Two cosegregating single-nucleotide polymorphisms (SNPs) in human TLR4, an A896G transition at SNP rs4986790 (D299G) and a C1196T transition at SNP rs4986791 (T399I), have been associated with LPS hyporesponsiveness and differential susceptibility to many infectious or inflammatory diseases. However, many studies failed to confirm these associations, and transfection experiments resulted in conflicting conclusions about the impact of these SNPs on TLR4 signaling. Using advanced protein modeling from crystallographic data of human and murine TLR4, we identified homologous substitutions of these SNPs in murine Tlr4, engineered a knock-in strain expressing the D298G and N397I TLR4 SNPs homozygously, and characterized in vivo …

Defensive symbioses, in which microbes provide molecular defenses for an animal host, hold great potential as untapped sources of therapeutically useful antibiotics. Fungus-growing ants use antifungal defenses from bacterial symbionts to suppress pathogenic fungi in their nests. Preliminary chemical investigations of symbiotic bacteria from this large family of ants have uncovered novel antifungal molecules with therapeutic potential, such as dentigerumycin and selvamicin.
In this study, the bacterial symbionts of North American Trachymyrmex fungus-growing ants are investigated for antifungal molecules. Plate-based bioassays using ecologically-relevant fungal pathogens confirmed that these bacteria have antifungal activity. In order to purify and identify the antifungal …

Female meiotic drive is an unequal transmission of alleles that arises through the competition of paired chromosomes for inclusion in the egg, resulting in an increase in frequency of the driven alleles regardless of their effect on fitness of the individual. In Mimulus guttatus (monkeyflower) second filial generations, driven alleles display transmission advantages resulting in the virtual elimination of recessive homozygotes, while the equivalent lines lacking drive elements conform to traditional Mendelian segregation population ratios. Climate change has been considered a key driver for adaptations in water usage efficiency (WUE) in plants. Perceptions persist that C4 photosynthesis is indicative of some level of WUE. Here, we examined the interaction and evolution of WUE within the grass family, Poaceae, as it pertains to photosynthetic pathway. Unique to this study is the magnitude of taxa sampled, presence of intrageneric sampling, and use of complete plastid genomes (plastomes) for phylogenomic analyses.

Identifying the sex-determination region (SDR) and other genomic features of sex chromosomes are of great importance in the studies of the evolution of sex. However, the process of accurately identifying the size and location of the SDR is often difficult, even when a genomic sequence is available. This usually is hindered by large repetitive elements and a lack of recombination in the SDR. In this thesis, I assemble sex chromosomes with whole genomic sequencing data, identify SDRs and explore their genomic features in two sister species from the Salicaceae family. Glutamate Carboxypeptidase II (GCPII) is a transmembrane, zinc metallopeptidase that is expressed in a wide range of organisms, including roundworms, mice, and humans. In humans, GCPII is primarily expressed in the prostate, kidneys, small intestine, and central nervous system. Within the small intestine, the expected function of GCPII is to aid in the absorption of dietary folate from the intestinal lumen. GCPII cleaves excess glutamates from folate to yield monoglutamated folate which is then readily transported into the enterocyte. Folate can then be used through the one carbon metabolic cycle for the synthesis of nucleotides, conversion of homocysteine to methionine, …

INTRODUCTION: Women with pathogenic germline gene variants in
METHODS: We searched Medline, EMBASE, CENTRAL, PubMed, and clinicaltrials.gov up to October 3, 2019. We identified 2775 records and included 21.
RESULTS: There is limited evidence related to these factors and ovarian cancer risk. For breast cancer risk, evidence suggests higher diet quality, adulthood weight-loss of ≥10 pounds, and activity during adolescence and young-adulthood may be linked with decreased risk. Higher meat intake and higher daily energy intake may be linked with increased risk.
CONCLUSIONS: There is not enough evidence to suggest tailored recommendations for dietary habits or weight management among women …

Motor protein-based active transport is essential for mRNA localization and local translation in animal cells, yet how mRNA granules interact with motor proteins remains poorly understood. Using an unbiased yeast two–hybrid screen for interactions between murine RNA-binding proteins (RBPs) and motor proteins, here we identified protein interaction with APP tail-1 (PAT1) as a potential direct adapter between zipcode-binding protein 1 (ZBP1, a β-actin RBP) and the kinesin-I motor complex. The amino acid sequence of mouse PAT1 is similar to that of the kinesin light chain (KLC), and we found that PAT1 binds to KLC directly. Studying PAT1 in mouse …

