Executive Summary
This report documents an analysis of traffic crash data in Kentucky for the years of 2013-2017. A primary objective of this study was to determine average crash statistics for Kentucky highways. Rates were calculated for various types of highways and for counties and cities. Difference criteria were used for exposure.
Average and critical numbers and rates of crashes were calculated for various types of highways in rural and urban areas. Executive Summary
Fatalities resulting from traffic crashes in Kentucky increased from a low of 638 in 2013 to 834 in 2016 (a 31% increase), and there is not a full understanding of the underlying causes for this trend. This study analyzes fatal crashes using data from both Kentucky’s crash reports and the National Highway Traffic Safety Administration’s (NHTSA) Fatal Accident Reporting System (FARS) to identify countermeasures to combat increasing fatal crash rates. In addition, this study compares Kentucky’s crash reports to the fatal crash data in FARS to identify data inconsistencies and recommend procedures to eliminate these inconsistencies.

This paper examines existing and future direct current (DC) distribution systems with a wide range of applications in data centers, telecommunication systems, commercial buildings, residential homes, electric vehicles, spacecraft, and aircrafts. DC distribution systems have many advantages and disadvantages over their alternating current (AC) counterparts. There are a few surviving examples of DC distribution systems; among them are the telecommunication systems and data centers that use the low-voltage 48 Vdc systems. Previous research revealed that wrong identification of a failed engine during flight is not an uncommon event in twin-engine propeller aircraft. Most accidents of this type have happened on takeoff when pilot workload was at its highest level. This study was based on the assumption that the “dead leg – dead engine” method was not efficient enough. An alternative method of identification of a failed engine, which involved a visual indicator inside a cockpit, was introduced and tested. As unmanned aircraft operations become more ubiquitous in the National Airspace System, one of the key remaining challenges is instilling the precepts of safety culture, aviation professionalism, airmanship, and effective aeronautical decision-making among these non-traditional aviators. To address these challenges, researchers codified best practices and operational recommendations from across the UAS industry, collectively publishing them in a compendium titled the UAS Pilots Code (UASPC). Evaluating the safety maturity of construction contractors before awarding the contract is an effective strategy that many owners have started to implement. Given the importance of workplace safety, awarding the contract to safer contractors can enhance project performance outcomes in terms of safety, quality, cost, and schedule. Safety maturity is one quality of a construction organization encompassing interdependent factors that can be used to evaluate expected safety performance. The railroad industry currently utilizes two wayside detection systems to monitor the health of freight railcar bearings in service: The Trackside Acoustic Detection System (TADS™) and the wayside Hot-Box Detector (HBD). TADS™ uses wayside microphones to detect and alert the conductor of high risk defects. Many defective bearings may never be detected by TADS™ due to the fact that a high risk defect is considered a spall which spans more than 90% of a bearing’s raceway, and there are less than 20 systems in operation throughout the United States and Canada. Prevention of railroad bearing failures, which may lead to catastrophic derailments, is a central safety concern. Early detection of railway component defects, specifically bearing spalls, will improve overall system reliability by allowing proactive maintenance cycles rather than costly reactive replacement of failing components. A bearing health monitoring system will provide timely detection of flaws. However, absent a well verified model for defect propagation, detection can only be used to trigger an immediate component replacement. Wayside hot-box detectors (HBDs) are devices that are currently used to monitor bearing, axle, and brake temperatures as a way of assessing railcar component health and to indicate any possible overheating or abnormal operating conditions. Conventional hot-box detectors are set to alarm whenever a bearing is operating at a temperature that is 94.4°C (170°F) above ambient, or when there is a 52.8°C (95°F) temperature difference between two bearings that share an axle. These detectors are placed adjacent to the railway and utilize an infrared sensor in order to obtain temperature measurements. Bearings that trigger HBDs or display temperature trending behavior …

