Hepatic gluconeogenesis is a critical process that generates glucose from non-carbohydrate precursors during fasting to support vital organs like the brain and red blood cells. Postprandially, this process is rapidly suppressed to allow for glucose storage as glycogen and lipids in the liver. Failure to suppress gluconeogenesis after meals leads to elevated postprandial glucose levels, a key feature of type 2 diabetes. This dynamic switch is regulated by insulin and glucagon, but insulin resistance impairs this regulation. In this study, we identified a novel mechanism involving postprandial circulating hyaluronan (HA) and lysosomal hyaluronidase-1 (HYAL1) that suppresses hepatic gluconeogenesis by rewiring …

Salvage endoscopic resection of residual esophageal neoplasia for patients that prefer to avoid surgery or are poor surgical candidates has been reported. We report a case of gastric signet ring cell carcinoma with residual disease after definitive chemoradiotherapy, that was treated with nonexposed endoscopic full thickness resection. This endoscopic resection achieved negative margins, with postresection surveillance endoscopy and imaging showing no evidence of disease. This case highlights the potential use of salvage endoscopic resection as a therapy for residual gastric cancer after chemoradiation in patients where surgery is not an option.

Objective: Weight regain following bariatric surgery remains a clinical challenge, with limited understanding of contributing environmental factors. Per- and polyfluoroalkyl substances (PFAS), persistent chemicals linked to metabolic dysfunction, may influence long-term weight trajectories. This study aimed to evaluate associations between PFAS exposure and changes in BMI, percent weight loss, and waist circumference among adolescents after bariatric surgery.
Methods: We included 186 adolescents (mean age: 17.1 years; 76.3% female; 72.0% White) from the Teen-Longitudinal Assessment of Bariatric Surgery (Teen-LABS) cohort who underwent surgery between 2007 and 2012. Ileal pouch-anal anastomosis is a surgical intervention in patients with medically refractory inflammatory bowel disease and familial adenomatous polyposis syndrome. Reported outcomes in children have been limited by both the retrospective nature and sample size of the investigations. In addition, there is a lack of consensus on surveillance guidelines in this patient population. Delay or lack of surveillance may increase the risk of complications (pouchitis, Crohn's disease-like inflammation, dysplasia, etc.). Background: Diet quality is a modifiable factor impacting treatment outcomes in oropharyngeal cancer (OPC), yet the effect of dysphagia and self-reported eating-related symptoms remains unclear. This study examined their associations with diet quality in OPC patients.
Methods: A cross-sectional analysis was conducted with two parallel cohorts from MD Anderson Cancer Center and Michael E. DeBakey Veterans Affairs Medical Center as part of the Uncovering the Long-Term Impact of OPC and Dysphagia on Dietary Quality and Nutrition Among Cancer Survivors (U-DINE) Study. Traumatic brain injury (TBI) is less common in pregnancy than in the general population; however, it poses significant challenges in diagnosis and management. These include the potential teratogenicity of medications, radiation exposure from imaging modalities, physiological changes associated with pregnancy, and obstetric complications that may arise. Although there are general guidelines for trauma in the obstetric patient, they are not specific to TBI, and most recommendations are based on low-level evidence or institutional experiences.

Lactate-derived N-lactoyl-phenylalanine (Lac-Phe) was recently identified as an lactate-derived circulating metabolite that reduces feeding and obesity, but mechanisms that underlie the metabolic benefits of Lac-Phe remain unknown. Here we demonstrated that Lac-Phe directly inhibits hypothalamic neurons that express Agouti-related protein (AgRP), resulting in an indirect activation of anorexigenic neurons in the paraventricular nucleus of the hypothalamus (PVH). We also found that both AgRP inhibition and PVH activation are required to mediate Lac-Phe-induced hypophagia. Further, we show that Lac-Phe inhibits AgRP neurons via activating the ATP-sensitive potassium (KATP) channel, and inhibition of the KATP channel blunts effects of Lac-Phe to suppress …

Peritoneal malignancy often indicates disruptions in multiple physiological systems resulting from widespread cancer. The heterogenous origin and dynamic nature of peritoneal cancer make it difficult to treat with standard approaches that fit into guidelines. We describe how successful treatment should address the underlying pathology, the systemic response to surgical treatments and target the immune perturbations that facilitate the establishment and propagation of this multifaceted disease.