Carbohydrates are immensely important biomolecule found in all organisms on earth, serving roles in structure, energy storage, protection and/or cell signaling. Carbohydrates are composed of an immense diversity of linkages, monosaccharide moieties and substitutions making them likely the most diverse biomolecule on earth, as well as the most abundant, being synthesized by plants and algae. Plant biomass is a renewable and sustainable feedstock for biofuel production that can reduce societal dependence on fossil fuels. However, the production of liquid biofuels from the non-starch (i.e. lignocellulosic) material through fermentation technology is limited due to the complexity of the cell wall structure. This necessitates the use of chemical, thermal, and/or mechanical pretreatment technologies, which adds significant capital, operational, and environmental costs. Human induced pluripotent stem cells offer a model for human brain development and disease by differentiation into brain organoids; however, current neural culture systems lack the microenvironment, neuronal circuits and connectivity, vascular circulation, and immune system that exist in vivo. After differentiation and development of neuronal and non-neuronal cell types within two formats of cell cultures, we can visualize and recapitulate in vivo protein accumulation, gene expression, and degradative processes such as autophagy. Steller’s jays (Cyanocitta stelleri) with swollen legs and feet resembling the signs of scaly leg have been commonly seen around Arcata, California, USA. These signs are thought to be caused by knemidokoptic mites, a group of parasites specialized at exploiting a range of avian hosts. Detailed comprehensive knowledge of the structures of individual long-range telomere-terminal haplotypes are needed to understand their impact on telomere function, and to delineate the population structure and evolution of subtelomere regions. However, the abundance of large evolutionarily recent segmental duplications and high levels of large structural variations have complicated both the mapping and sequence characterization of human subtelomere regions. Here, we use high throughput optical mapping of large single DNA molecules in nanochannel arrays for 154 human genomes from 26 populations to present a comprehensive look at human subtelomere structure and variation. Background: Phenotypic heterogeneity and complicated gene-environment interplay in etiology are among the primary factors that hinder the identification of genetic variants associated with cocaine use disorder. Methods: To detect novel genetic variants associated with cocaine use disorder, we derived disease traits with reduced phenotypic heterogeneity using cluster analysis of a study sample (n = 9965). We then used these traits in genome-wide association tests, performed separately for 2070 African Americans and 1570 European Americans, using a new mixed model that accounted for the moderating effects of 5 childhood environmental factors. Skeletal muscle atrophy can result from a range of physiological conditions, including denervation, immobilization, hindlimb unweighting, and aging. To better characterize the molecular genetic events of atrophy, a microarray was performed using skeletal muscle isolated from mice after 3 and 14 days of denervation and compared to control muscle to identify novel atrophy-induced genes. The microarray revealed that FGGY carbohydrate kinase domain containing (Fggy) is expressed in skeletal muscle and is induced in response to denervation. Bioinformatic analysis of the Fggy gene locus revealed two validated alternative isoforms, that we have termed Fggy-L-552 and Fggy-S-387, which have distinct transcription initiation …

Functional Analysis Of Fam83d And Dupd1 – Two Novel Neruogenic Skeletal Muscle Atrophy-Induced Genes, Lisa Michelle Cooper

Fam83d and Dupd1 have been identified as novel genes in skeletal muscle that are upregulated in response to neurogenic atrophy in a mouse model. qPCR analysis reveaed both genes are expressed in skeletal muscle with Fam83d expression being highest during myoblast proliferation, while Dupd1 expression is highest during myotube differentiation. Overexpression of either protein results in inhibition of proper muscle cell differentiation as evidenced by repression of both myosin heavy chain and myogenin expression. Characterization of transcriptional activity revealed both genes are modulated by myogenic regulatory factors and additionally, Dupd1 expression is enhanced by dexamethasone treatment. Klebsiella pneumoniae is a Gram-negative bacterium responsible for nosocomial infections such as blood stream infections, meningitis, and septicemia. Clinical isolates confirm K. pneumoniae is increasingly antibiotic resistant (AR) making treatment more difficult. While bacterial exposure to antimicrobial substances is a known cause of the AR phenotype, there still exists gaps in knowledge about the genetic changes responsible for this fitness change. The laboratory Klebsiella pneumoniae 43816 (ATCC 43816) was exposed to sublethal concentrations of Cephalothin for a 14-day period. It is usually associated with DNA methylation changes during tumorigenesis. ความเป็นมา: การเพิ่มขึ้นของสาร trimethylamine N-oxide (TMAO) ซึ่งเป็นสารพิษที่มาจากแบคทีเรียในลำไส้ของผู้ป่วยโรคไตเรื้อรัง (chronic kidney disease; CKD) อาจช่วยเพิ่มการผลิต TMAO reductase ซึ่งเป็นเอนไซม์ของแบคทีเรียในลำไส้และมีการแทรกผ่านไปยังกระแสเลือดอันเนื่องมาจากข้อบกพร่องในการซึมผ่านของลำไส้ (gut leakage) และอาจนำมาใช้เป็นตัวบ่งชี้ภาวะหลอดเลือดแข็งตัวในผู้ป่วยโรคไตเรื้อรัง วัตถุประสงค์: เพื่อสำรวจความสัมพันธ์ระหว่าง serum TMAO, serum TMAO reductase, gut leakage, ระบบการอักเสบและภาวะหลอดเลือดแข็งตัวในผู้ป่วยโรคไตเรื้อรัง วิธีการ: ทำการวิเคราะห์แบบตัดขวางในตัวอย่างเลือดจากผู้ป่วยโรคไตเรื้อรังที่เข้ารับการฟอกเลือดเป็นประจำ (n = 48) และใช้ตัวอย่างของผู้ที่มีสุขภาพดีในการควบคุม (n = 20) โดยวัดค่าการทำงานของตับ (serum glutamic-oxaloacetic transaminase; SGOT และ serum glutamate-pyruvate transaminase; SGPT) วิเคราะห์หาค่าสารพิษทั้งหมดของภาวะ uremia toxin ได้แก่ สารที่มีโมเลกุลขนาดเล็ก (BUN และ creatinine) สารที่มีโมเลกุลขนาดกลาง (beta-2 microglobulin; B2M) สารที่มีโมเลกุลที่จับกับโปรตีน (serum TMAO) วิเคราะห์หาข้อบกพร่องในการซึมผ่านของลำไส้ได้แก่ serum LPS และ serum TMAO reductase วิเคราะห์หาการตอบสนองต่อการอักเสบ (serum C-reactive protein, serum TNF-α และ serum IL-6) และประเมินภาวะหลอดเลือดแข็งตัวโดยใช้ดัชนี ankle-brachial index (ABI) และดัชนี cardio-ankle vascular index (CAVI) ผลการวิจัย: ในผู้ป่วยโรคไตเรื้อรังมีระดับ serum TMAO และ serum TMAO reductase รวมทั้งระดับสารพิษทั้งหมดของภาวะ uremia toxin และระดับการตอบสนองต่อการอักเสบสูงขึ้นมากกว่ากลุ่มควบคุม นอกจากนี้ยังตรวจพบภาวะ endotoxemia …

