Background and study aims Endoscopic submucosal dissection (ESD) allows removal of tumors en-bloc. Western adoption of ESD has been hindered by its steep learning curve. Western data regarding ESD learning curve are limited. We analyzed the learning curve of a single endoscopist at a tertiary referral center in the United States.
Patients and methods All consecutive ESDs performed by a single endoscopist at a tertiary referral center in the United States from 2015 through 2022 were identified. Descriptive statistics and CUSUM analysis were used to describe the learning curve for en-bloc, R0 resection, and resection speed.\

OBJECTIVES: To derive systematic-review informed, modified Delphi consensus regarding the medications used for anticoagulation for pediatric extracorporeal membrane oxygenation (ECMO) for the Pediatric ECMO Anticoagulation CollaborativE (PEACE).
DATA SOURCES: A structured literature search was performed using PubMed, EMBASE, and Cochrane Library (CENTRAL) databases from January 1988 to May 2021.
STUDY SELECTION: Included studies assessed anticoagulation used in pediatric ECMO.
DATA EXTRACTION: Two authors reviewed all citations independently, with a third reviewer adjudicating any conflicts. Eighteen references were used for data extraction as well as for creation of recommendations. Evidence tables were constructed using a standardized data extraction form.

A 4-month-old full-term female presented with growth faltering associated with progressive feeding difficulty, rash, abdominal distension, and developmental delays. She was found to have disconjugate gaze, abnormal visual tracking, mixed tone, bruising, and splenomegaly on examination. Initial workup was notable for thrombocytopenia and positive cytomegalovirus (CMV) immunoglobulin G and immunoglobulin M antibodies. She initially presented to the infectious diseases CMV clinic, where she was noted to have severe malnutrition, prompting referral to the emergency department for hospital admission to optimize nutrition with nasogastric tube feeding and facilitate additional evaluation. Background and objectives Dual-trained medicine-pediatrics physicians (med-peds) play an important role in the healthcare ecosystem. Little is known about the subspecialty choices of med-peds residency graduates. This study aims to characterize the subspecialty choices of med-peds residency graduates. Methods The Medicine-Pediatrics Program Directors Association (MPPDA) administers an annual survey to the program directors of all med-peds residency programs accredited by the Accreditation Council for Graduate Medical Education (ACGME). This project represents aggregate survey data from 2020-2023. Results The number of program directors responding to the survey ranged from 80.8% (63/78) to 85.7% (66/77; mean response rate: 82.8%). RATIONALE & OBJECTIVE: In the wake of the coronavirus disease 2019 (COVID-19) pandemic, the United States federal government expanded originating telemedicine sites to include outpatient dialysis units. For the first time, nephrology practitioners across the United States could replace face-to-face visits with telemedicine for patients receiving in-center hemodialysis. This study describes patients' perspectives on the use of telemedicine during in-center hemodialysis.
STUDY DESIGN: A qualitative study.
SETTING & PARTICIPANTS: Thirty-two patients from underserved populations (older, less educated, unemployed, persons of color) receiving in-center hemodialysis who used telemedicine with their nephrologist during the COVID-19 pandemic.

INTRODUCTION: In low- and middle-income countries (LMICs), which are disproportionately affected by the HIV epidemic and manage limited resources, optimized implementation strategies are needed to enhance the efficiency of the HIV response. Assessing strategy usage to date could identify research gaps and inform future implementation efforts. We conducted a systematic review to describe the features and distributions of published implementation strategies attempting to improve HIV treatment service delivery and outcomes.

OBJECTIVES: The diagnostic yield and clinical impact of image-guided core needle biopsy (ICNB) of suspected vertebral osteomyelitis in adults is heterogenous in published studies owing to small sample sizes, indicating the need for large cohort studies.
METHODS: A retrospective analysis of ICNBs was performed from 2010 to 2021 for patients with imaging findings consistent with vertebral osteomyelitis. For each biopsy, a series of factors were analyzed, as well as if histopathology was diagnostic of osteomyelitis and if microbiological cultures were positive. This study aims to explore the impact of Covid-19 confirmed cases and Covid-19 death cases on stock return of healthcare, technology, finance, transportation, and logistic sector companies listed on Indonesia Stock Exchange in January – May 2020. Using panel data regression method, the study found that Covid-19 confirmed cases significantly affect stock return of healthcare, technology, finance, transportation, and logistic sector companies. However, there are no significant impact of Covid-19 death cases on stock return of healthcare, technology, finance, transportation, and logistic sector companies.

While there has been a significant increase in healthcare demands and the need for precise clinical judgment, it is important to note that the performance enhancement of photonic crystal sensors is not solely attributed to this surge in demand. Instead, photonic crystal-based biosensors have demonstrated improved performance over conventional biosensors. In this study, a 2-D photonic crystal-based biosensor with a square lattice structure, measuring 6 x 6 µm in length and width respectively, was proposed. This study evaluates the economic implications of TB incidence on gross domestic product (GDP) growth across eight ASEAN countries from 2000 to 2020. Background: Leprosy is a chronic infectious disease caused by Mycobacterium leprae. Current therapeutic regimen, like the multidrug therapy (MDT), are effective in treating most cases, but new cases continue to emerge in Indonesia every year. Trypanosomiasis, a parasitic disease caused by protozoan parasites of the genus Trypanosoma, poses a significant threat to camel populations worldwide. This study designed for the diagnostic techniques used for detecting Trypanosoma infections specifically in camels. The disease, commonly known as camel trypanosomiasis or surra, adversely impacts the health and productivity of camels, thereby affecting livelihoods dependent on these animals. This study investigated the contagion effect of other tourists' risk-seeking behavior on tourists' risk decision-making. It further examined the mediation of two contrasting motivational systems (approach-avoidance), and the moderation of destination management deterrence. Four experiments were conducted focusing on the risk decision-making of tourists in the context of undeveloped natural areas and tidal-bore watching settings. Independent samples t-tests, regression, and multivariate analysis of covariance were utilized to examine the proposed conceptual model. BACKGROUND: Immune checkpoint inhibitors (ICIs), agents that stimulate T-cell function, have become the standard first-line treatment for unresectable hepatocellular carcinoma (HCC). However, they may also cause immune-related adverse events (irAEs), which are rare and have not been extensively reported. Here, we describe a case of severe febrile neutropenia and pancytopenia after atezolizumab plus bevacizumab (atezo/bev) therapy and its treatment course.

Exclusive breastfeeding for the first 6 months and continued breastfeeding with complementary foods up to 2 years are emphasized by the World Health Organization. Amid the COVID-19 pandemic, maternal concerns about virus transmission through breastfeeding have surfaced. Despite these concerns, breastfeeding remains the primary recommendation due to its manifold health benefits. This research, conducted in Tegal City's Margadana sub-district, investigates factors influencing exclusive breastfeeding success among mothers recovered from COVID-19. Using an analytical observational design with a cross-sectional approach, the study explores the link between COVID-19 symptoms in infants and exclusive breastfeeding success, revealing a significant correlation (p-value=0.042).
