Purpose: The sustainability of clinical pharmacogenomics requires further study of clinical education on the topic, its effects on clinical workflow, and the responsibilities of different providers for its delivery. Tools from the discipline of implementation science were utilized herein to help achieve the purposes of the three studies. The broad purpose of this dissertation is to advance the work of clinical pharmacogenomic implementation through a more rigorous convergence with implementation science.

The health of a newborn baby is inextricably linked to the health status of its mother and in turn the mother’s diet during pregnancy. Leptin (LEP) is an adipokine hormone involved in metabolism regulation and has been linked fetal development through the hypothalamic-pituitary-adrenal axis (HPA). Prior work suggests that gestational epigenetic alterations the LEP gene may be sensitive to adverse exposures during pregnancy, which in turn could explain variation in neonate outcomes. Transcription termination factor Rho is an essential protein in Escherichia coli and related bacteria. The primary function of Rho is to clear unproductive RNA polymerases from the DNA template to minimize negative effects associated with uncontrolled transcription. Although most of the Rho termination events are constitutive, premature Rho-mediated termination was observed at 3% of all affected transcripts indicating active regulation of Rho activity. In this work, we investigated the regulatory mechanism behind premature Rho-dependent transcription termination in two unrelated genes: suhB and topAI. Genomic instability, in the form of gene mutations, insertions/deletions, and gene amplifications, is one of the hallmarks in many types of cancers and other inheritable genetic disorders. Trinucleotide repeat (TNR) disorders, such as Huntington’s disease (HD) and Myotonic dystrophy (DM) can be inherited and repeats may be extended through subsequent generations. However, it is not clear how the CAG repeats expand through generations in HD. In the past 20 years, the number of subterranean taxa discovered in Australia, especially in the Pilbara bioregion, has considerably increased due to incidental environmental surveys often associated with mining development. Bathynellidae are an important component of stygofauna and they occur in most Australian aquifers, but their collection and identification are difficult due to their habitat, and small and fragile bodies with conservative morphology. Genetic studies of dispersal patterns in wild populations of orangutans (Pongo spp.) have sought to confirm behavioral observations that female orangutans tend to stay near their natal range while males disperse. In order to genotype a previously unsampled wild population of endangered orangutans at Tanjung Puting National Park I developed novel application of a methyl based magnetic capture for enrichment of fecal DNA and commercial human targeted single nucleotide polymorphism (SNP) microarray technology. Alzheimer’s disease (hereafter AD) is a progressive neurodegenerative disorder that affects the central nervous system. There are multiple factors that cause AD, viz., accumulation of extracellular Amyloid-beta 42 plaques, intracellular hyper-phosphorylated Tau tangles, generation of reactive oxygen species due to mitochondrial dysfunction and genetic mutations. The plaques and tau tangles trigger aberrant signaling, which eventually cause cell death of the neurons. "Insertion sequences (IS) are the smallest prokaryotic transposable elements. These mobile genes are highly diverse in size and structure, making them difficult to study. The transposition activity of insertion sequences has a significant role in evolution by enabling genomic plasticity via genome rearrangements. Insertion sequences are largely uncharacterized and thus methods to improve the ability to accurately identify, annotate, and infer genomic impact of insertion sequences is limited. Short Tandem Target Mimic (STTM) is a commonly technique used for functional studies of a number of genes in several plant model systems. However, very little is known about application of STTM technique in tree species. In this study, STTM was applied to knock down microRNA396 (miR396) in transgenic poplar trees for the first time. STTM396 expression resulted in dramatic decrease in miR396 expression levels leading to taller plants with larger leaves and larger leaf cell size. Foxtail pine (Pinus balfouriana) is a subalpine conifer endemic to California, notably separated into two disjunct subspecies. Previous studies have described the northern subspecies,Pinus balfouriana subsp. balfouriana,as having an uncommonly high level of genetic differentiation and no discernible spatial patterns in phenotypic variation. This study seeks to characterize the spatial genetic structure and patterns of selection of the northern subspecies (Pinus balfouriana