Introduction: Meridianin C (MC) is a marine-derived indole alkaloid that has demonstrated kinase inhibitory and anti-tumor activities. Despite its diverse biological properties, no previous reports have systematically evaluated the in vivo quantitative analysis of MC and its metabolites. Methods: In this study, MC was synthesized following a previously reported procedure with slight modifications. A sensitive, accurate, and reliable ultra-high performance liquid chromatography-tandem mass spectrometry (UHPLC–MS/MS) method was developed to simultaneously detect MC and its five major metabolites (MC-1-N-O-GluA, MC-1-N-O-SO3H, MC-2′-N-O-GluA, MC-2′-N-O-SO3H, and MC-O-GluA-didehydration) in rat plasma. BACKGROUND: Readmission following endoscopic endonasal transsphenoidal surgery (EETS) for pituitary neuroendocrine tumor (PitNET) and other sellar pathology is most commonly due to delayed hyponatremia. Studies suggest postoperative fluid restriction (FR) reduces delayed hyponatremia. We present a prospective randomized controlled study evaluating post-EETS FR.

METHODS: 300 participants were scheduled for EETS (2016-2023) at a single institution. Patients with CKD, CHF, arginine vasopressin deficiency on postoperative day (POD) 3, chronic hyponatremia, and untreated adrenal insufficiency or hypothyroidism were excluded. Objectives: Anthracyclines are widely used in the treatment of breast cancer. One of the known adverse effects of anthracyclines is anthracycline-induced cardiomyopathy. Studies done worldwide showed that the prevalence of chemotherapy-induced cardiomyopathy is approximately 9%. However, studies evaluating this complication prevalence, risk factors and cumulative dose are lacking in Oman. Electric Submersible Pumps (ESPs) are one of the important artificial lift methods for sustaining production in mature and high-water-cut wells; but may suffer frequent failures due to mechanical, electrical, hydraulic, chemical, and operational failures. These failures can yield substantial deferred production and intervention costs. Plenty of ESP installations are fitted with downhole sensors. Yet, it is observed that the current industry practice underutilizes the wealth of available sensor and operational data and lacks standardized, explainable failure-type identification and classification.

BACKGROUND: Unsupervised cognitive assessments are becoming commonly used in studies of aging and neurodegenerative diseases. As assessments are completed in everyday environments and without a proctor, there are concerns about how common distractions may impact performance and whether these distractions may differentially impact those experiencing the earliest symptoms of dementia.

OBJECTIVE: We examined the impact of self-reported interruptions, testing location, and social context during testing on remote cognitive assessments in older adults.

While micro- and nanoplastics are now found in water, soil, plants, animals, and even human bodies, their long-term effects on ecosystems, food and water safety, and human health remain poorly understood. These particles can infiltrate critical systems, potentially affecting troop readiness, infrastructure resilience, and national food security. This research assesses how advancements in composites material research affect communities from lower socioeconomic backgrounds, using a dual track strategy to quantitatively and qualitatively examine their impact. A keyword-based classification algorithm was applied to a sample of research papers from ScienceDirect spanning the last 24 years to quantify which engineering industries benefit most from this research. The findings indicate that the construction industry benefits the most, followed by the automotive, defense, renewable energy, and biomedical sectors. Unpredictability in early life is an understudied aspect of early life adversity that provides important signals to the child about their environment. Recent work shows the predictability of patterns of parental signals on a moment-to-moment timescale sculpts the developing brain during sensitive windows, offering a potential pathway through which early life experiences shape child development. Both experimental non-human animal models and observational human research show sex-specific links between early life exposure to unpredictable patterns of parental sensory signals and aberrant offspring development. Depression is common yet frequently underdiagnosed in group home settings serving adult and geriatric residents. This Doctor of Nursing Practice (DNP) project aimed to improve staff knowledge and promote routine use of the Patient Health Questionnaire-9 (PHQ-9), a validated depression screening tool, through an evidence-based educational intervention. A practice gap was identified at the project site due to inconsistent use and documentation of the PHQ-9, primarily linked to staff knowledge deficits. The problem explored in this study was the low graduation rate of first-generation college students at the university study site. Using Bandura’s self-efficacy theory, the purpose of this basic qualitative study was to explore first-generation college graduates’ beliefs about their educational experience as students upon entering a bachelor’s degree program, identify obstacles, and understand ways to master those obstacles to successfully graduate. This dissertation examines the role literature plays in understanding black life post-reconstruction and post-civil rights movement. Specifically, I explore the continuation of injustices and mistreatment for black workers, the role black labor plays during the first and second post-reconstructions, and how literature can be used in framing this discussion. The similarities between these periods help us to better understand the cycle of racist societal expectations and the challenges black workers face integrating into society, discriminatory mandates required of black workers, the unfair job market, and forced labor. This dissertation analyzes conceptual metaphors of psychological motion and stability in medieval English religious literature. I trace how language itself structures cultural approaches to epistemology, spiritual knowing, and virtue. Chapter One examines Alfred’s translation of Neoplatonic metaphors of psychological movement in the OE translations of The Consolation of Philosophy and Augustine’s Soliloquies. I argue that the reconfiguration of these metaphors into images of stability reveals an epistemology centered on certainty. Chapter Two compares the omission of motive psychological metaphor in Cynewulf’s Juliana and Ælfric’s “St. Agatha and St. Lucy” in process of translation. Single-cell studies on breast tissue have contributed to a change in our understanding of breast epithelial diversity that has, in turn, precipitated a lack of consensus on breast cell types. The confusion surrounding this issue highlights a possible challenge for advancing breast atlas efforts. In this perspective, we present our consensus on the identities, properties, and naming conventions for breast epithelial cell types and propose goals for future atlas endeavors. Our proposals and their underlying thought processes aim to catalyze the adoption of a shared model for this tissue and to serve as guidance for other investigators facing similar challenges.