Introduction: Chronic pancreatitis (CP) is a disease associated with chronic inflammation, fibrosis, and pain. There is a lack of tools available that facilitate early diagnosis, when intervention could prevent irreversible damage. Pilot data suggested prostaglandin E2 (PGE2) as a candidate biomarker for early CP. PGE2 activates signaling pathways that promote inflammation, pain, and fibrosis.
Methods: We assessed PGE2, metabolites, and downstream targets in pancreatic fluid collected endoscopically 0-10 (n = 110) and 10-20 (n = 111) minutes after intravenous secretin administration. PGE2 and metabolites were measured in plasma (n = 75) and urine (n = 71) from the same subjects. …

Background: Metabolic dysfunction-associated steatotic liver disease (MASLD) is increasingly recognized in people with HIV (PWH), with both HIV and antiretroviral therapy contributing to liver damage and glucose intolerance. However, the role of viral proteins derived from reservoirs in this process remains unclear.
Methods: Adeno-associated virus (AAV) constructs encoding a control protein or HIV-1 viral protein R (Vpr) driven by the thyroxine-binding globulin promoter were administered to male mice (n = 5 per group) fed regular chow or a high-fat diet (HFD). GLP-1-based therapy is highly effective in combating aging-associated metabolic diseases. However, the metabolic effects of frequent withdrawal from this therapy in aged, obese mice have not been previously studied. In this study, aged obese UM-HET3 mice were assigned to three groups: Group 1 received no liraglutide treatment (Lira OFF); Group 2 underwent 3 cycles of treatment followed by withdrawal (Lira ON/OFF); and Group 3 remained on continuous treatment (Lira ON). Purpose: The relationship between engaging in two domains of cancer-preventive behaviors, lifestyle behaviors and colonoscopy screening, is unknown in Hispanic adults. Accordingly, the study examined the association between lifestyle and colonoscopy screening in Hispanic adults along the Texas–Mexico border, where there is suboptimal colorectal cancer prevention.
Methods: Lifestyle behavior adherence and compliance with colonoscopy screening schedules were assessed using 2013–2023 data from the Cameron County Hispanic Cohorta populationbased sample of Hispanic adults living along the Texas–Mexico border. This report describes the case of a 21-year-old male with 95% total body surface area full-thickness burns who developed polymicrobial bacteremia involving nine distinct bacterial species, the highest number of diverse bacterial species documented to simultaneously coexist in a patient’s bloodstream. Despite aggressive surgical, antimicrobial, and multidisciplinary interventions, the patient succumbed to sepsis after 12 days. The case highlights the diagnostic and therapeutic challenges of managing complex infections in burn patients and underscores the critical importance of early detection, prompt treatment, and coordinated care. Background: Pediatric cholestatic liver diseases can be characterized by rapidly progressive fibrosis. A multicenter cross-sectional analysis of vibration-controlled elastography in biliary atresia (BA), alpha-1 antitrypsin deficiency (A1AT), and Alagille syndrome (ALGS) was leveraged to interrogate the plasma proteome relative to liver stiffness measurements (LSM).
Methods: Slow off-rate modified aptamer scanning profiling of >7000 proteins in plasma from 187 children with BA (n=93), A1AT (n=31), ALGS (n=46), and healthy pediatric controls (n=17) was performed, and correlations with LSM were undertaken.

The plant-based milk alternative (PBMA) market is expanding rapidly, offering a diverse range of options. However, the mineral composition of PBMA varies significantly across brands and milk types, posing challenges in educating individuals with renal disease. This study aimed to quantify and compare key minerals of concern for renal patients in commercially available PBMA using inductively coupled plasma optical emission spectrometry.
PBMA products available in the San Francisco Bay Area were purchased from chain grocery stores in spring 2023 and early January 2024. A total of 134 unique PBMA products from 28 brands and 11 different milk-alternative sources were analyzed. …

Nutrition intervention programs can improve dietary patterns, habits, and health outcomes in communities, but a long-term, integrative approach is essential for success. Barriers such as limited time, resources, lack of cultural sensitivity, and competing priorities hinder their effectiveness. Research highlights the importance of environmental support, particularly from peers and employers, in improving dietary habits and nutrition literacy.

