In this research, we explore a data-driven approach for drug repurposing to enhance the immunomodulatory effect by integrating pattern-based search and genome-scale metabolic modeling. This research helps to find a solution to the preexisting problems of drug discovery which includes expenses and a high amount of time consumption. By leveraging the pre-existing data of the approved drugs to identify the major metabolic pathway changes, we can find new drugs with similar effects with less off-target effects.

The rise of antibiotic-resistant bacteria is a growing concern for the medical and biological community. Increased use of antibiotics has led to increased development of bacterial tolerances. This phenomenon, driven by the principles of evolution, has been exacerbated by the improper antibiotic use of these antibiotics. Often, individuals stop taking antibiotics once symptoms subside, mistakenly believing the infection is cured. This practice allows bacteria or fungi to develop resistance.

Broadly neutralizing antibodies are proposed as therapeutic and prophylactic agents against HIV‐1, but their potency and breadth are less than optimal. This study describes the immunization of a llama with the prefusion‐stabilized HIV‐1 envelope (Env) trimer, BG505 DS‐SOSIP, and the identification and improvement of potent neutralizing nanobodies recognizing the CD4‐binding site (CD4bs) of vulnerability. Two of the vaccine‐elicited CD4bs‐targeting nanobodies, G36 and R27, when engineered into a triple tandem format with llama IgG2a‐hinge region and human IgG1‐constant region (G36×3‐IgG2a and R27×3‐IgG2a), neutralized 96% of a multiclade 208‐strain panel at geometric mean IC80s of 0.314 and 0.033 µg mL−1, respectively. Community awareness is a key component of disaster resilience and numerous agencies developed a plethora of preparedness materials. Previous studies evaluated national-level resources and social media content indicating that these efforts are not effectively reaching vulnerable populations, as the information is provided at or above the secondary education level. However, the local-specific content has been mostly overlooked in the analyses, subsequently neglecting the needs of areas with specific demographics and hazards, like predominantly Hispanic border communities. Retrospective Analysis Of The Clinical Utility Of Multi-Cytokine Profiles In Smear-Negative Pulmonary Tuberculosis, Xuemei Li, Weidong Zhao, Hong Han, Zhi Yang, Fengqing Bi, Yingchun He

Objectives: To evaluate cytokine profiles and interferon-gamma release assay (IGRA) for their diagnostic capabilities in the differentiation of tuberculosis (TB) from non-TB conditions, as well as smear-negative pulmonary tuberculosis (SNPT) from smear-positive pulmonary tuberculosis (SPPT). Methods: A total of 125 participants were included, 77 of whom had TB and 48 who didn’t, and demographic, clinical, and laboratory data were collected, including cytokine levels and IGRA results. The TB patients were further divided into 2 subgroups: SNPT (n=42) and SPPT (n=35). Results: Compared to non-TB, the TB group had lower BMI, higher WBC, neutrophils, monocytes, ESR and CRP ( p <0.05). TB patients showed higher IL-2, IL-6, IFN-γ, IL-8 ( p <0.001) and higher IGRA positivity (88.3% versus [vs.] 29.2%, p <0.001). Between SNPT and SPPT, moderate effect sizes were observed for IFN-α, IL-2, IL-10, IL-8 (Cohen’s d 0.59-0.76), with lower IGRA positivity in SNPT (81.0% vs. 97.1%, p =0.015). ROC analysis indicated IFN-α, IL-2, IL-10, IL-8 had moderate accuracy for SNPT diagnosis (AUCs 0.668-0.734), and combining these improved accuracy (AUC 0.759, 80% sensitivity, 64.2% specificity). Conclusion: A multi-biomarker approach combining these cytokines demonstrates enhanced diagnostic accuracy for tuberculosis.

