Hospital discharge planning is a critical determinant of patient outcomes, organizational performance, and financial sustainability. Many non-teaching urban hospitals continue to struggle with high readmission rates, especially among socially vulnerable populations such as the homeless. The purpose of this integrative review was to examine discharge planning strategies that would reduce readmissions, enhancing patient throughput, and mitigating financial penalties in non-teaching urban hospitals. The review question focused on effective discharge planning strategies that would improve readmission rates in non-teaching urban hospitals. Teachers are inundated with education legislation and the policy texts and discourses that surround them. Over nine months beginning in March 2024, I collaborated with three Florida P-12 teachers to explore their literacy practices and stories as they engaged with policy in a state that has recently passed an astounding amount of education legislation. This study explored how (1) teachers story the current policy context and their roles within it; (2) teachers use literacy practices to understand and respond to P/policies that impact their labor/working lives; and (3) teachers’ literate identities, agency, and collectivity intersect and shape one another. Accurate diagnosis of furcation defect is essential in periodontal therapy planning. Cone beam computed tomography (CBCT) provides three-dimensional visualization, but higher radiation doses remain a concern. This study aims to assess and compare the diagnostic accuracy of low-dose CBCT (LD-CBCT) and high-dose CBCT (HD-CBCT) in detecting and measuring furcation defect using a pig mandible. Detection and accurate assessment of periodontal disease is important to determine the tooth prognosis and treatment. This study investigates the associations of state-level regional economic indicators about the labor market and bankruptcy rates in United States. This research aims to fill a gap in the bankruptcy literature by analyzing how these factors are related to corporate and individual bankruptcy filings. The unemployment rate is the primary labor-market predictor of financial distress; I argue that a comprehensive analysis incorporating broader labor market statistics provides a more complete picture of bankruptcy trends.

OBJECTIVES: Acute laryngeal injury (ALgI) is created as a result of endotracheal tube pressure ulcer formation leading to fibrosis and inflammation. This condition often leads to airway obstruction and voice and swallowing dysfunction. This study demonstrates a reliable animal model of ALgI to reproduce the acute wound process seen clinically, to explore the pathophysiology of this disease process, and to serve as a reproducible injury suitable for the evaluation of therapeutic interventions.

In today’s landscape, copious amounts of unlabeled data continue to be generated. This data has the potential to accelerate machine learning research; however, supervised methods require labels and unsupervised methods often require expert fine-tuning to be reliable, both of which can impose significant cost. In addition to not requiring labels, another benefit of unsupervised learning is the protection of privacy since it does not require human annotation. In addition, class imbalance, where one class has significantly more instances, can complicate model training and reduce performance. Public School Districts Should Close Their “Alternative” And “Continuation” High Schools, Sean J. Drake

Public school districts across the country funnel academically struggling students into "alternative" or "continuation" high schools designed to help them catch up on credits and graduate on time. However, these schools often reproduce more inequality than they alleviate. This brief summarizes findings from a two-year ethnographic case study of an alternative high school in suburban Los Angeles, California. The author finds that these schools offer limited curricula and no extracurricular opportunities, while Black, Latino, and low-income students are vastly overrepresented compared to district averages. Students transferred to alternative schools are also more likely to drop out than low-performing peers who …

BACKGROUND: The field of implementation research has recently seen much interest in equity, with a strong emphasis on recognising and responding to disparities in care. Recent studies highlight the role of macro-level processes that translate meso-level institutional behaviours to micro-level healthcare practices, and that are generative of health and care inequities. They emphasise challenges patient-centredness and underscore the need for justice-oriented intervention design to address disparities and promote equitable care.

The number of students with disabilities is increasing, yet little is known about how graduate social work students navigate accommodation during field placements. This study addressed the limited understanding of Master of Social Work (MSW) students’ experiences applying Americans with Disabilities Act (ADA) accommodations in practicum settings. How accommodations are managed can determine whether disabled students gain equitable access to training and the profession. Aimed at promoting cross-cultural dialogue and exchange of ideas in the humanities, this article provides a Confucian response to the Stoic viewpoints expounded by Kutch in this journal. Two arguments are made in this paper: first, Confucius shares the Stoics’ emphasis on being mindful. Secondly, a significant difference between Confucius and the Stoics is that the former replaces the Stoics’ fate with Heaven (tian), which is a central Confucian spiritual ideal. Applying Confucian and Stoic insights to burnout, medical professionals are encouraged to (re)gain strength by reflecting on and embracing spiritual ideals, whether it is fate or Heaven. …