การตรวจความเข้ากันได้ของ Human Leukocyte Antigen (HLA) ก่อนการปลูกถ่ายไตทำขึ้นเพื่อตรวจจับ Donor Specific Antibody (DSA) ต่อผู้บริจาค ที่อาจก่อให้เกิดปฏิกิริยาปฏิเสธอวัยวะ โดยเทคนิคที่ใช้ในปัจจุบันคือเทคนิค Complement-dependent microcytotoxicity crossmatch (CDC-XM) และ Anti-Human Globulin Complement-dependent microcytotoxicity crossmatch (AHG-CDC) มีความไวจำกัด ไม่สามารถตรวจจับ DSA ปริมาณน้อยที่ส่งผลต่ออัตราการรอดของไตได้ ขณะที่เทคนิคโฟลไซโตเมทรี (Flow cytometry crossmactch, FCXM) มีความไวสูงกว่า แต่พบปัญหาผลบวกปลอมจากแอนติบอดีที่ไม่เกี่ยวข้องทำให้ผู้ป่วยเสียโอกาสที่จะได้รับการปลูกถ่ายไต งานวิจัยนี้จึงมีวัตถุประสงค์เพื่อปรับปรุงความไวและความจำเพาะของเทคนิค FCXM โดยศึกษาในลิมโฟซัยต์จากผู้บริจาคไตสมองตายกับน้ำเหลืองตัวอย่างที่ทราบผล DSA จำนวน 20 ตัวอย่างเพื่อประเมินเทคนิคแล้วทดสอบเปรียบเทียบกับเทคนิค CDC,AHG-CDC ในกลุ่มผู้ป่วยจำนวน 199 ตัวอย่าง พบว่าในขั้นตอนการประเมินผลเทคนิค FCXM มีความไวและความจำเพาะในกลุ่ม T cell 71% และ 100% ตามลำดับ ส่วน B cell พบว่าความไวเท่ากับ 75% และความจำเพาะ 100% และสอดคล้องกับผล DSA อย่างมีนัยสำคัญทางสถิติ (p< 0.05) เมื่อทดสอบกับผู้ป่วยให้ผลสอดคล้องกับเทคนิค CDC, AHG-CDC จำนวน 190 ตัวอย่าง คิดเป็น 95.47% พบผลบวกปลอม จำนวน 8 ตัวอย่าง คิดเป็น 4.0% ผลการทดสอบความเข้ากันได้และประเมินประสิทธิภาพเทคนิค FCXM นี้สรุปได้ว่าเหมาะสมที่จะนำไปทดลองใช้ทดแทนเทคนิค CDC และ AHG-CDC แต่ควรทำการศึกษาเพิ่มเติมในกลุ่มที่มี DSA ก่อนนำไปใช้จริงในอนาคต

วัคซีนที่มีความปลอดภัยก่อนนำไปใช้ในมนุษย์ต้องปราศจากเชื้อปนเปื้อนแฝง (extraneous agents) โดยเชื้อ Avian adenoviruses เป็นเชื้อปนเปื้อนแฝงชนิดหนึ่งที่อาจปนเปื้อนมากับไข่ไก่ฟักซึ่งเป็นวัตถุดิบในการผลิตวัคซีนไข้หวัดใหญ่ชนิดเชื้อตาย (Inactivated Influenza Vaccine, IIV) การตรวจหาเชื้อ Avian adenoviruses ในปัจจุบันทำการทดสอบโดยวิธีเซลล์เพาะเลี้ยง ซึ่งวิธีดังกล่าวสามารถตรวจได้เฉพาะเชื้อ Avian adenovirus ในกลุ่มที่ 1 (Fowl adenovirus) ใช้แรงงานจำนวนมาก และใช้เวลาในการทดสอบนาน ด้วยเหตุนี้จึงได้พัฒนาวิธี nested PCR และ real-time PCR เปรียบเทียบกับวิธีเซลล์เพาะเลี้ยงในการตรวจหาการปนเปื้อนของเชื้อ Avian adenoviruses ในวัคซีนไข้หวัดใหญ่ชนิดเชื้อตาย โดยพบว่าวิธี nested PCR และ real-time PCR สามารถตรวจหาเชื้อ Avian adenoviruses ในกลุ่มที่ 1 ได้แก่ Fowl adenovirus ซีโรไทป์ 1 2 และ 4 เชื้อ Hemorrhagic enteritis virus ในกลุ่มที่ 2 และเชื้อ Egg drop syndrome ในกลุ่มที่ 3 ได้อย่างจำเพาะ โดยไม่เกิดปฏิกิริยาข้ามกัน (cross-reaction) กับเชื้อก่อโรคชนิดอื่นในไก่ ได้แก่เชื้อ Mycoplasma gallisepticum Mycoplasma synoviae Infectious laryngotracheitis virus Chicken anemia virus Chicken bronchitis virus โดยวิธีเซลล์เพาะเลี้ยง nested PCR และ real-time PCR มีความไวในการตรวจหาเชื้อ Fowl adenovirus 1 ซึ่งมีการติดเชื้อในไก่แบบแฝงและตรวจพบได้ยาก ได้เท่ากับ 10-2 10-8 และ 10-18 TCID50/0.1ml …