Soft robotics is an emerging industry, largely dominated by companies which hand mold their actuators. Our team set out to design an entirely 3D printed soft robotic hand, powered by a pneumatic control system which will prove both the capabilities of soft robots and those of 3D printing. Through research, computer aided design, finite element analysis, and experimental testing, a functioning actuator was created capable of a deflection of 2.17” at a maximum pressure input of 15 psi. Since its inception in 1970, the Occupational Safety and Health Act has steadily decreased the rate in which construction injuries, illnesses, and deaths occur in the industry. There were studied the physicochemical indices of quality of natural honey normalized in Ukraine and abroad. The dominant ones that determine the level of its quality are the mass fractions of sugars, proline and diastase number, and the principal compound which presence determines the level of safety of honey for health of consumers is hydroxymethylfurfural. There was carried out the analysis of nine specimens of natural honey sampled in all regions of Ukraine and shown that their quality conforms to norms of European directive and international standard for honey.

At a construction site, workers mainly rely on two senses, which are sight and sound, in order to perceive their physical surroundings. However, they are often hindered by the nature of most construction sites, which are usually dynamic, loud, and complicated. To overcome these challenges, this research explored a method using an embedded sensory system that might offer construction workers an artificial sensing ability to better perceive their surroundings. Preliminary research found that jobsite safety practices are not a hinderance on a project schedule when managed well. A survey of students and industry professionals was the methodology used to gather this information. This survey consisted of various questions about the perception of safety practices on jobsites the respondent has worked on themselves, as well as their perception about safety in the construction industry as a whole. The Midwest Pooled Fund Program has been developing a prototype design for a non-proprietary, high-tension, cable median barrier for use in a 6H:1V median V-ditch. This system incorporates four evenly spaced cables, Midwest Weak Posts (MWPs) spaced at 8 to 16 ft (2.4 to 4.9 m) intervals, and a bolted, tabbed bracket to attach the cables to each post. Full-scale crash testing was needed to evaluate the barrier’s safety performance. The objective of this research effort was to mitigate the potential for floor pan tearing and penetration into the occupant compartment by modifying the posts utilized in a prototype cable barrier system. A series of dynamic component tests were conducted on the modified Midwest Weak Post (MWP). A bogie vehicle was equipped with a simulated floor pan designed to replicate the height, thickness, and strength of the floor pan of a Kia Rio. Two methods of post modification were investigated, including edge protection on the top of the MWP as well as weakening of the MWP at the ground line. …

Studies examining aviation accidents have not found differences in accident rates by gender, though there may be gender differences in the types of accident. Baker, Lamb, Grabowski, and Rebok (2001) examined fixed-wing aviation accident rates of male and female private pilots and found that males were more likely to have accidents related to inattention or poor planning while female pilots were more likely to have accidents due to mishandling the aircraft. This research analyzed the National Transportation Safety Board’s aviation accident database system to examine the severity of injury and aircraft damage in rotary-wing (helicopter) accidents by gender. The introduction of Vehicle-to-Vehicle (V2V) communication has the promise of decreasing vehicle collisions, congestion, and emissions. However, this technology places safety and privacy at odds; an increase of safety applications will likely result in the decrease of consumer privacy. Stone countertops are very heavy and fragile. When installing a finished countertop, up to six workers are required to lift the stone onto the countertop, risking their safety and possibly breaking the stone. The safety of both the workers and the stone are very important, so there needs to be a better way. NSI Solutions came up with an idea for a mechanism to aid in the insulation process while limiting the number of workers required, and keeping the workers safer while adequately supporting the stone to prevent it from breaking. Passengers rank safety as a key factor in airline choice. Thus, safety performance impacts an airline's ability to attract customers. The purpose of this correlational study was to examine the relationship and difference between airline category low-cost carriers (LCCs) and full-service carriers (FSCs), geographical region, and safety performance measured by accident rates. The target population comprised all airlines in all countries that had an accident during the 14-year period 2004 to 2017. KENTUCKY’S TRAFFIC COLLISION FACTS report is based on collision reports submitted to the Kentucky State Police Records Branch. As required by Kentucky Revised Statutes 189.635, “every law enforcement agency whose officers investigate a vehicle accident of which a report must be made...shall file a report of the accident...within ten days after investigation of the accident upon forms supplied by the bureau.” The stated purpose of this requirement is to utilize data on traffic collisions for such purposes as will improve the traffic safety program in the Commonwealth.
