OBJECTIVES: Enabling the unambiguous communication of cancer genetic testing results and the corresponding therapeutic implications of identified genetic variants is of major clinical importance. In this report, we assess the adequacy of the Health Level Seven Fast Healthcare Interoperability Resources (FHIR) Genomics Reporting Implementation Guide, version 3 (also known as "FHIR Genomics") for the structured communication of semantic objects typically appearing in real-world somatic testing reports.

Atomoxetine, a selective norepinephrine reuptake inhibitor, is a nonstimulant alternative to treat attention-deficit/hyperactivity disorder in children and adolescents. Atomoxetine exposure is highly variable due to cytochrome P450 polymorphism; however, the impact of these genetic variations has not been adequately characterized in the pediatric population. A population pharmacokinetic (PK) model was developed to evaluate steady-state atomoxetine exposures after oral administration in a large, heterogeneous population of children and adolescents with different metabolic phenotypes. BACKGROUND: Pediatric oncology and hematopoietic stem cell transplant (HSCT) patients have elevated risk for Clostridioides difficile infection (CDI), which can prolong hospitalization and delay chemotherapy. Colonization is an important prelude to symptomatic CDI. We sought to characterize colonization status in these patients.
METHODS: We retrospectively studied 276 stools longitudinally collected over 34 months from 32 HSCT and 12 oncology patients treated at a single tertiary center. Specimens were cultured for C difficile and compared by whole genome sequencing. The fecal microbiome was characterized by 16S rRNA gene sequencing.

BACKGROUND: Activated T-cell pediatric acute liver failure (TC-PALF) is the most common cause of non-acetaminophen PALF, with poor transplant-free survival. Livers in TC-PALF are infiltrated by effector cytotoxic T lymphocytes with markers of tissue-resident memory function (CD8 Trm) and an interferon gamma (IFNγ) transcriptional signature. The PALF-Immune Response Network (PALF-IRN) and the prospective TReatment for ImmUne-Mediated PathopHysiology (TRIUMPH) clinical trial (NCT04862221) aim to characterize the TC-PALF immune pathology and utility of T-cell directed therapy to improve transplant-free survival.

Optimal nutrition is central to growth and development in all children, and is particularly relevant in those with chronic illnesses. This document provides 10 practical, evidence-informed tips on the assessment and management of the nutritional prescription in children with chronic kidney disease (CKD) and those receiving dialysis. OBJECTIVE: Many young children with isolated skull fractures are referred to child protective services (CPS) despite being assessed to have a low likelihood of abuse. We sought to identify factors associated with CPS referrals in this population and to quantify hospital-level variation.
METHODS: We performed a multicenter retrospective cross-sectional study of children < 2 years with skull fractures with or without a small underlying intracranial hemorrhage (ICH) and no additional injuries undergoing a child abuse pediatrics (CAP) subspecialty evaluation. We explored associations between demographic factors, clinical characteristics, psychosocial risk factors, and hospital site with CPS referral status. We performed multivariable logistic regression, adjusting for transfer status and all significant covariates from unadjusted analyses. We utilized marginal standardization to calculate the estimated probability of CPS referrals at each site, adjusting for all factors in the final model.
RESULTS: Of 528 children, 303 (57.4%) were referred to CPS, with 86.5% referred before CAP consultation. OBJECTIVE: Prior research on confessions of physical abuse (PA) has explored mechanisms of injury. Little is known about conditions supporting a confession of abuse or how confessions may influence case outcomes.
METHODS: This cross-sectional study of suspected PA in children aged < 10 years at 10 centers participating in CAPNET, a multicenter child PA research network, included children with in-person Child Abuse Pediatrics (CAP) consultations between February 2021 and December 2022 and excluded children without injury. Our focus was a confession of inflicted injury known to the CAP during clinical involvement. We compared child and clinical characteristics, out-of-home (OOH) placements, and arrests between cases with and without confessions. Multivariable models using generalized estimating equations (GEE) produced adjusted predicted probabilities (APP) clustering by CAPNET site.
RESULTS: Confessions were known to CAPs in 115/4297 (2.7%) cases, with significant site variability. In a multivariable GEE model, confessions were more likely in cases with injuries with high specificity for abuse (APP 6.0% vs 1.6%, P < .001), near-fatality (APP 5.6% vs 2.3%, P < .001), and older children (APP 4.8% vs 2.5%, P = .025). OOH placements and arrests were more common in cases with confessions. CAP awareness of confessions, OOH placements, and arrests did not follow racial or ethnic patterns seen in the US child welfare system. Lower socioeconomic status was associated with OOH placements and arrests but not with CAP awareness of confessions.
CONCLUSIONS: Our findings highlight differences in case characteristics and outcomes where a confession is known to the CAP and raise questions about …

