Approach To Management Of Grown-Up Cyanotic Congenital Heart Disease, Pak-Cheong Chow

Journal of the Hong Kong College of Cardiology

Despite of advances in cardiac and surgical care, some children with complex congenital heart disease remain cyanotic in adulthood. The grown-up cyanotic congenital heart diseases are heterogeneous due to their underlying cardiac anatomy and physiology. Other than cardiovascular problems, they patients are also faced with multi-system complications in the long run. Therefore, these patients need nuanced and comprehensive assessment, regular follow-up monitoring, and multi-facet management. In this article, the types and classification of cyanotic congenital heart diseases, the multi-system complications and the approach to assessment and treatment will be presented.

The agricultural sector is the primary source of livelihood for communities in Southeast Asia; however, these activities increase the risk of exposure to zoonotic diseases, particularly leptospirosis. This literature review aims to identify risk factors for leptospirosis among agricultural workers in Southeast Asia. Photonic systems with exceptional points, where eigenvalues and corresponding eigenstates coalesce, have attracted interest due to their topological features and enhanced sensitivity to external perturbations. Non-Hermitian mode-coupling matrices provide a tractable analytic framework to model gain, loss, and chirality across optical, electronic, and mechanical platforms without the complexity of full open-system dynamics. Exceptional points define their spectral topology, and enable applications in mode control, amplification, and sensing. The problem studied in this paper is related to the f-harmonicity with potential of a vector field from a Riemannian to its tangent bundle TM equipped with the Sasaki metric. We show that an f-harmonic vector field with potential is parallel if and only if f is constant function. We also characterize the f-harmonic vector fields with potential on some three Riemannian Lie groups.

Due process guarantees lie at the heart of any legitimate legal system, serving as a critical safeguard against arbitrary state power and as a foundation for the rule of law. Yet their meaning and implementation vary significantly across legal traditions and political contexts.

This article examines the scope and limitations of due process guarantees in the Middle East, focusing on Egypt and Saudi Arabia as illustrative case studies. This Note examines the collapse of Chevron deference and its profound implications for federal tax law, where complexity and ambiguity are the rule rather than the exception. For decades, Chevron allowed courts to defer to the Internal Revenue Service’s reasonable interpretations of ambiguous statutes, promoting uniformity, predictability, and administrability across the tax system. In Loper Bright Enterprises v. Raimondo, the U.S. Supreme Court dismantled that framework, shifting interpretive authority decisively to the judiciary. This Note argues that the loss of Chevron destabilizes tax administration by inviting inconsistent judicial interpretations, increasing taxpayer challenges, and undermining reliance on agency guidance. This qualitative study examined how school-based professional development reflection programs influence teachers’ empathy across cultural differences within an independent school context. Grounded in Multicultural Theory and scholarship on empathy, the study explored how structured, sustained reflective practices shape educators’ understanding, relational engagement, and collective organizational culture. The problem of practice addressed in this study was the need for independent school leaders to cultivate culturally responsive environments within limited time and resource constraints, while serving increasingly diverse school communities. Could there be quantum superpositions of conscious states, as suggested by the Wigner’s friend thought experiment? Mathematical theories of consciousness, notably integrated information theory (IIT), make this question more precise by associating physical systems with both quantitative amounts of consciousness and structural characterizations of conscious states. Motivated by a recent proposal that ties wave-function collapse to integrated information, we construct a simple quantum circuit that would, on that proposal, place a minimal system—a feedback dyad—into a superposition of states that differ in their associated conscious states. We introduce Saturated Hierarchical Atomic Incremental Learning (sHAIL), a learning paradigm in which complex tasks are approached through a sequence of simpler atomic subtasks, each mastered to saturation before progression. The central mechanism is a saturation criterion that detects when learning dynamics enter a plateau region, triggering consolidation and subsequent ascent to a higher level of task complexity. We develop a theoretical framework for sHAIL and show that it naturally gives rise to \emph{staircased convergence}: alternating phases of rapid improvement and genuine plateau. Within each level, classical convergence guarantees apply under standard smoothness conditions, while the hierarchical transitions are driven …

Looking back over the decade that has elapsed since the Supreme Court’s landmark marriage equality decisions in United States v. Windsor and Obergefell v. Hodges, this article explores the complex relationship between legal and social change through the prism of the impact that these decisions have had on the tax landscape for the LGBTQ+ community. The article examines the gap between the promise of “equal” treatment and respect for members of the LGBTQ+ community embodied in these legal decisions and a social reality in which discrimination—tax and otherwise—stubbornly persists.

On January 30, 2026, Mayor Mamdani and Department of Consumer and Worker Protection (“DCWP”) Commissioner Samuel Levine announced that food delivery app companies Uber Eats, Fantuan, and Hungry Panda owed more than $5 million in worker restitution, civil penalties, and damages to resolve violations of the Minimum Pay Rate for delivery workers. Like New York City, other municipalities across the United States should enact statutory protections for delivery workers, investigate compliance, and enforce penalties to ensure workers' rights and justice.

Capitalism began in murder. Bodies were marked by slavery, colonialism, and genocide, white to have, and black to have not. Capitalism, “covered in dirt and dripping with blood,” was born of these three “primitive accumulations.” Slavery is death, and just like colonialism and genocide, it is a capital sentence. The law of capital punishment continues this legacy of murder and racism, as if on “rails to infinity.” This article uses insights from the critique of political economy to examine the death penalty, and the role of law generally, in maintaining racial and other inequalities. This essay describes how the Judicial Conference of the United States elected to reform the provision of transcripts to indigent persons appealing criminal convictions in federal court. A conviction cannot be appealed without a transcript, but, by definition, indigent persons cannot afford to purchase them. A series of Supreme Court decisions held that indigent persons had to be provided transcripts without charge. However, the practical implementation of that right was blocked. In the federal system, appointed counsel had to request production of transcripts using Criminal Justice Act Form 24. That form required special permission for the court reporter to produce …