This Doctor of Nursing Practice (DNP) project addressed a gap in practice in an outpatient treatment center for discharging patients with substance use disorder (SUD), addressing the need for follow-up care and relapse prevention due to the widespread impact of SUD. Clinical practice guidelines (CPG) help healthcare professionals make informed decisions for specific clinical situations. The guiding question asked whether an expert panel would evaluate and validate a CPG developed from current, evidence-based literature and recommendations with 70% agreement using the AGREE II tool. Timely digital surveillance is essential because clinical reporting often lags. This quantitative, longitudinal correlational study tested whether weekly Google searches in Austria signal changes in COVID-19 cases and vaccination uptake, and short-term case-vaccination dynamics, guided by Rogers’ Diffusion of Innovations Theory. Weekly Google Trends indices (0-100) and nationally reported surveillance totals (weekly counts of new cases and vaccine doses) were aggregated; cases: N = 175 weeks (Feb 2020–Jun 2023); vaccinations: N = 132 weeks (Dec 2020–Jun 2023). This Doctor of Nursing Practice (DNP) staff education initiative addressed the problem of polypharmacy, a common issue in home health care that increases the risk of drug interactions, side effects, and hospitalizations. Nurses and aides often provide daily care but may not recognize high-risk medications or warning symptoms. Addressing this gap was vital to nursing practice because staff are on the front line of patient monitoring and safety. Evidence for an association between insulin-like growth factors (IGF) and multiple myeloma (MM) is inconsistent. We examined total IGF-I concentrations and risk of MM by combining baseline serological data among UK Biobank participants (n = 444 187; 732 incident MM) with a two-sample Mendelian randomisation (MR) analysis using identified genetic variants associated with circulating total IGF-I and IGF-binding protein 3 (IGFBP-3) in the InterLymph consortium (2434 MM and their 2567 controls). Finally, additional lymphoid neoplasm (LN) subtypes were included for comparison with the main hypothesis. Chilaiditi sign is a rare radiographic finding demonstrating interposition of the colon between the liver and the diaphragm; Chilaiditi syndrome is the manifestation of gastrointestinal or respiratory symptoms secondary to this finding. This syndrome rarely can be associated with acute and chronic respiratory failure. We report a case of a 66-year-old woman with chronic hypercapnic and hypoxemic respiratory failure due to severe restrictive pulmonary mechanics in the setting of Chilaiditi syndrome. Imaging showed an elevated right hemidiaphragm initially concerning for hemidiaphragm paralysis; however a fluoroscopic Sniff test suggested normal diaphragm function. Background: Fam-trastuzumab deruxtecan (T-DXd) is an antibody-drug conjugate (ADC) that targets human epidermal growth factor receptor 2 (HER2) and delivers a topoisomerase inhibitor payload. T-DXd has been effectively used to treat metastatic breast cancer but causes pneumonitis in 10–15% of cases. Risk factors associated with T-DXd pneumonitis are not well described.

Research question: What are the major clinical risk factors for T-DXd pneumonitis?

Major depressive disorder (MDD) is a common and debilitating mental disorder associated with a significant disease burden and economic cost. Despite ample evidence documenting the efficacy of a diverse array of established MDD therapies, a large proportion of patients are inadequately responsive to initial treatment attempts, with first-line treatments leading to remission in only ~30% of the patients. Importantly, about a third of patients even fail to achieve symptom improvement after two or more antidepressant trials, resulting in what is commonly defined as treatment-resistant depression (TRD). More than 2.8 million infections due to antibiotic-resistant bacteria occur annually in the United States, and more than 35,000 people die as a result. Domestic wastewater (sewage), which contains antibiotic-resistant bacteria and antibiotic resistance genes, is collected at treatment plants for removal of contaminants, e.g. pathogenic microorganisms and nutrients. While wastewater treatment is designed to remove antibiotic-resistant bacteria and other microorganisms through treatment and disinfection, some bacteria may escape treatment. Treated wastewater effluent is generally discharged into surface waters; however, some fraction of the effluent may be used as recycled water. Influenza remains a leading cause of morbidity and mortality in the United States, with Black individuals disproportionately affected. Pregnant women are particularly vulnerable due to their modulated immune state, placing them at increased risk of complications, hospitalization, and intensive care admission. Despite strong evidence supporting the safety and efficacy of maternal seasonal influenza vaccination (SIV), uptake remains suboptimal. This study employed a multi-method design to identify barriers and facilitators of maternal SIV uptake, with a particular focus on racial disparities.