BACKGROUND: Congenital heart disease (CHD) is the most common congenital malformation, and the most severe type, single ventricle (SV) heart disease, requires the Fontan surgical palliation. Fontan palliation minimizes hypoxemia and volume overload by separating systemic and pulmonary circulations, resulting in the absence of a sub-pulmonary pump. Skeletal muscle health may have a greater relative contribution to overall functional capacity of those with SV physiology, via the skeletal muscle (SkM) pump mechanism and oxidative capacity. Purpose: More than 50 inherited retinal diseases are known, with rod photoreceptor functional loss serving as an early indicator in nearly half of them. The “optoretinogram” is a relatively new assay that detects optical changes in cells in response to stimuli. This tool has excellent potential for providing insights into the earliest functional changes of rod photoreceptors, with the potential to assist in the early detection, monitoring, and treatment of retinal diseases.

Multi-sector partnerships (MSPs) are problem-solving groups composed of government agencies, nonprofit organizations, private firms, community groups, and individuals. These partnerships have often struggled to achieve consistent success, with performance rates estimated at 30%–50%, largely because MSPs are tailored to the specific interests, resources, and implementation contexts of the constellation of stakeholders involved in each project. This situation raised important questions regarding how MSP performance can be predicted and whether the likelihood of success can be enhanced during the early planning stages of partnership formation.

Parenting stress and negative parental attributions are key contributors to child abuse risk. Early parenting stress may increase a parent’s risk of negatively appraising their child’s behaviors, which, in turn, may influence their likelihood of harsh or abusive parenting. Grounded in Social Information Processing (SIP) Theory, the present study explored how these processes evolve over time and differ between mothers and fathers. This study characterised polyhydroxybutyrate (PHB)-producing bacteria from the culture water of genetically improved farmed tilapia (GIFT) in biofloc ponds and investigated the influence of PHB on the growth and immune performance of GIFT. Out of 40 bacterial isolates, Bacillus infantis, Exiguobacterium profundum, Bacillus subtilis, Bacillus megaterium, Klebsiella pneumoniae and Enterococcus hirae showed positive results for PHB against fluorescent stains. The PHB yield in the range of 5%–95.45% (w/w) was recorded, with the highest from K. pneumoniae (95.45% w/w). Spider mites (Oligonychus trichardti) are emerging as a major constraint to Urochloa forage productivity in East Africa; however, knowledge of genotypic variation and tolerance remains limited. Herein, 55 Urochloa genotypes were evaluated under field-infested and non-infested conditions across two seasons using an alpha-lattice design. Agronomic and physiological traits, including plant height (PH), tiller number (TN), the Normalized Difference Vegetation Index (NDVI), total dry weight (TDW), and mite damage indices (visual severity index (VSI) and stress tolerance index (STI)) were assessed. Okra is increasingly recognized as a climate-resilient crop within diversified agroecological systems; however, an integrated synthesis of its biophysical, agronomic, and socioeconomic dimensions is lacking. This review consolidates the current knowledge on okra physiology, production ecology, and value chains, with a focus on climate resilience under heat, drought, salinity, pest, and disease pressures. Evidence across regions demonstrates that agroecological practices such as crop diversification, mulching, conservation tillage, integrated pest management, and the use of organic and biological inputs consistently improve soil health, stabilize yields under climatic stress, and reduce dependence on external inputs, although with trade-offs in labor demand and …

Herpes simplex virus (HSV) is a highly prevalent human pathogen whose symptoms are characterized by the formation of painful blisters and ulcers. There are two major families of HSV: HSV-1, primarily transmitted through oral contact, and HSV-2, predominantly spread through sexual contact. Periodic reactivation of the virus enables continued transmission and recurrent disease. Antiviral drugs that suppress viral replication are available to the public; however, no curative therapy exists.

