Hypophosphatasia (HPP) is a rare, inherited metabolic disorder caused by deficient tissue-nonspecific alkaline phosphatase activity. This study assessed the impact of treatment with asfotase alfa on patient-reported outcomes (PROs) in adults with pediatric-onset HPP. A longitudinal, telephone-based survey was administered to eligible individuals enrolled in a patient support program. Interviews were conducted at study entry (prior to asfotase alfa initiation) and after 3, 6, and 12 mo. In this study, we leveraged machine-learning tools by evaluating expression of genes of pharmacological relevance to standard-AML chemotherapy (ara-C/daunorubicin/etoposide) in a discovery-cohort of pediatric AML patients (N = 163; NCT00136084 ) and defined a 5-gene-drug resistance score (ADE-RS5) that was predictive of outcome (high MRD1 positivity p = 0.013; lower EFS p < 0.0001 and OS p < 0.0001). ADE-RS5 was integrated with a previously defined leukemic-stemness signature (pLSC6) to classify patients into four groups. ADE-RS5, pLSC6 and integrated-score was evaluated for association with outcome in one of the largest assembly of ~3600 AML patients from 10 independent cohorts (1861 pediatric and 1773 adult AML). Patients with high ADE-RS5 had poor outcome in validation cohorts and the previously reported pLSC6 maintained strong significant association in all validation cohorts. For pLSC6/ADE-RS5-integrated-score analysis, using Group-1 (low-scores for ADE-RS5 and pLSC6) as reference, Group-4 (high-scores for ADE-RS5 and pLSC6) showed worst outcome (EFS: p < 0.0001 and OS: p < 0.0001). Groups-2/3 (one high and one low-score) showed intermediate outcome (p < 0.001). Integrated score groups remained an independent predictor of outcome in multivariable-analysis after adjusting for established prognostic factors (EFS: Group 2 vs. 1, HR = 4.68, p < 0.001, Group 3 vs. 1, HR = 3.22, p = 0.01, and Group 4 vs. 1, HR = 7.26, p < 0.001). These results highlight the significant prognostic value of transcriptomics-based scores capturing disease aggressiveness through pLSC6 and drug resistance via ADE-RS5. The pLSC6 stemness score is a significant predictor of outcome and associates with high-risk group features, the ADE-RS5 drug resistance score adds further value, reflecting the clinical utility of simultaneous testing of both for optimizing treatment strategies.

BACKGROUND: Rebound hyperglycemia following the resolution of diabetic ketoacidosis (DKA) is common in pediatric patients with type 1 diabetes, increasing the risk of recurrent DKA and complicating the transition to subcutaneous insulin. Multiple studies suggest that early administration of long-acting insulin analogs during DKA management safely improves this transition.

INTRODUCTION: The Pediatric Rheumatology Care and Outcomes Improvement Network (PR-COIN) is a North American learning health network focused on improving outcomes of children with juvenile idiopathic arthritis (JIA). JIA is a chronic autoimmune disease that can lead to morbidity related to persistent joint and ocular inflammation. INTRODUCTION: Chronic inflammation of the gastrointestinal tissues underlies gastrointestinal inflammatory disorders, leading to tissue damage and a constellation of painful and debilitating symptoms. These disorders include inflammatory bowel diseases (Crohn's disease and ulcerative colitis), and eosinophilic disorders (eosinophilic esophagitis and eosinophilic duodenitis). Gastrointestinal inflammatory disorders can often present with overlapping symptoms necessitating the use of invasive procedures to give an accurate diagnosis.

Electrical muscle stimulation (EMS) has been shown to stimulate the production of myokines (i.e., brain-derived neurotrophic factor (BDNF)), but the most effective EMS parameters for myokine production have not been fully elucidated. The purpose of this study was to quantify the optimal EMS frequency for stimulating myokine production. This study included sixteen young adults (male, n = 13, age = 27.3 ± 5.5 years). This study investigated transparasocial relationships as they relate to identity management for content creators. Unlike traditional parasocial relationships, transparasocial relationships indicate high levels of interactivity between the creator and audience members. This study investigates how this interactivity affects the identity of the creators, specifically content creators on TikTok. A diverse set of 21 creators were reached out to and interviewed in a semi-structured respondent style over videoconference calls. Enamel lesions are common findings in primary teeth suffering from developmental defects of enamel (DDE). Silver Diamine Fluoride (SDF) has been used to reduce the sensitivity of hypomineralized teeth, also as a temporary non-invasive treatment of enamel demineralization by arresting the demineralization process and triggering the remineralization. Retirement represents a significant life transition typically occurring in later adulthood, often accompanied by substantial lifestyle changes. Several theoretical frameworks suggest that these changes present both opportunities and challenges for well-being, and the extent to which individuals experience positive versus negative well-being outcomes may be influenced by various factors. To study such heterogeneity in retirement experiences, researchers have embraced person-centered methodologies. “Buy-now-pay-later” (BNPL) is a relatively unregulated FinTech innovation that provides consumers with easy access to credit for retail purchases. BNPL spending is projected to reach $1 trillion by 2025, but we know little about its effects. Using banking data for10.6 million U.S. consumers, we investigate the effects of BNPL on leading indicators of users’ financial health. We find that new BNPL users experience rapid increases in bank overdraft charges and credit card interest and fees compared with nonusers, consistent with BNPL facilitating overborrowing. An instrumental variable exploiting consumers’ pre-BNPL shopping habits bolsters our inferences. Our results inform regulatory investigations into …

