Objective:

Unpredictability in early life is an understudied aspect of early life adversity that provides important signals to the child about their environment. Recent work shows the predictability of patterns of parental signals on a moment-to-moment timescale sculpts the developing brain during sensitive windows, offering a potential pathway through which early life experiences shape child development. Both experimental non-human animal models and observational human research show sex-specific links between early life exposure to unpredictable patterns of parental sensory signals and aberrant offspring development. Depression is common yet frequently underdiagnosed in group home settings serving adult and geriatric residents. This Doctor of Nursing Practice (DNP) project aimed to improve staff knowledge and promote routine use of the Patient Health Questionnaire-9 (PHQ-9), a validated depression screening tool, through an evidence-based educational intervention. A practice gap was identified at the project site due to inconsistent use and documentation of the PHQ-9, primarily linked to staff knowledge deficits. The project explored whether educating staff on PHQ-9 administration, scoring, and documentation would lead to improved knowledge, as measured by pre- and post-intervention assessments. The problem explored in this study was the low graduation rate of first-generation college students at the university study site. Using Bandura’s self-efficacy theory, the purpose of this basic qualitative study was to explore first-generation college graduates’ beliefs about their educational experience as students upon entering a bachelor’s degree program, identify obstacles, and understand ways to master those obstacles to successfully graduate. Six first-generation college graduates from the study site were recruited from FIRST, an on-campus support organization, for semistructured interviews using purposeful sampling. Cyclical Matters: Black Labor, Temporality, And The Incomplete Reconstructions Within African American Literature, Courtney Walton

This dissertation examines the role literature plays in understanding black life post-reconstruction and post-civil rights movement. Specifically, I explore the continuation of injustices and mistreatment for black workers, the role black labor plays during the first and second post-reconstructions, and how literature can be used in framing this discussion. The similarities between these periods help us to better understand the cycle of racist societal expectations and the challenges black workers face integrating into society, discriminatory mandates required of black workers, the unfair job market, and forced labor. These periods interact cyclically in that the second post-reconstruction models the first post-reconstruction. …

Economic inequality is a global concern with many adverse consequences for people across class divides. These consequences vary in scope—from negative effects on individual’s physical, mental, and social health to large-scale economic shifts which result from climate change (Galbraith & Hale, 2008; Hiesh & Pugh, 1993; Kaplan et al., 1996; La Ferrara & Alesina, 2000; Solt, 2008; Vilhjalmsdottir et al., 2016; Wilkinson & Pickett, 2007). “The Wonderful Swiftness Of The Soul”: Mental Movement And Religious Ethics In Medieval English Literature, Abigail Palmisano

This dissertation analyzes conceptual metaphors of psychological motion and stability in medieval English religious literature. I trace how language itself structures cultural approaches to epistemology, spiritual knowing, and virtue. Chapter One examines Alfred’s translation of Neoplatonic metaphors of psychological movement in the OE translations of The Consolation of Philosophy and Augustine’s Soliloquies. I argue that the reconfiguration of these metaphors into images of stability reveals an epistemology centered on certainty. Chapter Two compares the omission of motive psychological metaphor in Cynewulf’s Juliana and Ælfric’s “St. Agatha and St. Lucy” in process of translation. Single-cell studies on breast tissue have contributed to a change in our understanding of breast epithelial diversity that has, in turn, precipitated a lack of consensus on breast cell types. The confusion surrounding this issue highlights a possible challenge for advancing breast atlas efforts. In this perspective, we present our consensus on the identities, properties, and naming conventions for breast epithelial cell types and propose goals for future atlas endeavors. Our proposals and their underlying thought processes aim to catalyze the adoption of a shared model for this tissue and to serve as guidance for other investigators facing similar challenges.

Drills Or Distress? Understanding The Psychological Effects Of Active Shooter Drills On Adolescents, Jeff R Temple, Elizabeth Baumler, Christian Paige Owen, Leila Wood

Background: We sought to better understand the impact of exposure to active shooter drills (ASDs) on students' perceived stress, including those with disabilities and prior experiences with trauma.