Fc optimization can significantly enhance therapeutic efficacy of monoclonal antibodies. However, existing Fc engineering approaches are sub-optimal with noted limitations, such as inappropriate glycosylation, polyclonal libraries, and utilizing fragment but not full-length IgG display. Applying cell cycle arrested recombinase-mediated cassette exchange, this study constructed high-quality monoclonal Fc libraries in CHO cells, displayed full-length IgG on cell surface, and preformed ratiometric fluorescence activated cell sorting (FACS) with the antigen and individual FcγRs. China implemented catastrophic medical insurance (CMI) program to provide additional reimbursement for individuals requiring extensive healthcare services due to severe illnesses. This study empirically assesses the impact of the CMI program on the healthcare utilization and quality of care for severely ill patients, using medical insurance claim data from 2012 to 2018 in a capital city in China. The analysis yields the following conclusions: First, CMI decreased the medical expenditure of severe patients, but it had no impact on alleviating the financial burden. Propolis, known for its antibacterial, antifungal, antioxidant, antiviral, immunomodulatory, and anti-parasitic properties, has been the subject of this study to evaluate its efficacy and safety in the treatment of bacterial external otitis in domestic canines. Through questionnaires administered to veterinary experts and the use of neutrosophic logic for hypothesis test-ing, perceptions based on clinical experiences and scientific literature were analyzed. The results indicate strong agree-ment that propolis is effective and safe, surpassing some aspects of conventional antibiotic treatments and demonstrating high tolerability without serious side effects. This study examines the role of higher education in personality formation, highlighting its influence on the development of critical thinking, autonomy in decision-making, social skills, and creativity. While there is a favorable influence in these areas, the data also highlight limitations in developing problem-solving skills and cementing ethical and moral ideals. Emotional management has a neutral influence, whereas social awareness and citizenship reveal modest improvement, suggesting a need for increased attention on building civic involvement. Background: During public health emergencies, demand for N95 filtering facepiece respirators (N95 FFRs) can outpace supply. Elastomeric half-mask respirators (EHMRs) are a potential alternative that are reusable and provide the same or higher levels of protection. This study sought to examine the practical aspects of EHMR use among health care personnel (HCP).

Methods: Between September and December 2021, 183 HCPs at 2 tertiary referral centers participated in this 3-month EHMR deployment, wearing the EHMR whenever respiratory protection was required according to hospital protocols (ie, when an N95 FFR would typically be worn) and responding to surveys about their experience.

Context: Cardiovascular disease (CVD) in transgender women (TW) may be affected by gender-affirming hormone therapy (GAHT) and HIV, but few data compare TW on contemporary GAHT to well-matched controls.

Objective: We compared CVD burden and biomarker profiles between TW and matched cisgender men (CM).

Methods: Adult TW on GAHT (n = 29) were recruited for a cross-sectional study (2018-2020). CM (n = 48) from the former Multicenter AIDS Cohort Study were matched 2:1 to TW on HIV serostatus, age ±5 years, race/ethnicity, BMI category and antiretroviral therapy (ART) type. Recently published near full-length KSHV genomes from a Cameroon Kaposi sarcoma case-control study showed strong evidence of viral recombination and mixed infections, but no sequence variations associated with disease. Using the same methodology, an additional 102 KSHV genomes from 76 individuals with KSHV-associated diseases have been sequenced. Diagnoses comprise all KSHV-associated diseases (KAD): Kaposi sarcoma (KS), primary effusion lymphoma (PEL), KSHV-associated large cell lymphoma (KSHV-LCL), a type of multicentric Castleman disease (KSHV-MCD), and KSHV inflammatory cytokine syndrome (KICS). Participants originated from 22 different countries, providing the opportunity to obtain new near full-length sequences of a wide diversity of KSHV genomes. …

Platelet homeostasis is essential for vascular integrity and immune defence1,2. Although the process of platelet formation by fragmenting megakaryocytes (MKs; thrombopoiesis) has been extensively studied, the cellular and molecular mechanisms required to constantly replenish the pool of MKs by their progenitor cells (megakaryopoiesis) remains unclear3,4. Here we use intravital imaging to track the cellular dynamics of megakaryopoiesis over days. We identify plasmacytoid dendritic cells (pDCs) as homeostatic sensors that monitor the bone marrow for apoptotic MKs and deliver IFNα to the MK niche triggering local on-demand proliferation and maturation of MK progenitors. In this work, polybutadiene (PBD) was prepared from waste tires. The halogenated of polybutadiene was grafted with mono, di, and triethanolamine, the grafting was studied by X-ray diffraction and scanning electron microscopy. And according to the thermal characteristics (TGA and DSC), the produced compound was thermally stable up to 202 °C. Using potentiodynamic polarization techniques, the adsorption and corrosion inhibiting effects of three grafting polymers on N80 steel in 0.1 N H3PO4 as aggressive environments were measured at four different temperatures: 25, 35, 45, and 55°C. Nanopore sensors have garnered significant attention recently, not only for their groundbreaking advancements in single-molecule DNA sequencing but also for their diverse applications in stochastic sensing, single-molecule chemistry, and protein unfolding. The interaction of single molecules with the nanopore sensing interface leads to characteristic ionic current modulations, reflecting the composition, charge, distribution, structure, and sequence of the translocating molecule. With the electrical resistive pulse nanopore sensing, its single-molecule sensitivity and specificity have opened doors to numerous potential applications in personalized medicine. Background: Early syphilitic lesions are typically painless; however, several recent case studies have included patients with tender lesions and no evidence of concurrent infections. Here we present the manifestations and serological and molecular findings of a patient from New York State with a painful tongue lesion.