Propafenone is a class 1C antiarrhythmic and is one of the first-line drugs used in the management of atrial fibrillation. Its toxicity is rare, yet potentially life-threatening. Common clinical findings could range from hypotension, dysrhythmias, and conduction disturbances to cardiac arrest. We present a case of an 80-year-old male who presented with generalized weakness and polyuria secondary to over-diuresis leading to dehydration. Electrocardiogram on admission showed a first-degree atrioventricular block, QRS complex widening, and QTC interval prolongation. This paper examines the political constructions of people who use drugs in the Philippines throughout the presidency of Rodrigo Duterte (2016–2022), during which the government engaged in a ‘war on drugs’ and promoted a punitive drug regime.

Methods

Building on and drawing inspiration from the global drug policy scholarship that has looked at the ways in which drugs are framed and problematised in various domains, this study used qualitative content analysis to review 96 documents from national government agencies - including strategic action plans, directives, memorandums, guidelines, annual reports, and legislative measures.

Elexacaftor/tezacaftor/ivacaftor (ETI) therapy has revolutionized the treatment of cystic fibrosis (CF) for most affected individuals but the effects of treatment on sinus microbiota are still unknown. Changes to the airway microbiota in CF are associated with disease state and alterations to the bacterial community after ETI initiation may require changes to clinical management regimens. Previous research suggests dogs with adverse life experiences (e.g., abuse, neglect) have altered HPA-axis activity and attachment styles. Although research on stress biomarkers in dogs has primarily focused on the HPA-axis and cortisol, human research suggests the sympathoadrenal medullary (SAM) pathway is also altered by acute stress and associated with changes in salivary α-amylase. Background: Physical violence and aggression (PVA), defined as behaviors with the potential to cause bodily injury, are unfortunate risks in the management of all-cause neurodegenerative dementias. While dementia in Parkinson's disease (PD) may not be evident for many years after clinical onset, neuropsychiatric disturbances occur at all stages of the disease. At issue is whether PVA in PD is associated with clinical factors that can be targets for prevention and management in the absence of a prevailing dementia syndrome.

Microbial contamination of food processing facilities is a major cause of human disease and economic loss. There are approximately 48,000,000 cases of foodborne illness in the United States annually, leading to 128,000 hospitalizations and 1,300 deaths. The total economic damages are estimated at $36 billion each year, including productive hours, medical expenses, and the disposal of contaminated foodstuffs. Current methods to reduce contamination on food processing surfaces utilize steam or concentrated chemicals (hydrogen peroxide, hypochlorite, strong acids, strong bases) that are hazardous to workers and required large volumes of water to remove prior to resuming contact with food. A Nudge In The Right Direction: Nudging College Students In A Flipped Class To Become More Self-Regulatory, Jilane Michelle Richardson

This dissertation presents a multiple-case study investigating the influence of using nudges to encourage students in a flipped classroom to complete coursework in a timely manner. Using two sections of the same course, the study explored the effectiveness of reminder and self-regulation nudges in promoting on-time completion of assignments among undergraduate college students. The research questions addressed the influence of reminder nudges on coursework completion and the efficacy of self-regulation nudges in encouraging timely completion behaviors. Findings revealed that while reminder nudges initially encouraged accountability and on-time coursework submission rates, students experienced nudge fatigue over time. This three-article dissertation addresses how Indigenous and non-Indigenous state and non-state policy actors collaborating on Idaho missing and murdered Indigenous persons (MMIP) policy shift the Integrative Framework for Collaborative Governance (IFCG) policy context to the ‘third space of sovereignty’ (Bruyneel, 2007). In a space of competing narratives of authority across time, and space, paper one addresses how Indigenous and non-Indigenous policy actors are shaped by the “drivers” of collaboration. Large areas of the sagebrush steppe are destroyed by wildfire, which catalyzes invasive species growth, increases soil nitrogen availability, reduces soil water retention, and disrupts soil symbiont-plant interactions. These changes in biogeochemical properties may explain why restoration success remains limited. Biochar is a soil amendment created from pyrolyzed plant biomass that may reduce soil N availability and increase water retention. Our study aims to evaluate whether changing soil properties using biochar may enhance the survival of sagebrush seedlings. The purpose of this research is to create visual and spatial cues for cello students to better learn the instrument through a notation system developed by the author. The research and notation are based on Noa Eshkol and Abraham Wachman's movement notation, which notates general body movement. The new movement notation, developed by the author, is directed to specific movements for cello players. Learning to play the cello by ear has been a primary pedagogical method. Concern over academic entitlement has grown in higher education in the last two decades. Academic entitlement, defined as unfounded expectations of academic success, undeserved services and unrealistic accommodations (McLellan & Jackson, 2017), in nursing students is an under investigated phenomena. The purpose of this study was to examine the impacts of academic stress and general student satisfaction on academic entitlement beliefs in baccalaureate, junior, and senior level nursing students.