เชื้อแบคทีเรีย Mycobacterium tuberculosis เป็นสาเหตุของวัณโรค ซึ่งเป็นโรคติดเชื้อที่เป็นปัญหาทางสาธารณสุขทั้งในประเทศไทยและในระดับโลก โดยเฉพาะเมื่อโรคพัฒนาสู่ภาวะวัณโรคดื้อยาหลายขนาน (Multidrug-resistant Tuberculosis: MDR-TB) ยา Isoniazid เป็นหนึ่งในยาขนานแรกที่มีประสิทธิภาพสูงสุดในการรักษาผู้ป่วยวัณโรค ดังนั้นการดื้อยา Isoniazid จึงเป็นอุปสรรคที่สำคัญในการควบคุมการแพร่ระบาดของวัณโรค และยังพบว่าผู้ป่วยวัณโรคที่ดื้อต่อยา Isoniazid มีโอกาสสูงที่จะพัฒนาไปเป็นผู้ป่วยวัณโรคดื้อยาหลายขนาน และมีโอกาสในการรักษาสำเร็จที่ลดลง สาเหตุหลักของการดื้อยา Isoniazid จากการกลายพันธุ์ของยีน katG และยีน inhA ตามลำดับ การศึกษานี้ได้พัฒนาเทคนิค Multiplex Allele Specific Polymerase Chain Reaction (MAS-PCR) ร่วมกับแผ่นทดสอบ Dipstick Chromatography เพื่อใช้วินิจฉัยการกลายพันธุ์ของยีน katG ที่ตำแหน่งโคดอน 315 และยีน inhA ที่ตำแหน่งเหนือยีน (-15) ของเชื้อ M. tuberculosis ซึ่งสัมพันธ์กับการดื้อยา Isoniazid ผลการทดสอบกับดีเอ็นเอที่สกัดได้จากโคโลนีของเชื้อ M. tuberculosis จำนวน 250 ตัวอย่าง พบว่าเทคนิค MAS-PCR ร่วมกับแผ่นทดสอบ Dipstick Chromatography มีความไวและความจำเพาะในการวินิจฉัยการกลายพันธุ์ที่สัมพันธ์กับการดื้อยา Isoniazid เท่ากับ 91.67% และ 100% ตามลำดับ เมื่อเปรียบเทียบกับวิธีทดสอบทางฟีโนไทป์ ในขณะที่เมื่อเปรียบเทียบกับเทคนิค Sanger DNA sequencing ในการตรวจหากลายพันธุ์ของยีน katG ณ ตำแหน่งโคดอน 315 มีค่าความไวและความจำเพาะเท่ากับ 100% และ 99.28% ตามลำดับ และในการตรวจหายีน inhA ที่ตำแหน่งเหนือยีน (-15) มีค่าความไวและความจำเพาะเท่ากับ 100% และ 99.51% ตามลำดับ โดยเทคนิค MAS-PCR ร่วมกับแผ่นทดสอบ Dipstick Chromatography มีค่าความสอดคล้องในระดับดีมากกับทุกเทคนิคที่นำมาเปรียบเทียบ เทคนิค MAS-PCR …

วัณโรคเป็นโรคติดต่อที่มีการระบาดทั่วโลก และยังคงเป็นปัญหาสาธารณสุขที่สำคัญของประเทศไทย วัณโรคยังเป็นสาเหตุของการเสียชีวิตจากโรคติดต่อสูงเป็นอันดับสองรองจากโรคเอดส์ โดยมีสาเหตุมาจากเชื้อแบคทีเรีย Mycobacterium tuberculosis complex (MTBC) การตรวจวินิจฉัยวัณโรคทางห้องปฏิบัติการ เป็นหนึ่งปัจจัยที่สำคัญในการควบคุมการแพร่ระบาดของโรค การนำเทคนิคทางอณูชีววิทยาเข้ามาใช้วินิจฉัยทางห้องปฏิบัติการ ซึ่งมีความรวดเร็วมากกว่าการเพาะเลี้ยงเชื้อด้วยวิธีดั้งเดิมและสามารถวินิจฉัยโรคได้ภายใน 1 วัน มีบทบาทที่สำคัญในการวินิจวัณโรคในปัจจุบัน การศึกษานี้มีวัตถุประสงค์ในการพัฒนาแถบตรวจ MTB Strip แบบอ่านผลด้วยตาเปล่าอย่างรวดเร็ว สำหรับวินิจฉัยวัณโรคจากปฏิกิริยา Multiplex-recombinase polymerase amplification (M-RPA) โดยใช้ไพรเมอร์ที่จำเพาะต่อ IS1081 และ IS6110 ซึ่งมีความจำเพาะต่อเชื้อ MTBC และตรวจสอบผลผลิตของทั้งสอง IS ร่วมกัน เพื่อเพิ่มความไวของเทคนิคในการวินิจฉัยวัณโรค การทดสอบเบื้องต้นกับดีเอ็นเอที่สกัดจากโคโลนีของเชื้อ แสดงให้เห็นว่าเทคนิค M-RPA ร่วมกับแถบตรวจ MTB Strip สามารถจำแนกเชื้อ MTBC ออกจาก NTM ได้อย่างถูกต้อง เมื่อนำเทคนิค M-RPA ร่วมกับแถบตรวจ MTB Strip มาวินิจฉัยกับดีเอ็นเอที่สกัดจากสิ่งส่งตรวจเสมหะจำนวน 131 ตัวอย่าง พบว่ามีความไวและความจำเพาะ เท่ากับ 91.03% และ 84.91% ตามลำดับ โดยมีค่าความสอดคล้องในเกณฑ์ดี (κ=0.762) เมื่อเปรียบเทียบกับการย้อมสีทนกรด ในขณะที่มีความไวและความจำเพาะเท่ากับ 100.0% และ 94.55% ตามลำดับ และมีค่าความสอดคล้องในเกณฑ์ดีมาก (κ=0.953) เมื่อเปรียบเทียบกับเทคนิค Real-time PCR เทคนิค M-RPA ร่วมกับแถบตรวจ MTB Strip ใช้ระยะเวลาทำปฏิกิริยาเพียง 25 นาที ภายใต้อุณหภูมิ 37 องศาเซลเซียส และตรวจสอบผลผลิตด้วยแถบตรวจ MTB Strip ภายในเวลา 15 นาที โดยตรวจสอบความเข้มข้นดีเอ็นเอตั้งต้นน้อยที่สุดได้เท่ากับ 0.1 ng/µl และไม่เกิดปฏิกิริยาข้ามกลุ่มเชื้อแบคทีเรียสายพันธุ์อื่นที่มักก่อโรคในระบบทางเดินหายใจ ดังนั้นเทคนิค M-RPA ร่วมกับแถบตรวจ MTB Strip จึงเป็นเทคนิคที่มีความรวดเร็ว ไม่ซับซ้อน …