subsp. balfouriana) using genome-wide data and to identify the influence of ecology and environment on the unique genetic patterns. Birdstrikes to aircrafts are increasing on an annual basis and pose significant aviation safety risks. Identification of the birds involved is key to developing mitigation strategies. Often the only information available to make identifications are feather and/or tissue samples. Relying on feathers alone to identify the bird species requires special expertise and access to museum collections for specimen comparisons. Glutaraldehyde and DBNPA are commonly used to manage microbial growth in different industries. It is possible that biocide use may result in the development of bacterial resistance. However, resistance to DBNPA is believed to be limited under normal use conditions. We isolated a number of bacteria from produced water from a hydraulically fractured site in West Texas. While traditional thermomechanical pretreatments are effective at helping to deconstruct lignocellulose, they are often expensive and tend to generate environmentally dangerous waste materials. In this study, I investigated the role of two different regulatory genes in two different species of pathogenic fungi from the genus Aspergillus. The first study involves a transcriptome analysis of the epigenetic regulator rmtA in the plant pathogen Aspergillus flavus. A. flavus colonizes numerous oil seed crops such as corn, peanuts, treenuts and cotton worldwide, contaminating them with aflatoxins and other harmful potent toxins. Abdominal aortic aneurysm (AAA) is a life-threatening immunological disease responsible for 1 to 2% of all deaths in 65 year old or older individuals. Although mononuclear cell infiltrates have been demonstrated in AAA lesions and autoimmunity may be responsible for the initiation and account for the propagation of the disease, the information available about the pathogenesis of AAA is limited. To examine whether AAA lesions from patients with AAA contain clonally expanded α-chain TCR transcripts, we amplified by the non-palindromic adaptor-PCR (NPA-PCR)/Vα-specific PCR and/or the Vα-specific PCR these α-chain TCR transcripts. โรคติดเชื้อที่มีสาเหตุมาจาก Mycobacterium avium complex (MAC) มีอุบัติการณ์เพิ่มสูงขึ้นทั่วโลกรวมทั้งในประเทศไทย เนื่องมาจากการเพิ่มขึ้นของจำนวนผู้ป่วยที่มีภาวะภูมิคุ้มกันของร่ายกายต่ำ การวินิจฉัยเชื้อ MAC ในปัจจุบันที่อาศัยการเพาะเลี้ยงเชื้อและทดสอบปฏิกิริยาชีวเคมี นอกจากใช้ระยะเวลานานแล้ว ยังไม่สามารถจำแนกเชื้อได้ถึงระดับสปีชี่ส์ โดยเฉพาะอย่างยิ่งเชื้อ Mycobacterium avium และเชื้อ Mycobacterium intracellulare ซึ่งเป็นเชื้อ MAC ที่สำคัญและเพาะแยกได้บ่อยจากสิ่งส่งตรวจ ดังนั้นวัตถุประสงค์ของการศึกษาในครั้งนี้ คือการพัฒนาเทคนิค Recombinase Polymerase Amplification ร่วมกับการอ่านผลด้วยสี SYBR Green I (RPA/SYBR) ในการตรวจวินิจฉัยเชื้อ MAC ซึ่งอาศัยการเพิ่มปริมาณสารพันธุกรรมภายใต้อุณหภูมิคงที่ ร่วมกับไพรเมอร์ที่จำเพาะต่อเชื้อกลุ่ม MAC เชื้อ M. avium และเชื้อ M. intracellulare และอ่านผลที่เกิดขึ้นด้วยตาเปล่าได้ในทันทีภายหลังการเติมสี SYBR Green I ผลจากการทดสอบกับตัวอย่าง DNA ของเชื้อ MAC จำนวน 120 ตัวอย่าง พบว่าเทคนิค RPA/SYBR สามารถตรวจวินิจฉัยจำแนกสปีชี่ส์ของเชื้อ M. avium และเชื้อ M. intracellulare ได้อย่างมีประสิทธิภาพ โดยมีความไว 100% และความจำเพาะตั้งแต่ 80.7% ขึ้นไป ซึ่งจัดว่ามีความสอดคล้องในเกณฑ์ดีมากเมื่อเปรียบเทียบกับวิธีหาลำดับเบส (สถิติ Kappa มีค่าตั้งแต่ 0.801 ขึ้นไป) ในขณะที่เมื่อเปรียบเทียบกับเทคนิค PCR มีความไวตั้งแต่ 89.2% ขึ้นไปและความจำเพาะตั้งแต่ 97.5% ขึ้นไป และมีความสอดคล้องในเกณฑ์ดีมากเช่นเดียวกัน (สถิติ Kappa มีค่าตั้งแต่ 0.863 ขึ้นไป) นอกจากนี้เทคนิค RPA/SYBR ยังมีค่าความเข้มข้นของ DNA ต้นแบบน้อยที่สุดที่สามารถตรวจได้ เท่ากับ 1 ng/µl และไม่เกิดปฏิกิริยาข้ามกลุ่มกับเชื้อส่วนใหญ่ของ Mycobacterium แม้ว่าเทคนิค RPA/SYBR ที่ได้พัฒนาขึ้น มีขั้นตอนการทดสอบที่ทำง่าย …

เชื้อ Escherichia coli ที่สร้างเอนไซม์ Extended-spectrum β-lactamase (ESBLs) เป็นปัญหาสำคัญที่พบในโรงพยาบาลทั่วโลก การตรวจจีโนไทป์ของเอนไซม์ ESBLs เป็นวิธีที่สำคัญและมีประโยชน์ในการควบคุมการแพร่กระจายของเชื้อดื้อยาและการเลือกใช้ยาต้านจุลชีพอย่างเหมาะสม การศึกษานี้มุ่งหวังจะพัฒนาเทคนิค Multiplex Recombinase polymerase amplification (Multiplex RPA) และ Helicase dependent amplification (HDA) สำหรับตรวจหายีนดื้อยา blaCTX-M, blaOXA, blaSHV และ blaTEM ในเชื้อ E. coli ในสถาบันมะเร็งแห่งชาติ ประเทศไทย เชื้อ E. coli จำนวน 144 ตัวอย่างที่แยกได้จากสิ่งส่งตรวจภายในงานจุลชีววิทยา สถาบันมะเร็งแห่งชาติ ช่วงระหว่างเดือนกุมภาพันธ์ 2017 ถึงเดือนกันยายน 2018 ถูกนำมาทดสอบความไวต่อยาต้านจุลชีพและตรวจคัดกรองการสร้างเอนไซม์ ESBLs ด้วยวิธี Disk diffusion ตามวิธีมาตรฐานของ CLSI ปี ค.ศ. 2017 เชื้อที่สร้างเอนไซม์ ESBLs จำนวน 95 ตัวอย่าง พบดื้อยาสูงสุดในยา Ampicillin (100%), Cefotaxime (100%), Cefdinir (100%) และ Ceftriaxone (99.9%) และเชื้อ E. coli จำนวน 144 ตัวอย่าง มียีน blaCTX-M (100%) รองลงมาคือ blaTEM (42.1%), blaOXA (30.5%) และ blaSHV (2.1%) ตามลำดับ โดยเป็นยีน ESBLs ชนิด CTX-M-15 สูงสุด (51.1%) รองลงมาคือ CTX-M-55 (18.1%), CTX-M-27 (17.0%), CTX-M-14 …