Methods: The diagnosis of syphilis was based on a combination of physical examination, serologic, pathologic, and immunohistochemical findings. DNA obtained from a formalin-fixed, paraffin-embedded biopsy was used to characterize the infecting pathogen using polymerase chain reaction, multilocus sequence typing, and whole-genome sequencing methods.

Objective: To assess how antiretroviral therapy (ART) initiation during acute or early HIV infection (AEHI) affects the viral reservoir and host immune responses.

Design: Single-arm trial of ART initiation during AEHI at 30 sites in the Americas, Africa, and Asia.

Methods: HIV DNA was measured at week 48 of ART in 5 million CD4 + T cells by sensitive qPCR assays targeting HIV gag and pol . Infectious diseases caused by microbial pathogens remain a primary contributor to global health burdens. Prompt control and effective prevention of these pathogens are critical for public health and medical diagnostics. Conventional microbial detection methods suffer from high complexity, low sensitivity, and poor selectivity. Therefore, developing rapid and reliable methods for microbial pathogen detection has become imperative. Climate Change will increase the frequency and intensity of disasters necessitating United States Department of Defense (DoD) intervention. The US Congress and DoD have produced reports and demanded action for climate change-related security concerns, but have largely left foreign disaster relief capacity unaddressed. The DoD does not have dedicated force structure for humanitarian assistance and disaster relief (HA/DR), potentially compromising mission readiness and operational effectiveness as well as jeopardizing U.S. foreign interests. The salamander, Ambystoma annulatum, is considered a “species of special concern” in the state of Arkansas, USA, due to its limited geographic range, specialized habitat requirements and low population size. Although metazoan parasites have been documented in this salamander species, neither its native protists nor microbiome have yet been evaluated. This is likely due to the elusive nature and under-sampling of the animal. Here, we initiate the cataloguing of microbial associates with the identification of a new heterlobosean species, Naegleria lustrarea n. sp. (Excavata, Discoba, Heterolobosea), isolated from feces of an adult A. annulatum.

White supremacy consists of intricate ideologies, systems, privileges, and personal beliefs that result in unequal outcomes based on race in various aspects of life, such as wealth, freedom, health, and happiness (Belew & Gutiérrez, 2021). While considerable scholarship has explored responsibilities owed to research participants at the conclusion of explanatory clinical trials, no guidance exists regarding responsibilities owed at the conclusion of a pragmatic clinical trial (PCT). Yet post-trial responsibilities in PCTs present distinct considerations from those emphasized in existing guidance and prior scholarship. INTRODUCTION: Improving the delivery of existing evidence-based interventions to prevent and diagnose HIV is key to Ending the HIV Epidemic in the United States. BACKGROUND: Valganciclovir is the only approved antiviral for cytomegalovirus (CMV) prevention in pediatric solid organ transplantation (SOT). Additional approaches may be needed to improve outcomes.

METHODS: A multicenter retrospective study from 2016 to 2019 was conducted of pediatric SOT recipients in whom at least 3 months of valganciclovir prophylaxis was planned. Episodes of CMV DNA in blood (DNAemia), CMV disease, drug-related toxicities, as well as other infections in the first year posttransplant and demographic and clinical data were collected. CMV DNAemia in the first year after prophylaxis or during prophylaxis (breakthrough) was analyzed by multivariate hazard models.