ในปี 2018 มีการค้นพบ hybrid GP(B-A-B) ชนิดใหม่ซึ่งเกิดจากจีโนไทป์ แบบ homozygous GYP*Mur สร้างเป็นแอนติเจนใหม่เรียก JENU ลบ ในประชากรชาวไทย ดังนั้นเพื่อศึกษาความถี่ของจีโนไทป์แบบ hydrid GYP*Mur และ hybrid GYP ชนิดอื่นๆ ผู้วิจัยอาศัยเทคนิค Polymerase chain reaction High-Resolution Melting (PCR HRM) ตัวอย่างดีเอ็นเอที่สกัดได้จากผู้บริจาคโลหิตชาวไทย จำนวน 60 ราย ที่ให้ผลตรวจแอนติเจน s เป็นลบปลอมกับน้ำยา monoclonal IgM clone P3BER ผลค่า melting temperature จากตัวอย่างพีซีอาร์ที่ได้จะนำมาเปรียบเทียบกับตัวอย่าง homozygous GYP*Mur การหาลำดับเบสใช้ในตัวอย่างที่เป็น non-homozygous GYP*Mur ทุกตัวอย่าง จากผลการทดลอง พบว่า ผู้บริจาคโลหิตจำนวน 49 รายเป็น homozygous GYP*Mur (Tm เฉลี่ย 79.63±0.03, 90% Cl =79.337-81.302 ) และ อีก 11 รายเป็น non-homozygous GYP*Mur เมื่อเปรียบเทียบลำดับเบสของตัวอย่างดังกล่าวกับฐานข้อมูล พบว่า ตัวอย่างพบว่า 4 รายเป็น heterozygous GYP*Mur/Bun, 2 ราย เป็น homozygous GYP*Bun และ 5 ราย เป็น heterozygous GYP*BS/GYP*Bun สรุปผลการศึกษาครั้งนี้ คือ ความถี่ของแอนติเจน JENU ลบ คิดเป็นร้อยละ 0.65 ของประชากรชาวไทย โดยเทคนิค PCR HRM สามารถนำไปใช้ในงานประจำวันทางธนาคารเลือดได้จริง

โรคเบาหวานเป็นหนึ่งในปัญหาด้านสาธารณสุขทั่วโลกโดยพบผู้ป่วยโรคเบาหวานเพิ่มขึ้นอย่างต่อเนื่อง โรคเบาหวานเพิ่มความเสี่ยงต่อการติดเชื้อแบคทีเรีย เชื้อรา ไวรัสและปรสิต การศึกษาแบบภาคตัดขวางครั้งนี้มีวัตถุประสงค์เพื่อศึกษาความชุก ชนิดสายพันธุ์ และปัจจัยเสี่ยงต่อการติดเชื้อบลาสโตซิสติสของผู้ป่วยโรคเบาหวานและผู้ที่ไม่เป็นโรคเบาหวานในพื้นที่อำเภอบางปะอิน จังหวัดพระนครศรีอยุธยา ในช่วงเดือนพฤศจิกายน พ.ศ. 2562 ถึงเดือนกุมภาพันธ์ พ.ศ. 2563 ตัวอย่างอุจจาระและแบบสอบถามเก็บรวบรวมจากผู้ป่วยโรคเบาหวาน 130 คนและผู้ที่ไม่เป็นโรคเบาหวาน 100 คน โดยนำตัวอย่างอุจจาระทั้งหมดมาตรวจหาเชื้อบลาสโตซิสติส ด้วยวิธี simple smear technique และตรวจหายีน small subunit ribosomal DNA ของเชื้อบลาสโตซิสติสด้วยเทคนิค nested PCR และจำแนกสายพันธุ์ด้วยเทคนิค Sanger sequencing วิเคราะห์ความสัมพันธ์ของปัจจัยเสี่ยงต่อการติดเชื้อบลาสโตซิสติสโดยใช้การทดสอบไคสแควร์และอัตราส่วน odds (odds ratio) ที่ช่วงเชื่อมั่น 95% ผลการศึกษาพบว่า พบผู้ติดเชื้อบลาสโตซิสติสจำนวน 25 คน จากตัวอย่างอุจจาระทั้งหมด 230 คน คิดเป็นความชุกของการติดเชื้อบลาสโตซิสติสร้อยละ 10.9 การติดเชื้อชนิดนี้ในกลุ่มผู้ป่วยโรคเบาหวานและผู้ที่ไม่เป็นโรคเบาหวานคิดเป็นร้อยละ 12.3 และร้อยละ 9 ตามลำดับ ในขณะที่สายพันธุ์ของเชื้อบลาสโตซิสติสที่ตรวจมากที่สุดประกอบด้วย สายพันธุ์ที่ 3 สายพันธุ์ที่ 1 และสายพันธุ์ที่ 4 ตามลำดับ การศึกษาปัจจัยเสี่ยง ได้แก่ ภาวะโรคเบาหวาน เพศ อายุ ระดับการศึกษา อาชีพ ประเภทห้องน้ำ การมีสัตว์เลี้ยง ความถี่ของการรับประทานอาหารสุกๆดิบๆ การล้างผักและผลไม้ก่อนรับประทาน การล้างมือก่อนและหลังทำกิจกรรมต่างๆ และการขับถ่ายโดยใช้ห้องส้วม ไม่มีความสัมพันธ์ต่อการติดเชื้อบลาสโตซิสติสอย่างมีนัยสำคัญทางสถิติ แต่พบว่ามีแนวโน้มการติดเชื้อบลาสโตซิสติสที่สูงในเพศชาย ผู้ที่มีอายุมากกว่าหรือเท่ากับ 65 ปี ผู้ที่เป็นโรคเบาหวาน ระยะเวลาการเป็นโรคเบาหวานตั้งแต่ 10 ปีขึ้นไป ผู้ที่ใช้ห้องส้วมแบบราดน้ำหรือนั่งยอง รวมทั้งผู้ที่มีสัตว์เลี้ยง ผลของการศึกษานี้เป็นข้อมูลสำคัญที่เป็นประโยชน์เพื่อใช้ในการกำหนดแนวทางการควบคุมและป้องกันการติดเชื้อดังกล่าวในกลุ่มผู้ป่วยโรคเบาหวานและผู้ที่ไม่เป็นโรคเบาหวานในชุมชนต่อไป

