moving vaccine candidates from preclinical animal studies to clinical trials.

The cell wall β-glucans of Pneumocystis cysts have been shown to stimulate immune responses in lung epithelial cells, dendritic cells, and alveolar macrophages. Little is known about how the trophic life forms, which do not have a fungal cell wall, interact with these innate immune cells. Here we report differences in the responses of both neonatal and adult mice to the trophic and cystic life cycle stages of Pneumocystis murina. Gene content determination and variant calling in the complex KIR genomic region are useful for immune system function analysis, pathogenesis and disease risk factor elucidation, immunotherapy development, evolutionary investigations, and human migration modeling. Sequence-specific oligonucleotide and sequence-specific primer PCR methods are the de facto standards for KIR presence/absence identification, but the current platforms are unsuitable for SNP calling, impractical for KIR typing large cohorts of DNA samples, and inapplicable for typing repositories in which sequence data, but not cells or cell analytes, are available. Vaccinia virus (VACV) is a large double-stranded DNA virus and the prototypical member of the family Poxviridae and is most notable for its use as the vaccine that eradicated smallpox (variola virus). More recently, VACV has been used to develop recombinant vaccines and immunotherapies. However, many of these processes require VACV replication to be tightly controlled. Chagas disease is caused by the protozoan parasite Trypanosoma cruzi, which is transmitted to domestic and sylvatic mammals via the feces of hematophagous hemiptera of the subfamily Triatominae (Reduviidae). Trypanosoma cruzi is found only in the Americas and displays remarkable genetic diversity. Seven discrete typing units (DTUs) are currently recognized (TcI–TcVI and TcBat). In Jalisco, Mexico, where Chagas disease has a high prevalence rate, TcI has historically been the only DTU reported.
