Background. Living kidney donor transplantation provides recipients with kidney failure with an excellent graft function and survival. Donation of a kidney entails a thorough medical and psychosocial evaluation. Obesity is an increasingly prevalent medical condition and a known risk factor for the development of chronic kidney disease post-donation. Accepting overweight and obese individuals as living kidney donors remains controversial. The purpose of this study was to review the impact of obesity on the determination of candidates for living kidney donation at a single transplant center.
Methods. We performed a retrospective chart review of living kidney donors evaluated over 5 …
Hand Knob Stroke, The Ultimate Mimic Of Peripheral Mononeuropathy: A Case Report, Larry A. Osborne Jr, Stephen Chien
Hand Knob Stroke, The Ultimate Mimic Of Peripheral Mononeuropathy: A Case Report, Larry A. Osborne Jr, Stephen Chien
Graduate Medical Education Research Journal
Background. Hand knob strokes present with unilateral motor weakness of the hand, distal arm, or a particular group of fingers with or without paresthesia. This condition can easily be misdiagnosed as a peripheral mononeuropathy without a high level of suspicion and a thorough history and physical exam. We present a case of hand knob stroke that mimicked an ulnar mononeuropathy.
Case. A 100-year-old male with a history of untreated hypertension, Stage 3A Chronic Kidney Disease, and prediabetes presented to the hospital five hours after symptom onset with resolving acute left hand weakness and paresthesia. Symptoms began the night prior with …

Ai-Based Simulated Patient Encounters For Clinical Reasoning: A Needs Assessment, Alex Sorrick, David Liebovitz, Sarah Vick, Andrew S. Parsons, Brian W. Locke, Jonathan Herskovitz, Cory Rohlfsen
Graduate Medical Education Research Journal
Background. Artificial Intelligence (AI) offers an opportunity to enhance formative assessment of clinical reasoning skills. Traditionally, these skills have been assessed through workplace-based assessment or simulated patient encounters, both of which are limited by variability in case exposure, faculty resources, scheduling logistics, and cost. AI-based simulated patient encounters provide a scalable solution for convenient, low-stakes practice. At the University of Nebraska Medical Center (UNMC), we piloted an AI-based simulated patient case and conducted a needs assessment to explore current practices and user experiences.
Methods. From June to August 2025, participants trialed an AI-based simulated patient case and received feedback on …

Major advancements in domains like diagnosis and personalised treatment will be seen because of the rapid growth of artificial intelligence (AI) in healthcare. But this shift additionally presents significant ethical concerns that need to be carefully considered at every stage of AI's research, use, and impact on society in medical settings.
This review summarises the findings of multiple systematic and scoping reviews, as well as studies of specific AI applications published between 2013 and 2025. Additionally, it examines ethical norms from different countries and international organisations objectively. Consistent ethical challenges identified include the "black box" problem (lack of transparency and …

Background. Hypertensive disorders of pregnancy (HDP) are a major contributor to rising maternal mortality in the United States. The postpartum period represents a critical window to provide education aimed at reducing the risks of complications. This study evaluates the patients’ understanding of HDP and their retention of key education concepts during the first six weeks postpartum after participation in a structured quality improvement program.
Methods. Prospective cohort study of patients diagnosed with HDP were enrolled in the remote blood pressure monitoring program (RPM) and delivered between June 2023 and January 2024. Baseline surveys were administered prior to discharge and repeated …

Graduate Medical Education Research Journal
Background. Burnout poses considerable challenges in workplaces, particularly for resident physicians. This intricate phenomenon stems from a myriad of factors that include, but are not limited to, staff shortages, excessive workloads, specific job responsibilities, work environments, and individual psychological factors. The Accreditation Council for Graduate Medical Education encourages institutions to incorporate wellness as a fundamental component of residency training programs.
Methods. The aim of this review was to investigate burnout among resident physicians via a comprehensive literature review to elucidate this significant issue and identify potential preventative intervention strategies. We analyzed the issue of burnout across various residency specialties, providing …

Salt Vs. Scalpel: A Surprisingly Simple Solution To Pyogenic Granulomas, Richard M. Regester, Kimberly Eisenstein
Salt Vs. Scalpel: A Surprisingly Simple Solution To Pyogenic Granulomas, Richard M. Regester, Kimberly Eisenstein
Graduate Medical Education Research Journal
Background. Pyogenic granulomas (PGs) are benign vascular lesions commonly encountered in primary care settings. They are often linked to trauma, hormonal changes, infections, or medications. While traditional treatments such as excision, curettage, and silver nitrate application are the treatment of choice, recurrence is possible, and they may result in discomfort, anxiety, or scarring. This case highlights the successful use of salt packing, a low-cost, minimally invasive alternative, for a PG resistant to conventional therapy.
Case. A 57-year-old female presented with a 10 mm PG on the right ventral thumb, which bled intermittently and increased in size over three months. An …

Urology Resident Associate: A Promising Alternative For The Unmatched Urology Applicant, Jayson Kemble, Grace Handleman, Chad A. Lagrange, Christopher M. Deibert
Graduate Medical Education Research Journal
Background. Demand for urology residency spots frequently outstrips supply, with unmatched urology applicants having limited options while waiting for reapplication. We aim to demonstrate our experience with a year-long Urology Resident Associate (URA) position, which merges the role of junior resident and APP with the mission to reduce resident workload and further the training of an unmatched urology applicant.
Methods. A year-long, non-Accreditation Council for Graduate Medical Education (ACGME) accredited URA position was created at our institution in 2023 and filled by an unmatched applicant to our program. The URA was given all the roles and responsibilities of a …

Improving Resident Orientation To Anesthesiology Subspecialties Using A Structured “Passport”, Ky Raymond, Adam Jackson, Angela Mcelrath, Kathryne Corrigan, Ivan Barannikov, Kayla Knuf, Kelly Ivins-O'Keefe
Graduate Medical Education Research Journal
Background. On an initial pediatric, cardiothoracic, or obstetric anesthesiology rotation, a resident faces new knowledge and procedural expectations. Lack of guidance to direct learning may lead residents to feel unprepared. The optimal way to prepare residents is unknown. This initiative included the introduction of a structured “passport” for each subspecialty specifying the knowledge and procedural goals of an introductory rotation. This intervention aimed to improve resident and attending perceptions of resident readiness on these anesthesiology subspecialty rotations, as measured using pre- and post-initiative surveys of residents and attendings.
Methods. This initiative occurred at a large anesthesiology residency with 53 residents …

Human Papillomavirus Vaccine Uptake Among U.S. Medical Residents In High-Exposure-Risk Specialties – A National Study, Madison Oxford, Christina Stetter, Arpit Dave
Graduate Medical Education Research Journal
Background. HPV vaccination is not required for the United States (U.S.) medical residents, despite their potentially high risk of iatrogenic exposure to the virus. We aim to evaluate vaccination status, patterns of HPV vaccine uptake, and perceived risk of iatrogenic infection among residents in high-exposure-risk specialties.
Methods. In spring 2023, an anonymous online survey was distributed via program directors and coordinators of all Accreditation Council for Graduate Medical Education (ACGME)-accredited Obstetrics & Gynecology, Family Medicine, Otolaryngology, and Dermatology training programs to assess residents’ vaccination status, HPV exposure, and HPV vaccine knowledge.
Results. Responses from 537 residents at programs located in …