Methods: We used data from a longitudinal study of ethnically diverse students (n = 2,033; 53.5% female) originally recruited in 2018 and followed annually thereafter. In addition to self-reporting on their perceived stress with ASDs, participants responded to questions about adverse childhood experiences, anxiety, suicidal ideation, and disability.

Results: Female students and those with a disability, probable anxiety, and suicidal ideation were more likely to be anxious after an ASD.

Hypertension has been previously associated with higher intraoperative bleeding risk in patients undergoing Mohs micrographic surgery. However, the risks of common postoperative Mohs micrographic surgery complications have not been substantiated by large studies. Our study aimed to assess the risks of common Mohs micrographic surgery complications for patients with hypertension utilizing the TriNetX database, a global health records database encompassing over 105 million patients' data, from January 2009 to January 2024. After 1:1 propensity score matching for age, sex, race, diabetes, nicotine dependence, obesity, and anticoagulant use, the final analysis included 80,739 patients in each cohort of patients with hypertension …

The prevalence of tobacco and alcohol consumption among adolescents in Southeast Asia remains a public health concern despite the introduction of several national policies. This study assessed the relationship between policy implementation and consumption trends of tobacco and alcohol in selected ASEAN countries. Using a descriptive ecological design, we analyzed secondary data from the Global School-based Student Health Survey (GSHS) and policy documents from official government sources. We observed a rise in tobacco consumption in Indonesia (+4.2%) and Brunei (+5.5%), while Thailand experienced a slight increase (+0.3%). The Effects Of Input And Characteristic Factors On Contact Rate Achievement Among Puskesmas In Indonesia, Iqbal Andri Makarim, Pujiyanto Pujiyanto

The contact rate is the lowest-performing indicator in the Performance-Based Capitation Payment system initiated by BPJS Kesehatan. Analysis of the 2019 National Health Facility Research (Rifaskes) data showed that less than half of Puskesmas in Indonesia achieved the target. This cross-sectional study analyzed 5,384 Puskesmas from the 2019 Rifaskes to examine the effect of input and characteristic factors on achieving the contact rate. Independent variables included health worker ratios (doctors, nurses, midwives per 5,000 participants), completeness of essential medical equipment, communication facilities, P-Care application use, and Puskesmas characteristics. Multivariate logistic regression identified four significant factors: doctor ratio (OR=1.06; p<0.0001), midwife ratio (OR=1.01; p=0.033), completeness of essential medical equipment (OR=1.16; p=0.029), and BLUD-based financial management (OR=1.13; p=0.029). These factors explained only 2.6% of the variance (Nagelkerke R²=0.026), suggesting other unmeasured influences. Policy implications include equitable distribution of health personnel, targeted procurement of essential equipment, strengthening financial autonomy through BLUD, and optimizing P-Care data utilization to improve service accessibility and performance.

Cancer treatment often fails because combinations of different therapies evoke complex resistance mechanisms that are hard to predict. We introduce REsistance through COntext DRift (RECODR): a computational pipeline that combines co-expression graph networks of single-cell RNA sequencing profiles with a graph-embedding approach to measure changes in gene co-expression context during cancer treatment. RECODR is based on the idea that gene co-expression context, rather than expression level alone, reveals important information about treatment resistance. Chronic urinary tract infections are persistent bacterial infections with the potential to drive antibiotic resistance. Like other persistent bacterial infections, intracellular bacterial reservoirs and biofilm formation hinder the clearance of pathogens despite long courses of antibiotic therapy. New strategies for treatment of these persistent infections are needed. Here we describe the results of an open-label individual patient study using bacteriophage therapy to treat a chronic urinary tract infection in an immunocompetent 72-year-old woman without underlying urolithiasis or indwelling devices. We co-administered HP3.1, HP3, and ES19 bacteriophages with demonstrated in vitro activity via bladder instillation, orally, and as a topical formulation. …

