We present a woman in her 60s who underwent bilateral mastectomy with left axillary targeted lymph node biopsy in the setting of high grade invasive ductal carcinoma of the left breast with metastasis to a left axillary lymph node. Her postoperative course was complicated by high output milky drainage from her left axillary surgical drain, consistent with a chyle leak, after confirmatory testing demonstrated drain fluid with high triglyceride content. The patient failed non-operative management consisting of low-fat diet and compressive measures. Ultimately, the patient returned to the operating room where indocyanine green near-infrared fluorescence lymphography was used to localized …

Objective This study aimed to assess the reliability and purity of technetium-99m (99mTc) generators produced by the Pakistan Institute of Nuclear Science and Technology (PINSTECH), known as PAKGEN, by evaluating the molybdenum (99Mo) breakthrough levels over five years. Maintaining low 99Mo levels is crucial for patient safety and high-quality diagnostic imaging in nuclear medicine. Methods: 189 PAKGEN 99m Tc generators were tested at the Centre for Nuclear Medicine and Radiotherapy (CENAR) in Quetta, Pakistan, from December 2019 to December 2023. Gated recurrent neural networks such as Long Short-Term Memory (LSTM) and Gated Recurrent Units (GRU) help fix instability present in normal recurrent neural networks. This allows them to be used for various real-world tasks, and due to their architecture, they are uniquely qualified to handle variable sized input such as text. However, even before training can begin on a machine learning model, various hyperparameters must be chosen to decide how the model will be architectured. Shoplifting has contributed to significant losses in Oregon, reaching nearly $956 million in 2022. While overall property crime in Portland, Oregon, dropped between 2023 and 2024, reported shoplifting incidents increased by 50.0%. In Oregon, stealing property worth $100 or less is third-degree theft and a Class C misdemeanor. Second-degree theft involving stealing property between $100 and $1,000 is a Class A misdemeanor. Although often classified as petty or minor theft, shoplifting imposes strain on retailers, consumers, and communities, which raises questions about the spatial and temporal distribution of this crime. Librarians providing evidence synthesis services often face project management challenges, from tracking progress on search strategy development to clarifying team roles. Librarians have a variety of tools to address these challenges.1 We propose using electronic lab notebooks (ELNs), which standardize how scientists record research progress. Institutions have adapted ELNs to protect intellectual property and improve rigor and reproducibility.

As technology evolves, so do K-12 educational platforms and policies. However, research into broadband access’s influence on student academic achievement is limited and some are outdated. Without adequate knowledge on the topic, educational and policy leaders are at risk of misallocating educational technology funds. Multiracial Asian American college students endure pervasive and commonplace monoracism, multiracial microaggressions, and deficit-based narratives in higher education, which negatively impact their sense of self, belonging, and mental health. Through narrative methodology, the present qualitative study captures the stories of three multiracial Asian American undergraduate students and how they are making sense of their racialized experiences and cultural wealth in higher education. The results of this study suggest that (a) multiracial Asian Americans experience monoracism and multiracial microaggressions, which shape their understanding of cultural legitimacy and impact their behavior, sense of self, and relationships with others; (b) monoracism and multiracial …

Mastery learning has been one of the most extensively researched instructional strategies in education, studied continuously since the 1960s. Prior meta-analyses have consistently found positive effects of mastery learning on student learning outcomes, however, none have examined its effectiveness on student achievement explicitly within science, technology, engineering, and mathematics (STEM) disciplines.
This comprehensive meta-analysis investigated the effectiveness of the mastery-learning instructional strategy on student achievement across STEM disciplines in K-12 and higher-education contexts. There is growing evidence that green jobs have higher skill requirements, but whether they offer sufficient wage incentives to encourage workers to acquire those skills remains unclear. We study the green wage premium and its drivers to isolate the average return to green tasks using online job vacancy (OJV) data for EU countries over the period 2018-2023. We develop a transparent LLM-based approach to classify job vacancies as green when they list at least one green task. Green jobs pay a premium of 5.5% relative to comparable postings within the same occupation, and this estimate is little changed when controlling …

