High-Salt Tumor Microenvironment: Not As Bad As It Sounds, Not As Good As It Seems, Umer Ali, Venkataswarup Tiriveedhi

Recent evidence suggests a high-sodium microenvironment in breast tumors. However, the exact role of this high-sodium microenvironment on tumorigenesis is unknown. Salt (sodium chloride, NaCl) is a well-known inflammatory molecule playing a significant role in various chronic ailments like cardiovascular and autoimmune diseases. Importantly, chronic inflammation is recognized as one of the major hallmarks of carcinogenesis. Breast cancer cell culture-based studies demonstrated that high-salt (HS) treatment (Δ35–50 mM NaCl) induced cancer cell proliferation. However, preclinical murine research showed reduced tumor progression kinetics in mice fed a short-term HS diet (4% NaCl diet, 0–2 weeks prior to the injection of tumor …

This Doctor of Nursing Practice (DNP) project is a staff education program aimed at improving nursing care for elderly patients with post-stroke depression (PSD) through education on the collaborative care model (CCM). The practice concern addressed is the underrecognition and undertreatment of PSD in elderly patients in a rehabilitation setting due to a lack of staff knowledge and confidence in screening for PSD in the elderly. Students suffer from achievement gaps in higher education. Achievement gaps refer to differences in graduation rates, retention rates, persistence rates, and student's grades. Achievement gaps may negatively affect students who are culturally or ethnically diverse and/or have a lower socioeconomic status. The use of open educational resources (OER) may mitigate achievement gaps as OER assists student success through higher academic performance and cost savings for students. Average quiz scores were obtained at different higher education institutions using an Introduction to Psychology OER. The scores from each course were analyzed using hierarchical linear modeling. High-risk myelodysplastic syndromes (MDS) and acute myeloid leukemia (AML) are aggressive hematologic malignancies with poor prognoses, where extending survival often comes at the cost of substantial morbidity. This study was grounded in the quality of survival (QoS) framework, which emphasizes that treatment outcomes should balance longevity with the health-related quality of life (HRQoL) experienced during the period of survival. In this context, patient-reported outcomes are crucial to capturing what truly matters to patients. However, existing patient-reported outcome measures (PROMs) used in MDS/AML clinical trials may not fully reflect patient priorities. The TYDE programme is a college-in-school initiative which aims to develop partnerships between Technological University Dublin (TU Dublin) and local schools. It provides second-level students with an authentic experience of what is required to succeed in university programmes within the familiar environment of school. This experience supports students in transitioning from secondary school to university life, encourages participation, and supports good college and career choices. Transition-year students (15–16 year-olds) take core university modules from engineering programmes running at TU Dublin. Correlative studies have linked human gut microbes to specific health conditions. Alistipes is one such microbial genus negatively linked to inflammatory bowel disease (IBD). However, the protective role of Alistipes in IBD is understudied, and the underlying molecular mechanisms remain unknown. In this study, colonization of Il10-deficient mice with Alistipes timonensis DSM 27924 delays colitis development. Colonization does not significantly alter the gut microbiome composition, but instead shifts the host plasma lipidome, increasing phosphatidic acids while decreasing triglycerides. Outer membrane vesicles (OMVs) derived from Alistipes are detected in the plasma of colonized mice, carrying potentially immunomodulatory metabolites into the …

Pyruvate kinase deficiency (PKD) is an autosomal recessive disorder and the most common cause of chronic hemolytic anemia secondary to red blood cell (RBC) enzyme defects. Treatment is mainly supportive including splenectomy and transfusions. Mitapivat, an oral allosteric activator of defective pyruvate kinase, was approved by the FDA in February 2022 for treatment of adults with PKD. However, because Mitapivat undergoes liver metabolism, patients must have stable liver profiles prior to initiation of therapy. We present a case of PKD complicated by hemophagocytic lymphohistiocytosis (HLH). We initiated steroid therapy for his HLH with the intent to subsequently start Mitapivat. We …

Since HUD Secretary Henry Cisneros' pledge in 1995 to “end public housing as we know it, ” the United States has seen the rapid depletion of public housing stock. Alongside this mass demolition, a new redevelopment paradigm emerged: a community-oriented planning model that requires resident participation. HUD’ s shift toward participatory planning began with HOPE VI in 1992, which sought private investment to demolish public housing, and was formalized in 2010 through the Choice Neighborhoods Initiative (CNI). Adolescents are a developmentally distinctive population with unique primary health care needs; they comprise about 10% of Oregon's population and more than half are covered by the Oregon Health Plan (OHP). This dissertation investigated significant declines in preventive care utilization (well visits and HPV immunizations) among adolescents enrolled in OHP during the COVID-19 pandemic and the relationship between the decline in utilization and the rise in telehealth availability. The study focused specifically on the role of race/ethnicity, geography, and access to health systems in mitigating or exacerbating decreases in preventive care. The Portland study examined past planning regulations and greenspace distribution over the period of fifty years, with particular attention given to neighborhood differences between invested and non-invested neighborhoods. This research utilized a mixed-methods approach through the synthesis of qualitative themes from oral narratives and archival records. Furthermore, I examined spatial greenspace using Normalized Difference Vegetation Index (NDVI) imagery. I tested the study hypothesis using the GIS Ordinary Least Squares (OLS) regression test to predict how key sociodemographic characteristics affect the provision of greenspace in non-white disinvested neighborhoods. PURPOSE: Hematopoietic stem cell transplantation (HSCT) is used as consolidation for pediatric patients with high-risk AML in first complete remission (CR1). The definition of high-risk AML has evolved considerably over the past two decades with the successive identification of new unfavorable risk factors. We conducted a cross-study analysis to determine whether HSCT improves the outcomes of patients with contemporarily defined high-risk AML.