โรคหนองในเป็นโรคติดต่อทางเพศสัมพันธ์ที่เกิดจากการติดเชื้อไนซีเรีย โกโนเรีย (Neisseria gonorrhoeae) โดยมีอุบัติการณ์มากที่สุดในกลุ่มของโรคติดต่อทางเพศสัมพันธ์ในประเทศไทยและทั่วโลก จากการรายงานขององค์การอนามัยโลก ผู้ป่วยจำนวนมากไม่แสดงอาการของโรคโดยเฉพาะในผู้ป่วยหญิง ผู้ป่วยที่ไม่ได้รับการรักษาจะทำให้เกิดการติดเชื้อของระบบสืบพันธุ์ส่วนบนและนำไปสู่การเป็นหมันและการตั้งครรภ์นอกมดลูก พยาธิสภาพของโรคเกิดจากการเหนี่ยวนำให้เกิดการรวมกลุ่มของเซลล์ในระบบภูมิคุ้มกันและกระบวนการอักเสบโดยเซลล์แมคโครฟาจซึ่งเป็นเซลล์สำคัญในระบบภูมิคุ้มกันแบบกำเนิด การตอบสนองของเซลล์ต่อการติดเชื้อภายในเซลล์สามารถติดตามได้จากเอ็กต์ตราเซลลูลาร์เวสิเคิลที่ผลิตออกจากเซลล์แมคโครฟาจ เช่น การติดเชื้อวัณโรค ลิชมาเนีย และซัลโมเนลา ที่พบการเปลี่ยนแปลงส่วนประกอบโปรตีนของเซลล์แมคโครฟาจและของเชื้อภายในเซลล์ การปลดปล่อยเวสิเคิลนั้นจะมีผลต่อการกระตุ้นการทำงาน กระบวนการอักเสบ และปรับเปลี่ยนการทำงานของเซลล์ในระบบภูมิคุ้มกันได้ ในการศึกษานี้จึงมุ่งเน้นการแสดงออกและค้นหาโปรตีนในเอ็กต์ตราเซลลูลาร์เวสิเคิลจากเซลล์แมคโครฟาจทั้งชนิดติดเชื้อและไม่ติดเชื้อ N. gonorrhoeae โดยนำเซลล์เม็ดเลือดขาว THP-1 ที่ถูกกระตุ้นให้เป็นเซลล์แมคโครฟาจเกิดกระบวนการฟาโกไซโทซิสเชื้อ N. gonorrhoeae เข้าไปภายในเซลล์ ติดตามการแสดงออกของโปรตีนภายในเวสิเคิล และวิเคราะห์โปรตีนด้วย Tandem mass spectrometry ซึ่งพบการเปลี่ยนแปลงส่วนประกอบโปรตีนภายในเวสิเคิลที่ครอบคลุมโปรตีนของเซลล์แมคโครฟาจและของเชื้อ ทั้งยังพบว่ามีความแตกต่างกันทั้งชนิดและการแสดงออกจำเพาะในช่วงเวลาของการติดเชื้อ เมื่อนำผลวิเคราะห์เปรียบเทียบกับฐานข้อมูลชีวสารสนเทศทั้ง Vesiclepedia, PANTHER และ Uniprot database ศึกษาคุณสมบัติและหน้าที่ของโปรตีนภายในเวสิเคิล โปรตีนของมนุษย์ และโปรตีนของเชื้อ N. gonorrhoeae ตามลำดับ ทำให้บ่งชี้ถึงความสัมพันธ์และบทบาทต่อเซลล์แมคโครฟาจในช่วงเวลาที่ทดสอบ ผลการศึกษานี้สามารถนำไปเป็นข้อมูลเพื่อพัฒนาการตรวจติดตามเชื้อก่อโรคหนองในและองค์ความรู้ในความเข้าใจบทบาทของเอ็กต์ตราเซลลูลาร์เวสิเคิลต่อพยาธิสภาพและการดำเนินไปของโรคหนองใน