Lung adenocarcinomas (LUAD) are a pressing global health problem with enduring lethality and rapidly shifting epidemiology. Proteogenomic studies integrating proteomics and post-translational modifications with genomics can identify clinical strata and oncogenic mechanisms, but have been underpowered to examine effects of ethnicity, smoking and environmental exposures, or sex on this heterogeneous disease. Introduction: Snakebite-related deaths are major concerns in populations worldwide that lack access to medical care. Current treatment of snakebites includes administration of polyspecific and polyvalent heterologous equine antiserum. Swift diagnosis of the species of snake responsible is essential to initiate a specific treatment.

Methods: We generated murine monoclonal and rabbit polyclonal antibodies against the venom of the 3 most common venomous snakes in southern India: the Russell viper, the saw-scaled viper, and the Indian cobra. We developed an enzyme-linked immunosorbent assay to distinguish the snake by analyzing the exudates from the snakebite wound.

Background: Children born with a congenital anomaly have a higher risk of developing a brain tumor during childhood or adolescence, but the co-occurrence between specific types of congenital anomalies and specific types of childhood brain tumors (CBTs) is not well described. This study characterized the associations between specific congenital anomalies and CBTs.

Methods: We leveraged a population-based registry linkage study of births (1990-2018), congenital anomalies, and cancer from 9 states (n = 22,599,099 births). Congenital anomalies were classified as major structural without a known chromosomal or genetic syndrome, chromosomal, neurofibromatosis, and/or tuberous sclerosis complex. Federal probation officers supervise millions of offenders who must each comply with a multitude of probation conditions. These officers need enough flexibility to deal with the evolving needs of each offender, without having to clog the court system with repeated requests for modifications. Yet federal courts differ in the amount of discretionary authority they grant to federal probation officers. Scholars and policymakers from multiple disciplines have long debated whether and when paternalistic intervention might be appropriate to guide ordinary decisionmakers choices and behaviors. Recently, the use of empirical data has begun to inform this debate. Some such research has demonstrated that individuals' susceptibility to cognitive and emotional biases leads to nonoptimal decisions in a variety of areas, including health, finance, and safety, among others. This has led some scholars to suggest a role for third-party intervention to help protect citizens from their own biased decisionmaking.

States often attempt to regulate political speech in the form of deliberate lies related to ballot initiatives, referenda, candidates, or their political positions. Some courts focus on the various harms of electoral lies, while others focus more on the risks of bias and partisan abuse involved in such speech regulations; the cases are in disarray.

This Article argues, however, that the most important problem underlying this case confusion is inherent in the widely utilized constitutional standard of strict scrutiny. Strict scrutiny typically requires a compelling governmental interest, "narrow tailoring," and some causal relation between the regulation and the compelling interest. …

As the lawsuits challenging the Patient Protection and Affordable Care Act (ACA) have evolved, one feature of the litigation has proven especially rankling to the legal academy: the courts' incorporation of substantive libertarian concerns into their structural federalism analyses. The breadth and depth of scholarly criticism is surprising, especially given that judges frequently choose indirect methods, including the structural and process-based methods at issue in the ACA litigation, for protecting substantive constitutional values. Spatial Stochastic Frontier Model With Stochastic Weighting Matrix, Ming-Yu Deng, Levent Kutlu

This paper develops a spatial autoregressive stochastic frontier model that allows the spatial weighting matrix to be stochastic. This approach captures spatial heterogeneity in the interactions among production units and quantifies random spatial dependencies arising from this variability. The model parameters are estimated using a simulated maximum likelihood approach. We propose a method to simulate spillover-corrected direct, indirect, and total efficiency estimates. Additionally, we conduct Monte Carlo simulations to evaluate the finite-sample performance of the proposed model. Finally, an empirical application to the efficiency of Spanish dairy farms is presented to demonstrate the practical implementation of our methodology.
