A school district located in the southeastern United States uses benchmark tests as formative assessment to provide teachers with data to differentiate their instruction to meet the individual needs of students in their classrooms. Despite this effort, student achievement in mathematics in this school district has not improved. The purpose of this study was to gain an understanding of how middle-grade mathematics teachers used the benchmark results as formative data to guide instruction and meet student needs. In a southeastern state school district, the educators understood little about the scaffolding practices of ninth-grade teachers of Foundations of Algebra (FOA), a remedial course. FOA is a mathematics course designed for students who need substantial help to master the required standards. Students at an alternative high school located in a northern Midwest state demonstrated low science achievement and high rates of student absenteeism. North Carolina's community colleges redesigned developmental math programs in 2011. The overall effectiveness of the redesign has not yet been evaluated. A concurrent mixed-methods study was conducted at Mid-Atlantic Community College (MACC) for a formative and summative evaluation of the redesigned program. Mezirow's transformative learning theory, along with an emphasis on designing individualized methods of instruction as outlined by Keller were the theoretical foundations of the evaluation. The evaluation examined success, retention, and persistence outcomes of the redesigned course compared to the previous developmental math course. In a midsized urban school district in the Northeastern United States, all high school mathematics teachers attended a 3-year mandatory PD program. K-5 teachers attended district training to improve mathematics instruction and content mastery, but the training omitted teachers' affective domain in teaching. Teachers' affective relationships with mathematics (ARM) affects content delivery, instructional decisions, and teachers' confidence levels and motivation. The problem of low achievement and failure of 8th-grade students to attain state proficiency level in science in a local school district was addressed in this case study. Teachers and administrators in a Title I elementary school in a southeastern state are concerned that there has been a trend over the past 3 years of declining standardized assessment scores in mathematics for students in Grades 3, 4, and 5. The purpose of this qualitative case study was to explore teachers' perceptions of practices, and professional development (PD) for differentiating mathematics instruction.