Acinetobacter baumannii เป็นเชื้อที่สำคัญที่ก่อให้เกิดการติดเชื้อภายในโรงพยาบาลและมีการดื้อยาปฏิชีวนะหลายชนิดเพิ่มมากขึ้น โดยการสร้างเอนไซม์ทำลายยาเป็นกลไกการดื้อยาที่มีความสำคัญและพบได้บ่อย ในการศึกษาครั้งนี้ได้ทำในเชื้อ A. baumannii ที่แยกได้จากสิ่งส่งตรวจตั้งแต่เดือนกรกฎาคม 2561 ถึง กุมภาพันธ์ 2562 จำนวน 257 ตัวอย่าง นำมาตรวจหาความชุกของยีนที่สร้างเอนไซม์ beta-lactamase เอนไซม์ cephalosporinase เอนไซม์ aminoglycoside-modifying enzymes (AMEs) และอินเซอร์ชันซีเควน (ISAba125) โดยปฏิกิริยาลูกโซ่พอลิเมอเรส และหาความหลากหลายทางสายพันธุ์ของเชื้อด้วยวิธี repetitive element polymerase chain reaction (REP-PCR) ผลการศึกษาพบว่าเชื้อ A. baumannii 221 ตัวอย่าง (ร้อยละ 86.0) เป็นเชื้อที่ดื้อยาหลายชนิด (multidrug-resistant A. baumannii : MDR-AB) โดยเชื้อ A. baumannii 257 ตัวอย่าง ให้ผลบวกกับยีน blaOXA-51 คิดเป็นร้อยละ 100.0 รองลงมาได้แก่ ยีน blaADC ยีน blaNDM-1 ยีน ISAba125 และยีน aphA6 คิดเป็นร้อยละ 38.5, 8.2, 6.2 และ 2.3 ตามลำดับ โดยพบ blaOXA-51 ร่วมกับยีน blaADC มากที่สุดคิดเป็นร้อยละ 35.0 ในการศึกษาครั้งนี้สามารถจำแนกความหลากหลายทางสายพันธุ์ของเชื้อ A. baumannii ด้วยวิธี rep-pcr ได้ 18 กลุ่ม โดยพบว่ากลุ่มที่ 1 และ กลุ่มที่ 2 ซึ่งเป็นกลุ่มหลักที่ใหญ่ที่สุด กลุ่มที่ 1 พบยีน blaOXA-51 (ร้อยละ 59.7) และ blaOXA-51 ร่วมกับยีน …

เชื้อ Helicobacter pylori เป็นแบคทีเรียก่อโรคในระบบทางเดินอาหารของมนุษย์ที่มีความสำคัญทางคลินิก การติดเชื้อ H. pylori มีความสัมพันธ์กับการเกิดเป็นโรคมะเร็งกระเพาอาหาร เนื่องจากเชื้อมีปัจจัยความรุนแรงในการก่อโรคหลายชนิด อาทิ ความสามารถในการสร้างเอนไซม์ยูรีเอสปริมาณมาก การสร้างสารพิษประเภท CagA และ VacA การใช้แฟลเจลลาในการเคลื่อนที่แบบควงสว่านเข้าสู่ชั้นเยื่อเมือกของกระเพาะอาหาร การยึดเกาะกับเซลล์เยื่อบุกระเพาะอาหาร รวมทั้งการกระตุ้นให้เซลล์เกิดการตายแบบอะพอพโทซิสที่นำมาสู่การพัฒนากลายเป็นโรคมะเร็งกระเพาะอาหารได้ แฟลเจลลาของเชื้อ H. pylori มีส่วนช่วยในการเคลื่อนที่และเป็นสารที่ใช้ในการยึดเกาะที่สำคัญของเชื้อ อีกทั้งยังถูกสันนิษฐานว่าอาจมีส่วนกระตุ้นในการทำให้เซลล์เยื่อบุผิวเกิดการตาย การควบคุมการสร้างและการทำงานของแฟลเจลลาเกิดขึ้นจากการทำงานร่วมกันของยีนหลายชนิด โดยมียีน flaA ทำหน้าที่ควบคุมการสร้างโปรตีนแฟลเจลลินหลักของแฟลเจลลา และยีน fliD ทำหน้าที่ควบคุมการสร้าง capping protein หุ้มส่วนปลายของแฟลเจลลา การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลกระทบของยีน flaA และยีน fliD ของเชื้อ H. pylori ต่อการเคลื่อนที่ของเชื้อ การยึดเกาะและการกระตุ้นให้เกิดการตายแบบอะพอพโทซิสของเซลล์เยื่อบุผิว Human gastric adenocarcinoma (AGS) โดยทำการศึกษาลักษณะของสายแฟลเจลลาวิธีย้อมด้วยสี leifson-tannic acid fuchsin และด้วยกล้องจุลทรรศน์อิเล็กตรอนแบบส่องผ่าน เปรียบเทียบกันระหว่างเชื้อ H. pylori สายพันธุ์มาตรฐาน ATCC 43504 และเชื้อ H. pylori สายพันธุ์ที่มีการกลายพันธุ์ของยีน flaA และ ยีน fliD เชื้อดังกล่าวทั้งหมดยังถูกนำมาทดสอบการเคลื่อนที่ในอาหารกึ่งแข็งกึ่งเหลว และเพาะเลี้ยงร่วมกับเซลล์ AGS เพื่อทดสอบการยึดเกาะกับเซลล์ด้วยวิธี adhesion assay และการกระตุ้นให้เซลล์ที่ติดเชื้อตายด้วยการย้อมสี Annexin/ PI และวัดสัญญาณด้วย flow cytometry พบว่าเชื้อ H. pylori ที่มีการกลายพันธุ์ทั้งสองสายพันธุ์ไม่สามารถสร้างสายแฟลเจลลาที่มีรูปร่างสมบูรณ์ได้ ทั้งนี้เชื้อ H. pylori ที่มีการกลายพันธุ์ของยีน flaA มีความสามารถในการเคลื่อนที่และการยึดเกาะกับเซลล์ AGS ลดลงอย่างมีนัยสำคัญ ในขณะที่เชื้อ H. pylori ที่มีการกลายพันธุ์ของยีน fliD สามารถยึดเกาะเซลล์ AGS ได้มากขึ้นเมื่อเปรียบเทียบกับเชื้อ H. pylori สายพันธุ์มาตรฐาน …