Tisagenlecleucel (tisa-cel) and brexucabtagene autoleucel (brexu-cel) are approved CD19 chimeric antigen receptor T-cell therapy (CAR T) products for young adults (YA) with relapsed/refractory B-cell acute lymphoblastic leukemia. A distinct analysis of YAs receiving commercial CD19 CAR T has not been reported. Using retrospective data from the Pediatric Real-World CAR T Consortium and the Real-World Outcomes of CAR T in Adult ALL collaboration, we describe the efficacy and safety of tisa-cel and brexu-cel in 70 YAs (18-26 years; tisa-cel, n = 50; brexu-cel, n = 20). OBJECTIVES: Status epilepticus (SE) is a neurologic emergency that requires urgent recognition and medical management. SE management remains heterogeneous across centers.
METHODS: We analyzed SE treatment protocols from level 3 and level 4 epilepsy centers. Discrete data including stabilization measures, timing of treatment phases, medications, doses, and routes of administration were collected from each protocol and described using frequency for categorical variables and median for continuous variables. The distribution of treatment times and dosing were compared with the AES guideline.

In 1957, the Brazilian government approved Law No.1310, establishing a free port in the city of Manaus, the capital of the state of Amazonas. Politicians at the time argued that such a project was essential to help mitigate the unfortunate economic circumstances the state faced due to its geographical isolation. Ten years later, the military dictatorship that ruled Brazil expanded that mission through Decree No.288, thus creating what is now known as the Zona Franca de Manaus (ZFM, Free Trade Zone, or FTZ), an area with a series of fiscal incentives to foment industrialization in the Amazon region. This thesis examines the catastrophic 2024 flooding in Rio Grande do Sul, Brazil—locally known as "The Flood"—through the lens of young environmental activists' experiences in Porto Alegre. Drawing on urban political ecology, disaster studies, and climate justice frameworks, this research examines how colonial-era urban planning and infrastructure influenced the socio-environmental conditions that led to The Flood, how the disaster affected young activists' relationships with their city, and what climate justice might entail in this context. As videoconferencing psychotherapy (VCP) becomes an increasingly common component of mental health service delivery, little is known about how trainees experience VCP training during their graduate education. Gaining insight into trainees’ lived experiences is essential to understanding how they navigate virtual care and how training programs can better prepare future clinicians. This study employed Consensual Qualitative Research (CQR) methodology to explore how trainees experienced their VCP training and how it shaped their clinical development. Wireless Emergency Alerts are an important way people learn about impending threats. Yet, these messages typically lack information about the hazard that will endanger people. In this study, we examine how naming the hazard and its associated cues in a Wireless Emergency Alert influences people’s perceptions that help them make protective action decisions, including understanding, belief, personalization, deciding, response efficacy, and self-efficacy. We also measure the extent to which naming the hazard and its cues affects an organization’s reputation and perceived transparency. The Elder Justice Act (EJA) was a significant policy that passed in 2010 as part of the Patient Protection and Affordable Care Act. The goal of the EJA was to provide public health and social service programs for preventing, detecting, and treating elder abuse. How can we lower Oregon’s youth substance use disorder (SUD) and overdose rates? Oregon is third in the country for SUD, with 5.77% of adolescents (12-17) having documented SUD. Evidence-based policy making is the notion that policy decisions should be based on (1) rigorous data collection and research and (2) focus on ‘what works.’ This approach aligns with the broader theory of New Public Management (NPM), which contends that the public sector should adopt the practices of and operate more like the private sector, including a reliance on measuring performance. Zoonoses are infectious diseases transmitted between animals and people that disproportionately affect low- and middle-income countries (LMICs). The One Health (OH) approach aims to integrate systems, incentivizing collaboration among animal, human, and environmental systems to address zoonoses and improve health. Innovative solutions for effectively and sustainably achieving systems integration are needed. Organizational theories can facilitate the study of the context of OH implementation, applying comprehensive approaches that recognize the interconnections among organisms, the environment, and social systems, including the diverse entities needing integration (e.g., policy, community, public organizations). This research explores hybristophilia (an individual’s sexual interest in and attraction to those who commit crimes) among Generation Z women, particularly through investigating how Gen Z female users interact with and feel about hybristophilic content and how this content impacts their attraction to offenders. Study 1 involved a content analysis of 66 videos and 91 comments on TikTok, identifying seven main themes: The Halo Effect, Actor-Offender Transference, Sympathy, Romance and APD, Protection and Loyalty, “I Can Fix Him,” Gen Z Irony, and Victim Fantasy.