Graft-versus-host disease (GvHD) is a common and serious complication following allogeneic hematopoietic stem cell transplantation (HSCT). Accurate documentation of GvHD severity and management is essential for clinical decision-making, interdisciplinary communication, and regulatory reporting to organizations such as the Foundation for the Accreditation of Cellular Therapy (FACT) and the Center for International Blood and Marrow Transplant Research (CIBMTR). Dementia remains under-detected in primary care despite recommendations to assess cognitive health during Medicare’s Annual Wellness Visit (AWV). Persistent gaps in clinician knowledge and workflow standardization contribute to delayed diagnosis and missed opportunities for early intervention. Coffee is a key agricultural commodity in Southeast Asia, with Vietnam and Indonesia ranking among the world's leading producers, while the Philippines faces declining output and fragmented research efforts. This study compares coffee-related research in Indonesia, Philippines, and Vietnam from 2015 to 2024 to inform strategies for strengthening Philippine coffee science. A mixed-methods scientometric and thematic approach was applied to a corpus of Scopus-indexed journal articles. Descriptive bibliometric indicators were used to examine research productivity, citation visibility, collaboration structures, institutional participation, funding composition, and disciplinary coverage. An Integrated Value Chain Analysis Of Large Cardmom (Amomum Subulatum Roxb): A Case Of Bhutan, Laxmi Chetri, Muneer Ahmad Magry

This study explores the value chain of large cardamom in Tsirang, Bhutan, through a field survey of 260 households engaged in cardamom farming, using a semi-structured questionnaire. The findings reveal a relatively simple value chain: upstream actors include both male and female farmers who cultivate, harvest, and dry the cardamom, while downstream actors are primarily male local and regional traders who purchase, transport, store, and export the product. Women are largely confined to upstream roles, with limited or no participation in downstream activities. The relationship between farmers and traders is predominantly transactional, lacking formal contracts or advance financing. A cost-benefit …

Clonal hematopoiesis of indeterminate potential (CHIP) and mosaic chromosomal alterations (mCAs) are two types of clonal hematopoiesis (CH) associated with hematological parameters and malignancy risk. Here we show, in genomic data from 546,090 biobank participants, that co-occurring CH (≥2 CH mutations detected) is present in 1.6% of cancer-free individuals and shows strong evidence for selection (up to 804x enrichment). Co-occurrence is more frequent in those with a prior cancer (3.6%), suggesting treatment-induced selection. Acquisition of CHIP usually precedes mCAs with co-occurrences manifesting stronger phenotypic disruptions in telomere attrition and hematologic parameters than component CH events. IMPORTANCE: Understanding approaches to managing treatment-related adverse events (AEs) in immunotherapy and tyrosine kinase inhibitor-based treatments in a clinical setting is critical for reaching optimal patient outcomes.
OBJECTIVE: To investigate the management of AEs in community practices during first-line axitinib plus pembrolizumab therapy for advanced renal cell carcinoma (aRCC).
DESIGN, SETTING, AND PARTICIPANTS: This study comprised a cross-sectional electronic physician survey and a multisite, retrospective cohort study of the medical records of US patients with aRCC who initiated first-line axitinib-pembrolizumab between April 22, 2019, and January 31, 2024. Titanium dioxide (TiO2) is one of the most prevalent food additives utilized in commercial products in the United States. While TiO2 was originally hailed as a nontoxic and cost-effective additive, studies are beginning to reveal that TiO2 is not as safe as it once seemed. TiO2 may contribute to the development of insulin resistance and metabolic disorders through direct and indirect mechanisms. The compound induces gut dysbiosis, leading to alterations in bacterial metabolites and pro-inflammatory cytokines, which results in the selective resistance of glucocorticoid receptors.