South Asians have a higher risk of developing type 2 diabetes than manyother racial or ethnic groups. Dietary patterns are a key, modifiable risk factor, and cultural influences strongly shape food choices and lifestyle decisions. Effective, long-term behavior change requires culturally meaningful interventions. This study aimed to develop culturally tailored recipes that comply with the American Diabetes Association (ADA) and American Heart Association (AHA) guidelines.

Cognitive abilities are heritable and influenced by socioeconomic status (SES). Mitochondrial membrane phospholipids impact mitochondrial structure and function by influencing the assembly and activity of membrane proteins. Although the specific roles of the three most abundant mitochondrial phospholipids, phosphatidylcholine (PC), phosphatidylethanolamine (PE), and cardiolipin (CL), have been extensively studied, the precise function of less abundant phosphatidylserine (PS) is not yet determined. Here, we used genetic and nutritional manipulation to engineer a set of yeast mutants, including a mutant completely devoid of PS, to assess its role in mitochondrial bioenergetics and lipid homeostasis. Numerous exercise oncology trials have been completed, greatly informing exercise recommendations for patients with cancer. Exercise medicine can be administered in various types, doses, and schedules at various time points. This thesis focuses on the optimization of date (Phoenix dactylifera L.) syrup, an emerging natural sweetener rich in glucose, fructose, and bioactive compounds, by investigating how extraction temperature and pectin incorporation influence its physicochemical and structural properties. Therefore, the main objective of this study was to examine how extraction temperatures (25°C, 50°C, 75°C, and 90°C), and the incorporation of lowmethoxyl pectin (LMP, DE 31%) and high-methoxyl pectin (HMP, DE 85%) prior to concentration, affect the quality, stability, and functionality of date syrup. Among the potential mechanisms, inflammation plays a key role in recurrence by promoting tumor progression. Objectives Rapid discrimination of infections caused by Mycobacterium tuberculosis (MTB) and non-tuberculous mycobacteria (NTM) is crucial in clinical settings. Despite overlapping clinical and radiological features, the two require markedly different therapeutic approaches and public health responses. Current laboratory methods are time-consuming and complex, underscoring the urgent need for a simple and efficient diagnostic tool to inform public health decision-making. Background/Objective: Temporal eating patterns and ultra-processed food (UPF) consumption have independently been associated with obesity and non-communicable diseases. Little is known about the temporal patterns of UPF consumption, as data is challenging to collect. Temporal data can be extracted from mobile food records (mFRs). The aim of this study was to identify the temporal eating patterns of those consuming UPFs using an mFR. Introduction: Higher adherence to the Mediterranean-DASH (Dietary Approaches to Stop Hypertension) Intervention for Neurodegenerative Delay (MIND) diet has been associated with reduced Alzheimer's disease (AD) risk. This study assessed the validity of a brief 15-item MIND diet screener compared to a comprehensive food frequency questionnaire (FFQ). Methods: The validity of an adapted MIND diet screener relative to the VioScreen FFQ was evaluated in 92 older adults from the Indiana Alzheimer's Disease Research Center (IADRC). Background: Concerns have been raised around whether calcium supplements increase dementia risk. This post-hoc analysis of a five-year double-blind, placebo-controlled randomised trial of calcium supplements for primary fracture prevention evaluated the long-term risk for dementia in older women, randomised to either calcium supplements or placebo. Methods: 1460 community-dwelling dementia-free Australian women (≥70 years) were randomised to 1200 mg/day calcium carbonate (n = 730) or placebo (n = 730) for five years and were observed for an additional 9.5 years afterwards. Introduction: This study explores the collaborative co-design process for developing a weekly meal box tailored for individuals with neurological conditions. Recognising the critical role of nutrition for this community, the research addresses the challenges posed by cognitive and physical impairments in meal preparation. Methods: Through two co-design workshops involving consumers, healthcare providers and industry experts, insights were gathered on dietary preferences, nutritional needs and practical challenges. Results: The workshops emphasised the importance of convenient, easy-to-prepare meals with simple instructions, flexibility and customisability.
