A Causal Model Of Performance Shaping Factors For Human Reliability Analysis In Manufacturing., Prameet Ranjan Jha

Human reliability analysis is a critical component of probabilistic risk assessment, aimed at predicting and mitigating human errors in complex systems. This dissertation develops a novel approach to human reliability analysis in manufacturing by integrating structural equation modeling and Bayesian networks to improve the estimation of human error probabilities. Traditional human reliability assessment methods, such as the Standardized Plant Analysis Risk-Human Reliability Analysis (SPAR-H) and the Technique for Human Error Rate Prediction (THERP), provide structured techniques for estimating human error probabilities. This research introduces an unstable slope management program (USMP) for Tennessee based on federal slope management standards, along with improved methods for landslide monitoring with unmanned aerial systems (UAS) lidar and photogrammetry. In mountainous regions, monitoring slope hazards is a critical function of transportation management. A mobile field assessment form created with Survey123 was used to collect 22 unstable slope ratings in eastern Tennessee. Location points were appended with photographs, notes, and site information. Landslide scores ranged from 325 (Fair) to 1005 (Poor). UAS monitoring of a slow-moving soil landslide along I-40 near Rockwood, TN, produced high-resolution lidar and photogrammetry …

This thesis explores the transformative potential of data-driven approaches in addressing key operational and reliability issues in power systems. The first part of this thesis addresses a prevalent problem in power distribution networks: the accurate identification of load phases. This study develops a data-driven model leveraging consumption measurements from smart meters and corresponding substation data to reconstruct topology information in low-voltage distribution networks. The proposed model is extensively tested using a dataset with more than 5,000 real load profiles, demonstrating satisfactory performance for large-scale networks. An inventory of unstable slopes along transportation corridors and performance modeling are important components of geotechnical asset management in Great Smoky Mountains National Park (GRSM). Hazards and risk were assessed for 285 unstable slopes along 151 miles of roadway. A multi-criteria model was created to select fourteen sites for two-dimensional probabilistic rockfall simulations and Acid Base Accounting (ABA) tests. Simulations indicate that rock material would likely enter the roadway at all fourteen sites. ABA test results indicate that influence of significant acid-producing potential is generally confined to slaty rocks of the Anakeesta Formation and graphitic schist of the Wehutty Formation. …

This thesis proposes a risk based price adjustment method for a day ahead scheduling of a distribution side tiered community microgrid within a transactive energy style market. The system is organized into a master controller and local controller confïguration to simulate behind meter activity that is not visible to the master controller. Concerns were that suspect couplings might not meet the engineering specifications and could be vulnerable to sudden failure. A testing company in Oak Ridge, TN that specializes in in-situ and laboratory mechanical testing was contacted by the manufacturer for help in this endeavor. This study was conducted to provide a quantitative risk assessment for road transportation, railway transportation, and storage of hazardous materials in Washington County, Tennessee. A roadside survey, railroad hazardous materials survey, and a chemical inventory were conducted. It revealed an estimated 267,858 HAZMAT shipments per year on primary and secondary roadways and an estimated total of 9.4 x 109 tons of HAZMAT shipped by rail per year. There are 49 facilities that store a daily average of 33,722,700 lbs of HAZMAT. The risk of a hazardous materials incident occurring in Washington County is highest for Interstate transportation.