ซิฟิลิสเป็นโรคติดเชื้อที่มีสาเหตุมาจากเชื้อแบคทีเรีย Treponema pallidum ผ่านทางกระแสโลหิต สารคัดหลั่งของร่างกาย ซึ่งสามารถติดต่อจากมารดาไปยังทารกในครรภ์ อีกทั้งสามารถติดต่อทางเพศสัมพันธ์ได้ ซึ่งการติดเชื้อดังกล่าวส่งผลกระทบต่อพยาธิสภาพของร่างกายหลายระบบ นอกจากนี้ซิฟิลิสยังสามารถถ่ายทอดผ่านการรับโลหิตและผลิตภัณฑ์ของโลหิตจากผู้บริจาคที่ติดเชื้อ ด้วยเหตุนี้งานทางธนาคารโลหิตจึงจำเป็นต้องตรวจคัดกรองซิฟิลิสในโลหิตของผู้บริจาคโลหิตทุกราย โดยนิยมใช้เทคนิคการตรวจทางภูมิคุ้มกันวิทยา การศึกษาครั้งนี้มีวัตถุประสงค์เพื่อสร้างเครื่องต้นแบบบันทึกภาพและซอฟต์แวร์ที่ออกแบบโดยใช้ภาษา C# ที่สามารถบันทึกภาพ อ่านและแปลผลผลปฏิกิริยา ของการทดสอบทางภูมิคุ้มกันวิทยาด้วยวิธี Treponema pallidum Particle Agglutination (TPPA) และเปรียบเทียบประสิทธิภาพการอ่านและแปลผลระหว่างเครื่องต้นแบบ กับการอ่านและแปลผลด้วยตาเปล่าโดยผู้ปฏิบัติงาน การศึกษาใช้ตัวอย่างพลาสมาของผู้ป่วยที่เข้ารับการรักษาที่คลีนิคนิรนาม ศูนย์วิจัยโรคเอดส์ สภากาชาดไทย และพลาสมาของผู้บริจาคโลหิต ที่ได้รับความอนุเคราะห์จาก ศูนย์บริการโลหิตแห่งชาติ สภากาชาดไทย จำนวนทั้งสิ้น 65 ตัวอย่าง ซึ่งทั้งหมดได้ผ่านการตรวจซิฟิลิสด้วยวิธี Chemiluminescent Microparticle Immunoassay (CMIA) พบว่า เมื่อนำเครื่องบันทึกภาพต้นแบบและซอฟท์แวร์ที่พัฒนาขึ้น มาอ่านปฏิกิริยา TPPA เปรียบเทียบกับการอ่านผลด้วยตาเปล่าโดยเจ้าหน้าที่ผู้ปฏิบัติงาน พบว่าให้ผลตรงกันในแต่ละระดับของปฏิกิริยาจำนวน 60 ตัวอย่าง และไม่ตรงกันในแต่ละระดับของปฏิกิริยาจำนวน 5 ตัวอย่าง ส่งผลให้มีความสอดคล้องของทั้งสองเมื่อวิเคราะห์ด้วยสถิติ Kappa = 0.88 แสดงให้เห็นถึงความสอดคล้องในระดับดีมาก อย่างไรก็ตามหากพิจารณาเฉพาะการอ่านผล reactive โดยมิได้คำนึงถึงระดับปฏิกิริยา พบว่าทั้งสองวิธีอ่านให้ผลปฏิกิริยาตรงกัน 64 ตัวอย่าง และไม่ตรงกันจำนวนเพียง 1 ตัวอย่าง ส่งผลให้มีความสอดคล้องของทั้งสองเมื่อวิเคราะห์ด้วยสถิติ Kappa = 0.98 แสดงให้เห็นถึงว่าความสอดคล้องในระดับดีมาก ดังนั้น เครื่องบันทึกภาพต้นแบบและซอฟท์แวร์ที่พัฒนาขึ้น สามารถช่วยอ่านและแปลผลปฏิกิริยาของการทดสอบด้วยวิธี TPPA ในการตรวจวินิจฉัยโรคซิฟิลิสได้อย่างมีประสิทธิภาพ ใช้เวลาอ่านผลที่รวดเร็วกว่าการอ่านผลโดยผู้ปฏิบัติงาน ไม่มีความคลาดเคลื่อนในการอ่านผลที่อาจเกิดจากปัจจัยของผู้ปฏิบัติงาน ใช้วัสดุอุปกรณ์ราคาถูก สามารถนำไปพัฒนาต่อยอดและประยุกต์ใช้ยังห้องปฏิบัติการทั่วไปได้