Drills Or Distress? Understanding The Psychological Effects Of Active Shooter Drills On Adolescents, Jeff R Temple, Elizabeth Baumler, Christian Paige Owen, Leila Wood

Background: We sought to better understand the impact of exposure to active shooter drills (ASDs) on students' perceived stress, including those with disabilities and prior experiences with trauma.

Methods: We used data from a longitudinal study of ethnically diverse students (n = 2,033; 53.5% female) originally recruited in 2018 and followed annually thereafter. In addition to self-reporting on their perceived stress with ASDs, participants responded to questions about adverse childhood experiences, anxiety, suicidal ideation, and disability.

Results: Female students and those with a disability, probable anxiety, and suicidal ideation were more likely to be anxious after an ASD.

Hypertension has been previously associated with higher intraoperative bleeding risk in patients undergoing Mohs micrographic surgery. However, the risks of common postoperative Mohs micrographic surgery complications have not been substantiated by large studies. Our study aimed to assess the risks of common Mohs micrographic surgery complications for patients with hypertension utilizing the TriNetX database, a global health records database encompassing over 105 million patients' data, from January 2009 to January 2024. After 1:1 propensity score matching for age, sex, race, diabetes, nicotine dependence, obesity, and anticoagulant use, the final analysis included 80,739 patients in each cohort of patients with hypertension …

The prevalence of tobacco and alcohol consumption among adolescents in Southeast Asia remains a public health concern despite the introduction of several national policies. This study assessed the relationship between policy implementation and consumption trends of tobacco and alcohol in selected ASEAN countries. Using a descriptive ecological design, we analyzed secondary data from the Global School-based Student Health Survey (GSHS) and policy documents from official government sources. We observed a rise in tobacco consumption in Indonesia (+4.2%) and Brunei (+5.5%), while Thailand experienced a slight increase (+0.3%). The Effects Of Input And Characteristic Factors On Contact Rate Achievement Among Puskesmas In Indonesia, Iqbal Andri Makarim, Pujiyanto Pujiyanto

The contact rate is the lowest-performing indicator in the Performance-Based Capitation Payment system initiated by BPJS Kesehatan. Analysis of the 2019 National Health Facility Research (Rifaskes) data showed that less than half of Puskesmas in Indonesia achieved the target. This cross-sectional study analyzed 5,384 Puskesmas from the 2019 Rifaskes to examine the effect of input and characteristic factors on achieving the contact rate. Independent variables included health worker ratios (doctors, nurses, midwives per 5,000 participants), completeness of essential medical equipment, communication facilities, P-Care application use, and Puskesmas characteristics. Multivariate logistic regression identified four significant factors: doctor ratio (OR=1.06; p<0.0001), midwife ratio (OR=1.01; p=0.033), completeness of essential medical equipment (OR=1.16; p=0.029), and BLUD-based financial management (OR=1.13; p=0.029). These factors explained only 2.6% of the variance (Nagelkerke R²=0.026), suggesting other unmeasured influences. Policy implications include equitable distribution of health personnel, targeted procurement of essential equipment, strengthening financial autonomy through BLUD, and optimizing P-Care data utilization to improve service accessibility and performance.

Cancer treatment often fails because combinations of different therapies evoke complex resistance mechanisms that are hard to predict. We introduce REsistance through COntext DRift (RECODR): a computational pipeline that combines co-expression graph networks of single-cell RNA sequencing profiles with a graph-embedding approach to measure changes in gene co-expression context during cancer treatment. RECODR is based on the idea that gene co-expression context, rather than expression level alone, reveals important information about treatment resistance. Chronic urinary tract infections are persistent bacterial infections with the potential to drive antibiotic resistance. Like other persistent bacterial infections, intracellular bacterial reservoirs and biofilm formation hinder the clearance of pathogens despite long courses of antibiotic therapy. New strategies for treatment of these persistent infections are needed. Here we describe the results of an open-label individual patient study using bacteriophage therapy to treat a chronic urinary tract infection in an immunocompetent 72-year-old woman without underlying urolithiasis or indwelling devices. We co-administered HP3.1, HP3, and ES19 bacteriophages with demonstrated in vitro activity via bladder instillation, orally, and as a topical formulation. …

Lung adenocarcinomas (LUAD) are a pressing global health problem with enduring lethality and rapidly shifting epidemiology. Proteogenomic studies integrating proteomics and post-translational modifications with genomics can identify clinical strata and oncogenic mechanisms, but have been underpowered to examine effects of ethnicity, smoking and environmental exposures, or sex on this heterogeneous disease. This comprehensive proteogenomic analysis of LUAD tumors and matched normal adjacent tissues from 406 patients across diverse geographic and demographic backgrounds explores the impact of understudied driver mutations, prognostic role of chromosomal instability, patterns of immune signaling, differential and sex-specific effects of endogenous mutagens and environmental carcinogens, and pathobiology …