BACKGROUND: Postnatal care (PNC) plays a crucial role in averting newborn mortality, yet its use remains low, particularly in regions with the highest mortality. While demographic and social determinants of PNC use have been well studied and have informed current strategies focused on changing care-seeking behaviors, the stagnating decline in neonatal mortality highlights the need for upstream and system-wide approaches to increase PNC uptake. Limited evidence exists to guide system-level reforms; therefore, we investigated whether health systems that ensure high-quality perinatal care are associated with increased use of PNC.

The antiviral response to several clinically significant viruses, including respiratory syncytial virus and parainfluenza virus, is driven by copy-back viral genomes (cbVGs) generated during virus replication. However, the broader impact of cbVGs on the functional states of host cells remains undefined. Here, we developed a single-cell RNA-sequencing and computational framework to map cbVG-driven host responses during Sendai virus infection. Unsupervised profiling identified distinct transcriptional states throughout the course of infection, highlighting a shift from early antiviral signaling to later inflammatory and remodeling programs. Recent figures indicate that there are just over 441,000 people living in long-term residential care homes across the United Kingdom (UK) (Berg, 2025), of which 70% are living with dementia or a severe cognitive impairment (Alzheimer’s Society, 2024). Recent studies have demonstrated that a subset of long “noncoding” RNAs (lncRNAs) produce functional polypeptides and proteins. In this study, we discovered a 132 amino acid protein in human breast cancer cells named XCP (X-linked Cancer-associated Polypeptide), which is encoded by lncRNA1456 (a.k.a. RHOXF1P3), a transcript previously thought to be noncoding. lncRNA1456 is a pancreas- and testis-specific RNA whose gene is located on chromosome X. We found that the expression of lncRNA1456 and XCP is highly upregulated in the luminal A, luminal B, and HER2 molecular subtypes of breast cancer. This study examined how internal public relations encourages employees to participate in organizational citizenship behaviors. Using a national survey of employees (N = 339), we tested a conceptual model that proposed linkages among dialogic internal communication, organizational social capital, communal relationships, and organizational citizenship behaviors. The results suggest dialogic internal communication can enhance organizational social capital and encourage internal publics to participate in organizational citizenship behaviors. Additionally, organizational social capital mediated the effects of dialogic internal communication on communal relationships and organizational citizenship behaviors. This study clusters consumers according to their hourly online shopping consumption patterns and develops optimized advertising strategies that can be practically applied by companies. Specifically, we considered multidimensional variables related to consumers, including demographic characteristics, shopping frequency, information search and content consumption characteristics, and lifestyle traits. We explored consumer types by comparing various clustering algorithms and applying the optimal clustering method for each time period. The analysis results show significant differences in payment amounts according to payment time periods. Recognizing the growing potential of AI-generated chatbots in healthcare and healthcare marketing, this study investigates how different types of chatbot-expressed empathy influence user responses in stigmatized versus non-stigmatized health contexts. Perceived authenticity is introduced as a key mediator. Drawing on Mind Perception Theory, a 3 (empathy: none, cognitive, affective) × 2 (illness type: stigmatized, non-stigmatized) online experiment was conducted. The findings show that affective empathy reduced perceived authenticity, which subsequently decreased user satisfaction, perceived understanding, reuse intention, and follow intention. The dusky spinefoot, Siganus luridus (Rüppel, 1829), stands as one of the most economically significant herbivorous species inhabiting the coral reefs of the Gulf of Aqaba, Egypt. Despite its prominence, our understanding of its biology and population structure remains limited.
